Genes play an important role in the development of diabetes mellitus. Putative susceptibility genes could be the key to the development of diabetes. Type 1 diabetes mellitus is one of the most common chronic diseases of childhood. A combination of genetic and environmental factors is most likely the cause of Type 1 diabetes. The pathogenetic sequence leading to the selective autoimmune destruction of islet beta-cells and development of Type 1 diabetes involves genetic factors, environmental factors, immune regulation and chemical mediators. Unlike Type 1 diabetes mellitus, Type 2 diabetes is often considered a polygenic disorder with multiple genes located on different chromosomes being associated with this condition. This is further complicated by numerous environmental factors which also contribute to the clinical manifestation of the disorder in genetically predisposed persons. Only a minority of cases of type 2 diabetes are caused by single gene defects such as maturity onset diabetes of the young (MODY), syndrome of insulin resistance (insulin receptor defect) and maternally inherited diabetes and deafness (mitochondrial gene defect). Although Type 2 diabetes mellitus appears in almost epidemic proportions our knowledge of the mechanism of this disease is limited. More information about insulin secretion and action and the genetic variability of the various factors involved will contribute to better understanding and classification of this group of diseases. This article discusses the results of various genetic studies on diabetes with special reference to Indian population.

The Gulf is experiencing a pandemic of lifestyle-induced obesity and type 2 diabetes mellitus (T2DM), with rates exceeding 50 and 30%, respectively. It is likely that T2DM represents the tip of a very large metabolic syndrome iceberg, which precedes T2DM by many years and is associated with abnormal/ectopic fat distribution, pathological systemic oxidative stress and inflammation. However, the definitions are still evolving with the role of different fat depots being critical. Hormetic stimuli, which include exercise, calorie restriction, temperature extremes, dehydration and even some dietary components (such as plant polyphenols), may well modulate fat deposition. All induce physiological levels of oxidative stress, which results in mitochondrial biogenesis and increased anti-oxidant capacity, improving metabolic flexibility and the ability to deal with lipids. We propose that the Gulf Metabolic Syndrome results from an unusually rapid loss of hormetic stimuli within an epigenetically important time frame of 2-3 generations. Epigenetics indicates that thriftiness can be programmed by the environment and passed down through several generations. Thus this loss of hormesis can result in continuation of metabolic inflexibility, with mothers exposing the foetus to a milieu that perpetuates a stressed epigenotype. As the metabolic syndrome increases oxidative stress and reduces life expectancy, a better descriptor may therefore be the Lifestyle-Induced Metabolic Inflexibility and accelerated AGEing syndrome – LIMIT-AGE. As life expectancy in the Gulf begins to fall, with perhaps a third of this life being unhealthy – including premature loss of sexual function, it is vital to detect evidence of this condition as early in life as possible. One effective way to do this is by detecting evidence of metabolic inflexibility by studying body fat content and distribution by magnetic resonance (MR). The Gulf Metabolic Syndrome thus represents an accelerated form of the metabolic syndrome induced by the unprecedented rapidity of lifestyle change in the region, the stress of which is being passed from generation to generation and may be accumulative. The fundamental cause is probably due to a rapid increase in countrywide wealth. This has benefited most socioeconomic groups, resulting in the development of an obesogenic environment as the result of the rapid adoption of Western labour saving and stress relieving devices (e.g. cars and air conditioning), as well as the associated high calorie diet.

Syfte: Syftet med denna studie var att undersöka vilka metoder som kan användas av sjuksköterskor för att uppnå följsamhet gällande genomförande av livsstilsförändringar hospatienter med diabetes typ 2 samt effekten av dessa. Metod: Uppsatsen har genomförts somen systematisk litteraturstudie. Artiklar har sökts i databaserna ELIN@Dalarna, Cinahl samtPubMed. Endast vetenskapliga artiklar som har publicerats mellan 2003 och 2008, tillgängligai fulltext, har inkluderats. Sexton kvantitativa artiklar ligger till grund för resultatdelen.Resultat: Användandet av telefonsamtal som hjälpmedel för sjuksköterskan att nå ut tilldiabetespatienterna har visat sig kunna öka diabetespatienters följsamhet vad gäller dieten.Automatiserade telefonsamtal har påvisats kunna hjälpa till att öka följsamheten gällandefysisk aktivitet. Även SMS som kommunikationsmedel mellan sjuksköterskan och patientenhar visat sig kunna öka följsamheten gällande fysisk aktivitet. Genom användandet utavvideokonferensutrustning för informering kring bland annat kost, motion och diabetessymtomhar en påtagbar livsstilsförändring kring diet kunnat påvisas.

Syfte: Syftet var att undersöka vilka kunskaper vårdpersonal upplever sig ha om äldre personer med diabetes typ 2. Resultatet av undersökningen ska ligga till grund för utvecklandet av ett instrument för kunskapskontroll inför delegering.Metod: Denna studie genomfördes med kvalitativ ansats med hjälp av fokusgruppsintervjuer.Den kvalitativa intervjumetoden användes för att ta reda på vilka kunskaper om diabetes som vårdpersonal i kommunal vård upplever sig ha. Studien innefattade intervjuer vid tre tillfällen. Sammanlagt deltog 22 personer i intervjuerna.Resultat: Studien visar att vårdpersonalen upplever sin kunskap om diabetes som bristfällig. Detta gäller framför allt symtom, behandling samt vilka åtgärder som ska vidtas vid förändring i hälsotillståndet hos den äldre personen med diabetes typ 2. Den största osäkerheten gäller kunskapen om blodsockervärden, vad som är normalvärde, höga respektive låga värden. De upplever sig också ha dålig kunskap när det gäller individuella målvärden. Detta tyder på att det är svårt för vårdpersonalen att arbeta enligt de nationella riktlinjerna som fokuserar på symtomfrihet och livskvalitet. Studien visar däremot att vårdpersonalen upplever sig ha goda kunskaper när det gäller omvårdnadsåtgärder som leder till ett gott förebyggande arbete, framför allt när det gäller fotkomplikationer. Sammantaget visar studien att vårdpersonalen behöver regelbunden strukturerad undervisning om diabetes för att de ska kunna ge en god och säker omvårdnad.

Aims: To explore the relationship between family average income (FAI; an index of socio-economic status) and Type 2 diabetes in a region of mainland China. Methods: Population-based cross-sectional study, conducted between October 2000 and March 2001 in administrative villages (n = 45) randomly selected from three urban districts and two rural counties of NanJing municipality, mainland China, with a regional population of 5.6 million. Participants were all local residents aged ≥ 35 years old (n = 29 340); 67.7% from urban areas, 32.3% from rural areas, 49.8% male and 50.2% female. Results: The response rate of eligible participants was 90.1%. The overall prevalence of self-reported Type 2 diabetes was 1.9%. After adjustment for possible confounding variables (age, gender, area of residence, body mass index, educational level, smoking status, occupational and leisure-time physical activity), participants in the higher and middle FAI categories were more than twice as likely to have Type 2 diabetes as those in the lower FAI category. Conclusions: The prevalence of Type 2 diabetes is positively related to socio-economic status (indexed by FAI) in Chinese at the population level. After controlling for potential confounding factors, people in higher socio-economic status groups are more likely to have Type 2 diabetes. These associations are consistent with other effects of epidemiological transition and identify a need for preventive initiatives.

Aims To determine the prevalence and risk factors for neuropathy and peripheral vascular disease (PVD) in the Australian diabetic population and identify those at high risk of foot ulceration.

Methods The Australian Diabetes Obesity and Lifestyle study included 11 247 adults aged ≥ 25 years in 42 randomly selected areas of Australia. Neuropathy and PVD were assessed in participants identified as having diabetes (based on self report and oral glucose tolerance test), impaired fasting glucose, impaired glucose tolerance and in a random sample with normal glucose tolerance (total n = 2436).

Results The prevalence of peripheral neuropathy was 13.1% in those with known diabetes (KDM) and 7.1% in those with newly diagnosed (NDM). The prevalence of PVD was 13.9% in KDM and 6.9% in NDM. Of those with diabetes, 19.6% were at risk of foot ulceration. Independent risk factors for peripheral neuropathy were diabetes duration (odds ratio (95% CI) 1.73 (1.33–2.28) per 10 years), height (1.42 (1.08–1.88) per 10 cm), age (2.57 (1.94–3.40) per 10 years) and uric acid (1.59 (1.21–2.09) per 0.1 mmol/l). Risk factors for PVD were diabetes duration (1.64 (1.25–2.16) per 10 years), age (2.45 (1.86–3.22) per 10 years), smoking (2.07 (1.00–4.28)), uric acid (1.03 (1.00–1.06) per 0.1 mmol/l) and urinary albumin/creatinine ratio (1.11 (1.011.21) per 1 mg/mmol).

Conclusions The prevalence of neuropathy and PVD was lower in this population than has been reported in other populations. This may reflect differences in sampling methods between community and hospital-based populations. Nevertheless, a substantial proportion of the diabetic population had risk factors for foot ulceration.

Background: The effectiveness of lifestyle interventions in reducing diabetes incidence has been well established. Little is known, however, about factors influencing the reach of diabetes prevention programs. This study examines the predictors of enrolment in the Sydney Diabetes Prevention Program (SDPP), a community-based diabetes prevention program conducted in general practice, New South Wales, Australia from 2008–2011.

Methods:
SDPP was an effectiveness trial. Participating general practitioners (GPs) from three Divisions of General Practice invited individuals aged 50–65 years without known diabetes to complete the Australian Type 2 Diabetes Risk Assessment tool. Individuals at high risk of diabetes were invited to participate in a lifestyle modification program. A multivariate model using generalized estimating equations to control for clustering of enrolment outcomes by GPs was used to examine independent predictors of enrolment in the program. Predictors included age, gender, indigenous status, region of birth, socio-economic status, family history of diabetes, history of high glucose, use of anti-hypertensive medication, smoking status, fruit and vegetable intake, physical activity level and waist measurement.

Results:
Of the 1821 eligible people identified as high risk, one third chose not to enrol in the lifestyle program. In multivariant analysis, physically inactive individuals (OR: 1.48, P = 0.004) and those with a family history of diabetes (OR: 1.67, P = 0.000) and history of high blood glucose levels (OR: 1.48, P = 0.001) were significantly more likely to enrol in the program. However, high risk individuals who smoked (OR: 0.52, P = 0.000), were born in a country with high diabetes risk (OR: 0.52, P = 0.000), were taking blood pressure lowering medications (OR: 0.80, P = 0.040) and consumed little fruit and vegetables (OR: 0.76, P = 0.047) were significantly less likely to take up the program.

Conclusions: Targeted strategies are likely to be needed to engage groups such as smokers and high risk ethnic groups. Further research is required to better understand factors influencing enrolment in diabetes prevention programs in the primary health care setting, both at the GP and individual level.

People with diabetes have a higher risk of emotional distress (anxiety, depression) than non-diabetic or healthy controls. Therefore, identification of factors that can decrease emotional distress is relevant. The aim of the present study was to examine (1) the association between facets of mindfulness and emotional distress; and (2) whether mindfulness might moderate the association between potential adverse conditions (stressful life events and comorbidity) and emotional distress. Analyses were conducted using cross-sectional data (Management and Impact for Long-term Empowerment and Success-Netherlands): 666 participants with diabetes (type 1 or type 2) completed measures of mindfulness (Five Facet Mindfulness Questionnaire-Short Form; FFMQ-SF), depressive symptoms (Patient Health Questionnaire; PHQ-9), and anxiety symptoms (General Anxiety Disorder assessment; GAD-7). Hierarchical multiple regression analyses showed significant associations between mindfulness facets (acting with awareness, non-judging, and non-reacting) and symptoms of anxiety and depression (β = -0.20 to -0.33, all p < 0.001). These mindfulness facets appeared to have a moderating effect on the association between stressful life events and depression and anxiety (all p < 0.01). However, the association between co-morbidity and emotional distress was largely not moderated by mindfulness. In conclusion, mindfulness is negatively related to both depression and anxiety symptoms in people with diabetes and shows promise as a potentially protective characteristic against the influence of stressful events on emotional well-being. © 2014 Springer Science+Business Media New York.

Jones and colleagues recently made a plea for the prioritization of psychological well-being in diabetes care [1]. Such calls to action began over 20 years ago with the 'St Vincent Declaration' guidelines [2]. A 'plea' two decades later emphasizes that this paradigm shift is a long, slow burn. In the 21st century, people with diabetes are now adding their powerful voices via social media, advocating for better psychological support, as active consumers (and constructive critics) of health care [3]. We can learn considerably from organizations such as Cancer Voices (www.cancervoicesaustralia.org) - they demonstrate how consumer representation and involvement in research agendas, policy and service provision play an integral part in shaping holistic health care. Durante o exercício, indivíduos com diabetes tipo 1 podem necessitar um aporte maior de carboidratos (CHO), do que os contidos nas bebidas esportivas, para manter os níveis de glicose sanguínea. OBJETIVO: Verificar a resposta glicêmica em adolescentes diabéticos tipo 1, durante 60 minutos e após 60 minutos do término de exercício submáximo (55-65% do VO2max) em ciclo ergômetro em 2 situações: (1) com a utilização de bebida carboidratada a 8% (CHO 8%) e (2) com a utilização de bebida carboidratada a 10% (CHO 10%). MÉTODOS: Dezesseis adolescentes (10 meninos e 6 meninas – 16,25 ± 2,65 anos), com diabetes tipo 1 controlada (HbA1c< 7,31%) e sem complicações da doença, pedalaram a 55-65% do VO2max por 60 min em dois dias separados. Os sujeitos ingeriram tanto a bebida com CHO 8% como a CHO 10% (2,62 g e 3,28 g de frutose; e 5,38 g e 6,72 g de glicose em 100 ml, respectivamente) em cada uma das duas sessões de exercício. As duas bebidas eram similares na cor e no sabor. O volume ingerido das bebidas foi de 5 ml·kg-1 15 min antes do exercício, e 2 ml·kg-1 a cada 15 min de exercício, oferecidos de forma randomizada e duplo-cega. RESULTADOS: Após 60 min de bicicleta, houve uma redução não significativa de 20,06 mg·dL-1 (p>0,05) e a manutenção (-0,533 mg·dL- 1)(p>0,05) da glicemia capilar com a ingestão das bebida CHO 8% e CHO 10%, respectivamente. Durante o exercício, a diferença entre os deltas das bebidas também não foi significativa (p=0,056). No período de recuperação, não foram encontradas diferenças significativas na glicemia entre as sessões. Também não foram encontradas diferenças significativas entre as sessões na freqüência cardíaca, taxa de percepção ao esforço, peso pré e pós-exercício e nos sintomas gastrointestinais. CONCLUSÃO: A ingestão de bebida contendo 8% e 10% de CHO preveniu uma redução significativa na glicemia induzida por uma hora de exercício contínuo em adolescentes com diabetes tipo 1.
