Cette étude comparative analyse le management au sein de cinq juridictions du pouvoir judiciaire genevois. Elle s'intéresse en particulier au président et au greffier de juridiction, qui, dans ces structures, sont investis de cette mission, et se fonde sur une conception actualisée du cycle traditionnel de management. Comment fonctionnent et s'organisent les juridictions du point de vue managérial ? Comment les managers se répartissent-ils leur rôle et leurs responsabilités ? Quelles sont les pratiques managériales utilisées, et comment sont-elles perçues par les collaborateurs et les managers ? Cet ouvrage a pour objectif de répondre à ces questions et de formuler une série de recommandations visant l'optimisation du management juridictionnel au sein du pouvoir judiciaire à Genève.

A l'occasion d'une intervention en santé au travail au siège d'une ONG humanitaire, nous avons constaté la présence d'une souffrance au travail importante parmi le personnel. Nous avons alors effectué une recherche sur l'organisation du travail et l'activité des employés dans le but de proposer des pistes de compréhension de cette souffrance. Nous avons collecté des données à l'aide d'un questionnaire, de 20 entretiens semi-directifs et de 26 jours d'observation non-participante. Nous avons mis en évidence un type de fonctionnement spécifique s'articulant en permanence autour de l'urgence. Celui-ci se nourrit de la nature des interventions de cette ONG ainsi que de la très forte valorisation des activités liées aux missions sur les terrains : tout se passe comme si le mode d'intervention " en situation d'urgence ", propre aux activités du terrain, contaminait l'organisation du travail au niveau du siège. Les employés sont pris dans un rythme effréné, ce qui les empêche de rester attentif à l'importance des tâches et de discriminer entre ce qui relève de " vraies " urgences et d'urgences " fantasmées ". Ceci génère un environnement potentiellement pathogène, où il est d'une part peu légitime de se préoccuper de sa propre santé et où d'autre part la charge de travail et le rythme ne laissent pas le temps à l'employé de mettre en place des stratégies de préservation de sa santé. The functional architecture of the occipital cortex is being studied with increasing detail. Functional and structural MR based imaging are altering views about the organisation of the human visual system. Recent advances have ranged from comparative studies with non-human primates to predictive scanning. The latter multivariate technique describes with sub-voxel resolution patterns of activity that are characteristic of specific visual experiences. One can deduce what a subject experienced visually from the pattern of cortical activity recorded. The challenge for the future is to understand visual functions in terms of cerebral computations at a mesoscopic level of description and to relate this information to electrophysiology. The principal medical application of this new knowledge has focused to a large extent on plasticity and the capacity for functional reorganisation. Crossmodality visual-sensory interactions and cross-correlations between visual and other cerebral areas in the resting state are areas of considerable current interest. The lecture will review findings over the last two decades and reflect on possible roles for imaging studies in the future.

Head and neck squamous cell cancer (HNSCC) is the sixth leading cause of cancer-related deaths worldwide. These tumors are commonly diagnosed at advanced stages and mortality rates remain high. Even cured patients suffer the consequences of aggressive treatment that includes surgery, chemotherapy, and radiotherapy. In the past, in clinical trials, HNSCC was considered as a single disease entity. Advances in molecular biology with the development of genomic and proteomic approaches have demonstrated distinct prognostic HNSCC patient subsets beyond those defined by traditional clinical-pathological factors such as tumor subsite and stage [Cho W (ed). An Omics Perspective on Cancer Research. New York/Berlin: Springer 2010]. Validation of these biomarkers in large prospective clinical trials is required before their clinical implementation. To promote this research, the European Organisation for Research and Treatment of Cancer (EORTC) Head and Neck Cancer Program will develop the following strategies-(i) biobanking: prospective tissue collection from uniformly treated patients in the setting of clinical trials; (ii) a group of physicians, physician-scientists, and EORTC Headquarters staff devoted to patient-oriented head and neck cancer research; (iii) a collaboration between the basic scientists of the Translational Research Division interested in head and neck cancer research and the physicians of the Head and Neck Cancer Group; and (iv) funding through the EORTC Grant Program and the Network Core Institutions Consortium. In the present report, we summarize our strategic plans to promote head and neck cancer research within the EORTC framework.

There are different approaches to dealing with alcohol related problems in the workplace. A literature review indicates that two of the models that underpin programmes to deal with alcohol related problems in the workplace are the disease model and the health promotion model. The disease model considers alcoholism as an illness and uses curative techniques to restore the individual to sobriety. The health promotion model looks at the determinants of health and promotes changes in the environment and structures, which would support healthy behaviour in relation to alcohol. Employee Assistance Programmes (EAPs) may have elements of both theses models. Dealing with alcohol problems at work involves a captive audience and the workplace as a setting can be used to influence healthier lifestyles. A workplace alcohol policy is a mechanism through which alcohol related issues might be dealt with, and the necessary resources and commitment of managers and staff channelled to this end. The policy aims should be clear and unambiguous, and specific plans put in place for implementing all aspects of the policy. In the case of the alcohol policy in the organisation under study, the policy was underpinned by a health promotion ethos and the policy document reflects broad aims and objectives to support this. The steering group that oversaw the development of the policy had particular needs of their own which they brought to the development process. The common theme in their needs was how to identify and support employees with alcohol related problems within an equitable staff welfare system. The role of the supervisor was recognised as crucial and training was provided to introduce the skills needed for an early intervention and constructive confrontation with employees who had alcohol related problems. Opportunities provided by this policy initiative to deal with broader issues around alcohol and to consider the determinants of health in relation to alcohol were not fully utilised. The policy formalised the procedures for dealing with people who have alcohol related problems in an equitable and supportive manner. The wider aspect of the health promotion approach does not appear to have been a priority in the development and implementation of the policy.This resource was contributed by The National Documentation Centre on Drug Use.

Le décalage entre la sacralisation du principe du développement durable et la subsistance de pratiques néfastes pour l'environnement et les ressources naturelles ne va pas sans poser un certain nombre de questions sur les raisons qui expliquent cette situation; cela soulève en particulier la question de la définition exacte de ce qui peut et doit être considéré comme une exploitation durable des ressources. Dans ce contexte, ce travail se propose d'évaluer un mode de gestion dont il a été affirmé à plusieurs reprises qu'il s'agissait d'un «modèle de gestion durable»: les modes d'organisation communautaires liés à l'irrigation par les bisses, ces canaux à ciel ouvert très répandus en Valais. Notre approche, qui se situe dans la continuité des travaux sur les Régimes Institutionnels de Ressources (RIR) et repose sur une conception indéniablement forte de la durabilité, propose une grille d'analyse novatrice à travers l'assimilation des réseaux d'irrigation à des complexes multiressourciels composés des ressources eau, infrastructures (bisse) et sol. L'application rigoureuse de cette grille au cas de Savièse permet - en mettant en évidence les lacunes et les incohérences des modes de gestion qui, en 2010, sont mis en place autour du réseau encore partiellement composé de bisses - de démontrer que leur caractère durable ne va pas de soi et nécessite sans aucun doute une analyse au cas par cas.

This report presents the results of the second national audit which examines the organisation of services provided to older people for falls prevention and bone health. Falls and fractures are a common and serious problem affecting older people, with high levels of personal and financial cost. National guidelines, supported by the research evidence, require the provision of integrated services for falls and fracture prevention and treatment. Effective commissioning is needed to produce such high quality services.��This audit was commissioned by the Healthcare Quality Improvement Partnership (HQIP) as part of the second cycle of audits on services for the prevention of falls and fractures in older people. It follows the first organisational audit, performed in 2005, and the clinical audit of 2007. All were audited against specific standards from the National Service Framework for Older People (NSF) and guidance from the National Institute for Health and Clinical Excellence (NICE). Since the first audit, indicators have been added or updated in line with new guidance including that on falls prevention of inpatients following the National Patient Safety Agency (NPSA) report on slips, trips and falls in hospital (2007). For the first time, the audit also looks specifically at falls and fracture prevention in mental healthcare and a sample of care homes.
