OBJETIVO: Identificar as principais características socioeconômicas, demográficas, antropométricas e comportamentais, bem como os resultados perinatais da gravidez na adolescência. Conhecer os tipos de serviços de saúde utilizados pelas gestantes adolescentes. MÉTODOS: A amostra de 2.429 partos de mulheres residentes em São Luís, MA, abrangeu 94% dos nascimentos hospitalares. As mulheres foram separadas em seis grupos de idade para melhor avaliação do comportamento das variáveis entre os dois grupos de adolescentes (abaixo de 18 anos; 18 a 19 anos) e entre as adolescentes e as demais mulheres. O teste do qui-quadrado foi utilizado para a comparação de proporções e a razão de prevalências foi empregada como medida de efeito. RESULTADOS: Das 2.429 mulheres, 714 eram adolescentes (29,4%). Seu coeficiente específico de fecundidade, 72,2 por mil, foi mais elevado que em outras regiões do País. As adolescentes apresentaram piores condições socioeconômicas e reprodutivas que as demais mulheres, maior proporção de pré-natal inadequado (39,2%) e muitas não tinham companheiro (34,5%). Por outro lado, tiveram menor proporção de parto cesáreo (23,0%) e de fumantes (3.5%). CONCLUSÕES: Apesar da situação socioeconômica igualmente desfavorável, as adolescentes de 18 a 19 anos apresentaram resultados perinatais semelhantes às mulheres de 25 a 29 anos. Já as menores de 18 anos tiveram maiores proporções de filhos com baixo peso ao nascer, prematuros e com maior risco de mortalidade infantil. Isto sugere que a imaturidade biológica possa estar associada a maiores taxas de prematuridade, baixo peso ao nascer e mortalidade infantil.

This paper presents a project consisting on the development of an Intelligent Tutoring System, for training and support concerning the development of electrical installation projects to be used by electrical engineers, technicians and students. One of the major goals of this project is to devise a teaching model based on Intelligent Tutoring techniques, considering not only academic knowledge but also other types of more empirical knowledge, able to achieve successfully the training of electrical installation design.

OBJECTIVE: To compare estimates of low birth weight (LBW), preterm birth, small for gestational age (SGA), and infant mortality in two birth cohorts in Brazil. METHODS: The two cohorts were performed during the 1990s, in São Luís, located in a less developed area in Northeastern Brazil, and Ribeirão Preto, situated in a more developed region in Southeastern Brazil. Data from one-third of all live births in Ribeirão Preto in 1994 were collected (2,839 single deliveries). In São Luís, systematic sampling of deliveries stratified by maternity hospital was performed from 1997 to 1998 (2,439 single deliveries). The chi-squared (for categories and trends) and Student t tests were used in the statistical analyses. RESULTS: The LBW rate was lower in São Luís, thus presenting an epidemiological paradox. The preterm birth rates were similar, although expected to be higher in Ribeirão Preto because of the direct relationship between preterm birth and LBW. Dissociation between LBW and infant mortality was observed, since São Luís showed a lower LBW rate and higher infant mortality, while the opposite occurred in Ribeirão Preto. CONCLUSIONS: Higher prevalence of maternal smoking and better access to and quality of perinatal care, thereby leading to earlier medical interventions (cesarean section and induced preterm births) that resulted in more low weight live births than stillbirths in Ribeirão Preto, may explain these paradoxes. The ecological dissociation observed between LBW and infant mortality indicates that the LBW rate should no longer be systematically considered as an indicator of social development.

This paper describes an architecture conceived to integrate Power Sys-tems tools in a Power System Control Centre, based on an Ambient Intelligent (AmI) paradigm. This architecture is an instantiation of the generic architecture proposed in [1] for developing systems that interact with AmI environments. This architecture has been proposed as a consequence of a methodology for the inclu-sion of Artificial Intelligence in AmI environments (ISyRAmI - Intelligent Sys-tems Research for Ambient Intelligence). The architecture presented in the paper will be able to integrate two applications in the control room of a power system transmission network. The first is SPARSE expert system, used to get diagnosis of incidents and to support power restoration. The second application is an Intelligent Tutoring System (ITS) incorporating two training tools. The first tutoring tool is used to train operators to get the diagnosis of incidents. The second one is another tutoring tool used to train operators to perform restoration procedures.

OBJECTIVE: To obtain population estimates and profile risk factors for infant mortality in two birth cohorts and compare them among cities of different regions in Brazil. METHODS: In Ribeirão Preto, southeast Brazil, infant mortality was determined in a third of hospital live births (2,846 singleton deliveries) in 1994. In São Luís, northeast Brazil, data were obtained using systematic sampling of births stratified by maternity unit (2,443 singleton deliveries) in 1997-1998. Mothers answered standardized questionnaires shortly after delivery and information on infant deaths was retrieved from hospitals, registries and the States Health Secretarys' Office. The relative risk (RR) was estimated by Poisson regression. RESULTS: In São Luís, the infant mortality rate was 26.6/1,000 live births, the neonatal mortality rate was 18.4/1,000 and the post-neonatal mortality rate was 8.2/1,000, all higher than those observed in Ribeirão Preto (16.9, 10.9 and 6.0 per 1,000, respectively). Adjusted analysis revealed that previous stillbirths (RR=3.67 vs 4.13) and maternal age <18 years (RR=2.62 vs 2.59) were risk factors for infant mortality in the two cities. Inadequate prenatal care (RR=2.00) and male sex (RR=1.79) were risk factors in São Luís only, and a dwelling with 5 or more residents was a protective factor (RR=0.53). In Ribeirão Preto, maternal smoking was associated with infant mortality (RR=2.64). CONCLUSIONS: In addition to socioeconomic inequalities, differences in access to and quality of medical care between cities had an impact on infant mortality rates.

The activity of Control Center operators is important to guarantee the effective performance of Power Systems. Operators’ actions are crucial to deal with incidents, especially severe faults, like blackouts. In this paper we present an Intelligent Tutoring approach for training Portuguese Control Centre operators in tasks like incident analysis and diagnosis, and service restoration of Power Systems. Intelligent Tutoring System (ITS) approach is used in the training of the operators, taking into account context awareness and the unobtrusive integration in the working environment.

This article describes a new approach in the Intelligent Training of Operators in Power Systems Control Centres, considering the new reality of Renewable Sources, Distributed Generation, and Electricity Markets, under the emerging paradigms of Cyber-Physical Systems and Ambient Intelligence. We propose Intelligent Tutoring Systems as the approach to deal with the intelligent training of operators in these new circumstances.

In this paper we present an Intelligent Tutoring approach for training Portuguese Control Centre operators in incident analysis and diagnosis, and service restoration of Power Systems, offering context awareness and an easy integration in the working environment.

OBJECTIVE: To assess HIV testing rate and determine risk factors for not have been tested during pregnancy. METHODS: A cross-sectional study was carried out in Porto Alegre, Southern Brazil, from December 2000 to February 2001. Socioeconomic, maternal and healthcare variables were obtained by means of a standardized questionnaire. Crude and adjusted odds ratios and their 95% confidence intervals were obtained in logistic regression models. RESULTS: A total of 1,642 mothers were interviewed. Of them, 94.3% reported being offered HIV testing before or during pregnancy or during labor; 89 mothers (5.4%) were not tested or did not know if they were tested. Attending fewer than six prenatal visits, being single and younger than 18 years old were relevant barriers preventing HIV testing. There was found a relationship between maternal schooling and the category of prenatal care provider. Having low 22.20 (12.43-39.67) or high 3.38 (1.86-7.68). schooling and being cared in the private sector strongly reduced the likelihood of being HIV tested. CONCLUSIONS: The Brazilian Health Ministry's recommendation for universal counseling and HIV testing has been successfully implemented in the public sector. In order to improve HIV testing coverage, new strategies need to target women cared in the private sector especially those of low schooling.

Considering ancient monuments and historical buildings, it seems that these mortars have proved to be durable and reliable materials. The restoration and maintenance of old renders is one of the key aspects of correct rehabilitation practice. The ideal course of action is to replace the damaged material by a material with compatible characteristics.The study in development presents the chemical, physical and morphologic analysis performed for ancient air lime mortars belonging to historical monuments: Santa Marta Fortress in the coast line Lisbon-Cascais dated from XVII century and Defense Wall of Lisbon dated from XI century, which layout could be associated to roman period. It is important to underline that the studied samples of ancient portuguese air lime mortars, have been submitted during centuries to very severe maritime environment that includes daily cycles of wet/dry, wind, friction and the constant presence of salts, generally aggressive. However, they show very good performance and conservation state, unlike most of the new air lime mortars, which are generally considered weak, not very durable, materials. This work is included in a study intending to determine key factors to the durability of these ancient materials in presence of water. Visible reaction rims around some aggregates suggests the occurrence of pozzolanic reactions between aggregates and the lime binder that creates neoformation products, such as calcium-silico-aluminates, which seems be, besides the pores filling, the responsible for the resistance and cohesion of these ancient mortars submitted to aggressive humid environments.

OBJECTIVE: Low birth weight children are unusual among well-off families. However, in Brazil, low birth weight rate was higher in a more developed city than in a less developed one. The study objective was to find out the reasons to explain this paradox. METHODS: A study was carried out in two municipalities, Ribeirão Preto (Southeastern Brazil) and São Luís (Northeastern Brazil), which low birth weight rates were 10.7% and 7.6% respectively. Data from two birth cohorts were analyzed: 2,839 newborns in Ribeirão Preto in 1994 and 2,439 births in São Luís in 1997-1998. Multiple logistic regression analysis was performed, adjusted for confounders. RESULTS: Low birth weight risk factors in São Luís were primiparity, maternal smoking and maternal age less than 18 years. In Ribeirão Preto, the associated variables were family income between one and three minimum wages, maternal age less than 18 and equal to or more than 35 years, maternal smoking and cesarean section. In a combined model including both cohorts, Ribeirão Preto presented a 45% higher risk of low birth weight than São Luís. When adjusted for maternal smoking habit, the excess risk for low birth weight in Ribeirão Preto compared to São Luís was reduced by 49%, but the confidence interval was marginally significant. Differences in cesarean section rates between both cities contributed to partially explain the paradox. CONCLUSIONS: Maternal smoking was the most important risk factor for explaining the difference in low birth weight between both cities. The other factors contributed little to explain the difference in low birth weight rates.

The aim of the present study was to test a hypothetical model to examine if dispositional optimism exerts a moderating or a mediating effect between personality traits and quality of life, in Portuguese patients with chronic diseases. A sample of 540 patients was recruited from central hospitals in various districts of Portugal. All patients completed self-reported questionnaires assessing socio-demographic and clinical variables, personality, dispositional optimism, and quality of life. Structural equation modeling (SEM) was used to analyze the moderating and mediating effects. Results suggest that dispositional optimism exerts a mediator rather than a moderator role between personality traits and quality of life, suggesting that “the expectation that good things will happen” contributes to a better general well-being and better mental functioning.

Estuda-se o papel das sondagens eleitorais no processo eleitoral, abrangendo os domínios da comunicação política e do comportamento eleitoral. Analisam-se aspetos de interface entre o processo da consulta de opinião, através das sondagens eleitorais, e o processo da consulta eleitoral, através do voto. Procede-se à revisão da literatura na área da especialidade e desenvolvem-se três estudos empíricos sobre a realização e divulgação de sondagens em Portugal entre 1991 e 2012: (1) como são divulgados os resultados das sondagens através das notícias (n=489) publicadas nos média; (2) como são realizadas as sondagens eleitorais (n=561) em termos metodológicos e com que qualidade preditiva; (3) de que forma a sua divulgação influencia o comportamento de voto dos eleitores. A revisão de literatura confirma o interesse da investigação: as sondagens eleitorais têm duas décadas de história em Portugal e é necessário caracterizar sistematicamente o corpo que constituem; são escassos os estudos sobre a metodologia das sondagens e o conteúdo das notícias que as divulgam, sendo ainda fundamental, estudar o papel que as sondagens representam face ao comportamento de voto. Principais resultados: (1) elevada frequência de notícias na imprensa portuguesa com resultados de sondagens apresentados ou interpretados incorretamente e generalizado incumprimento da lei no que respeita ao conteúdo obrigatório da ficha técnica, verificando-se, no entanto, evolução positiva; (2) desempenho muito positivo das empresas de sondagens portuguesas que alcançaram desempenho próximo das suas congéneres europeias e norte-americanas, quer em termos metodológicos, quer em termos da qualidade preditiva; (3) rejeição da existência de efeitos sistemáticos de mobilização dos eleitores (bandwagon ou underdog), porém, aceitação da possibilidade de desmobilização por certeza de vitória em eleições de segunda ordem. Os resultados são discutidos à luz da investigação nacional e internacional e são propostas algumas implicações para os diversos agentes envolvidos na tríade notícias, sondagens, eleições: jornalistas, empresas, cidadãos eleitores, classe política e órgão regulador. São também sugeridas algumas linhas de investigação futura.

Doenças crónicas (DC) são doenças que têm de ser geridas em vez de curadas. Giovannini, Bitti, Sarchielli e Speltini (1986) caracterizam as doenças crónicas como: a) doenças de longa duração, b) que tendem a prolongar-se por toda a vida do doente, c) que provocam invalidez em graus variáveis, d) são devidas a causas não reversíveis, e) que exigem formas particulares de reeducação, f) que obrigam o doente a seguir determinadas prescrições terapêuticas, g) que normalmente exigem a aprendizagem de um novo estilo de vida, h) que necessitam de controlo periódico, de observação e de tratamento regulares. As DC não se definem pela sua aparente ou real gravidade. As pessoas que têm DC podem fazer a vida do dia-a-dia como qualquer outro cidadão, e grande parte deles acaba por falecer de velhice ou de outras doenças, que não a DC que os acompanhou grande parte da vida. Para estas pessoas viverem uma boa vida o processo de ajustamento é decisivo. O ajustamento à DC torna-se então um objectivo fundamental para as pessoas e para a sociedade. O ajustamento pode definir-se como uma resposta a uma alteração do meio ambiente, que leva um organismo a adaptar-se a essa alteração. Esta definição, explicam as autoras, implica que ele ocorre ao longo do tempo e, também, que é um resultado desejável. Para Stanton, Revenson, e Tennen, (2007), o ajustamento engloba inúmeros componentes que cruzam domínios interpessoais, cognitivos, emocionais, físicos e comportamentais. Estes autores referem que da revisão da literatura se pode concluir que, grosso modo a DC requer, ajustamento em múltiplos domínios, que o ajustamento se desenrola ao longo do tempo, e que existe uma heterogeneidade acentuada no modo como os indivíduos se ajustam à DC. O ajustamento é um conceito do senso comum que tanto é verbo como substantivo.
