Este estudo avaliou os efeitos do laser de baixa intensidade na velocidade de movimentação ortodôntica dos caninos durante a fase de retratação e na ancoragem dos molares por meio de tomografia computadorizada de feixe cônico. Foram selecionados 21 pacientes que necessitavam de extração de primeiros pré-molares sendo que 15 apresentavam maloclusão de classe I de Angle e 6 classe II de Angle. Foram avaliados 72 caninos e 72 molares , sendo 72 molares, sendo 42 caninos e 42 molares superiores, e 30 caninos e 30 molares inferiores. A retratação inicial dos caninos foi realizada com arco segmentado 0,014, por mio de mola de níquel titâneo com uma força de 150 g. Um dos caninos foi irradiado com laser de diodo, com o protocolo de aplicação de 0nm/40mW/10J/cm2/04,J por ponto, sendo cinco pontos por vestibular e cinco pontos por lingual na região radicular , totalizando Et=4J. Esta aplicação foi realizada no dia em que foi ativada a mola de retratação, sendo que no canino contralateral foi realizada uma simulação (placebo). Decorridos trinta dias em média, houve nova ativação da mola e aplicação do laser no mesmo canino já irradiado.(AU)

O presente estudo objetivou avaliar a eficácia do Laser de Baixa Intensidade (LBI) na aceleração da movimentação dentária e na diminuição da dor frente à aplicação de força ortodôntica. A amostra foi composta por 19 pacientes, sendo doze do sexo feminino e sete do sexo masculino, com idade inicial média de 14,69 anos, todos com indicação para extrações de primeiros pré-molares. Destes, 66 caninos foram submetidos à retração inicial, sendo que 33 receberam aplicação de laser e 33 foram considerados controle. Utilizou-se o Laser de Baixa Intensidade de arseneto de gálio e alumínio, com comprimento de onda de 780nm, na dosimetria de 40mW;10J/cm2;10s/ponto, aplicado apenas uma vez ao mês em dez pontos, sendo cinco por vestibular e cinco por lingual/palatino. Modelos de gesso foram confeccionados durante todos os meses de retração dos caninos, que teve duração de quatro meses, sendo, posteriormente, digitalizados para se mensurar a quantidade de movimentação de um lado em relação ao outro, utilizando-se como referência as papilas incisivas. Para a avaliação da dor experimentada pelos pacientes, os mesmos foram orientados a preencher uma escala analógica visual (VAS) que variava de 0 a 10 , em que zero significava nenhuma dor e dez significava dor insuportável, nos intervalos de 12, 24, 48 e 72 horas após a aplicação da força ortodôntica. Foi mensurado o apinhamento de todas as hemiarcadas dos pacientes na fase inicial, medindo-se a distância entre os pontos de contato de cada dente. Para a verificação do padrão de normalidade, empregou-se o teste de Kolmogorov-Smirnov, sendo que para comparar o lado irradiado com o lado não irradiado foi utilizado o teste t pareado, exceto para a variável razão caninos/molares , analisada pelo teste não paramétrico de Wilcoxon (p<0,05). Os resultados mostraram que em relação ao apinhamento dentário, os lados irradiado e não irradiado apresentaram-se compatíveis. Além disso, não houve diferença estatisticamente significante entre a quantidade de retração dos caninos irradiados comparados aos não irradiados, o mesmo acontecendo com a sensibilidade dolorosa experimentada pelos pacientes. Concluiu-se assim que o LBI na dosimetria e forma como foi utilizado não foi eficiente na aceleração da movimentação dentária nem na redução da dor experimentada pelos pacientes frente às forças ortodônticas.

Diabetes Mellitus (DM ) is a complex disease that requires continuous medical care for the reduction of risk factors in addition to glycemic control. The typical hyperglycemia of this disease produces glycosylation of proteins and so the consequence is the accumulation of glycosylation final products in various human tissues, among them, the tendon. The aerobic exercise (AE) and the low level laser therapy (LLLT) have been used to treat tendinopathies in individuals with or without DM. Objective: The aim of this study was to watch the effect of the LLLT and the AE, in association, in partial tenotomy of the tissue repair of the Achilles tendon (AT) of diabetic rats. Methods: 91 animals were utilized and divided in to the following groups: control group (GC), injured control group (GCL), diabetic group (GD), diabetic group LLLT (GD – TLBI), diabetic group trained (GD - EX) and diabetic group trained laser (GD-EX+TLBI). The animals were submitted to intervention with AE, using a protocol with a progressive increase of time (12 to 60 min) and speed of (4 to 9 m/min), and the LLLT (660 nm laser, 10mW, 4 J/cm², single point for 16 seconds, three times for week). It was analyzed morphological, biomechanical and molecular characteristics. For data showing normal distribution was used one-way ANOVA test and post hoc Tukey and data without normal distribution was used Mann Whitney test and post hoc Dunn's. It was accepted p <0.05 for statistical significance Results: The biomechanical tests indicated major improvement in the GC and GD-EX+TLBI groups when compared with the diabetic groups in the following variables: maximum load, strain, absorbed energy, stress, cross section area, elastic modulus and energy density (p<0.05). The analysis through molecular biology indicated that the association of aerobic exercise and LLLT generated an increase of the collagen I gene expression and modulated the expression of the MMP2 and MMP9 (p<0.05). No observed any major improvement in the morphological variable studied. Conclusion: the LLLT associated with aerobic exercise promotes and increase of the mechanical properties, in the control of collagen I gene expression and of the MMP2 and MMP9 of the diabetic rats.

Esta investigación tiene como objetivo determinar la efectividad de incorporar un programa de terapia asistida con animales (TAA) al tratamiento farmacológico y programa individualizado de rehabilitación psicosociolaboral en pacientes con esquizofrenia que acuden a recursos de atención social de la red de atención pública de la Comunidad de Madrid. Hay dos momentos, un primer estudio en el que la investigación se centra en evaluar la mejora de dominios clínicos, psicosociales y cognitivos y un segundo estudio en el que la evaluación se centra en comprobar la mejora en dominios cognitivos utilizando una prueba específica de evaluación de procesos cognitivos en pacientes con esquizofrenia (MATRICS). Se plantean dos hipótesis en la medida en que se han realizado dos estudios. En un primer estudio se plantea que los cambios en las puntuaciones de variables clínicas, psicosociales y cognitivas del grupo con TAA serán mayores y significativas entre evaluaciones pre y post y frente a las del grupo sin TAA. El segundo estudio se centra, casi exclusivamente, en que la mejora de los dominios cognitivos del grupo con TAA será mayor y significativa entre evaluaciones pre y post y frente a las del grupo sin TAA...

La Terapia biológica aplicada a pacientes diagnosticados de Artritis Reumatoide ha demostrado tener buenos resultados paraclínicos en estudios realizados en otros países. Ejecutamos esta investigación para conocer mediante Laboratorio (PCR-VSG y FR), si dichos cambios se presentan en pacientes con AR que reciben tratamiento biológico en nuestro medio. Métodos: Se realizó un estudio cuasiexperimental, seleccionando al azar 50 pacientes con diagnóstico de Artritis Reumatoide sin enfermedad concomitante, que reciben tratamiento biológico en el Hospital José Carrasco de la Ciudad de Cuenca, se revisó en las Historias Clínicas los valores preterapéuticos de PCR, VSG y FR, luego se realizaron controles a partir de los 5 meses de inicio de la terapia biológica y se confrontaron resultados. El análisis incluyó Chi cuadrado, Valor p, Desvio Estándar y diferencia de medias. Resultados: De los 50 pacientes estudiados, el PCR fue positivo en el 100de pacientes antes y después de la Terapia biológica. Los valores de VSG fueron positivos preterapia en el 70y post-terapia en el 40, (p= 0.003). El FR fue positivo en el 84antes del tratamiento, luego del mismo el porcentaje de FR positivo fue 60, (p= 0.008). La media de los valores de PCR antes del tratamiento fue 12.8ñ14.2DE y luego del tratamiento 5.6 ñ 6.5DE (p= 0.002) la media para la VSG preterapia fue 33.4 ñ 21.3DE y post-terapia 17.2 ñ 6.7DE (p= 0,0001). Para el FR la media antes de la terapia fue 120.6 ñ 97.9 y luego de la terapia 100.2ñ 122.3DE (p= 0.360). Conclusiones: Los valores de PCR, VSG y FR de cincuenta pacientes con Artritis Reumatoide, presentaron un descenso significativo, luego de recibir tratamiento biológico en el Hospital José Carrasco Arteaga de la Ciudad de Cuenca

Gene therapy is one of the major challenges of the post-genomic research and it is based on the transfer of genetic material into a cell, tissue or organ in order to cure or improve the patient s clinical status. In general, gene therapy consists in the insertion of functional genes aiming substitute, complement or inhibit defective genes. The achievement of a foreigner DNA expression into a population of cells requires its transfer to the target. Therefore, a key issue is to create systems, vectors, able to transfer and protect the DNA until it reaches the target. The disadvantages related to the use of viral vectors have encouraged efforts to develop emulsions as non-viral vectors. In fact, they are easy to produce, present suitable stability and enable transfection. The aim of this work was to evaluate two different non-viral vectors, cationic liposomes and nanoemulsions, and the possibility of their use in gene therapy. For the two systems, cationic lipids and helper lipids were used. Nanoemulsions were prepared using sonication method and were composed of Captex® 355; Tween® 80; Spam® 80; cationic lipid, Stearylamine (SA) or 1,2-dioleoyl-3-trimethylammoniumpropane (DOTAP) and water (Milli-Q®). These systems were characterized by average droplet size, Polidispersion Index (PI) and Zeta Potential. The stability of the systems; as well as the DNA compaction capacity; their cytotoxicity and the cytotoxicity of the isolated components; and their transfection capacity; were also evaluated. Liposomes were made by hydration film method and were composed of DOTAP; 1,2-dioleoyl-sn-glycero-3-phosphoethanolamine (DOPE), containing or not Rhodaminephosphatidylethanolamine (PE- Rhodamine) and the conjugate Hyaluronic Acid DOPE (HA-DOPE). These systems were also characterized as nanoemulsions. Stability of the systems and the influence of time, size of plasmid and presence or absence of endotoxin in the formation of lipoplexes were also analyzed. Besides, the ophthalmic biodistribution of PE-Rhodamine containing liposomes was studied after intravitreal injection. The obtained results show that these systems are promising non-viral vector for further utilization in gene therapy and that this field seems to be very important in the clinical practice in this century. Propósito y Método del Estudio: Determinar los perfiles de expresión de proteínas en orina en pacientes agrupados de acuerdo a las complicaciones presentadas post trasplante (TR) y detectar su variación al modificar la terapia. Fue un estudio observacional, longitudinal, analítico y retrospectivo. Se incluyeron pacientes que fueron sometidos a trasplante y estuvieron de acuerdo en participar en el protocolo. Se recolectaron muestras de orina pretrasplante y cada tercer día desde el momento del trasplante. Las muestras fueron almacenadas a -70ºC hasta el momento de su análisis por marcaje peptídico mediante isótopos isobáricos para la cuantificación relativa (iTRAQ). Se agrupó a los pacientes con complicaciones por infección confirmado por cultivos y rechazo agudo confirmado por biopsia. Se establecieron 4 fases de estudio: pre trasplante, post TR previo a complicación, post TR con complicación en curso y post TR complicación tratada. Contribuciones y Conclusiones: De Enero de 2009 a Mayo de 2013 se incluyeron a 22 pacientes: 10 mujeres (45%) y 12 hombres (55%) con una edad promedio de 45+ 15 años. Solo 12 pacientes presentaron complicaciones en el post TR: 2 pacientes con rechazo agudo al injerto (GR) (1hombre, 1 mujer); y 10 pacientes (6 hombres, 4 mujeres) en el grupo de infecciones (GI). Para el análisis por iTRAQ se hizo la cuantificación relativa comparando la presencia de las proteínas en las diferentes fases de estudio. Para el grupo de rechazo agudo, se encontraron 345 proteínas, de las cuales solo 15 cumplieron los criterios de aceptación de la técnica (score >30, > 2 péptidos identificados con el 95% de confianza). Para el grupo de infecciones se encontraron 113 de las cuales 28 cumplieron los criterios de aceptación de la técnica. Conclusiones: La albúmina fue la única proteína encontrada en ambos grupos de estudio, el resto de las proteínas 14 en el GR y 27 en GI fueron diferentes. Las 5 proteínas con mayor scores en GR fueron alfa 1 microglobulina, 5' nucleosidasa citosólica, Proteína 4 de unión a retinol, proteína de membrana 4 palmitolada y serin carboxipeptidasa mientras que en GI: acetil coenzima A sintetasa mitocondrial, adenosil homocisteinasa 2, proteína de dedo de zinc GLIS1isoforma X1, proteína putativa de la isoforma FAM157B, proteína de dedo de zinc 615 isoforma X6. Queda por dilucidar la participación de cada una de éstas en los pacientes con trasplante renal.

Language is a unique aspect of human communication because it can be used to discuss itself in its own terms. For this reason, human societies potentially have superior capacities of co-ordination, reflexive self-correction, and innovation than other animal, physical or cybernetic systems. However, this analysis also reveals that language is interconnected with the economically and technologically mediated social sphere and hence is vulnerable to abstraction, objectification, reification, and therefore ideology – all of which are antithetical to its reflexive function, whilst paradoxically being a fundamental part of it. In particular, in capitalism, language is increasingly commodified within the social domains created and affected by ubiquitous communication technologies. The advent of the so-called ‘knowledge economy’ implicates exchangeable forms of thought (language) as the fundamental commodities of this emerging system. The historical point at which a ‘knowledge economy’ emerges, then, is the critical point at which thought itself becomes a commodified ‘thing’, and language becomes its “objective” means of exchange. However, the processes by which such commodification and objectification occurs obscures the unique social relations within which these language commodities are produced. The latest economic phase of capitalism – the knowledge economy – and the obfuscating trajectory which accompanies it, we argue, is destroying the reflexive capacity of language particularly through the process of commodification. This can be seen in that the language practices that have emerged in conjunction with digital technologies are increasingly non-reflexive and therefore less capable of self-critical, conscious change.

The primary aims of scoliosis surgery are to halt the progression of the deformity, and to reduce its severity (cosmesis). Currently, deformity correction is measured in terms of posterior parameters (Cobb angles and rib hump), even though the cosmetic concern for most patients is anterior chest wall deformity. In this study, we propose a new measure for assessing anterior chest wall deformity and examine the correlation between rib hump and the new measure. 22 sets of CT scans were retrieved from the QUT/Mater Paediatric Spinal Research Database. The Image J software (NIH) was used to manipulate formatted CT scans into 3-dimensional anterior chest wall reconstructions. A ‘chest wall angle’ was then measured in relation to the first sacral vertebral body. The chest wall angle was found to be a reliable tool in the analysis of chest wall deformity. No correlation was found between the new measure and rib hump angle. Since rib hump has been shown to correlate with vertebral rotation on CT, this suggests that there maybe no correlation between anterior and posterior deformity measures. While most surgical procedures will adequately address the coronal imbalance & posterior rib hump elements of scoliosis, they do not reliably alter the anterior chest wall shape. This implies that anterior chest wall deformity is to a large degree an intrinsic deformity, not directly related to vertebral rotation.
