OBJECTIVE: Therapeutic temperature modulation is recommended after cardiac arrest (CA). However, body temperature (BT) regulation has not been extensively studied in this setting. We investigated BT variation in CA patients treated with therapeutic hypothermia (TH) and analyzed its impact on outcome. METHODS: A prospective cohort of comatose CA patients treated with TH (32-34°C, 24h) at the medical/surgical intensive care unit of the Lausanne University Hospital was studied. Spontaneous BT was recorded on hospital admission. The following variables were measured during and after TH: time to target temperature (TTT=time from hospital admission to induced BT target <34°C), cooling rate (spontaneous BT-induced BT target/TTT) and time of passive rewarming to normothermia. Associations of spontaneous and induced BT with in-hospital mortality were examined. RESULTS: A total of 177 patients (median age 61 years; median time to ROSC 25 min) were studied. Non-survivors (N=90, 51%) had lower spontaneous admission BT than survivors (median 34.5 [interquartile range 33.7-35.9]°C vs. 35.1 [34.4-35.8]°C, p=0.04). Accordingly, time to target temperature was shorter among non-survivors (200 [25-363]min vs. 270 [158-375]min, p=0.03); however, when adjusting for admission BT, cooling rates were comparable between the two outcome groups (0.4 [0.2-0.5]°C/h vs. 0.3 [0.2-0.4]°C/h, p=0.65). Longer duration of passive rewarming (600 [464-744]min vs. 479 [360-600]min, p<0.001) was associated with mortality. CONCLUSIONS: Lower spontaneous admission BT and longer time of passive rewarming were associated with in-hospital mortality after CA and TH. Impaired thermoregulation may be an important physiologic determinant of post-resuscitation disease and CA prognosis. When assessing the benefit of early cooling on outcome, future trials should adjust for patient admission temperature and use the cooling rate rather than the time to target temperature.

We report the effect of Stryphnodendron adstringens on the trypanosomatid Herpetomonas samuelpessoai. The parasites were grown at 28ºC in a chemically defined medium containing crude extract and fractions at concentrations from 100 to 5000 µg/ml obtained from S. adstringens. Concentrations of 500, 1000, 2500, and 5000 µg/ml both crude extract and semi-purified fraction progressively inhibited the protozoans' growth. At a concentration of 100 µg/ml, crude extract or a semi-purified (F3) fraction did not affect the growth of the protozoans. The F3-9 - F3-12 sub-fractions, at a concentration of 1000 µg/ml, also showed increased inhibitory activity on H. samuelpessoai. The IC50 of the crude extract and the F3 fraction were 538 and 634 µg/ml, respectively. Ultrastructural and enzymatic alterations in the trypanosomatids were also evaluated. H. samuelpessoai cultivated in the presence of IC50 crude extract showed considerable ultrastructural alterations, such as marked mitochondrial swelling with a large number of cristae and evident Golgi complex vesiculation, as observed by transmission electron microscopy. Cells exposed to 538 µg/ml of crude extract at 28ºC for 72 h, showed decreased activity of the enzyme succinate cytochrome c reductase, a typical mitochondrion marker, as compared to untreated cells

Several studies have analyzed the relationship between androgenetic alopecia and cardiovascular disease (mainly heart disease). However few studies have analyzed lipid values in men and women separately. This case-control study included 300 patients consecutively admitted to an outpatient clinic, 150 with early onset androgenetic alopecia (80 males and 70 females) and 150 controls (80 males and 70 females) with other skin diseases. Female patients with androgenic alopecia showed significant higher triglycerides values (123.8 vs 89.43 mg/dl, p = 0.006), total cholesterol values (196.1 vs 182.3 mg/dl, p = 0.014), LDL-C values (114.1 vs 98.8 mg/dl, p = 0.0006) and lower HDL-C values (56.8 vs 67.7 mg/dl, p <0.0001) versus controls respectively. Men with androgenic alopecia showed significant higher triglycerides values (159.7 vs 128.7 mg/dl, p = 0.04) total cholesterol values (198.3 vs 181.4 mg/dl, p = 0.006) and LDL-C values (124.3 vs 106.2, p = 0.0013) versus non-alopecic men. A higher prevalence of dyslipidemia in women and men with androgenic alopecia has been found. The elevated lipid values in these patients may contribute, alongside other mechanisms, to the development of cardiovascular disease in patient with androgenic alopecia.

A cohort of 123 adult contacts was followed for 18‐24 months (86 completed the follow-up) to compare conversion and reversion rates based on two serial measures of QuantiFERON (QFT) and tuberculin skin test (TST) (PPD from TUBERSOL, Aventis Pasteur, Canada) for diagnosing latent tuberculosis (TB) in household contacts of TB patients using conventional (C) and borderline zone (BZ) definitions. Questionnaires were used to obtain information regarding TB exposure, TB risk factors and socio-demographic data. QFT (IU/mL) conversion was defined as <0.35 to ≥0.35 (C) or <0.35 to >0.70 (BZ) and reversion was defined as ≥0.35 to <0.35 (C) or ≥0.35 to <0.20 (BZ); TST (mm) conversion was defined as <5 to ≥5 (C) or <5 to >10 (BZ) and reversion was defined as ≥5 to <5 (C). The QFT conversion and reversion rates were 10.5% and 7% with C and 8.1% and 4.7% with the BZ definitions, respectively. The TST rates were higher compared with QFT, especially with the C definitions (conversion 23.3%, reversion 9.3%). The QFT conversion and reversion rates were higher for TST ≥5; for TST, both rates were lower for QFT <0.35. No risk factors were associated with the probability of converting or reverting. The inconsistency and apparent randomness of serial testing is confusing and adds to the limitations of these tests and definitions to follow-up close TB contacts.

The dynamics of homogeneously heated granular gases which fragment due to particle collisions is analyzed. We introduce a kinetic model which accounts for correlations induced at the grain collisions and analyze both the kinetics and relevant distribution functions these systems develop. The work combines analytical and numerical studies based on direct simulation Monte Carlo calculations. A broad family of fragmentation probabilities is considered, and its implications for the system kinetics are discussed. We show that generically these driven materials evolve asymptotically into a dynamical scaling regime. If the fragmentation probability tends to a constant, the grain number diverges at a finite time, leading to a shattering singularity. If the fragmentation probability vanishes, then the number of grains grows monotonously as a power law. We consider different homogeneous thermostats and show that the kinetics of these systems depends weakly on both the grain inelasticity and driving. We observe that fragmentation plays a relevant role in the shape of the velocity distribution of the particles. When the fragmentation is driven by local stochastic events, the longvelocity tail is essentially exponential independently of the heating frequency and the breaking rule. However, for a Lowe-Andersen thermostat, numerical evidence strongly supports the conjecture that the scaled velocity distribution follows a generalized exponential behavior f (c)~exp (−cⁿ), with n ≈1.2, regarding less the fragmentation mechanisms

BACKGROUND & AIMS: In the last decade, pegylated interferon-α (PegIFN-α) plus ribavirin (RBV) was the standard treatment of chronic hepatitis C for genotype 1, and it remains the standard for genotypes 2 and 3. Recent studies reported associations between RBV-induced anemia and genetic polymorphisms of concentrative nucleoside transporters such as CNT3 (encoded by SLC28A3) and inosine triphosphatase (encoded by ITPA). We aimed at studying genetic determinants of RBV kinetics, efficacy and treatment-associated anemia. METHODS: We included 216 patients from two Swiss study cohorts (61% HCV genotype 1, 39% genotypes 2 or 3). Patients were analyzed for SLC28A2 single nucleotide polymorphism (SNP) rs11854484, SLC28A3 rs56350726, and SLC28A3 rs10868138 as well as ITPA SNPs rs1127354 and rs7270101, and followed for treatment-associated hemoglobin changes and sustained virological response (SVR). In 67 patients, RBV serum levels were additionally measured during treatment. RESULTS: Patients with SLC28A2 rs11854484 genotype TT had higher dosage- and body weight-adjusted RBV levels than those with genotypes TC or CC (p=0.02 and p=0.06 at weeks 4 and 8, respectively). ITPA SNP rs1127354 was associated with hemoglobin drop ≥3 g/dl during treatment, in genotype (relative risk (RR)=2.1, 95% CI 1.3-3.5) as well as allelic analyses (RR=2.0, 95%CI 1.2-3.4). SLC28A3 rs56350726 was associated with SVR in genotype (RR=2.2; 95% CI 1.1-4.3) as well as allelic analyses (RR=2.0, 95% CI 1.1-3.4). CONCLUSIONS: The newly identified association between RBV serum levels and SLC28A2 rs11854484 genotype, as well as the replicated association of ITPA and SLC28A3 genetic polymorphisms with RBV-induced anemia and treatment response, may support individualized treatment of chronic hepatitis C and warrant further investigation in larger studies.

Y-a-t-il des risques à prendre en compte, des opposants ? 6. Une perspective de santé publique. 7.Qu'est-ce qui devrait être fait maintenant ? [Introduction (extrait)] Depuis 2000, la notion de «bientraitance » est de plus en plus couramment utilisée, notamment en France et en Belgique. Pourtant, les contours de cette notion sont encore flous. Les définitions sont nombreuses et les points de vue diffèrent selon les corps professionnels. Cet ouvrage entend préciser les contours de cette notion tout en rendant compte de toute la complexité que suppose une telle entreprise. Il relève les différentes perspectives adoptées autour du thème de la bientraitance des enfants et soulève les interrogations inhérentes à toute tentative de définition. En plus de soulever de nombreuses pistes de réflexion, cette Revue de la Littérature a servi de base au développement d'un projet pilote mené depuis décembre 2006 dans le canton de Fribourg. Le projet « Bientraitance : le rôle de l'éducation non formelle », mené par la Fondation Charlotte Olivier, cherche à optimiser l'utilisation des ressources de bientraitance endogènes. Il poursuit un double objectif : augmenter l'accès et la participation des enfants et des adolescents à des activités collectives (sport, art, culture, scouts / éclaireurs,...) considérées comme "bientraitantes" (donc en principe favorables au développement et à la santé au sens large) et évaluer les résultats de cette participation.

Fecundity and longevity of Aphis gossypii Glover, 1877 (Hemiptera, Aphididae) at different temperatures and commercial chrysanthemum cultivars (Dendranthema grandiflora Tzvelev). The aphid A. gossypii is one of the main pests in a number of crops both under field and protected conditions. The objective of the present study was to evaluate the fecundity and longevity of A. gossypii under different temperatures and commercial chrysanthemum cultivars (Yellow Snowdon, White Reagan and Dark Splendid Reagan) with different trichomes densities (11.3; 16.6 and 21.6 trichome/mm² of the leaf, respectively) The trials were carried out in climatic chambers, at four temperatures (15, 20, 25 and 30 ±1 °C), 70 ± 10% RH and photophase 10h. The reproductive period significantly decreased with increase of temperature in the three cultivars. In Yellow Snowdon cultivar average duration of the reproductive period was 14.3 days at 25 °C. The maximum fecundity was obtained at the temperature of 25 ºC with 3,1; 2,8 and 3,6 nymphs/female/day in the Yellow Snowdon, White Reagan and Dark S. Reagan cultivars, respectively. The total fecundity was reduced by extreme temperatures (15 and 30 °C), and was obtained at 25 °C with 35,9 nymphs/female. Females maintained in Yellow Snowdon cultivar significantly showed superiority (30,7 nymphs/female) in total fecundity in relation to White Reagan (22,1 nymphs/female) and Dark S. Reagan (22,9 nymphs/female). The Yellow Snowdon cultivar (with a lower trichome density) had a significant influence in daily and total capacity of nymphs production, showing a higher fecundity of A. gossypii females. The aphid's longevity was affected by cultivars and temperature, and this longevity decreased whit increase of temperature. The results showed that there was an interaction between the temperature and host plant on reproductive parameters of A. gossypii.

Los registros se efectuaron del 12 agosto al 22 setiembre de 1996, durante el Crucero BIC Humboldt 9608-09 de Evaluación de la Biomasa Desovante de Anchoveta desde Tambo de Mora hasta Paita y desde la costa hasta las 120 mn en promedio. Las características térmicas superficiales se presentaron frías para la época (14,5° C a 18,1 °C). Las anomalías térmicas tuvieron una variación de -2,0 °C en la zona costera entre Chimbote y Salaverry a +0,1 °C en la zona costera frente a Pucusana. Toda el área prospectada presentó anomalías térmicas negativas. La salinidad superficial del mar tuvo valores que oscilaron de 35,2 % (propio de las aguas oceánicas) a 34,95% (propio de las aguas costeras frías). La estructura vertical de salinidad mostró un mayor predominio de las aguas costeras frías desde la costa hasta las 60 mn y hasta una profundidad de 50 m. En la superficie del mar el oxígeno fluctuó entre 1,611 a 6,02 mL/L; los valores de 5,5 mL/L indicaron la presencia de las ASS y se ubicaron a 120 mn.

El Crucero se desarrolló desde Puerto Pizarro a Pacasmayo hasta las 60 mn, del 14 de enero al 7 febrero 2004. En la columna de agua se obtuvieron promedios de oxígeno disuelto en el nivel de los 100 m: para Puerto Pizarro (1,86 mL/L), para Paita (1,57 mL/L) para Punta Falsa (1,05 mL/L) para Eten (1,59 mL/L). En el fondo, los altos valores de oxígeno (0,5 – 2,0 mL/L) se debieron a la presencia de la Extensión Sur de la Corriente de Cromwell, que favorecieron las concentraciones de la merluza, destacando las zonas frente a Puerto Pizarro y Pimentel. En las secciones de Puerto Pizarro a Punta Falsa entre 75 a 150 m de profundidad, se registró la temperatura (15,12 °C); oxígeno disuelto (1,45 mL/L); fosfatos (1,74 μg-at/L); silicatos (16,06 μg-at/L); nitratos (20,09 μg-at/L).

We have recently shown that at isotopic steady state (13)C NMR can provide a direct measurement of glycogen concentration changes, but that the turnover of glycogen was not accessible with this protocol. The aim of the present study was to design, implement and apply a novel dual-tracer infusion protocol to simultaneously measure glycogen concentration and turnover. After reaching isotopic steady state for glycogen C1 using [1-(13)C] glucose administration, [1,6-(13)C(2)] glucose was infused such that isotopic steady state was maintained at the C1 position, but the C6 position reflected (13)C label incorporation. To overcome the large chemical shift displacement error between the C1 and C6 resonances of glycogen, we implemented 2D gradient based localization using the Fourier series window approach, in conjunction with time-domain analysis of the resulting FIDs using jMRUI. The glycogen concentration of 5.1 +/- 1.6 mM measured from the C1 position was in excellent agreement with concomitant biochemical determinations. Glycogen turnover measured from the rate of label incorporation into the C6 position of glycogen in the alpha-chloralose anesthetized rat was 0.7 micromol/g/h.

PURPOSE: This study assessed whether a cycle of "routine" therapeutic drug monitoring (TDM) for imatinib dosage individualization, targeting an imatinib trough plasma concentration (C min) of 1,000 ng/ml (tolerance: 750-1,500 ng/ml), could improve clinical outcomes in chronic myelogenous leukemia (CML) patients, compared with TDM use only in case of problems ("rescue" TDM). METHODS: Imatinib concentration monitoring evaluation was a multicenter randomized controlled trial including adult patients in chronic or accelerated phase CML receiving imatinib since less than 5 years. Patients were allocated 1:1 to "routine TDM" or "rescue TDM." The primary endpoint was a combined outcome (failure- and toxicity-free survival with continuation on imatinib) over 1-year follow-up, analyzed in intention-to-treat (ISRCTN31181395). RESULTS: Among 56 patients (55 evaluable), 14/27 (52 %) receiving "routine TDM" remained event-free versus 16/28 (57 %) "rescue TDM" controls (P = 0.69). In the "routine TDM" arm, dosage recommendations were correctly adopted in 14 patients (median C min: 895 ng/ml), who had fewer unfavorable events (28 %) than the 13 not receiving the advised dosage (77 %; P = 0.03; median C min: 648 ng/ml). CONCLUSIONS: This first target concentration intervention trial could not formally demonstrate a benefit of "routine TDM" because of small patient number and surprisingly limited prescriber's adherence to dosage recommendations. Favorable outcomes were, however, found in patients actually elected for target dosing. This study thus shows first prospective indication for TDM being a useful tool to guide drug dosage and shift decisions. The study design and analysis provide an interesting paradigm for future randomized TDM trials on targeted anticancer agents.

This study addressed the contribution of acidic sphingomyelinase (ASMase) in TNF-alpha-mediated hepatocellular apoptosis. Cultured hepatocytes depleted of mitochondrial glutathione (mGSH) became sensitive to TNF-alpha, undergoing a time-dependent apoptotic cell death preceded by mitochondrial membrane depolarization, cytochrome c release, and caspase activation. Cyclosporin A treatment rescued mGSH-depleted hepatocytes from TNF-alpha-induced cell death. In contrast, mGSH-depleted hepatocytes deficient in ASMase were resistant to TNF-alpha-mediated cell death but sensitive to exogenous ASMase. Furthermore, although in vivo administration of TNF-alpha or LPS to galactosamine-pretreated ASMase(+/+) mice caused liver damage, ASMase(-/-) mice exhibited minimal hepatocellular injury. To analyze the requirement of ASMase, we assessed the effect of glucosylceramide synthetase inhibition on TNF-alpha-mediated apoptosis. This approach, which blunted glycosphingolipid generation by TNF-alpha, protected mGSH-depleted ASMase(+/+) hepatocytes from TNF-alpha despite enhancement of TNF-alpha-stimulated ceramide formation. To further test the involvement of glycosphingolipids, we focused on ganglioside GD3 (GD3) because of its emerging role in apoptosis through interaction with mitochondria. Analysis of the cellular redistribution of GD3 by laser scanning confocal microscopy revealed the targeting of GD3 to mitochondria in ASMase(+/+) but not in ASMase(-/-) hepatocytes. However, treatment of ASMase(-/-) hepatocytes with exogenous ASMase induced the colocalization of GD3 and mitochondria. Thus, ASMase contributes to TNF-alpha-induced hepatocellular apoptosis by promoting the mitochondrial targeting of glycosphingolipids.

Com a correção da acidez, nutrientes importantes para o cultivo do milho tornam-se mais disponíveis à planta, promovendo o aumento de produtividade. Objetivou-se com este estudo avaliar as alterações químicas ocorrentes em dois solos e na produção de matéria seca de milho pela aplicação de corretivos com diferentes graus de reatividade. Foi realizado um primeiro experimento em casa de vegetação, aplicando-se doses de três calcários comerciais, sendo um com reatividade média, denominado A (RE = 85,7 %; PRNT = 88,3 %), outro com reatividade baixa denominado B (RE = 83,7 %; PRNT = 76,2 %), e outro, denominado C (RE = 99,1 %; PRNT = 105,3 %) altamente reativo, em quatro cultivos sucessivos de milho. Os solos utilizados foram Neossolo Quartzarênico e Latossolo Vermelho. Os tratamentos foram dispostos inteiramente ao acaso, com quatro repetições, constando de três doses de calcário, calculadas para elevar a saturação por bases para 40, 60 e 80 %, além da testemunha. Paralelamente, um experimento de incubação foi realizado para verificar o efeito residual dos corretivos ao longo do tempo. A calagem aumentou os valores de pH, Ca2+ e Mg2+ e reduziu os teores de Al3+ trocável. O melhor crescimento do milho foi observado no Latossolo Vermelho. Neste solo, os calcários A e B foram mais eficientes na elevação do pH durante os quatro cultivos, enquanto o produto C teve menor eficiência na redução do Al3+ nos últimos ciclos, devido à sua maior reatividade. Os calcários A e C tiveram comportamentos semelhantes nos dois solos estudados. Houve queda na produção de matéria seca aérea com os cultivos sucessivos de milho. No experimento de incubação, os teores de Ca trocável foram maiores que os residuais.
