Socioeconomic disadvantage is an important predictor of maternal harsh discipline, but few studies have examined risk mechanisms for harsh parenting within disadvantaged samples. In the present study, parenting stress, family conflict, and child difficult temperament are examined as predictors of maternal harsh discipline among a group of 58 mothers from socioeconomically disadvantaged backgrounds and their young children between the ages of 1- to 4-years-old. Maternal harsh discipline was measured using standardized observations, and mothers reported on parenting stress, family conflict, and child temperament. Severity of socioeconomic deprivation was included as a moderator in these associations. Results showed that parenting stress and family conflict predicted maternal harsh discipline, but only in the most severely deprived families. These findings extend prior research on the processes through which socioeconomic deprivation severity and family functioning impact maternal harsh discipline within a high-risk sample of low-income families. They suggest that the spillover of negative parental functioning into parent–child interactions is particularly likely under conditions of substantial socioeconomic deprivation. Severity of socioeconomic stress seems to undermine maternal adaptive forms of coping, resulting in harsh disciplining practices. Intervention efforts aimed at improving parenting and family relations, as well as an adaptive coping style assume especial relevance.

OBJECTIVE: To report the pattern of occurrence, diagnosis, and treatment of hypertension in a female nursing staff of an emergency hospital. METHODS: We carried out a cross-sectional study that included interviews and blood pressure measurements of 494 nursing professionals at an emergency hospital in the city of Salvador, in the state of Bahia, Brazil. We considered hypertensive all individual with blood pressure > or = 140/90 mmHg or normal pressure if on regular treatment. RESULTS: We found a prevalence of hypertension of 36.4%. Only 18.3% of the individuals ignored their hypertensive condition, and 64.2% admitted not being having regular treatment. Of those individuals who were having treatment, 69.4% had elevated blood pressure on examination. The major reasons for not being on treatment was the occasional elevation of blood pressure (22.2%) and medical counseling (20.0%). CONCLUSION: The results point to the need to introduce hypertension control measures in this occupational group, because of the magnitude of the disease and the potential impact on diffusion of knowledge and measures to control hypertension.

Los profesionales de enfermería sufren en este momento histórico el impacto de la crisis económica y los cambios en el contexto político y social en lo que se refiere a las políticas de empleo y ajustes salariales. El ambiente de trabajo en las unidades de hospitalización suele ser agobiante y las actividades que realizan los exponen a riesgos específicos tales como: los propios del ambiente, condiciones y tipo de trabajo y las relaciones interpersonales. Definimos la satisfacción laboral como el resultado de la apreciación que cada individuo tiene de su trabajo y la importancia para satisfacer sus necesidades básicas.Objetivo General: Conocer y describir el nivel de satisfacción laboral y la percepción de riesgos en el trabajo que manifiestan los profesionales de Enfermería.Objetivos Específicos: - Describir la percepción de riesgos del ambiente de trabajo - Mostrar el nivel de satisfacción laboral según el tipo y número de pacientes que atienden, antigüedad en la profesión, estabilidad laboral, sexo, edad, situación de pareja, número de hijos,- Describir la satisfacción laboral en cuanto a los recursos con los que cuenta para el cuidado y la seguridad laboral de la institución, al salario que recibe y la cantidad de empleos que tiene, - Determinar la satisfacción laboral con respecto al estímulo que reciben de los superiores, al reconocimiento de los pares, equipo de salud y usuarios.El encuadre metodológico es el siguiente:El tipo de estudio elegido es el considerado correlacional simple ya que busca posibles vínculos entre dos o más variables.El universo lo componen los profesionales de enfermería que se desempeñan en instituciones públicas de salud de la ciudad de Río Cuarto y Córdoba Capital.La técnica de recolección de datos es la encuesta en su modalidad de cuestionario autoadministrado.

BACKGROUND: The number of nonagenarians and centenarians is rising dramatically, and many of them live in nursing homes. Very little is known about psychiatric symptoms and cognitive abilities other than memory in this population. This exploratory study focuses on anosognosia and its relationship with common psychiatric and cognitive symptoms. METHODS: Fifty-eight subjects aged 90 years or older were recruited from geriatric nursing homes and divided into five groups according to Mini-Mental State Examination scores. Assessment included the five-word test, executive clock-drawing task, lexical and categorical fluencies, Anosognosia Questionnaire-Dementia, Neuropsychiatric Inventory, and Charlson Comorbidity Index. RESULTS: Subjects had moderate cognitive impairment, with mean ± SD Mini-Mental State Examination being 15.41 ± 7.04. Anosognosia increased with cognitive impairment and was associated with all cognitive domains, as well as with apathy and agitation. Subjects with mild global cognitive decline seemed less anosognosic than subjects with the least or no impairment. Neither anosognosia nor psychopathological features were related to physical conditions. CONCLUSIONS: Anosognosia in oldest-old nursing home residents was mostly mild. It was associated with both cognitive and psychopathological changes, but whether anosognosia is causal to the observed psychopathological features requires further investigation.

Modification of the immune response to schistosomal infection in children or offspring born to mother R infected with Schistosoma mansoni has been demonstrated in human and in experimental schistosomiasis. One of the hypothesis to explain this fact could be the transfer of circulating antigens and antibodies from mother to foetus through the placenta or from mother to child by milk. The results of this spontaneous transference are controversial in the literature. In an attempt to investigate these questions, we studied one hundred and twenty offspring (Swiss mice), sixty born to infected-mothers (group A) and sixty born to non-infected mothers (group B). These were percutaneously infected with 50 cercariae/mouse, and divided in six sub-groups (20 mice/sub-group), according to the following schedule: after birth (sub-groups A.I and B.I), 10 days old (sub-groups A.II and B.II) and 21 days old (sub-groups A.III and B.III). After the exposure period, the young mice returned to their own mothers for nursing. Six weeks later, the mice were killed. We obtained the following results: 1) There is transference of antibody to cercariae (CAP), adult worms (SWAP) and egg antigens (SEA) from the infected mothers to the offspring, probably through placenta and milk; 2) Offspring born to infected mothers exhibit much less coagulative hepatic necrosis and show a lower number of eggs in the small intestine and a less intense and predominant exsudative stage of the hepatic granulomas when compared with the exsudative-productive stage of the control groups. The findings suggest that congenital and nursing factors can interfere on the development of the schistosomiasis infection, causing an hyporesponse to the eggs.

Q-sort is a research method which allows defining profiles of attitudes toward a set of statements, ordered in relation to each other. Pertaining to the Q Methodology, the qualitative analysis of the Q-sorts is based on quantitative techniques. This method is of particular interest for research in health professions, a field in which attitudes of patients and professionals are very important. The method is presented in this article, along with an example of application in nursing in old age psychiatry.

Les addictions ont fortement consolidé leur place dans le champ de la médecine à la fin du 20e siècle parce que la certitude de leurs fondements neurobiologiques était désormais acquise et que les preuves de l'efficacité des stratégies de prévention et de traitement étaient suffisantes. La médecine de l'addiction couvre un champ très large de l'activité médicale allant de la prévention de l'usage à risque, en passant par le dépistage des formes peu sévères de dépendance au traitement des formes les plus graves, chroniques et invalidantes. Cette activité médicale prend place dans une grande diversité de lieux de soins, en médecine de premier recours, particulièrement dans les cabinets de médecine de famille, mais aussi aux urgences et en pédiatrie. La médecine de l'addiction est très présente en psychiatrie où ses formes sont souvent sévères. Malgré cela, aujourd'hui seul un patient sur dix touché par une problématique d'addiction est traité. C'est la raison pour laquelle la Suisse, comme c'est le cas aux Etats-Unis et aux Pays-Bas par exemple, doit se doter d'un système promouvant une médecine de l'addiction à «large spectre», permettant d'identifier et de traiter la grande diversité des troubles rencontrés dans les lieux de soins les plus touchés. En Suisse, les médecins sont actuellement insuffisamment préparés pour assumer ce rôle. C'est aux disciplines concernées de proposer des filières de formation. Cette stratégie contribuera à améliorer les compétences de l'ensemble du corps médical dans ce domaine, avec pour conséquence d'offrir au plus grand nombre de patients touchés par les addictions les traitements les plus efficaces

Whether or not macroeconomics is a science depends on the scientific nature of macroeconomic theories and how the discipline responds when the empirical evidence fails to match the underlying assumptions and predictions of the theories. By way of an example, four conditions for macroeconomics to be a science are developed and used to examine the 'modern' theories of the Phillips curve. It is found that while the discipline in general maintains one condition it routinely violates the other three. This suggests the macroeconomics discipline has some way to go before it can call itself a 'pure science.'

OBJECTIVES: To determine the relationship between infections and functional impairment in nursing home residents. DESIGN: Prospective cohort study (follow-up period, 6 months). SETTING: Thirty-nine nursing homes in western Switzerland. PARTICIPANTS: A total of 1,324 residents aged 65 and older (mean age 85.7; 76.6% female) who agreed to participate, or their proxies, by oral informed consent. MEASUREMENTS: Functional status measured every 3 months. Two different outcomes were used: (a) functional decline defined as death or decreased function at follow-up and (b) functional status score using a standardized measure. RESULTS: At the end of follow-up, mortality was 14.6%, not different for those with and without infection (16.2% vs 13.1%, P=.11). During both 3-month periods, subjects with infection had higher odds of functional decline, even after adjustment for baseline characteristics and occurrence of a new illness (adjusted odds ratio (AOR)=1.6, 95% confidence interval (CI)=1.2-2.2, P=.002, and AOR=1.5, 95% CI=1.1-2.0, P=.008, respectively). The odds of decline increased in a stepwise fashion in patients with zero, one, and two or more infections. The analyses predicting functional status score (restricted to subjects who survived) gave similar results. A survival analysis predicting time to first infection confirmed a stepwise greater likelihood of infection in subjects with moderate and severe impairment at baseline than in subjects with no or mild functional impairment at baseline. CONCLUSION: Infections appear to be both a cause and a consequence of functional impairment in nursing home residents. Further studies should be undertaken to investigate whether effective infection control programs can also contribute to preventing functional decline, an important component of these residents' quality of life.

This paper studies party discipline in a congress within a political agency framework with retrospective voting. Party discipline serves as an incentive device to induce office- motivated congress members to perform in line with the party leadership's objective of controlling both the executive and the legislative branches of government. I show fi rst that the same party is more likely to control both branches of government (i.e., uni ed government) the stronger the party discipline in the congress is. Second, the leader of the governing party imposes more party discipline under uni ed government than does the opposition leader under divided government. Moreover, the incumbents' aggregate performance increases with party discipline, so a representative voter becomes better off. JEL classi cation: D72. OBJECTIVE: To investigate the evolution of delirium of nursing home (NH) residents and their possible predictors. DESIGN: Post-hoc analysis of a prospective cohort assessment. SETTING: Ninety NHs in Switzerland. PARTICIPANTS: Included 14,771 NH residents. MEASUREMENTS: The Resident Assessment Instrument Minimum Data Set and the Nursing Home Confusion Assessment Method were used to determine follow-up of subsyndromal or full delirium in NH residents using discrete Markov chain modeling to describe long-term trajectories and multiple logistic regression analyses to determine predictors of the trajectories. RESULTS: We identified four major types of delirium time courses in NH. Increasing severity of cognitive impairment and of depressive symptoms at the initial assessment predicted the different delirium time courses. CONCLUSION: More pronounced cognitive impairment and depressive symptoms at the initial assessment are associated with different subsequent evolutions of delirium. The presence and evolution of delirium in the first year after NH admission predicted the subsequent course of delirium until death.
