This study evaluated the 10-year clinical performance of high-viscosity glass-ionomer cement placed in posterior permanent teeth by means of the Atraumatic Restorative Treatment (ART) approach. One operator placed 167 single- and 107 multiple-surface restorations in 43 high-risk caries pregnant women (mean decayed teeth = 9.8 +/- 5.5). Examinations were performed at 1-, 2-, and 10-year intervals according to ART criteria. In the last evaluation, the US Public Health Service (USPHS) criteria were also used. After 10 years, 129 restorations (47.1%) were evaluated and achieved a cumulative survival rate of 49.0% (SE 7.2%). The 10-year survival of single- and multiple-surface ART restorations assessed using the ART criteria were 65.2% (SE 7.3%) and 30.6% (SE 9.9%), respectively. This difference was significant (jackknife SE of difference; p < 0.05). Using the USPHS criteria, the 10-year survival of single- and multiple-surface ART restorations were 86.5% and 57.6%, respectively. The primary causes of failure were total loss (9.3%) and marginal defects (5.4%). The survival rates observed, especially for the single-surface restorations, confirm the potential of the ART approach for restoring and saving posterior permanent teeth.

Aims. This study aimed to investigate the dental caries status and salivary properties in 3- to 15-year-old children/adolescents. Methods. The sample was split in two groups: asthma group (AG), composed of 65 patients who attended Public Health Service; asthma-free group (AFG), composed of 65 nonasthmatic children/adolescents recruited in two public schools. Stimulated salivary samples were collected for 3 min. Buffering capacity and pH were ascertained in each salivary sample. A single trained and calibrated examiner (kappa = 0.98) performed the dental caries examination according to WHO criteria. Results. The AFG showed salivary flow rate (1.10 +/- 0.63 mL/min) higher (P = 0.002) than AG (0.80 +/- 0.50 mL/min). An inverse relationship was observed between asthma severity and salivary flow rate (Phi coefficient, r phi: 0.79, P = 0.0001). Children with moderate or severe asthma showed an increased risk for reduced salivary flow rate (OR: 17.15, P < 0.001). No association was observed between drug use frequency (P > 0.05) and drug type (P > 0.05) with salivary flow rate. Buffering capacity was similar in both groups. No significant differences were encountered in dental caries experience between AFG and AG groups. Conclusions. Although asthma can cause reduction in flow rate, the illness did not seem to influence dental caries experience in children with access to proper dental care.

Background. Research has suggested that packable resin-based composites inserted with a placement technique similar to amalgam condensation can reduce the sensitivity associated with posterior restorations. The authors evaluated the clinical performance, including associated sensitivity, of two packable composites in a randomized five-year clinical trial. Methods. A single operator randomly placed two restorations in each of 33 patients: one restoration consisting of Alert (Jeneric/Pentron, Wallingford, Conn.) and the other consisting of SureFil (Dentsply/Caulk, Milford, Del.). There were 30 Class I and 36 Class II restorations. Two independent evaluators evaluated the restorations by using modified U.S.; Public Health Service criteria. The authors analyzed data by means of the Fisher, chi(2) and McNemar tests at P < .05. Results. Of 60 restorations evaluated at five years, two Class II restorations (one SureFil, one Alert) failed. All other restorations received the highest score possible for sensitivity and vitality. The only difference between the composites at the five-year recall was the significantly better surface texture of SureFil. The authors observed significantly different scores between the baseline and at five years for marginal discoloration (Alert and SureFil), surface texture (Alert and SureFil) and color (SureFil). Conclusions. Both packable resin-based composites showed excellent durability during the five-year follow-up. Clinical Implications. The investigated resin-based composites are suitable for posterior restorations.

This study investigates the use of general practitioner services by women in Australia. Although there is a universal health insurance system (Medicare) in Australia, there are variations in access to services and out of pocket costs for services. Survey data from 2350 mid-age (45-50 years) and 2102 older (70-75 years) women participating in the Australian Longitudinal Study on Women's Health were linked with Medicare data to provide a range of individual and contextual variables hypothesised to explain general practitioner use. Structural equation modelling showed that physical health was the most powerful explanatory factor of general practitioner use. However, after adjusting for self-reported health, out of pocket cost per consultation was inversely associated with use of services. The out of pocket cost was generally lower for women with low socioeconomic status but cost was also directly related to geographical remoteness. Women living in more remote areas had higher out of pocket costs and poorer access to services. Women who reported better access to care were more likely to be satisfied with their most recent general practice consultation and less likely to be sceptical of the value of medical care. These results show the need for health policies that improve the equitable use of general practitioner services in Australia. (C) 2001 Elsevier Science Ltd. All rights reserved.

Parenthood is considered a major life role. Yet for people with a major mental illness, it is one that is fraught with difficulties and for which they receive the least support. Research on parenting and parenting programmes for people with a major mental illness is sparse and most of the papers presented do not provide a working model that can be easily replicated. This lack of support for parents or knowledge of working parenting programmes has often resulted in children being placed in care. Occupational therapists working in an Australian mental health service developed a two-stream programme which aimed to consolidate the parent/child relationship and enable the parents to develop effective parenting skills. This programme has a parents' educational stream and a stream with developmentally appropriate activities for the children. Observed outcomes have included the parents becoming more responsive to their children, increased treatment compliance, improved community access, and a decrease in the number of children in temporary foster care.

Teledermatology holds great potential for revolutionizing the delivery of dermatology services, providing equitable service to remote areas and allowing primary care physicians to refer patients to dermatology centres of excellence at a distance. However, before its routine application asa service tool, its reliability, accuracy and cost-effectiveness need to be verified by rigorous evaluation. Teledermatology can be applied in one of two ways: it may be conducted in real-time, utilizing videoconferencing equipment, or by store-and-forward methods, when transmitted digital images or photographs are submitted with a clinical history. While there is a considerable range of reported accuracy and reliability, evidence suggests that teledermatology will become increasingly utilized and incorporated into more conventional dermatology service delivery systems. Studies to date have generally found that real-time dermatology is likely to allow greater clinical information to be obtained from the patient. This may result in fewer patients requiring conventional consultations, but it is generally more time-consuming and costly to the health service provider It is often favoured by the patient because of the instantaneous nature of the diagnosis and management regimen for the condition, and it has educational value to the primary care physician. Store-and-forward systems of teledermatology often give high levels of diagnostic accuracy, and are cheaper and more convenient for the health care provider, but lack the immediacy of patient contact with the dermatologist, and involve a delay in obtaining the diagnosis and advice on management. It is increasingly likely that teledermatology will prove to be a significant tool in the provision of dermatology services in the future. These services will probably be provided by store-and-forward digital image systems, with real-time videoconferencing being used for case conferences and education. However, much more research is needed into the outcomes and Limitations of such a service and its effect on waiting lists, as well as possible cost benefits for patients, primary health care professionals and dermatology departments.

Networking North Queensland (NNQ) was a two-year project to improve access to health services in rural and remote communities. The project involved email and Internet access in 61 communities, in a region almost three times the size of the UK. Videoconferencing equipment was also installed at 21 sites and a total of 197 h of videoconferencing was recorded at 10 of the remote sites over 12 months. As a result of the project, health consumers enjoyed improved access to medical, specialist, allied health and primary health services. In addition, health service providers had better access to reliable, up-to-date health-care information via intranet and Internet services. Consideration of local issues-local needs and existing resources-was vital to the achievements of the project. Community involvement and community access were also important factors in its success.

O levantamento de dados foi realizado em livros, sites do Minist??rio da Sa??de, Legisla????es do Di??rio Oficial e artigos das bases de dados Biblioteca Virtual em Sa??de (BVS/Bireme), Medline e Scientific Eletronic Library (Scielo). As pol??ticas p??blicas de sa??de materno infantil foram definidas seguindo desde os princ??pios meramente reprodutivos at?? aqueles mais amplos, voltados para garantir uma melhor condi????o de sa??de e de vida. Nas ??ltimas d??cadas, com o aux??lio do governo e da sociedade, houve uma progress??o no atendimento da sa??de da mulher e da crian??a. No entanto, conclui-se que, apesar da evolu????o observada, o grande desafio ainda se encontra na redu????o da morbimortalidade materna, neonatal e infantil.

Trata-se de uma pesquisa-intervenção com o objetivo principal de colocar em análise a atividade na vigilância em saúde do trabalhador ancorada nos pressupostos da formação pelo trabalho. Tem como aporte conceitual o referencial teórico-metodológico da clínica da atividade proposta por Yves Clot e colaboradores. A pesquisa foi realizada com a equipe multiprofissional que atua na vigilância em saúde do trabalhador do Centro de Referência Estadual em Saúde do Trabalhador do Espírito Santo (Cerest/ES). As rodas de conversa, a partir da clínica da atividade, foram a estratégia metodológica privilegiada. O gênero profissional vigilante em saúde do trabalhador, ao se confrontar com o estudo das diretrizes da clínica da atividade, foi produzindo deslocamentos que o levou a (re)pensar, analisar a sua atividade e o processo de trabalho no qual estava inserido. A cada encontro no diálogo com a clínica da atividade, eram experimentados os modos de como esses profissionais atuavam na vigilância, como também todo o atravessamento desse gênero na assistência aos trabalhadores, nas atividades educativas e gestão do trabalho. No desenvolvimento das rodas, ficou evidenciado o quanto esses encontros se tornaram equipamentos para esses profissionais. As atividades desenvolvidas produziram análise coletiva do trabalho e um processo de formação pelo trabalho. Buscaram incorporar os métodos utilizados pela clínica da atividade no cotidiano do Cerest dialogando com os efeitos desse processo. Nas rodas também emergiram propostas de mudanças na maneira como vinham ocorrendo as atividades realizadas, a gestão do trabalho nessa equipe e as estratégias de atuação na vigilância, ou seja, intervenção no processo de trabalho do grupo, produzindo potência para o poder de agir do gênero. O referencial teórico-metodológico da clínica da atividade foi um dispositivo de formação importante. Permitiu a experimentação e o desenvolvimento do gênero vigilantes em saúde do trabalhador na produção de análise da atividade, na constituição de uma clínica dialógica em transformação. Também possibilitou contemplar na análise as dimensões integrantes da atividade de trabalho, fazendo a interlocução para o encontro entre os saberes da experiência instituídos no cotidiano do serviço de saúde e o saber acadêmico na produção de conhecimento.

A saúde e o uso do psicotrópico no sistema prisional habitam um paradoxo. O sistema penitenciário, nas últimas décadas, passou por algumas transformações. No mundo, as estatísticas apontam crescimento populacional carcerário e prisões superlotadas, em condições precárias. No Brasil, a situação não é diferente: em 10 anos a população prisional brasileira duplicou e as condições de confinamento são paupérrimas, o que acaba contribuindo para a prevalência de doenças infectocontagiosas. Diante desta realidade, em 2003 homologou-se o Plano Nacional de Saúde no Sistema Penitenciário (PNSSP) que, em consonância com os princípios do Sistema Único de Saúde, visa garantir a integralidade e a universalidade de acesso aos serviços de saúde para a população penitenciária. O estado do Espírito Santo aderiu ao PNSSP e formulou o Plano Operativo Estadual de Atenção Integral à Saúde da População Prisional (2004), contudo, foi a partir de 2010 que se efetivou o acesso aos serviços de saúde prisional capixaba. Neste contexto, a pesquisa de mestrado buscou investigar as práticas de saúde no sistema prisional e as formas de usos do psicotrópico por presos da Penitenciária de Segurança Máxima II (PSMA II), localizada no Complexo Penitenciário de Viana, Espírito Santo. Para tanto, foi necessário habitar o sistema penitenciário capixaba e realizar entrevistas semiestruturadas com profissionais da gestão de saúde prisional da Secretaria Estadual de Justiça do Espírito Santo, com profissional da área da medicina psiquiátrica e com presos da PSMA II. Dessa forma, foi possível observar que a saúde no sistema penitenciário, bem como os usos do psicotrópico, encontram-se em um espaço poroso. As práticas de saúde podem fortalecer estratégias de controle e produzir mortificação, como podem escapar dos investimentos biopolíticos e produzir resistência. O uso do medicamento psicotrópico por sujeitos privados de liberdade encontra-se nessa mesma ambivalência: podem servir como instrumentos regularizadores de captura, como podem produzir autonomia nas suas formas de uso pelos presos. Por fim, entre mortificações e resistências, afirma-se que é o próprio preso que administrará os tensionamentos desse paradoxo e irá produzir vida, potência de vida.

INTRODUCTION: Morbidity information is easily available from medical records but its scope is limited to the population attended by the health services. Information on the prevalence of diseases requires community surveys, which are not always feasible. These two sources of information represent two alternative assessments of disease occurrence, namely demand morbidity and perceived morbidity. The present study was conceived so as to elicit a potential relationship between them so that the former could be used in the absence of the latter. METHODS: A community of 13,365 families on the outskirts of S. Paulo, Brazil, was studied during the period from 15/Nov/1994 to 15/Jan/1995. Data regarding children less than 5 years old were collected from a household survey and from the 2 basic health units in the area. Prevalence of diseases was ascertained from perceived morbidity and compared to estimates computed from demand morbidity. RESULTS: Data analysis distinguished 2 age groups, infants less than 1 year old and children 1 to less than 5. The most important groups of diseases were respiratory diseases, diarrhoea, skin problems and infectious & parasitical diseases. Basic health units presented a better coverage for infants. Though disease frequencies were not different within or outside these units, a better coverage was found for diarrhoea and infectious & parasitical diseases in the infant group, and for diarrhoea in the older age group. Equivalence between the two types of morbidity was found to be limited to the infant group and concerned only the best covered diseases. The odds of a disease being seen at the health service should be of at least 4:10 to ensure this equivalence. CONCLUSION: It was concluded that, provided that health service coverage is good, demand morbidity can be taken as a reliable estimate of community morbidity.
