BACKGROUND: Previously we identified a DNA damage response-deficient (DDRD) molecular subtype within breast cancer. A 44-gene assay identifying this subtype was validated as predicting benefit from DNA-damaging chemotherapy. This subtype was defined by interferon signaling. In this study, we address the mechanism of this immune response and its possible clinical significance.

METHODS: We used immunohistochemistry (IHC) to characterize immune infiltration in 184 breast cancer samples, of which 65 were within the DDRD subtype. Isogenic cell lines, which represent DDRD-positive and -negative, were used to study the effects of chemokine release on peripheral blood mononuclear cell (PBMC) migration and the mechanism of immune signaling activation. Finally, we studied the association between the DDRD subtype and expression of the immune-checkpoint protein PD-L1 as detected by IHC. All statistical tests were two-sided.

RESULTS: We found that DDRD breast tumors were associated with CD4+ and CD8+ lymphocytic infiltration (Fisher's exact test P < .001) and that DDRD cells expressed the chemokines CXCL10 and CCL5 3.5- to 11.9-fold more than DNA damage response-proficient cells (P < .01). Conditioned medium from DDRD cells statistically significantly attracted PBMCs when compared with medium from DNA damage response-proficient cells (P < .05), and this was dependent on CXCL10 and CCL5. DDRD cells demonstrated increased cytosolic DNA and constitutive activation of the viral response cGAS/STING/TBK1/IRF3 pathway. Importantly, this pathway was activated in a cell cycle-specific manner. Finally, we demonstrated that S-phase DNA damage activated expression of PD-L1 in a STING-dependent manner.

CONCLUSIONS: We propose a novel mechanism of immune infiltration in DDRD tumors, independent of neoantigen production. Activation of this pathway and associated PD-L1 expression may explain the paradoxical lack of T-cell-mediated cytotoxicity observed in DDRD tumors. We provide a rationale for exploration of DDRD in the stratification of patients for immune checkpoint-based therapies.

Introduction: Breast Phyllodes tumors are rare breast tumors present in less than 1% of new cases of breast cancer, usually occurring among middle-aged women (40-50 yrs). Objective: This study shows diagnostic experience, surgical management and follows up of patients with this disease during a period of ten years in a oncology referral center. Methods: Retrospectively, breast cancer registries at the institution were reviewed, identifying 77 patients with Phyllodes tumors between 2002 and 2012, who had been operated on at the Instituto de Cancerología – Clínica Las Américas, in Medellín (Colombia). Clinical and histopathological data belonging to these cases was captured and analyzed and descriptive statistics were used. Results: The follow up median was 22.5 months (IQR: 10.5-60.0), average age was 47.2 yrs (SD: 12.4), mean tumor size was 3.6 cm (SD: 4.6), 88.3% of the patients (68 cases) presented negative margins and none of them received adjuvant chemotherapy. Of the patients with Phyllodes tumors; 33.8% had benign, 31.2% had borderline and 35.0% had malignant tumor. Disease-free survival was 85.8% and overall survival was 94.5%. Discussion: Reported data in this article is in accordance with what has been reported in worldwide literature. In our cohort even the high mean size of the tumors, the risk of local relapse and metastatic disease is low than previously reported in literature. Trials with longer follow up and molecular trials in Phyllodes tumors are necessary to understand the behavior of these tumors in Hispanics population.

The ADAM23 gene is frequently silenced in different types of tumors, and, in breast tumors, silencing is correlated with tumor progression, suggesting that it might be associated with the acquisition of a metastatic phenotype. ADAM23 exerts its function mainly through the disintegrin domain, because its metalloprotease domain is inactive. Analysis of ADAM23 binding to integrins has revealed a specific interaction with alpha(v)beta(3) integrin mediated by the disintegrin domain. Altered expression of alpha(v)beta(3) integrin has been observed in different types of tumors, and expression of this integrin in the activated form has been shown to promote metastasis formation. Here, we investigated the possibility that interaction between ADAM23 and alpha(v)beta(3) integrin might negatively modulate alpha(v)beta(3) activation during metastatic progression. ADAM23 expression was knocked down using short hairpin RNA in the MDA-MB-435 cell line, which has been extensively used as a model for alpha(v)beta(3) integrin activation. Ablation of ADAM23 enhanced alpha(v)beta(3) integrin activation by at least 2- to 4-fold and ADAM23 knockdown cells showed enhanced migration and adhesion to classic alpha(v)beta(3) integrin ligands. Ablation of ADAM23 expression also enhanced pulmonary tumor cell arrest in immunodeficient mice. To complement our findings with clinical evidence, we showed that silencing of ADAM23 gene by DNA promoter hypermethylation in a collection of 94 primary breast tumors was significantly associated with lower distant metastases-free and disease-specific survivals and was an independent prognostic factor for poor disease outcome. Our results strongly support a functional role of ADAM23 during metastatic progression by negatively modulating alpha(v)beta(3) integrin activation. [Cancer Res 2009;69(13):5546-52]

The taxane docetaxel is currently the most effective chemotherapeutic drug for the treatment of advanced breast cancer. However, a considerable proportion of breast cancer patients do not respond positively to docetaxel. The mechanisms of docetaxel resistance are poorly understood. Overexpression of ERBB2 occurs in 15-30% of breast tumors and is associated with chemoresistance to a variety of anticancer drugs. In the present study, we sought to identify genes involved in ERBB2-mediated chemoresistance to docetaxel. We generated SAGE libraries from two human mammary cell lines expressing basal (HB4a) and high (C5.2) levels of ERBB2 before and after intensive exposure to docetaxel and identified potential ERBB2 target genes implicated in a variety of cellular processes including cell proliferation, cell adhesion, apoptosis and cytoskeleton organization. Comparison of the transcriptome of the cell lines before and after docetaxel exposure revealed substantially different expression patterns. Twenty-one differentially expressed genes between HB4a and C5.2 cell lines, before and after docetaxel treatment, were further analyzed by qPCR. The alterations in the expression patterns in HB4a and C5.2 cell lines in response to docetaxel treatment observed by SAGE analysis were confirmed by qPCR for the majority of the genes analyzed. Our study provides a comprehensive view of the expression changes induced in two human mammary cells expressing different levels of ERBB2 in response to docetaxel that could contribute to the elucidation of the mechanisms involved in ERBB2-mediated chemoresistance in breast cancer.

Aims Claudins are integral transmembrane proteins of the tight junctions, critical for maintaining cell adhesion and polarity. Alterations in the expression of individual claudins have been detected in carcinomas and appear to correlate with tumour progression. Methods In this study, a panel of anti-claudin antibodies (anti-claudins 1, 2, 3, 4, 5 and 7) was employed to map claudin expression in 136 cases of oral squamous cell carcinoma (OSCC) organised in a tissue microarray. Results Claudins were expressed in a reticular pattern up to the prickle layer in normal mucosal epithelium. In OSCC, claudins were strongly present in well-differentiated tumours, they presented mild and low expression in moderately differentiated OSCC, and were negative in poorly differentiated OSCC; the absences of claudin 1 (p = 0.002) and claudin 4 (p<0.001) were associated with moderately/poorly differentiated tumours. Strong expression of claudin 4 was associated with decreased perineural infiltration (p = 0.024). Claudins 5 and 7 were mostly negative or weakly expressed in all cases studied. Expression of claudin 7 was associated with the early clinical stages of the disease, whereas loss of claudin 7 tended to be more frequent in advanced stages of OSCC (p = 0.054). Absence of claudin 7 was also associated with absent vascular infiltration (p = 0.045) and with presence of recurrence (p = 0.052). Conclusions Claudin expression patterns showed a strong correlation with histological type of OSCC; claudin expression was decreased in areas of invasion, and negative in poorly differentiated tumours. This pattern may be related to evolution and prognosis of these tumours, especially in the case of claudin 7, which seems to be associated with a poor prognosis in OSCC.

Introdução – O cancro de mama é a principal causa de morte por cancro na população feminina portuguesa. Assim, pretende-se descrever sucintamente o percurso de uma paciente com tumor de mama, evidenciando a interligação entre algumas áreas das tecnologias da saúde. Metodologia – Selecionou-se um caso clínico de uma paciente com cancro de mama e recorreu-se à pesquisa bibliográfica de forma a apoiar ou refutar os resultados obtidos com o referido caso clínico. Resultados e discussão de resultados – Em 1996 foi diagnosticado à paciente um carcinoma ductal da mama esquerda e posteriormente, em 2011, foi diagnosticada uma metástase do carcinoma mamário com padrão mucinoso. Neste caso, bem como em todos os tumores de mama, foi realizada uma abordagem multidisciplinar envolvendo diferentes áreas das tecnologias da saúde. Para deteção e diagnóstico do cancro de mama são utilizadas a mamografia e a ultrassonografia. Para casos de re-estadiamento e monitorização da terapêutica devem ser utilizadas a tomografia por emissão de positrões (TEP) e a ressonância magnética mamária. No que se refere à terapêutica, neste tipo de tumores recorre-se à cirurgia, à radioterapia e à quimioterapia. ABSTRACT - Introduction – The breast cancer is the main cause of death by cancer in the Portuguese female population. We intend to describe briefly the pathway of a patient with breast cancer, highlighting the connection between some of the health technology areas. Methodology – It was selected a clinical case of a patient with breast cancer and it was used a literature research to support or refute the results obtained with the mentioned clinical case. Results and discussion – In 1996, it was diagnosed to the patient a ductal carcinoma of the left breast and in 2011 it was diagnosed a breast metastases. In this case, as well as other breast tumours, it was done a multidisciplinary approach involving different areas of health technologies. For detection and diagnose of breast cancer it’s used the mammography and ultrasound. For cases of re-staging and treatment monitoring it should be used positron emission tomography and magnetic resonance imaging of the breast. Regarding the therapy approach, in this type of tumours, it is used surgery, radiotherapy and chemotherapy.

Gene expression signatures are used in the clinic as prognostic tools to determine the risk of individual patients with localized breast tumors developing distant metastasis. We lack a clear understanding, however, of whether these correlative biomarkers link to a common biological network that regulates metastasis. We find that the c-MYC oncoprotein coordinately regulates the expression of 13 different "poor-outcome" cancer signatures. In addition, functional inactivation of MYC in human breast cancer cells specifically inhibits distant metastasis in vivo and invasive behavior in vitro of these cells. These results suggest that MYC oncogene activity (as marked by "poor-prognosis" signature expression) may be necessary for the translocation of poor-outcome human breast tumors to distant sites.

Metastasis depends on the ability of tumor cells to establish a relationship with the newly seeded tissue that is conducive to their survival and proliferation. However, the factors that render tissues permissive for metastatic tumor growth have yet to be fully elucidated. Breast tumors arising during pregnancy display early metastatic proclivity, raising the possibility that pregnancy may constitute a physiological condition of permissiveness for tumor dissemination. Here we have shown that during murine gestation, metastasis is enhanced regardless of tumor type, and that decreased NK cell activity is responsible for the observed increase in experimental metastasis. Gene expression changes in pregnant mouse lung and liver were shown to be similar to those detected in premetastatic sites and indicative of myeloid cell infiltration. Indeed, myeloid-derived suppressor cells (MDSCs) accumulated in pregnant mice and exerted an inhibitory effect on NK cell activity, providing a candidate mechanism for the enhanced metastatic tumor growth observed in gestant mice. Although the functions of MDSCs are not yet understood in the context of pregnancy, our observations suggest that they may represent a shared mechanism of immune suppression occurring during gestation and tumor growth.

Objective: to discuss the participation of Plastic Surgery in the reconstruction of the chest wall, highlighting relevant aspects of interdisciplinaryness. Methods: we analyzed charts from 20 patients who underwent extensive resection of the thoracic integument, between 2000 and 2014, recording the indication of resection, the extent and depth of the raw areas, types of reconstructions performed and complications. Results: among the 20 patients, averaging 55 years old, five were males and 15 females. They resections were: one squamous cell carcinoma, two basal cell carcinomas, five chondrosarcomas and 12 breast tumors. The extent of the bloody areas ranged from 4x9 cm to 25x40 cm. In 12 patients the resection included the muscular plane. In the remaining eight, the tumor removal achieved a total wall thickness. For reconstruction we used: one muscular flap associated with skin grafting, nine flaps and ten regional fasciocutaneous flaps. Two patients undergoing reconstruction with fasciocutaneous flaps had partially suffering of the flap, solved with employment of a myocutaneous flap. The other patients displayed no complications with the techniques used, requiring only one surgery. Conclusion: the proper assessment of local tissues and flaps available for reconstruction, in addition to the successful integration of Plastic Surgery with the specialties involved in the treatment, enable extensive resections of the chest wall and reconstructions that provide patient recovery.

Au Canada, on estime qu'une femme sur neuf développera un cancer du sein et qu'une femme sur vingt-huit en mourra. Les carcinomes mammaires de phénotype basal qui comprennent environ 15 à 25% des cancers envahissants du sein sont des tumeurs malignes ayant un très mauvais pronostic. Dans ce type tumoral, les métastases sont fréquentes et les décès nombreux. La formation des métastases est un processus complexe qui comprend plusieurs étapes; chacune peut être étudiée par des marqueurs spécifiques. Notre hypothèse de recherche est que le carcinome mammaire de phénotype basal possède des caractéristiques spécifiques qui permettent d'expliquer son potentiel métastatique élevé. Six micromatrices tissulaires comprenant un total de 213 tumeurs mammaires ont été confectionnées. L'expression des marqueurs usuels de la métastase et celle de nouveaux marqueurs a été étudiée par des techniques d'immunohistochimie. L'étude comparative de l’expression des marqueurs du potentiel métastatique par les carcinomes mammaires de phénotype basal indique que les protéines Ki-67, EGFR, CD276 et galectine-7 sont étroitement reliées à ce type de cancer. De plus, l'expression du marqueur GATA-3, un marqueur anti-métastatique, fait complètement défaut. Nos résultats confirment que le cancer du sein de phénotype basal possède un plus grand potentiel métastatique que les autres sous-types génétiques et suggèrent que la galectine-7 puisse être impliquée.

L’acide rétinoïque (AR) est le ligand des récepteurs nucléaires RAR et RXR qui agissent comme facteurs de transcription ligand-inductibles et médient ses effets biologiques. Il est connu que l’AR a des propriétés prodifférenciatrices et antiprolifératives, notamment sur les cellules de l’épithélium mammaire. Une perte de sensibilité de l’AR a toutefois été mise en évidence dans plusieurs lignées cellulaires mammaires cancéreuses, ce qui pourrait faciliter la croissance des tumeurs. Or jusqu’ici cette perte de sensibilité avait été attribuée à des défauts de la voie de signalisation de l’AR, causée par la perte de l’expression des récepteurs à l’AR dans la tumeur, bien que plusieurs lignées de cellules cancéreuses y soient tout de même très sensibles. Peu d’études se sont intéressées au rôle de la voie de synthèse de l’AR dans la transformation des cellules mammaires. En effet, l’AR est synthétisé à partir de la vitamine A, ou rétinol, son précurseur sanguin provenant de la diète. Les cellules de l’épithélium mammaire normales ont la capacité de synthétiser l’AR à partir du rétinol. Nos rapportons pour la première fois que l’épithélium mammaire est probablement le siège de la synthèse et de la signalisation de l’AR. Cela est dû, au moins en partie, à l’expression d’une enzyme de synthèse de l’AR, RALDH3, dans l’épithélium mammaire normal. Dans cette étude, nous démontrons que les cellules cancéreuses de type luminal, qui ont sensibles à l’AR (et qui expriment le récepteur des estrogènes ER, catégorie qui regroupe 75 % des tumeurs diagnostiquées) n’ont au contraire pas la capacité de sythétiser l’AR, probablement en raison d’une faible expression de RALDH3 dans les tumeurs, sous l’effet des estrogènes. Cela pourrait représenter un nouveau mécanisme favorisant la croissance des tumeurs luminales dont les cellules proliférent en présence du rétinol sanguin. RALDH1, une autre enzyme de la voie de synthèse de l’AR, et qui partage 70 % d’identité de séquence avec RALDH3, est un marqueur de tumeurs plus agressives et de la formation de métastase. Nous montrons au contraire, que RALDH3 est un marqueur d’une moindre probabilité de développer des métastases chez les patientes atteintes d’une tumeur luminale. Cela suggère des rôles different pour ces deux enzymes dans la glande mammaire. Nos résultats indiquent que RALDH1 et 3 ont des propriétés enzymatiques très différentes, ce qui est en accord avec cette dernière hypothèse. Nos données suggèrent aussi que RALDH1 et 3 pourraient être des marqueurs de populations distinctes de cellules dans la glande mammaire normale. Nous proposons d’exploiter les diffèrences entre RALDH1 et 3 afin de mettre au point des méthodes de séparation des différentes population de cellules de l’épithélium mammaire ce qui pourrait aider à comprendre le rôle de la synthèse d’AR dans ce tissu.

Malgré les progrès des traitements des cancers du sein, ceux-ci demeurent la seconde cause de mortalité par cancer au Canada. Parmi les gènes associés aux cancers du sein, le récepteur des œstrogènes ERα est exprimé dans plus de 70% des tumeurs mammaires, qui prolifèrent en réponse aux œstrogènes, faisant de lui une cible de choix. ERα est un facteur de transcription ligand-dépendant, liant des éléments de réponse PuGGTCAnnnTGACCPy. Afin d’examiner la capacité des récepteurs nucléaires à reconnaitre de nouveaux motifs ADN, des mutants aux capacités de liaison modifiées ont été générés. Parmi les quatre résidus interagissant avec l’ADN, R211 ne peut pas être modifiée sans perdre complètement la liaison du récepteur à l’ADN. Néanmoins, les mutations combinées de plusieurs acides aminés contactant les bases de l’ERE ont généré des récepteurs capables de reconnaitre de nouveaux motifs, tout en conservant des niveaux de transactivation efficaces. L’utilisation potentielle des récepteurs nucléaires comme outils de thérapie génique hormono-dépendant, repose sur la prédiction des motifs de liaison efficaces. Étant donné son importance dans la carcinogenèse mammaire, ERα est une cible cruciale des thérapies anti-néoplastiques. L’anti-œstrogène total, ICI, induit la dégradation de ERα et l’arrêt de la croissance des cellules tumorales mammaires ERα-positives. De plus, la nouvelle drogue anti-tumorale HDACi, SAHA, module la voie de signalisation des œstrogènes et possède des propriétés prometteuses en association avec d’autres traitements anti-tumoraux. En effet, le co-traitement ICI et SAHA a un impact synergique sur l’inhibition de la prolifération des cellules mammaires tumorales ERα-positives. Cette synergie repose sur la coopération des effets de ICI et SAHA pour réduire les niveaux protéiques de ERα et bloquer la progression du cycle cellulaire via la modulation de la transcription des gènes cibles des œstrogènes. En fait, les fortes doses de HDACis masquent rapidement et complètement la signalisation transcriptionnelle des œstrogènes. De plus, les gènes cibles primaires des œstrogènes, contenant des EREs, présentent la même régulation transcriptionnelle en réponse aux fortes doses de SAHA ou du co-traitement, avec des doses utilisables en clinique de ICI et SAHA. En fait, ICI mime l’impact des fortes doses de SAHA, en dégradant ERα, potentialisant ainsi la répression de la transcription ERE-dépendante par SAHA. Finalement, la synergie des effets de ICI et SAHA pourrait augmenter l’efficacité des traitements des tumeurs mammaires.
