Dunite, wehrlite and websterite xenoliths occur amongst a large abundance of mantle xenoliths in kimberlites of the Kimberley cluster in South Africa. Up to know they have mostly been neglected. On the basis of texture, major and trace elements, oxygen isotopes as well as Re-Os isotope characteristics, they can be subdivided into two groups. A coarse-grained mantle peridotite group, comprising dunite, wehrlite and websterite xenoliths, that are similar to fertile peridotites and represent upper mantle assemblages that are differently influenced by mantle metasomatism. And a cumulate group, containing fine-grained Fe-rich dunite xenoliths that represent cumulates of flood basalt magmatism related to ~183 Ma Karoo and ~2.7 Ga Ventersdorp events in southern Africa. Dunite, wehrlite and websterite xenoliths have preserved a complex history of melt depletion and metasomatic re-enrichment events, which gives information about the different re-enrichment stages of the subcratonic lithospheric mantle and the spatial differences within the Kaapvaal craton upper mantle. Websterite xenoliths comprise orthopyroxene (40-85 Vol. %), clinopyroxene (5-42 Vol. %), garnet (4-10 Vol. %) and subordinately olivine, while dunite and wehrlite xenoliths contain predominantly olivine (65-100 Vol %) and subordinately orthopyroxene, clinopyroxene and garnet. High melt depletion and a dunitic to harzburgitic protolith composition are reflected by high forsterite (Fo90-92) and high olivine NiO contents (2800-5000 ppm) and high orthopyroxene Mg# (Mg/(Mg+Fe)) of 0.91-0.93. Re-depletion ages of predominantly 2.9 Ga reflect a minimum age of melt depletion. Melt depletion ceased in conjunction with collision of the Kimberley block with the Witwatersrand block ~2.9 Ga ago. Subduction related re-fertilisation of the previously depleted mantle xenoliths is documented by i) amoeboid textured orthopyroxene, clinopyroxene and garnet, which crystallized in schlieren along olivine grain boundaries, ii) high whole-rock SiO2, Al2O3, CaO, TiO2, FeO contents, iii) low oxygen isotope ratios in clinopyroxene and garnet of 4.8-5.4 ‰ and 4.7-5.3 ‰, respectively and iv) trace element compositions of wehrlitic clinopyroxene and garnet in equilibrium with high-pressure partial melts of eclogite. Trace element disequilibrium of orthopyroxene with clinopyroxene and garnet indicates a separate origin for orthopyroxene, on one side as primary mantle orthopyroxene in dunite and wehrlite xenoliths and on the other side as reaction product with Si-rich melts produced by partial melting of eclogite. This reaction triggered replacement of olivine by orthopyroxene in the surrounding mantle and produced the typical Si-rich composition of Kaapvaal mantle peridotites. Partial melting of eclogite at higher temperatures produced a second metasomatic melt with lower SiO2, but higher Al2O3, CaO, FeO, Ti, Zr, Hf and a low oxygen isotope ratio. This melt triggered clinopyroxene and locally garnet and rutile crystallization in percolation veins, replacing olivine and orthopyroxene in the Kaapvaal upper mantle. Additionally, websterite xenoliths have experienced late stage cryptic metasomatism by the host kimberlite melt, changing the trace element composition of clinopyroxene, orthopyroxene and garnet to different extent. Hence websterite and most fertile lherzolite xenoliths have experienced three metasomatic events: i) reaction with high-Si melt, ii) percolation of subduction related silica melt with lower SiO2 content and iii) cryptic metasomatism by kimberlite. In contrast, dunite and wehrlite xenoliths have only experienced the second metasomatic event. They represent mantle lithologies further away from metasomatising agents. The Fe-rich dunites comprise olivine neoblasts with subordinate olivine porphyroclasts and parallel-orientated needles of ilmenite, which may enclose spinel. The lower forsterite and NiO contents of olivine in Fe-rich dunites compared to mantle peridotite xenoliths (Fo87-89 vs. Fo93-95 and 1300-2800ppm vs. 2200-3900 ppm, respectively), rules out a restitic origin. Cr-rich spinels are remnants of the original cumulate mineralogy that survived a late stage metasomatic overprint related to the production of the host kimberlite, producing ilmenite and phlogopite in some samples. Olivine porphyroclasts and neoblasts have different trace element compositions, the latter having high Ti, V, Cr and Ni and low Zn, Zr and Nb contents, indicating contrasting origins for neoblasts and porphyroclasts. The dunites have high 187Os/188Os ratios (0.11-0.15) indicating young (Phanerozoic) model ages for most samples, whereas three samples show isotopic mixtures between Phanerozoic neoblasts and ancient porphyroclastic material. Most Fe-rich dunite xenoliths can be interpreted as cumulates of fractional crystallization of Karoo magmatism, whereas the porphyroclasts are interpreted to be remnants from the much earlier Archaean Ventersdorp magmatism.

This thesis presents a comparative developmental study of inflorescences and focuses on the production of the terminal flower (TF). Morphometric attributes of inflorescence meristems (IM) were obtained throughout the ontogeny of inflorescence buds with the aim of describing possible spatial constraints that could explain the failure in developing the TF. The study exposes the inflorescence ontogeny of 20 species from five families of the Eudicots (Berberidaceae, Papaveraceae-Fumarioideae, Rosaceae, Campanulaceae and Apiaceae) in which 745 buds of open (i.e. without TF) and closed (i.e. with TF) inflorescences were observed under the scanning electron microscope.rnThe study shows that TFs appear on IMs which are 2,75 (se = 0,38) times larger than the youngest lateral reproductive primordium. The shape of these IMs is characterized by a leaf arc (phyllotactic attribute) of 91,84° (se = 7,32) and a meristematic elevation of 27,93° (se = 5,42). IMs of open inflorescences show a significant lower relative surface, averaging 1,09 (se=0,26) times the youngest primordium size, which suggests their incapacity for producing TFs. The relative lower size of open IMs is either a condition throughout the complete ontogeny (‘open I’) or a result from the drastic reduction of the meristematic surface after flower segregation (‘open II’). rnIt is concluded that a suitable bulge configuration of the IM is a prerequisite for TF formation. Observations in the TF-facultative species Daucus carota support this view, as the absence of the TF in certain umbellets is correlated with a reduction of their IM dimensions. A review of literature regarding histological development of IMs and genetic regulation of inflorescences suggests that in ‘open I’ inflorescences, the histological composition and molecular activity at the tip of the IM could impede the TF differentiation. On the other side, in ‘open II’ inflorescences, the small final IM bulge could represent a spatial constraint that hinders the differentiation of the TF. The existence of two distinct kinds of ontogenies of open inflorescences suggests two ways in which the loss of the TF could have occurred in the course of evolution.rn

Obiettivi. Valutare l’angiogenesi tumorale mediante la Microvessel density (MVD) come fattore predittivo di mortalità per tumore polmonare non a piccole cellule (NSCLC) pT1aN0M0 trattato chirurgicamente. Metodi. I dati demografici, clinici e istopatologici sono stati registrati per 82 pazienti (60 maschi, 22 femmine) sottoposti a resezione chirurgica in due diverse Chirurgie Toraciche tra gennaio 2002 e dicembre 2007 per tumori polmonari non a piccole cellule pT1AN0M0. La MVD è stata valutata mediante il conteggio visivo dei microvasi positivi alla colorazione immunoistochimica con anticorpo monoclonale anti-CD31 e definita come il numero medio di microvasi per 1 mm2 di campo ottico. Risultati. Sono state eseguite 59 lobectomie (72%) e 23 resezioni sublobari (28%). Reperti istopatologici: 43 adenocarcinomi (52%) e 39 neoplasie non- adenocarcinoma (48%) pT1aN0M0; MVD media: 161 (CD31/mm2); mediana: 148; range 50-365, cut-off=150. Una MVD elevata (> 150 CD31/mm2) è stata osservata in 40 pazienti (49%), una MVD ridotta ( ≤ 150 CD31/mm2 ) in 42 pazienti (51%). Sopravvivenze a 5 anni: 70 % e 95%, rispettivamente per il gruppo ad elevata MVD vs il gruppo a ridotta MVD con una p = 0,0041, statisticamente significativa. Il tipo di resezione chirurgica, il diametro del tumore, le principali comorbidità e l’istotipo nono sono stati fattori predittivi significativi di mortalità correlata alla malattia. La MVD è risultata essere superiore nel gruppo “Adenocarcinoma” (MVD mediana=180) rispetto al gruppo “Non-Adenocarcinoma (MVD mediana=125), con un test di Mann-Whitney statisticamente significativo (p < 0,0001). Nel gruppo “Adenocarcinoma” la sopravvivenza a 5 anni è stata del 66% e 93 %, rispettivamente per i pazienti con MVD elevata e ridotta (p = 0.043. Conclusioni. Il nostro studio ha mostrato che la Microvessel density valutata con la colorazione immunoistochimica per CD31 ha un valore prognostico rilevante nel carcinoma polmonare in stadio precoce pT1aN0M0.

Hintergrund: Die antimetabolitgestützte Trabekulektomie stellt seit längeren denrnGoldstandard bei medikamentös nicht ausreichend therapierbaren Glaukomen dar. Kurz- und mittelfristige Erfolge wurden durch viele Studien bestätigt. Allerdings unterliegen diese sehr unterschiedlichen Erfolgsdefinitionen. Eine strikte Druckkontrolle ≤ 15 mm Hg ohne zusätzliche medikamentöse Therapie erscheint sinnvoll einen risikofreien Therapieerfolg zu bewerten. Es existieren nur wenige Langzeitstudien mit diesem Erfolgskriterium. Die durchgeführte Studie soll einen Eindruck der ophthalmologischen Versorgung trabekulektomierter Patienten an der Universitätsaugenklinik Mainz über einen bewusst langen Zeitraum bieten. Patienten und Methoden: In diese retrospektiven Studie wurden alle Patienten, die aufgrund einer fortgeschrittenen Glaukomerkrankung in den Jahren 1996, 2001 oder 2006 eine Trabekulektomie erhielten, aufgenommen. Von den 723 Augen der 664 Patienten dieser Jahrgänge konnten 447 (61,8%) nachverfolgt werden. Die Zusammensetzung der Patienten war mit anderen Studien vergleichbar. 28% konnten mindestens 7 Jahre, 10% sogar 10 Jahre nachverfolgt werden. Esrnwurde untersucht, ob ein signifikanter Zusammenhang zwischen dem ophthalomologisch-internistischem Entlassstatus (Visus, Tensio, Gesichtsfeld,rnGlaukomtyp, Voroperationen, Medikation, Vorerkrankungen, Art der Operation) undrnder erstrebten Kontrolle des Intraokulardruckes besteht. Ergebnisse: Die mittlere Nachbeobachtungszeit betrug 4,3 ± 3,4 Jahre. Nach 1, 3,rn5, 7 und 10 Jahren wiesen 217 (82,1%) (p < 0,001), 133 (67,7%) (p < 0,001), 70rn(50%) (p < 0,001), 59 (47,7%) (p = 0,056) und 16 (38,1%) (p = 0,06) Augen Intraokulardrücke ≤ 15 mm Hg ohne zusätzliche Antiglaukomatosa auf. Nichtrnstatistisch signifikant waren die 7- und 10-Jahresergebnisse. Mit Hilfe von Antiglaukomatosa waren es insgesamt, 225 (85,1%), 156 (79,7%), 87 (62,5%), 93 (75%) und 23 (54,7%) (alle p < 0,001). Die mediane Überlebenszeit für IOD ≤ 15 mm Hg ohne Medikation betrug 7,4 Jahre ± 5 Monate. Druckobergrenzen von ≤ 18 bzw. 21 mm Hg erfüllten bis zu 20% mehr Patienten. Der mittlere Visus von 0,32 ± 6 Stufen blieb nach einem mittleren postoperativen Abfall auf 0,25 ± 5 Stufen in den Folgeuntersuchungen stabil. Er zeigte ab dem 3-Jahresintervall keine statistisch signifikante Verschlechterung zum präoperativen Visus. 5,8 Jahre ± 80 Tage betrug die mediane Überlebenszeit für ein stabiles Gesichtsfeld. Gesichtsfelddaten, MD und PSD zeigten keine statistisch signifikante Verschlechterung (p > 0,05). Risikofaktoren für ein Scheitern der Operation waren Patientenalter (RR = 1,01, KI: 0,95 - 1,34, p = 0,043), arterielle Hypertonie (RR = 1,87, KI: 1,21-2,9, p = 0,005) und männliches Geschlecht (RR = 1,24; KI: 1,07 – 1,43; p = 0,004). Komplikationen waren passagere okuläre Hypotonien an 85 (19%), Fistulation an 46(10,2%), Aderhautschwellung an 29 (6,4%) –abhebung an 14 (3,1%), retinale Amotio an 9 (2%), hypotone Makulopathie an 5 (1,1%) und Hypertonien an 70 (15,6%) Augen. 150 (33,5%) Augen erhielten einen Folgeeingriff, 117 (26%) eine Phakoemulsifikation, 149 (33%) eine Fadenlockerung, 122 (27%) 5-FU-Injektionen, 42 (9,4%) eine Fadennachlegung, 33 (7,4%) ein Needling, 26 (5,8%) eine Zyklophotokoagulation, 19 (4,3%) eine Re-TE und 9 (2%) sonstige chirurgische Revisionen. Schlussfolgerung: Die Kontrolle des Augeninnendruckes ≤ 15 mm Hg ohne zusätzliche Medikation erreichten viele Patienten über einen langen Nachbeobachtungszeitraum. Die Häufigkeit der Komplikationen oder nötiger Folgeeingriffe war meist niedriger als in vergleichbaren Studien. Selbst Patienten mit hohem Risikoprofil hatten gute Ergebnisse. Aufgrund mangelnder Gesichtsfelddaten fanden sich keine Hinweise auf statistisch relevantes Fortschreiten des Glaukoms zur angestrebten medikationsfreien Druckkontrolle. Weitere Studien für einen Untersuchungszeitraum von 10 Jahren mit gleichen Erfolgskriterien wie in der vorliegenden Arbeit mit genauer Analyse der Gesichtsfelddaten wären wünschenswert, um zu belegen, dass die guten Langzeitergebnisse nach Trabekulektomie an der Universitätsaugenklinik Mainz auch eine Glaukomprogredienz dauerhaft verhindern. Damit stellt die an der Universitätsaugenklinik Mainz durchgeführte antimetabolitgestützte Trabekulektomie und deren postoperative Nachbetreuung an einer repräsentativen Population eine sichere und komplikationsarme Methode dar.

Lo sviluppo tecnologico e la nascita di Internet ha portato alla costituzione di un nuovo canale di comunicazione il quale costituisce, insieme all'uso dei sistemi informatici, un veicolo per la realizzazione di illeciti. Lo scopo di questa tesi di laurea è di presentare e analizzare in modo critico i principali reati informatici. Le ricerche sono state effettuate attraverso diverse banche dati, lettura di libri e vari articoli. La tesi espone al primo capitolo una panoramica storica e generale sui reati informatici. Nel secondo capitolo viene esaminata la Legge 23 dicembre 1993 n. 547. Il terzo capitolo è il cuore della tesi e analizza i principali reati informatici insieme a relativi casi giuridici. Il quarto ed ultimo capitolo, esamina l'esigenza di avere una normativa comune tra i vari stati del mondo. La trattazione fornisce un quadro d'insieme sul dilagante fenomeno dei crimini informatici, seguito da un'analisi approfondita dei singoli reati. La ricerca e lo studio dei casi reali hanno consentito di concretizzare la normativa teorica.

La documentazione tecnica ha assunto un ruolo fondamentale per i produttori sia per rispondere agli obblighi di legge (v. Direttiva Macchine 2006/42/CE) sia come strumento di marketing. Dato l’alto grado di ripetitività dei manuali di istruzioni e le numerose lingue di destinazione, per la loro traduzione è indispensabile l’uso dei CAT tool. In questo contesto, la presente tesi si pone l’obiettivo di individuare una serie di elementi linguistici che possano fornire informazioni sulla qualità dei testi di partenza in italiano e su cui si possa intervenire per migliorarli, nell’ottica di ottimizzare l’uso dei CAT tool e ridurre il costo delle traduzioni. L’idea alla base del progetto è infatti che le caratteristiche dei testi originali influiscano in maniera fondamentale sulla qualità del risultato finale, così come sul costo del servizio di traduzione. I contenuti sono suddivisi in cinque capitoli. Nel primo capitolo si analizzano gli studi sulla leggibilità dei testi, sulla semplificazione del linguaggio e sui linguaggi specialistici. Nel secondo capitolo si presenta il funzionamento dei CAT tool e si approfondiscono gli aspetti rilevanti per l’ottimizzazione del loro uso nella traduzione dei manuali di istruzioni. Nel terzo capitolo si combinano le conoscenze acquisite nei primi due capitoli e si propone un metodo di revisione dei manuali da sottoporre a traduzione. Tale revisione ha lo scopo di agevolare i CAT tool nel riconoscimento della somiglianza tra le frasi e di permettere di conseguenza un lavoro di traduzione a costi inferiori, senza però perdere di vista la qualità finale del manuale e la sua leggibilità e chiarezza. Nel quarto capitolo si espone l’applicazione pratica del metodo e si analizzano e discutono i risultati ottenuti. Nel quinto capitolo si traggono le conclusioni finali sul metodo proposto e i relativi risultati. Si propongono inoltre argomenti di riflessione e discussione che suggeriscono nuovi scenari e prospettive.

Le lagune costituiscono ecosistemi dinamici ed eterogenei, caratterizzati da comunità biologiche più ristrette rispetto agli ambienti marini confinanti. Si tratta di ambienti delicati e spesso soggetti a disturbo antropico, per cui è importante monitorare lo stato ecologico del loro sistema. Un ottimo strumento per questa valutazione è rappresentato dalle comunità di invertebrati che popolano i sedimenti. Nel presente studio ci si propone di indagare la struttura dei popolamenti macrobentonici nella laguna di Messolonghi (Grecia occidentale), parte di un sistema lagunare di importanza ecologica in tutto il Mediterraneo, come testimoniato dalla designazione di ‘important bird area’ (IPA) nell’ambito del programma sviluppato dall’organizzazione BirdLife International; dalla presenza di siti dichiarati di interesse comunitario (SIC) all’interno della rete di protezione europea “Natura 2000”, nell’ambito della Direttiva Habitat (92/43/CEE). In questo studio viene indagata la struttura della comunità di invertebrati nella laguna e il ruolo della vegetazione e delle condizioni ambientali nel condizionare la disposizione dei popolamenti. A tale scopo vengono applicati indici di diversità che vengono messi in relazione alle specie vegetali presenti e alle caratteristiche del sedimento. I risultati mostrano una comunità diversificata, non regolata da un preciso gradiente ambientale, su cui la principale influenza risulta essere la vegetazione nel suo complesso. Nelle stazioni vegetate si trova un’elevata ricchezza specifica mentre l’equidistribuzione rimane invariata tra habitat vegetato e non. Dalle possibili correlazioni esaminate, risulta che l’abbondanza di alcuni dei taxa più importanti nel differenziamento delle strutture di comunità osservato varia in relazione alla granulometria dei sedimenti e al loro contenuto in azoto e carbonio organico.

Pemphigus vulgaris (PV) is a severe autoimmune bullous skin disease and is primarily associated with IgG against desmoglein 3 (dsg3), a desmosomal adhesion protein. In light of the recent association of autoreactive T helper (Th) 2 cells with active PV, the present study sought to relate the occurrence of Th2-regulated dsg3-specific autoantibody subtypes, i.e. IgE and IgG4, in 93 well-characterized PV patients. Patients with acute onset PV (n=37) showed the highest concentrations of serum IgE and IgG4 autoantibodies, which were significantly lower in PV patients in remission (n=14). Furthermore, there was a strong correlation between dsg3-reactive IgE and IgG4 in acute onset, but not in chronic active (n=42) or remittent patients. Additionally, intercellular IgE deposits were detected in the epidermis of acute onset PV. Thus, dsg3-specific IgE and IgG4 autoantibodies are related to acute onset disease which provides additional support to the concept that PV is a Th2-driven autoimmune disorder.

The concept of chronic critical limb ischaemia (CLI) emerged late in the history of peripheral arterial occlusive disease (PAOD). The historical background and changing definitions of CLI over the last decades are important to know in order to understand why epidemiologic data are so difficult to compare between articles and over time. The prevalence of CLI is probably very high and largely underestimated, and significant differences exist between population studies and clinical series. The extremely high costs associated with management of these patients make CLI a real public health issue for the future. In the era of emerging vascular surgery in the 1950s, the initial classification of PAOD by Fontaine, with stages III and IV corresponding to CLI, was based only on clinical symptoms. Later, with increasing access to non-invasive haemodynamic measurements (ankle pressure, toe pressure), the need to prove a causal relationship between PAOD and clinical findings suggestive of CLI became a real concern, and the Rutherford classification published in 1986 included objective haemodynamic criteria. The first consensus document on CLI was published in 1991 and included clinical criteria associated with ankle and toe pressure and transcutaneous oxygen pressure (TcPO(2)) cut-off levels <50 mmHg, <30 mmHg and <10 mmHg respectively). This rigorous definition reflects an arterial insufficiency that is so severe as to cause microcirculatory changes and compromise tissue integrity, with a high rate of major amputation and mortality. The TASC I consensus document published in 2000 used less severe pressure cut-offs (≤ 50-70 mmHg, ≤ 30-50 mmHg and ≤ 30-50 mmHg respectively). The thresholds for toe pressure and especially TcPO(2) (which will be also included in TASC II consensus document) are however just below the lower limit of normality. It is therefore easy to infer that patients qualifying as CLI based on TASC criteria can suffer from far less severe disease than those qualifying as CLI in the initial 1991 consensus document. Furthermore, inclusion criteria of many recent interventional studies have even shifted further from the efforts of definition standardisation with objective criteria, by including patients as CLI based merely on Fontaine classification (stage III and IV) without haemodynamic criteria. The differences in the natural history of patients with CLI, including prognosis of the limb and the patient, are thus difficult to compare between studies in this context. Overall, CLI as defined by clinical and haemodynamic criteria remains a severe condition with poor prognosis, high medical costs and a major impact in terms of public health and patients' loss of functional capacity. The major progresses in best medical therapy of arterial disease and revascularisation procedures will certainly improve the outcome of CLI patients. In the future, an effort to apply a standardised definition with clinical and objective haemodynamic criteria will be needed to better demonstrate and compare the advances in management of these patients.

Human invariant natural killer T (NKT) cell TCRs bind to CD1d via an "invariant" Vα24-Jα18 chain (iNKTα) paired to semi-invariant Vβ11 chains (iNKTβ). Single-amino acid variations at position 93 (p93) of iNKTα, immediately upstream of the "invariant" CDR3α region, have been reported in a substantial proportion of human iNKT-cell clones (4-30%). Although p93, a serine in most human iNKT-cell TCRs, makes no contact with CD1d, it could affect CD1d binding by altering the conformation of the crucial CDR3α loop. By generating recombinant refolded iNKT-cell TCRs, we show that natural single-nucleotide variations in iNKTα, translating to serine, threonine, asparagine or isoleucine at p93, exert a powerful effect on CD1d binding, with up to 28-fold differences in affinity between these variants. This effect was observed with CD1d loaded with either the artificial α-galactosylceramide antigens KRN7000 or OCH, or the endogenous glycolipid β-galactosylceramide, and its importance for autoreactive recognition of endogenous lipids was demonstrated by the binding of variant iNKT-cell TCR tetramers to cell surface expressed CD1d. The serine-containing variant showed the strongest CD1d binding, offering an explanation for its predominance in vivo. Complementary molecular dynamics modeling studies were consistent with an impact of p93 on the conformation of the CDR3α loop.

Axillary clearance in early breast cancer aims to improve locoregional control and provide staging information but is associated with undesirable morbidity. We therefore investigated whether avoiding axillary surgery in older women would result in improved quality of life (QL) with similar disease-free survival (DFS) and overall survival (OS).

OBJECTIVES: To assess the microbiological outcome of local administration of minocycline hydrochloride microspheres 1 mg (Arestin) in cases with peri-implantitis and with a follow-up period of 12 months. MATERIAL AND METHODS: After debridement, and local administration of chlorhexidine gel, peri-implantitis cases were treated with local administration of minocycline microspheres (Arestin). The DNA-DNA checkerboard hybridization method was used to detect bacterial presence during the first 360 days of therapy. RESULTS: At Day 10, lower bacterial loads for 6/40 individual bacteria including Actinomyces gerensceriae (P<0.1), Actinomyces israelii (P<0.01), Actinomyces naeslundi type 1 (P<0.01) and type 2 (P<0.03), Actinomyces odontolyticus (P<0.01), Porphyromonas gingivalis (P<0.01) and Treponema socranskii (P<0.01) were found. At Day 360 only the levels of Actinobacillus actinomycetemcomitans were lower than at baseline (mean difference: 1x10(5); SE difference: 0.34x10(5), 95% CI: 0.2x10(5) to 1.2x10(5); P<0.03). Six implants were lost between Days 90 and 270. The microbiota was successfully controlled in 48%, and with definitive failures (implant loss and major increase in bacterial levels) in 32% of subjects. CONCLUSIONS: At study endpoint, the impact of Arestin on A. actinomycetemcomitans was greater than the impact on other pathogens. Up to Day 180 reductions in levels of Tannerella forsythia, P. gingivalis, and Treponema denticola were also found. Failures in treatment could not be associated with the presence of specific pathogens or by the total bacterial load at baseline. Statistical power analysis suggested that a case control study would require approximately 200 subjects.

OBJECTIVES: To compare the clinical outcomes of standard, cylindrical, screw-shaped to novel tapered, transmucosal (Straumann Dental implants immediately placed into extraction sockets. Material and methods: In this randomized-controlled clinical trial, outcomes were evaluated over a 3-year observation period. This report deals with the need for bone augmentation, healing events, implant stability and patient-centred outcomes up to 3 months only. Nine centres contributed a total of 208 immediate implant placements. All surgical and post-surgical procedures and the evaluation parameters were discussed with representatives of all centres during a calibration meeting. Following careful luxation of the designated tooth, allocation of the devices was randomly performed by a central study registrar. The allocated SLA titanium implant was installed at the bottom or in the palatal wall of the extraction socket until primary stability was reached. If the extraction socket was >or=1 mm larger than the implant, guided bone regeneration was performed simultaneously (Bio Oss and BioGide. The flaps were then sutured. During non-submerged transmucosal healing, everything was done to prevent infection. At surgery, the need for augmentation and the degree of wound closure was verified. Implant stability was assessed clinically and by means of resonance frequency analysis (RFA) at surgery and after 3 months. Wound healing was evaluated after 1, 2, 6 and 12 weeks post-operatively. RESULTS: The demographic data did not show any differences between the patients receiving either standard cylindrical or tapered implants. All implants yielded uneventful healing with 15% wound dehiscences after 1 week. After 2 weeks, 93%, after 6 weeks 96%, and after 12 weeks 100% of the flaps were closed. Ninety percent of both implant designs required bone augmentation. Immediately after implantation, RFA values were 55.8 and 56.7 and at 3 months 59.4 and 61.1 for cylindrical and tapered implants, respectively. Patient-centred outcomes did not differ between the two implant designs. However, a clear preference of the surgeon's perception for the appropriateness of the novel-tapered implant was evident. CONCLUSIONS: This RCT has demonstrated that tapered or standard cylindrical implants yielded clinically equivalent short-term outcomes after immediate implant placement into the extraction socket.

This study compared the performance of fluorescence-based methods, radiographic examination, and International Caries Detection and Assessment System (ICDAS) II on occlusal surfaces. One hundred and nineteen permanent human molars were assessed twice by 2 experienced dentists using the laser fluorescence (LF and LFpen) and fluorescence camera (FC) devices, ICDAS II and bitewing radiographs (BW). After measuring, the teeth were histologically prepared and assessed for caries extension. The sensitivities for dentine caries detection were 0.86 (FC), 0.78 (LFpen), 0.73 (ICDAS II), 0.51 (LF) and 0.34 (BW). The specificities were 0.97 (BW), 0.89 (LF), 0.65 (ICDAS II), 0.63 (FC) and 0.56 (LFpen). BW presented the highest values of likelihood ratio (LR)+ (12.47) and LR- (0.68). Rank correlations with histology were 0.53 (LF), 0.52 (LFpen), 0.41 (FC), 0.59 (ICDAS II) and 0.57 (BW). The area under the ROC curve varied from 0.72 to 0.83. Inter- and intraexaminer intraclass correlation values were respectively 0.90 and 0.85 (LF), 0.93 and 0.87 (LFpen) and 0.85 and 0.76 (FC). The ICDAS II kappa values were 0.51 (interexaminer) and 0.61 (intraexaminer). The BW kappa values were 0.50 (interexaminer) and 0.62 (intraexaminer). The Bland and Altman limits of agreement were 46.0 and 38.2 (LF), 55.6 and 40.0 (LFpen) and 1.12 and 0.80 (FC), for intra- and interexaminer reproducibilities. The posttest probability for dentine caries detection was high for BW and LF. In conclusion, LFpen, FC and ICDAS II presented better sensitivity and LF and BW better specificity. ICDAS II combined with BW showed the best performance and is the best combination for detecting caries on occlusal surfaces.

PURPOSE: We evaluated the long-term outcome after 1 cycle of adjuvant modified bleomycin, etoposide and cisplatin chemotherapy in patients who underwent orchiectomy for high risk clinical stage I nonseminomatous germ cell tumor of the testis. MATERIALS AND METHODS: Between 1995 and 1999 a consecutive series of 44 patients underwent orchiectomy for clinical stage I nonseminomatous germ cell tumor of the testis, followed by a single postoperative cycle of adjuvant modified bleomycin, etoposide and cisplatin for vascular or lymphatic tumor invasion, and/or a predominance (50% or greater) of embryonal carcinoma. RESULTS: Four of the 44 patients were excluded from analysis. Of the patients 35 had no evidence of disease at a median followup of 99 months (range 60 to 134). One patient with progression after 13 months showed complete remission after 3 cycles of salvage bleomycin, etoposide and cisplatin chemotherapy but he died of pneumonia 4 weeks after the third course. Two patients underwent orchiectomy for contralateral testis cancer at 18 and 42 months, respectively, followed by an additional 3 cycles of adjuvant chemotherapy. They remained relapse-free for 4 and 92 months, respectively. The former patient was lost to followup after 4 months. Two other patients were disease-free at 10 and 31 months, respectively, and were lost to followup thereafter. Late side effects were tinnitus in 3 patients and involuntary childlessness in 3, of whom 2 had cryptorchidism of the contralateral testis. Nine patients fathered children. CONCLUSIONS: One cycle of bleomycin, etoposide and cisplatin effectively decreases the risk of relapse in patients with high risk stage I nonseminomatous germ cell tumor of the testis. It has minimal side effects and can be a valuable alternative to retroperitoneal lymph node dissection.
