Syfte: Syftet med denna studie var att sammanställa patienters upplevelser av att leva med Multipel Skleros (MS) samt att undersöka vilka omvårdnadsbehov patienter med MS har. Metod: Uppsatsen har genomförts som en systematisk litteraturstudie. Inklusionskriterier var att artiklarna skulle vara vetenskapligt granskade och inte äldre än fem år. Sammanlagt har 17 stycken artiklar använts till resultatet, av dessa var nio stycken kvantitativa studier och åtta stycken kvalitativa studier. Resultat: Trötthet och depression var de mest besvärliga och immobiliserande symptomen för personer med MS. Sjukdomen inverkade negativt på sexlivet. Symptom som inkontinens och urinträngningar upplevdes som ett stort problem, men själv-kateterisering ledde till färre urinvägsproblem. Smärta fanns hos hälften av MS-patienterna, av dem upplevde 70,5 % neurogen smärta. Balanssvårigheter var det största problemet gällande rörligheten, och ovissheten om hur symptomen varierar från dag till dag upplevdes frustrerande. En stor del upplevde att andra människor har svårt att förstå deras situation och ta den på allvar. Kvaliteten på information och kommunikation mellan patienterna och vårdpersonalen hade stor betydelse. Delaktighet i planeringen av sin vård var av stor vikt för patienterna med MS.

The main purpose of this thesis project is to prediction of symptom severity and cause in data from test battery of the Parkinson’s disease patient, which is based on data mining. The collection of the data is from test battery on a hand in computer. We use the Chi-Square method and check which variables are important and which are not important. Then we apply different data mining techniques on our normalize data and check which technique or method gives good results.The implementation of this thesis is in WEKA. We normalize our data and then apply different methods on this data. The methods which we used are Naïve Bayes, CART and KNN. We draw the Bland Altman and Spearman’s Correlation for checking the final results and prediction of data. The Bland Altman tells how the percentage of our confident level in this data is correct and Spearman’s Correlation tells us our relationship is strong. On the basis of results and analysis we see all three methods give nearly same results. But if we see our CART (J48 Decision Tree) it gives good result of under predicted and over predicted values that’s lies between -2 to +2. The correlation between the Actual and Predicted values is 0,794in CART. Cause gives the better percentage classification result then disability because it can use two classes.

Under menstruationscykeln påverkas kvinnan av flera hormonstimulerande signalsubstanser och hormoner som i sin tur kan ha olika effekt på den psykiska och fysiska hälsan som PMS – premenstruellt syndrom eller PMDD – premenstruell dysforisk dysfunktion. Syftet med studien var att undersöka variationer i allmänt mående, irritationsnivå och energi under en menstruationscykel. I undersökningen deltog 2 grupper av kvinnor, de som inte använder, respektive de som använder hormonpreparat. Tjugofem undersökningsdeltagare (udd) ingick i undersökningen som pågick under 30 dagar. Undersökningen, som var webbaserad, bestod av att kvinnan dagligen fyllde i frågor om sitt mående. Resultatet visade inte på signifikanta medelvärdesskillnader mellan olika dagar i menscykeln eller mellan kvinnorna i gruppen oavsett grupp. Resultatet signalerade även att kvinnor som inte använder sig av något hormonpreparat svänger mer i sin psykiska hälsa.

Syfte: Syftet med studien var att belysa olika omvårdnadsåtgärder som bidrar till att minska beteendemässiga och psykiska symtom vid demens (BPSD). Metod: Examensarbetet har utförts som en litteraturstudie med 15 vetenskapliga artiklar varav fem kvantitativa och tio kvalitativa. Datainsamling har genomförts i de vårdvetenskapliga databaserna CINAHL och PubMed. Resultat: De olika omvårdnadsåtgärderna som bidrog till att minska BPSD var att anpassa vårdmiljön genom färgsättning och möjlighet till enkelrum samt egen toalett, även att använda sig av personcentrerad och behovsanpassad vård för att beakta hela människan och dess behov, att stimulera hörsel, lukt och känsel med hjälp av taktil och fotmassage, aromaterapi, akupressur och musik. Samt att använda sig av montessori baserade aktiviteter som innebar att personer fick ägna sig åt intressen som de haft innan sjukdomen uppkom och teoretiskt baserade aktiviteter såsom att läsa tidning och lösa korsord. Dessa omvårdnadsåtgärder bidrog till att minska symtom vid BPSD såsom agitation, aggression samt verbal aggression. Konklusion: Genom att anpassa vårdmiljön med hjälp av design och anpassning, att använda sig av personcentrerad och behovsanpassad vård, att stimulera hörsel, lukt och känsel som taktil och fotmassage, aromaterapi, akupressur och musik. Samt att använda sig av montessori baserade aktiviteter och teoretiskt baserade aktiviteter minskas BPSD samtidigt som möjligheten till en god omvårdnad förbättras och livskvaliteten hos personer med demenssjukdom kan öka.

Bakgrunden till projektet Möjligheternas Hus var att det fanns ett behov av att utveckla rehabiliteringsinsatser för individer med psykiska funktionsnedsättningar som står långt från arbetsmarknaden, men även att de skulle erbjudas tillgång till en meningsfull daglig verksamhet. Rapporten syftar till att beskriva, problematisera och analysera hur projektet Möjligheternas Hus genomförts och vad det resulterade i. Den handlar om hur projektet har förmått fungera som ett verktyg för att skapa ett socialt företag för personer med psykiska funktionsnedsättningar. Rapporten behandlar även hur deltagarna har påverkats av sin medverkan i projektet, både vad avser deras livssituation i stort och deras möjligheter att bryta ett utanförskap samt hur projektet lyckades med att uppnå de mål som sattes upp. Deltagarna har startat ett socialt företag och de upplever att de kommit att må bättre. Flertalet uppger att de trivs bättre med livet, att de har ett rikare socialt liv, självförtroendet och självkänslan har stärkts och att det sociala företaget ger hopp för framtiden. En fråga som dock fortfarande är obesvarad är om sociala företag är ett framgångsrikt sätt att hjälpa och stödja individer som befinner i ett utanförskap att kunna lämna detta? Den här rapporten har inga svar på detta, med visar samtidigt att det är en fråga som behöver studeras ytterligare.

Meta-analysis of the heterogeneous symptoms of obsessive-compulsive disorder (OCD) has found a four-factor structure of symptom dimensions consisting of cleaning, forbidden thoughts, symmetry, and hoarding. Research into age of onset of symptom dimensions has yielded inconsistent results, and it is unknown whether symptoms along these dimensions differ in their clinical course. We assessed age of onset and clinical course of different OCD symptom dimensions in a large cohort of adult patients. Nine-hundred fifty-five subjects were assessed using the Dimensional Yale-Brown Obsessive-Compulsive Scale. For age of onset analysis, we tested across three methods of classification: (1) primary (more severe) symptom dimension (2) clinically significant symptoms within a dimension or (3) any symptoms within a dimension. Age of onset was defined as the earliest age of onset reported for any individual item within a symptom dimension. For analysis of different types of clinical course, we used chi-square tests to assess for differences between primary symptom dimensions. OCD symptoms in the symmetry dimension had an earlier age of onset than other OCD symptom dimensions. These findings remained significant across all three methods of classification and controlling for gender and comorbid tics. No significant differences were found between the other dimensions. Subjects with primary OCD symptoms in the forbidden thoughts dimension were more likely to report a waxing-and-waning course, whereas symmetry symptoms were less likely to be associated with a waxing-and-waning course. © 2013.

The study aimed to compare male and female patients with obsessive-compulsive disorder (OCD) across symptom dimensions, clinical course and comorbidity. A cross-sectional study was undertaken with 858 adult OCD patients (DSM-IV) from the Brazilian Research Consortium on Obsessive-Compulsive Spectrum Disorders. Patients were evaluated using structured interviews, including the Dimensional Yale-Brown Obsessive-Compulsive Scale (DY-BOCS) and the Structured Clinical Interview for DSM-IV Axis I disorders (SCID-I). The sample was composed of 504 women (58.7%) and 354 men (41.3%) with a mean age of 35.4 years-old (range: 18-77). Men were younger, more frequently single and presented more tics, social phobia and alcohol use disorders. Among men, symptom interference occurred earlier and symptoms of the sexual/religious dimension were more common and more severe. Conversely, women were more likely to present symptoms of the aggressive, contamination/cleaning and hoarding dimension and comorbidity with specific phobias, anorexia nervosa, bulimia, trichotillomania, skin picking and compulsive buying. In the logistic regression, female gender remained independently associated with the aggressive and contamination/cleaning dimensions. In both genders the aggressive dimension remained associated with comorbid post-traumatic stress disorder, the sexual/religious dimension with major depression and the hoarding dimension with tic disorders. Gender seems to be relevant in the determination of OCD clinical presentation and course and should be considered an important aspect when defining more homogeneous OCD subgroups. © 2012 Elsevier B.V.

Since there was no Portuguese questionnaire to evaluate cutaneous allodynia, which has been pointed out as a risk factor of migraine, we aimed to perform the cross-cultural adaptation of the 12 item Allodynia Symptom Checklist for the Brazilian population and to test its measurement properties. It consisted in six stages: translation, synthesis, back translation, revision by a specialist committee, pretest and submission the documents to the committee. In the pretest stage, the questionnaire was applied to 30 migraineurs of both sexes, who had some difficulty in understanding it. Thus, a second version was applied to 30 additional subjects, with no difficulties being reported. The mean filling out time was 3'36", and the internal consistency was 0.76. To test reproducibility, 15 other subjects filled out the questionnaire at two different times, it was classified as moderate (weighted kappa=0.58). We made available to Brazilian population an easy, quick and reliable questionnaire.

Background: Clinical and sociodemographic findings have supported that OCD is heterogeneous and composed of multiple potentially overlapping and stable symptom dimensions. Previous neuroimaging investigations have correlated different patterns of OCD dimension scores and gray matter (GM) volumes. Despite their relevant contribution, some methodological limitations, such as patient's previous medication intake, may have contributed to inconsistent findings. Method: Voxel-based morphometry was used to investigate correlations between regional GM volumes and symptom dimensions severity scores in a sample of 38 treatment-naive OCD patients. Several standardized instruments were applied, including an interview exclusively developed for assessing symptom dimensions severity (DY-BOCS). Results: Scores on the "aggression" dimension were positively correlated with GM volumes in lateral parietal cortex in both hemispheres and negatively correlated with bilateral insula, left putamen and left inferior OFC. Scores on the "sexual/religious" dimension were positively correlated with GM volumes within the right middle lateral OFC and right DLPFC and negatively correlated with bilateral ACC. Scores on the "hoarding" dimension were positively correlated with GM volumes in the left superior lateral OFC and negatively correlated in the right parahippocampal gyrus. No significant correlations between GM volumes and the "contamination" or "symmetry" dimensions were found. Conclusions: Building upon preexisting findings, our data with treatment-naive OCD patients have demonstrated distinct GM substrates implicated in both cognitive and emotion processing across different OCS dimensions. OBJECTIVE: To investigate the influence of brain-derived neurotrophic factor (BDNF) gene variations on cognitive performance and clinical symptomatology in first-episode psychosis (FEP). METHODS: We performed BDNF val66met variant genotyping, cognitive testing (verbal fluency and digit spans) and assessments of symptom severity (as assessed with the PANSS) in a population-based sample of FEP patients (77 with schizophreniform psychosis and 53 with affective psychoses) and 191 neighboring healthy controls. RESULTS: There was no difference in the proportion of Met allele carriers between FEP patients and controls, and no significant influence of BDNF genotype on cognitive test scores in either of the psychosis groups. A decreased severity of negative symptoms was found in FEP subjects that carried a Met allele, and this finding reached significance for the subgroup with affective psychoses (p < 0.01, ANOVA). CONCLUSIONS: These results suggest that, in FEP, the BDNF gene Val66Met polymorphism does not exert a pervasive influence on cognitive functioning but may modulate the severity of negative symptoms.

Clinical manifestations of lactase (LCT) deficiency include intestinal and extra-intestinal symptoms. Lactose hydrogen breath test (H2-BT) is considered the gold standard to evaluate LCT deficiency (LD). Recently, the single-nucleotide polymorphism C/T(-13910) has been associated with LD. The objectives of the present study were to evaluate the agreement between genetic testing of LCT C/T(-13910) and lactose H2-BT, and the diagnostic value of extended symptom assessment. Of the 201 patients included in the study, 194 (139 females; mean age 38, range 17-79 years, and 55 males, mean age 38, range 18-68 years) patients with clinical suspicion of LD underwent a 3-4 h H2-BT and genetic testing for LCT C/T(-13910). Patients rated five intestinal and four extra-intestinal symptoms during the H2-BT and then at home for the following 48 h. Declaring H2-BT as the gold standard, the CC(-13910) genotype had a sensitivity of 97% and a specificity of 95% with a of 0.9 in diagnosing LCT deficiency. Patients with LD had more intense intestinal symptoms 4 h following the lactose challenge included in the H2-BT. We found no difference in the intensity of extra-intestinal symptoms between patients with and without LD. Symptom assessment yielded differences for intestinal symptoms abdominal pain, bloating, borborygmi and diarrhoea between 120 min and 4 h after oral lactose challenge. Extra-intestinal symptoms (dizziness, headache and myalgia) and extension of symptom assessment up to 48 h did not consistently show different results. In conclusion, genetic testing has an excellent agreement with the standard lactose H2-BT, and it may replace breath testing for the diagnosis of LD. Extended symptom scores and assessment of extra-intestinal symptoms have limited diagnostic value in the evaluation of LD.
