INTRODUCTION The dimensions of the thoracic intervertebral foramen in adolescent idiopathic scoliosis (AIS) have not previously been quantified. During posterior approach scoliosis correction surgery pedicle screws may occasionally breach into the foramen. Better understanding of the dimensions of the foramen may be useful in surgical planning. This study describes a reproducible method for measurement of the thoracic foramen in AIS using computerized tomography (CT). METHODS In 23 pre-operative female patients with Lenke 1 type AIS with right side convexity major curves confined to the thoracic spine the foraminal height (FH), foraminal width (FW), pedicle to superior articular process distance (P-SAP) and cross sectional foraminal area (FA) were measured using multiplanar reconstructed CT. Measurements were made at entrance, midpoint and exit of the thoracic foramina from T1/T2 to T11/T12. Results were correlated with potential dependent variables of major curve Cobb Angle measured on X-ray and CT, Age, Weight, Lenke classification subtype, Risser Grade and number of spinal levels in the major curve. RESULTS The FH, FW, P-SAP and FA dimensions and ratios are all significantly larger on the convexity of the major curve and maximal at or close to the apex. Mean thoracic foraminal dimensions change in a predictable manner relative to position on the major thoracic curve. There was no significant correlation with the measured foraminal dimensions or ratios and the potential dependent variables. The average ratio of convexity to concavity dimensions at the apex foramina for entrance, midpoint and exit respectively are FH (1.50, 1.38, 1.25), FW (1.28, 1.30, 0.98), FA (2.06, 1.84, 1.32), P-SAP (1.61, 1.47, 1.30). CONCLUSION Foraminal dimensions of the thoracic spine are significantly affected by AIS. Foraminal dimensions have a predictable convexity to concavity ratio relative to the proximity to the major curve apex. Surgeons should be aware of these anatomical differences during scoliosis correction surgery.

INTRODUCTION Adolescent idiopathic scoliosis (AIS) is a spinal deformity, which may require surgical correction by attaching rods to the patient’s spine using screws inserted into the vertebrae. Complication rates for deformity correction surgery are unacceptably high. Determining an achievable correction without overloading the adjacent spinal tissues or implants requires an understanding of the mechanical interaction between these components. Our novel patient specific modelling software creates individualized finite element models (FEM) representing the thoracolumbar spine and ribcage of scoliosis patients. This paper presents a novel algebraic formulation of the central problem of screw theory, namely the determination of the principal screws of a given system. Using the algebra of dual numbers, it shows that the principal screws can be determined via the solution of a generalised eigenproblem of two real, symmetric matrices. This approach allows the study of the principal screws of the general two-, three-systems associated with a manipulator of arbitrary geometry in terms of closed-form expressions of its architecture and configuration parameters. We also present novel methods for the determination of the principal screws for four-, five-systems which do not require the explicit computation of the reciprocal systems. Principal screws of the systems of different orders are identified from one uniform criterion, namely that the pitches of the principal screws are the extreme values of the pitch.The classical results of screw theory, namely the equations for the cylindroid and the pitch-hyperboloid associated with the two-and three-systems, respectively have been derived within the proposed framework. Algebraic conditions have been derived for some of the special screw systems. The formulation is also illustrated with several examples including two spatial manipulators of serial and parallel architecture, respectively.

Use of socket prostheses Currently, for individuals with limb loss, the conventional method of attaching a prosthetic limb relies on a socket that fits over the residual limb. However, there are a number of issues concerning the use of a socket (e.g., blisters, irritation, and discomfort) that result in dissatisfaction with socket prostheses, and these lead ultimately a significant decrease in quality of life. Bone-anchored prosthesis Alternatively, the concept of attaching artificial limbs directly to the skeletal system has been developed (bone anchored prostheses), as it alleviates many of the issues surrounding the conventional socket interface.Bone anchored prostheses rely on two critical components: the implant, and the percutaneous abutment or adapter, which forms the connection for the external prosthetic system (Figure 1). To date, an implant that screws into the long bone of the residual limb has been the most common intervention. However, more recently, press-fit implants have been introduced and their use is increasing. Several other devices are currently at various stages of development, particularly in Europe and the United States. Benefits of bone-anchored prostheses Several key studies have demonstrated that bone-anchored prostheses have major clinical benefits when compared to socket prostheses (e.g., quality of life, prosthetic use, body image, hip range of motion, sitting comfort, ease of donning and doffing, osseoperception (proprioception), walking ability) and acceptable safety, in terms of implant stability and infection. Additionally, this method of attachment allows amputees to participate in a wide range of daily activities for a substantially longer duration. Overall, the system has demonstrated a significant enhancement to quality of life. Challenges of direct skeletal attachment However, due to the direct skeletal attachment, serious injury and damage can occur through excessive loading events such as during a fall (e.g., component damage, peri-prosthetic fracture, hip dislocation, and femoral head fracture). These incidents are costly (e.g., replacement of components) and could require further surgical interventions. Currently, these risks are limiting the acceptance of bone-anchored technology and the substantial improvement to quality of life that this treatment offers. An in-depth investigation into these risks highlighted a clear need to re-design and improve the componentry in the system (Figure 2), to improve the overall safety during excessive loading events. Aim and purposes The ultimate aim of this doctoral research is to improve the loading safety of bone-anchored prostheses, to reduce the incidence of injury and damage through the design of load restricting components, enabling individuals fitted with the system to partake in everyday activities, with increased security and self-assurance. The safety component will be designed to release or ‘fail’ external to the limb, in a way that protects the internal bone-implant interface, thus removing the need for restorative surgery and potential damage to the bone. This requires detailed knowledge of the loads typically experienced by the limb and an understanding of potential overload situations that might occur. Hence, a comprehensive review of the loading literature surrounding bone anchored prostheses will be conducted as part of this project, with the potential for additional experimental studies of the loads during normal activities to fill in gaps in the literature. This information will be pivotal in determining the specifications for the properties of the safety component, and the bone-implant system. The project will follow the Stanford Biodesign process for the development of the safety component.

The purpose of the present study was to investigate the effects of low-intensity ultrasound on bioabsorbable self-reinforced poly-L-lactide (SR-PLLA) screws and on fracture healing after SR-PLLA device fixation in experimental and clinical cancellous bone fracture. In the first experimental study, the assessment of the mechanical strengths of the SR-PLLA screws was performed after 12 weeks of daily 20-minute ultrasound exposure in vitro. In the second experimental study, 32 male Wistar rats with an experimental distal femur osteotomy fixed with an SR-PLLA rod were exposed for daily low-intensity ultrasound treatment for 21 days. The effects on the healing bone were assessed. The clinical studies consist of three prospective, randomized, and placebo-controlled series of dislocated lateral malleolar fractures fixed with one SR-PLLA screw. The total number of the patients in these series was 52. Half of the patients were provided randomly with a sham ultrasound device. The patients underwent ultrasound therapy 20 minutes daily for six weeks. Radiological bone healing was assessed both by radiographs at two, six, nine, and 12 weeks and by multidetector computed tomography (MDCT) scans at two weeks, nine weeks, and 18 months. Bone mineral density was assessed by dual-energy X-ray absorptiometry (DXA). The clinical outcome was assessed by both Olerud-Molander scoring and clinical examination of the ankle. Low-intensity ultrasound had no effects on the mechanical properties and degradation behaviour of the SR-PLLA screws in vitro. There were no obvious signs of low-intensity ultrasound-induced enhancement in the bone healing in SR-PLLA-rod-fixed metaphyseal distal femur osteotomy in rats. The biocompatibility of low-intensity ultrasound treatment and SR-PLLA was found to be good. In the clinical series low-intensity ultrasound was observed to have no obvious effects on the bone mineral density of the fractured lateral malleolus. There were no obvious differences in the radiological bone healing times of the SR-PLLA-screw-fixed lateral malleolar fractures after low-intensity ultrasound treatment. Low-intensity ultrasound did not have any effects on radiological bone morphology, bone mineral density or clinical outcome 18 months after the injury. There were no obvious findings in the present study to support the hypothesis that low-intensity pulsed ultrasound enhances bone healing in SR-PLLA-rod-fixed experimental metaphyseal distal femur osteotomy in rats or in clinical SR-PLLA-screw-fixed lateral malleolar fractures. It is important to limit the conclusions of the present set of studies only to lateral malleolar fractures fixed with an SR-PLLA screw.

[EU]Hiru dimentsioko inprimaketa etorkizun handiko teknologia bezala azaltzen zaigu gaur egun. Esate baterako, biomedikuntza arloan aukera berritzaileak ekar ditzake, baina baita hezkuntza, heziketa eta ikerketa munduetan ere. Teknologia berri honen abantailarik nagusiena prototipatze azkarrean datza, eta honi esker, mikro- eta makro- egitura definituak dituzten objektuak diseinatu eta fabrikatu daitezke modu lehiakorrean. Lan honen helburua 3D inprimagailu baten bitartez inprimaturiko polimero biobateragarri eta biodegradagarrietan oinarrituriko ereduen garapen eta fabrikazioan datza. Hala ere, lehenik eta behin, lehengaiak bai fisikoki eta bai termikoki karakterizatu behar dira, ondoren, 3D inprimagailuaren parametroen arteko erlazioa ezarri, eta azkenik, produktu finalaren egitura propietateak eta kalitatea aztertu. O afrouxamento dos parafusos protéticos é descrito na literatura como uma das complicações mais frequentes das próteses sobre implantes. Durante sua confecção, os profissionais sentem necessidade de remover várias vezes as próteses e/ou componentes protéticos, soltando e re-apertando os parafusos repetidamente. O principal objetivo deste trabalho foi avaliar a variação do torque de remoção de parafusos de fixação de pilares protéticos a implantes osteointegráveis após sucessivos ciclos de parafusamento e desparafusamento. Outro objetivo foi avaliar a influência do hexágono da base do pilar no torque de remoção dos parafusos. Para isso, foram utilizados 20 implantes de plataforma regular com hexágono externo e 20 pilares protéticos sextavados, que foram parafusados aos implantes com um parafuso de titânio, aplicando-se a este um torque de 32Ncm, por meio de um torquímetro digital. Os conjuntos implante/pilar/parafuso foram divididos em dois grupos: (1) pilares cujo hexágono da base foram removidos e (2) pilares convencionais, com hexágono na base. Cada conjunto recebeu uma restauração provisória e foi submetido a ciclagem mecânica por 15 minutos. Depois, os parafusos foram removidos, medindo-se o torque de remoção. Esta sequência foi repetida dez vezes e então o parafuso foi trocado por outro sem uso, e mais um ciclo foi realizado. Uma análise de regressão linear demonstrou nos dois grupos uma queda do torque de remoção do parafuso ao longo dos repetidos ciclos de inserção/remoção. A comparação entre os coeficientes da regressão nos dois grupos não revelou diferença entre eles. Também não houve diferença entre as médias das 5 últimas repetições e o 11 ciclo, com o parafuso novo. Concluiu-se que (1) repetidos parafusamentos e desparafusamentos promoveram a diminuição progressiva do torque de remoção dos parafusos, (2) a troca do parafuso por outro sem uso após dez ciclos de inserção/remoção não aumentou sua resistência ao afrouxamento, e (3) a remoção do hexágono da base do pilar protético não exerceu nenhum efeito sobre o torque de remoção do parafuso.
Caracterización experimental del comportamiento dinámico de un accionamiento de máquina herramienta.

[ES]El objetivo de este proyecto es validar experimentalmente una serie de modelos que caracterizan el comportamiento de un accionamiento de máquina herramienta de husillo a bolas. El proyecto aparece como respuesta a un problema de precisión que se percibe en máquinas herramienta con pórticos y columnas verticales de varios metros. Para ello, es necesario diseñar un prototipo de esta máquina herramienta y un modelo de 3 grados de libertad (con un programa numérico y un simulador). Este modelo debe ser validado por medidas internas y externas para predecir como una columna real actuaria bajo condiciones predeterminadas.

Aplikazio hauek suposatzen duten inbertsio ekonomikoa dela eta, ihesak ekiditeko asmoz, ezinbestekoa da brida hauek elkarri lotzeko dituzten torlojuen kargak uniformeki bananduta egotea torloju guztietan, interakzio elastikoak eta pibotamendu mugimenduak helburu hori zailtzen badute ere. Gaur egun torlojutze sekuentziak ASME-k araututa baldin baditu ere, ez dira batere erosoak, torloju guztiak ordena zehatz batean estutu behar direlako, eta estutze hauek ez direlako egoerarik erosoenetan egiten, denbora asko galduz. Horregatik, sinpletasuna lortzeko eta denbora eta dirua aurrezteko, torlojutze sekuentzia horiek optimizatzea izango da helburu nagusia. Aipaturiko lana aurrera eramateko, bi brida modeloren torlojutze analisiak egin beharko dira, berauek parametro batzuen menpe jarrita, estutze sekuentzietan duten eragina ikusi ahal izateko.

[EU]Polimero biobateragarri eta biodegradagarrien erabilera medikuntza arloan aurrerapauso handia suposatu du. Inplanteen arloan (torlojuak, iltzeak, plakak, eta abar…) eta ehun ingeniaritzan garrantzia handia dute. Hala ere, polimero hauen inplanteek desabantaila nabaria aurkezten dute ohiko material metalikoekin alderatuz: erradiopazitate eza. Ikerketa lan honetan karga erradiopako baten adizioak polimero biobateragarri eta biodegradagarri baten giza gorputzaren baldintzetan (pH=7,2 eta 37ºC) burututako in vitro degradazioan duen eragina aztertu da, %70 poli(L-laktida) (PLLA) eta %30 bario sulfato (BaSO4) sistemaren degradazioa, hain zuzen ere. Aztertutako karga erradiopakoak PLLAren degradazioan eragin handirik ez duela ondorioztatu da, beraz, sistema bideragarria dela.

Os orifícios de acesso aos parafusos de retenção devem ser preenchidos para que o parafuso não seja danificado caso seja necessária a remoção da prótese. Dentre os materiais mais utilizados estão o algodão, a fita de politetrafluoretileno e a guta percha. O objetivo deste estudo é avaliar a formação de biofilme de Candida albicans nos materiais anteriormente descritos, buscando estabelecer um parâmetro que contribua para a escolha do tipo de material mais adequado a ser utilizado clinicamente. Foram utilizados UCLAs, análogos e parafusos sextavados, todos de titânio. Os conjuntos foram montados com torque de 32N. Os materiais foram condensados no interior dos UCLAs e colocados em meio de cultura com uma suspensão de 3x106 células/ml de Candida albicans. O sistema foi armazenado à 37C com agitação, por 15 dias e o meio foi renovado a cada 48 horas. A quantificação de biofilme foi realizada pelo ensaio de MTT e leitura à 490nm, resultando em diferentes valores de densidade óptica. A normalidade (p=0,304 - Kolmogorov-Smirnov) e a igualdade de variâncias (p=0,721 - Scheffe) foram testadas primeiramente. O teste de análise de variância demonstrou diferença significativa entre os grupos (p<0,001) e com o Holm-Sidak foi observada diferença significativa entre os grupos algodão e guta (p<0,05) e algodão e fita de politetrafluoetileno (p<0,05); não houve diferença significativa entre os grupos guta e fita de politatrafluoretileno (p>0,05), apesar dos valores da fita de politetrafluoetileno terem sido maiores. Considerando-se as limitações deste estudo in vitro, podemos concluir que tanto a guta-percha quanto a fita de politetrafluoretileno apresentaram menor formação de biofilme, não havendo diferença estatisticamente significativa entre os materiais. O algodão apresentou um nível de formação de biofilme significativamente maior que a fita de politetrafluoretileno e a guta percha. Diante disso, serão necessários novos estudos para confirmar as limitações que este tipo de material pode apresentar quando usado como material de preenchimento do acesso do parafuso da prótese sobre implante.

Os elásticos ortodônticos intermaxilares sintéticos vem sendo cada vez mais utilizados, sendo principalmente indicados para pacientes que apresentam hiper-sensibilidade ao látex. Afim de avaliar e comparar o comportamento de elásticos de látex e sintéticos quanto a perda de força ao longo do tempo, este estudo foi realizado tanto in vitro quanto in vivo. Para o estudo in vitro foram avaliados 15 elásticos de cada material, para cada tempo: 0, 1, 3, 12 e 24 horas. No estudo in vivo, pacientes foram avaliados (N=15), utilizando elásticos de ambos os materiais (látex e sintético), nos mesmos tempos do estudo in vitro. Os elásticos foram transferidos para a máquina de ensaios mecânicos (EMIC DL-500 MF). Os valores da força gerada foram registrados após a distensão dos elásticos a uma distância de 25mm. Foi aplicado o teste t pareado para a amostra clínica e independente para a amostra laboratorial. Foi utilizada a análise de variância (ANOVA) para verificar a variação das forças geradas entre os tempos determinados e o teste post-hoc para identificar entre quais tempos houve diferença significativa. Quanto às forças iniciais geradas (zero hora), os valores para os elásticos sintéticos foram bastante semelhantes entre os estudos laboratorial e clínico e ligeiramente superiores aos dos elásticos de látex. Nos tempos subsequentes, as forças geradas pelos elásticos de látex apresentaram valores superiores. Em relação à degradação do material, ao final de 24 horas, maior percentual foi observado para os elásticos sintéticos, tanto in vitro quanto in vivo. A maior queda nos valores das forças liberadas pelos elásticos de ambos os materiais e nos estudos clínico e laboratorial, ocorreu entre os tempos de 0 e 1 hora, seguida de uma queda gradativa e progressiva até o tempo de 24 horas. Os elásticos de látex apresentaram um comportamento mais estável no período estudado em relação aos sintéticos, em ambos os estudos.

A new electrochemical cell assembly with the combination of UV and amperometric detector (AD) based on their complementarity was described. A Nafion tubing junction was used to decouple the high voltage from the separation capillary in the rear of on-column UV detector. In this mode, the electroactive and inert compounds could be detected by UV and AD at the same time. Aromatic amines were determined with the UV and the end-column AD detection to evaluate the performances of the cell assembly. Such an improved electrochemical detector could match the capillary with different diameters. By simple adjustment of the screws, the positioning of the working electrode and the detection capillary was easily gained without microscope. 区别于现有的电动、气动螺丝刀工具,该螺丝刀系统在驱动螺丝时,能对螺钉头实现无轴向压力的调节。刀头在旋转的同时随螺钉轴向进给,转速与进给量匹配,保证刀头与螺钉不脱节。该系统调整精度高,刀头转动精度可达0.72°,其应用于断路器类低压电气产品的自动调试,对确保产品质量、提高生产效率具有积极意义。
