Objective and design: The effects of anesthetics on cytokine release in patients without comorbidities who undergo minor surgery are not well defined. We compared inflammatory cytokine profiles in adult patients undergoing minimally invasive surgery who received isoflurane or propofol anesthesia. Methods: Thirty-four patients without comorbidities undergoing minor surgery were randomly assigned to receive an inhaled anesthetic (isoflurane; n = 16) or an intravenous anesthetic (propofol; n = 18). Blood samples were drawn before premedication and anesthesia (T1), 120 min after anesthesia induction (T2), and on the first post-operative day (T3). Plasma concentrations of interleukins (IL-) 1β, 6, 8, 10 and 12 and tumor necrosis factor (TNF)-α were measured using flow cytometry. Results: The pro-inflammatory cytokine IL-6 was increased in the isoflurane group at T2 and T3 compared to T1 (P < 0.01). In the propofol group, IL-6 and IL-8 were significantly increased at T3 compared to T1. However, there were no significant differences in cytokine concentrations between the isoflurane and propofol groups. Conclusion: An inflammatory response occurred earlier in patients who received an inhaled agent compared with an intravenous anesthetic, but no differences in plasma cytokine profiles were evident between isoflurane and propofol anesthesia in patients without comorbidities undergoing minimally invasive surgeries. © 2013 Springer Basel.

This Manual is intended to guide individuals and institutions seeking to use the Aid for Trade Dashboard developed by the Economic Commission for Latin America and the Caribbean as part of the Development Project “Facilitating the Effective Integration of Developing Countries into the Global Economy through Aid for Trade Schemes”. This Dashboard presents a set of online indicators for all the countries comprising the five United Nations’ Regional Commissions which are currently participating in this Project.

These reports are the result of consultations which were conducted in 2008 in Aruba, Barbados, Netherlands Antilles, Dominican Republic, Guyana, Jamaica, Montserrat, Saint Lucia and Trinidad and Tobago. The objective was to obtain relevant information that would inform a Stern-type report where the economics of climate change would be examined for the Caribbean subregion. These reports will be complimented by future assessments of the costs of the “business as usual”, adaptation and mitigation responses to the potential impacts of climate change. It is anticipated that the information contained in each country report would provide a detailed account of the environmental profile and would, therefore, provide an easy point of reference for policymakers in adapting existing policy or in formulating new ones. ECLAC continues to be available to the CDCC countries to provide technical support in the area of sustainable development.

This manual contains information on the dataset compiled from the Survey of Living Conditions and Household Budgets (SLC/HBS) conducted in Saint Lucia by the Kairi Consultants Limited and National Assessment Team between 2005 and 2006. The SLC/HBS is a sample survey which generates data on households and individuals in the country. This manual was developed by the Economic Commission for Latin America and the Caribbean (ECLAC) – Subregional Headquarters in the Caribbean as a supplementary document for the Caribbean Household Surveys Database (CHSD). It is sectioned out into two main parts- section one provides brief description of the survey and section two contains a concise data dictionary of variables generated from the survey as well as additional variables created by ECLAC. In addition, for ease of reference, an index of all variables in the database is included at the end of the document.

ABSTRACT: The femtocell concept aims to combine fixed-line broadband access with mobile telephony using the deployment of low-cost, low-power third and fourth generation base stations in the subscribers' homes. While the self-configuration of femtocells is a plus, it can limit the quality of service (QoS) for the users and reduce the efficiency of the network, based on outdated allocation parameters such as signal power level. To this end, this paper presents a proposal for optimized allocation of users on a co-channel macro-femto network, that enable self-configuration and public access, aiming to maximize the quality of service of applications and using more efficiently the available energy, seeking the concept of Green networking. Thus, when the user needs to connect to make a voice or a data call, the mobile phone has to decide which network to connect, using the information of number of connections, the QoS parameters (packet loss and throughput) and the signal power level of each network. For this purpose, the system is modeled as a Markov Decision Process, which is formulated to obtain an optimal policy that can be applied on the mobile phone. The policy created is flexible, allowing different analyzes, and adaptive to the specific characteristics defined by the telephone company. The results show that compared to traditional QoS approaches, the policy proposed here can improve energy efficiency by up to 10%.
