O presente estudo, de que participaram 77.893 doadores de sangue que compareceram por primeira vez ao Hemocentro do Acre, de janeiro de 1997 a dezembro de 2008, teve por objetivos: 1) identificar indivíduos com sorologia positiva para doença de Chagas; 2) caracterizar, clinicamente, os indivíduos com sorologia positiva para doença de Chagas e 3) orientar adequadamente indivíduos com sorologia positiva quanto à terapêutica preconizada. A amostra se constituiu de 91,6% de pacientes do sexo masculino, com uma média de idade em torno de 47 anos, todos residentes do Estado do Acre. A triagem sorológica foi realizada com a aplicação do teste ELISA, com 102 resultados positivos; destes, doze foram incluídos no estudo e submetidos a testes confirmatórios, dos quais onze tiveram o resultado positivo confirmado. Segundo a avaliação de exames complementares realizados (ECG, ecocardiograma e endoscopia digestiva alta), um doador apresentava a forma cardíaca instalada e os demais a forma indeterminada da doença. É preciso oferecer o exame confirmatório da doença de Chagas na rotina dos bancos de sangue como forma de garantir o encaminhamento oportuno a uma assistência médica qualificada àquele doador de sangue que se tornou paciente chagásico.

BACKGROUND: The relevance of left ventricular (LV) geometric pattern after myocardial infarction is not known. OBJECTIVES: To analyze the presence of different LV geometric patterns and teir impact as a predictor of remodeling in patients with myocardial infarction. METHODS: Patients with anterior acute myocardial infarction (n = 80) were divided according to the geometric pattern: normal (normal left ventricular mass index [LVMI] and normal relative wall thickness [RWT]), concentric remodeling (normal LVMI and increased RWT), concentric hypertrophy (increased LVMI and RWT) and eccentric hypertrophy (increased LVMI and normal RWT). After six months, echocardiographic assessment was repeated. RESULTS: Four patients died. Of the survivors, 41 showed remodeling (R +), whereas 39 did not (R-). Considering the geometric pattern, the cases were distributed as follows: 24 patients with normal pattern, 13 with concentric remodeling, 29 with concentric hypertrophy and 14 with eccentric hypertrophy. Patients who showed remodeling had larger infarction sizes analyzed by peak CPK (R + = 4,610 (1,688-7,970), R- = 1,442 (775-4247), p <0.001) and CK-MB (R + = 441 (246 - 666), R- = 183 (101-465), p <0.001), trend towards higher prevalence of concentric remodeling (R+ = 10, R- = 3, p = 0.08) and lower prevalence of eccentric hypertrophy (R + = 2 R- = 12, p = 0.006). In the multivariate regression analysis, infarction size was a predictor (OR = 1.01, p = 0.020) and eccentric hypertrophy was a protective factor (OR = 0.189, p = 0.046) of ventricular remodeling after coronary occlusion. CONCLUSION: The LV geometric pattern of can have an impact on the remodeling process in patients with myocardial infarction.

Infective endocarditis is a process in which an infection attacks the heart endothelial surface, and is commonly caused by bacterial colonization, which is called bacterial endocarditis. It is a condition rarely found in dogs and cats, and is more prevalent in male dogs of large size. It mainly affects the left side of the heart, affecting the mitral and aortic valves with greater frequency. The circulation of the bacterium in the bloodstream is what gives rise endocarditis, and is caused by any non-aseptic process that serves as a gateway for bacterium in the body, as from a skin lesion, even as an invasive procedure, such as, catheterization and surgery. The ante-mortem diagnosis is difficult because the clinical signs of endocarditis are varied and common to other diseases, summing up the signs of infection (fever, lethargy, weight loss), and presence of heart murmur and may show signs of congestive heart failure. Thus, the diagnosis is most often through autopsy. To arrive at a diagnosis should be used, besides the history and physical examination, some laboratory tests, especially blood cultures and echocardiography. Treatment is accomplished through the use of antibiotics for long period of time, it is very important to follow the results of susceptibility after its outcome is revealed. The prognosis for bacterial endocarditis ranges from guarded to poor, and can be assessed mainly by the echocardiography. There are few studies in veterinary about the bacterial endocarditis, and the majority is case reports
