O vírus de Epstein Barr (EBV) é o agente causador da mononucleose infecciosa e está associado com várias desordens proliferativas malignas tais como: linfoma de Burkitt, linfoma de Hodgkin e linfomas não Hodgkin. Um total de 118 casos de linfomas diagnosticados no Hospital Ofir Loyola no período de 1996 e 2005 foram analisados no Instituto Evandro Chagas, Ananindeua, Brasil; com o objetivo de detectar o genoma do EBV mediante a identificação dos genes EBER 1 e EBNA1 em casos de doença de Hodgkin. Os espécimes parafinizados foram analisados por hibridização in situ (gene EBER 1) e PCR em tempo real (EBNA 1). Do total, 61% (72/118) dos pacientes eram do sexo masculino e 39% (46/118) do sexo feminino com faixa etária variando entre 3- 98 anos. Sessenta e cinco (55%) foram diagnosticados como doença de Hodgkin e cinqüenta e três (45%) como linfomas não-Hodgkin. O EBV foi identificado nas células Reed Sternberg e variantes em 76,9% (50/65) dos casos de linfoma de Hodgkin com idade média de 28,3 anos (variação, 2-84 anos). Os subtipos histológicos de casos EBV-positivos foram o seguinte: esclerose nodular em 50% (25/50), celularidade mista em 28% (14/50), depleção linfocitária em 14% (7/50) e predominância linfocitária em 8% (4/50). O DNA do EBV foi detectado em 53% (26/49) com um coeficiente de regressão para a curva padrão de 0,99. Este estudo foi a primeira descrição do vírus de Epstein Barr em casos de doença de Hodgkin na região Norte do Brasil; reforçando a hipótese de que o EBV seja um co-fator no processo de transformação neoplásica em conjunto com a predisposição genética e imunidade do paciente, justificando a condução de estudos posteriores a nível molecular.

INTRODUÇÃO: Nos últimos anos, um número crescente de estudos sugere a participação dos herpesvírus na doença periodontal. OBJETIVO: Este trabalho investiga a relação entre a presença do herpesvírus Epstein-Barr (EBV) e a infecção periodontal em pacientes com periodontite crônica. METODOLOGIA: Foram coletadas amostras de biofilme subgengival de sítios com profundidades de sondagem de 4 a 6 mm e > 7 mm, de 28 pacientes com periodontite crônica. Como controles, foram incluídos 16 indivíduos, sistemicamente saudáveis e sem doença periodontal. Adicionalmente, parâmetros clínicos de profundidade de sondagem (PS), nível clínico de inserção (NCI) e índice de sangramento à sondagem (SS) foram registrados. RESULTADO: Os resultados demonstraram médias de 2,7 mm PS, 1,7 mm NCI e 0,3% dos sítios apresentaram SS. A investigação do EBV no biofilme subgengival dos grupos foi realizada por meio da reação em cadeia da polimerase com primer espécie-específico. Os resultados da análise viral indicaram ausência de EBV em todas as amostras subgengivais analisadas. CONCLUSÃO: A partir destes resultados, não foi encontrada relação entre a presença do herpesvírus Epstein-Barr e a periodontite crônica.

O câncer bucal e de laringe representam um problema crescente de saúde pública no Brasil. O tabagismo e o álcool são considerados os principais causadores do câncer bucal e de laringe, porém uma parte da população desenvolve a doença sem estar exposta a estes fatores de risco, sugerindo a existência de outras causas como: predisposição genética, alteração de genes supressores tumorais, dieta e agentes virais, em particular o Papilomavírus humano (HPV) e o vírus Epstein-Barr (EBV). Este estudo teve como proposição verificar a prevalência do HPV e do EBV na mucosa oral normal e no câncer bucal e de laringe, e quais os tipos mais prevalentes nestas duas situações. Para este estudo foram estabelecidos dois grupos: um composto por 70 espécimes emblocados em parafina, com diagnóstico confirmado de câncer bucal e laringe e outro com 166 indivíduos sem presença de lesões na cavidade bucal. A análise laboratorial para detecção viral do HPV e a detecção e tipagem do EBV (EBV 1 ou tipo 2) foram realizadas através da técnica de PCR (Reação em Cadeia de Polimerase) convencional. Já as tipagens das amostras positivas para o HPV (tipos 6, 11, 16, 18, 33, 35, 38, 52 e 58) foram realizadas por PCR em tempo real, utilizando sondas específicas para cada tipo. A prevalência do HPV e EBV encontrada nas neoplasias orais e de laringe foi de 78,6% para HPV e de 84,3% para EBV e de 24,1% e 45,8% para HPV e EBV, respectivamente, em indivíduos sem lesões orais. Os tipos mais prevalentes de HPV foram HPV 58 (50,9%), HPV 6 (9,1%) e HPV 16 (9,1%) nas neoplasias e HPV 18 (12,5%), HPV 6 (7,5%) e HPV 58 (2,5%) no grupo com ausência de lesões. O EBV 2 foi mais prevalente tanto nas lesões neoplásicas quanto nos indivíduos sem lesões, com frequência de 94,9% e 82,9%, respectivamente. Não houve associação da infecção por HPV e EBV com o sexo, sendo a prevalência semelhante para homens e mulheres. Foi observada associação entre as prevalências de HPV e EBV e suas co-infecções com o grupo que desenvolveu câncer. A prevalência de infecção por HPV e EBV e a razão de chances na ocorrência do câncer foi de 8,86 (p<0,0001) nos indivíduos infectados pelo HPV e de 4,08 (p=0,0004) nos infectados pelo EBV. O valor probabilístico estimado para prevalência de HPV e EBV e co-infecção e a ocorrência de câncer, demonstrou que o indivíduo infectado pelos dois vírus tem 65,72% de probabilidade de desenvolver câncer, enquanto o infectado pelo HPV tem 31,94% e o infectado pelo EBV 17,79%. Os resultados encontrados neste estudo permitem sugerir que os agentes virais (HPV e EBV) são fatores de risco importantes para o desenvolvimento da carcinogênese, sendo o HPV mais efetivo que o EBV no desencadeamento da doença.

Introduction: the squamous cell carcinoma (SCC) is the head and neck cancer of higher occurrence, representing about 90% of all these tumors. The SCC has several risk factors as smoking, alcohol and some oncogenic viruses, including the Epstein Barr virus (EBV). The EBV is a member of Herpesviridae family and has tropism for B lymphocytes and also for epithelial cells. Aim: the aim of this study was accomplish a review of the literature about the presence of the EBV in oral carcinomas. Conclusion: EBV is closely related to nasopharyngeal carcinoma, a SCC of high incidence in Southeast Asia, however its role in others oral SCC has not been proved.

Epstein-Barr virus (EBV), the causative agent of infectious mononucleosis, is a human herpesvirus associated with epithelial cell malignancies (nasopharyngeal carcinoma) as well as B-cell malignancies. Understanding how viral latency is disrupted is a central issue in herpesvirus biology. Epithelial cells are the major site of lytic EBV replication within the human host, and viral reactivation occurs in EBV-associated nasopharyngeal carcinomas. It is known that expression of a single viral immediate-early protein, BZLF1, is sufficient to initiate the switch from latent to lytic infection in B cells. Cellular regulation of BZLF1 transcription is therefore thought to play a key role in regulating the stringency of viral latency. Here we show that, unexpectedly, expression of another viral immediate-early protein, BRLF1, can disrupt viral latency in an epithelial cell-specific fashion. Therefore, the mechanisms leading to disruption of EBV latency appear to be cell-type specific.

Antibodies to type II collagen, and to Epstein Barr virus nuclear antigen-1 (EBNA-1) have been associated with rheumatoid arthritis (RA). In studies involving probing of phage-displayed random peptide libraries with an antibody to type II collagen, CII-C1, we observed that among 17 phagotopes selected 5 expressed peptides with homology with the sequence of EBNA-1. The residues in common were RLPFG. Hence we tested sera from 50 patients with RA, of whom 26 had antibodies to native type II collagen, and 43 healthy controls, for reactivity by ELISA with a phagotope selected 4 times, which expressed the peptide RRLPFGSQM. Eight RA sera (16%) but no normal sera reacted with the phagotope (p = 0.025). This reactivity could not be correlated with reactivity of RA sera with EBNA-1 by semi-quantitative western blot, with which reactivity occurred in 78% of RA patients and 81% of controls. Evidence for molecular mimicry was not found insofar as the phagotope did not inhibit reactivity of RA sera with EBNA-1 and CII-C1 was not reactive with EBNA-1. We conclude that the reactivity of the RA sera with the phagotope is most likely due to the phagotope being a mimic of an epitope of type II collagen for a proportion of RA sera.

Le virus Epstein-Barr est un des virus dotés de propriétés oncogéniques. Ceci est inquiétant car le virus est présent sous forme d’infection latente dans 95% de la population adulte au niveau mondial. Bien que ce virus soit associé surtout aux lymphomes, d’autres types de cancer sont aussi connus par leur association à cette infection tels que le carcinome gastrique. En fait, 10% de tous les cas de carcinome gastrique sont associés à la présence du virus Epstein-Barr. Plusieurs protéines du virus ont été étudiées individuellement afin d’établir leurs propriétés oncogéniques. Parmi celles-ci, la protéine virale EBNA1 joue un rôle important au niveau de la carcinogénèse et son expression est détectée au niveau des tissus gastriques cancéreux associés à l’infection par le virus Epstein-Barr. Des études réalisées au cours de ces dernières années montrent la relation entre un patron aberrant de l’épissage alternatif des ARN messagers et différents types de cancer, comme le cancer du sein et de la prostate. Les travaux de recherche présentés dans ce mémoire visent à établir si le virus Epstein-Barr est capable de changer le patron d’épissage alternatif au niveau des tissus cancéreux de l’estomac. L’utilisation de données de séquençage à haut débit fait sur des tissus cancéreux et tissus sains d’estomac (infectés ou non par le virus Epstein-Barr) permettra d’estimer les changements au niveau du patron d’épissage alternatif en relation à l’état des tissus et de la présence du virus Epstein-Barr. Les résultats obtenus nous montrent que l’épissage alternatif de plus de 500 gènes est altéré lorsque le virus est présent. Parmi ces gènes plusieurs codent pour des facteurs d’épissage, des facteurs de transcription, et des suppresseurs de tumeurs qui pourraient être impliqués dans le processus de développement du cancer. Finalement, nos résultats montrent que le patron d’épissage alternatif d’une cellule est modifié lorsque celle-ci est infectée par le virus Epstein-Barr ou qu’elle exprime une de ses protéines virales EBNA1, et ces altérations touchent plusieurs gènes impliqués dans des processus biologiques et qui semblent favoriser le développement du cancer.

The absence of cellular immunity is central to the pathogenesis of herpesvirus-mediated diseases after allogeneic hemopoietic stem cell transplantation (HSCT). For both bone marrow (BM)– and granulocyte-colony stimulating factor–mobilized peripheral blood stem cells (PBSCs) HSCT, donor-derived Epstein-Barr virus (EBV) and cytomegalovirus (CMV) peptide–specific CD8+ T cells clones undergo early expansion and persist long-term, with additional diversification arising from novel antigen-specific clones from donor-derived progenitors. Whether BM or PBSC is the superior source of antiviral CD8+ T cells is unclear. Given that PBSC has largely replaced BM as a source of stem cells for HSCT, it is unlikely that herpesvirus effector T-cell reconstitution will ever be compared prospectively. PBSC grafts contain 10 to 30 times more T cells than BM and a randomized study found proven viral infections were more frequent in BM than PBSC recipients, suggesting viral-specific T-cell immunity is enhanced in PBSC. Recently Moss showed in lung cancer patients that herpesvirus-specific BM-derived CD8+ T cells have unique homing properties relative to herpesvirus-specific CD8+ T cells present in unmobilized peripheral blood (PB). Immunodominant EBV-lytic peptide–specific CD8+ T cells were enriched in BM but were reduced for CMV peptide–specific CD8+ T cells relative to PB. EBV-latent peptide–specific CD8+ T cells were equivalent, which has relevance in the context of posttransplantation lymphoproliferative disorder for which impaired EBV-latent CD8+ T-cell immunity is a risk-factor. A comparison of herpesvirus-specific cellular immunity in PBSC versus PB has yet to be performed.

Background: Infection with EBV and a lack in vitamin D may be important environmental triggers of MS. 1,25-(OH)2D3 mediates a shift of antigen presenting cells (APC) and CD4+ T cells to a less inflammatory profile. Although CD8+ T cells do express the vitamin D receptor, a direct effect of 1,25(OH)2D3 on these cells has not been demonstrated until now. Since CD8+ T cells are important immune mediators of the inflammatory response in MS, we examined whether vitamin D directly affects the CD8+ T cell response, and more specifically if it modulates the EBV-specific CD8+ T cell response. Material and Methods: To explore whether the vitamin D status may influence the pattern of the EBV-specific CD8+ T cell response, PBMC of 10 patients with early MS and 10 healthy controls (HC) were stimulated with a pool of immunodominant 8-10 mer peptide epitopes known to elicit CD8+ T cell responses. PBMC were stimulated with this EBV CD8 peptide pool, medium (negative control) or anti- CD3/anti-CD28 beads (positive control). The following assays were performed: ELISPOT to assess the secretion of IFN-gamma by T cells in general; cytometric beads array (CBA) and ELISA to determine whichcytokines were released by EBV-specific CD8+ T cells after six days of culture; and intracellular cytokine staining assay to determine by which subtype of T cells secreted given cytokines. To examine whether vitamin D could directly modulate CD8+ T cell immune responses, we depleted CD4+ T cells using negative selection. Results: We found that pre-treatment of vitamin D had an antiinflammatory action on both EBV-specific CD8+ T cells and on CD3/ CD28-stimulated T cells: secretion of pro-inflammatory cytokines (IFNgamma and TNF-alpha) was decreased, whereas secretion of antiinflammatory cytokines (IL-5 and TGF-beta) was increased. At baseline, CD8+ T cells of early MS patients showed a higher secretion of TNFalpha and lower secretion of IL-5. Addition of vitamin D did not restore the same levels of both cytokines as compared to HC. Vitamin D-pretreated CD8+T cells exhibited a decreased secretion of IFN-gamma and TNF-alpha, even after depletion of CD4+ T cells from culture. Conclusion: Vitamin D has a direct anti-inflammatory effect on CD8+ T cells independently from CD4+ T cells. CD8+ T cells of patients with earlyMS are less responsive to the inflammatory effect of vitamin D than HC, pointing toward an intrinsic dysregulation of CD8+ T cells. The modulation of EBV-specific CD8+T cells by vitaminDsuggests that there may be interplay between these twomajor environmental factors of MS. Indivíduos imunocomprometidos possuem maior risco de desenvolver linfomas associados ao EBV. A detecção desse vírus em sangue periférico e a determinação de sua carga viral podem ter importância na evolução clínica de indivíduos portadores do HIV. Foram avaliadas 156 amostras de pacientes HIVpositivos pela reação em cadeia da polimerase em tempo real (qPCR) para detecção e quantificação da carga viral do EBV. 123/156 (78,8%) casos apresentaram carga viral detectável para o EBV, sendo que a carga viral média foi de 6,9x10-3 cópias de EBV/célula. Foi detectada elevada carga viral do EBV em indivíduos com falha terapêutica ou sem HAART (p =0,0076), em coinfectados pelos EBVs 1 e 2 (p=0,0205), em pacientes com altas cargas de HIV (rho=0,27614, p=0,0005) e longos períodos de infecção pelo HIV (rho= 0,24164, p =0,0026) e os que apresentavam altos níveis de linfócitos T CD8 + (rho=0,19286, p =0,0159). A amplificação do gene EBNA-2 para realização da tipagem viral foi possível em 95/123 (77,2%) amostras, das quais 72 (75,8%) revelaram infecção pelo EBV-1, 9 (9,5%) pelo EBV-2 e 14 (14,7%) apresentavam coinfecção entre os EBVs 1 e 2. Esses dados estão de acordo com a literatura visto que o tipo 1 é predominante em países ocidentais e 70,0% da coorte era composta por indivíduos caucasianos e heterossexuais. A maioria dos pacientes que apresentaram coinfecção pelos EBVs 1 e 2 tiveram contagem de linfócitos T CD4 + entre 200 e 499 células/μL de sangue segundo classificação CDC (p =0,0272). Quanto a analise do gene BNLF-1, a amplificação foi possível em 99/123 (80,5%). Desses 50/99 (50,5%) apresentavam a deleção de 30pb no gene, enquanto 49/99 (49,5%) não a possuíam. Acquired thrombotic thrombocytopenic purpura (TTP) is the consequence of a severe ADAMTS13 deficiency resulting from autoantibodies inhibiting ADAMTS13 or accelerating its clearance. Despite the success of plasma exchange the risk of relapse is high. From 2 patients (A and B), splenectomized for recurrent episodes of acquired TTP, the splenic B-cell response against ADAMTS13 was characterized through generation of human monoclonal anti-ADAMTS13 autoantibodies (mAbs) by cloning an immunoglobulin G (IgG)4κ- and IgG4λ-Fab library using phage display technology and by Epstein-Barr virus transformation of switched memory B cells (CD19+/CD27+/IgG+). Sequence analysis of the anti-ADAMTS13 IgGs of both patients revealed that the VH gene use was limited in our patients to VH1-3 (55%), VH1-69 (17%), VH3-30 (7%), and VH4-28 (21%) and contained 8 unique and thus far not reported heavy-chain complementarity determining region 3 motifs, of which 4 were shared by the 2 patients. The discovery of several highly similar anti-ADAMTS13 autoantibodies in 2 unrelated TTP patients suggests that the autoimmune response is antigen driven, because the probability that such similar immunoglobulin rearrangements happen by chance is very low (< 10(-9)).

In Hodgkin lymphoma (HL), the malignant Hodgkin Reed-Sternberg (HRS) cells constitute only 0.5% of 10% of the diseased tissue. The surrounding cellular infiltrate is enriched with T cells that are hypothesized to modulate antitumor immunity. We show that a marker of regulatory T cells, LAG-3, is strongly expressed on infiltrating lymphocytes present in proximity to HRS cells. Circulating regulatory T cells (CD4(+) CD25(hi) CD45 ROhi, CD4(+) CTLA4(hi), and CD4(+) LAG-3(hi)) were elevated in HL patients with active disease when compared with remission. Longitudinal profiling of EBV-specific CD8(+) T-cell responses in 94 HL patients revealed a selective loss of interferon-gamma expression by CD8(+) T cells specific for latent membrane proteins 1 and 2 (LMP1/2), irrespective of EBV tissue status. Intratumoral LAG-3 expression was associated with EBV tissue positivity, whereas FOXP3 was linked with neither LAG-3 nor EBV tissue status. The level of LAG-3 and FOXP3 expression on the tumor-infiltrating lymphocytes was coincident with impairment of LMP1/2-specific T-cell function. In vitro pre-exposure of peripheral blood mono-nuclear cells to HRS cell line supernatant significantly increased the expansion of regulatory T cells and suppressed LMP-specific T-cell responses. Deletion of CD4(+) LAG-3(+) T cells enhanced LMP-specific reactivity. These findings indicate a pivotal role for regulatory T cells and LAG-3 in the suppression of EBV-specific cell-mediated immunity in HL.
