Introducción: Los factores de riesgo de la enfermedad cardiovascular (FRECV) pueden estar presentes desde la infancia y predicen la enfermedad cardiovascular del adulto. Objetivo: Evaluar la prevalencia de FRECV en niños de 3 a 17 años hijos de Enfermeras de la Fundación CardioInfantil - Instituto de cardiología (FCI). Métodos: Estudio de corte transversal analítico. Resultados: 118 niños, edad promedio 7,4 años, desviación estándar 3,86, la mayoría eutróficos 72,0%. Presentaron FRECV como malos hábitos alimenticios 89,0%, sedentarismo 78,8%, exposición a tabaco 19,5%, historia familiar de riesgo cardiovascular 16,1%, sobrepeso 15,3% y obesidad 12,7%. No se encontraron diferencias entre factores de riesgo entre niños y niñas.El sedentarismo en niños con sobrepeso u obesidad fue del 90,9% y en niños eutróficos del 36,5%. Los malos hábitos alimentarios en niños con sobrepeso u obesidad fueron 84,8% y en niños eutróficos 42,4%. Los adolescentes presentaron una mayor exposición a tabaco en comparación con los preescolares y escolares, al igual que una mayor proporción de malos hábitos alimenticios en comparación con ambos grupos. De la totalidad de la población de estudio, el 97,5% presentó al menos un FRECV, y el 42,4% 3 o más FRECV. La presencia de ≥3 FRECV fue mayor en obesos al compararlos con los niños en sobrepeso y eutróficos. Conclusiones: Los resultados del estudio indican que los niños de 3 a 17 años evaluados presentan una alta carga de FRECV, en especial en aquellos con sobrepeso y obesidad.

Introducción: Las enfermedades cardiovasculares han generado un alto impacto en morbimortalidad, la carga de enfermedad de la población y años de vida saludables, sufriendo mayor impacto en discapacidades, lo cual es imperativo el control de factores de riesgo cardiovascular. La principal causa de muerte en Colombia 2011 fue la enfermedad coronaria, según el instituto nacional de salud, lo cual implica la importancia de prevención y promoción. Objetivo: Estimar la prevalencia de factores de riesgo cardiovascular en trabajadores de una empresa hidroeléctrica de Colombia durante el año 2013, con mira a realizar intervención en estos. Metodología: Estudio descriptivo transversal, para determinar prevalencia de los factores de riesgo cardiovascular en una hidroeléctrica en Colombia. Muestra: 113 trabajadores a quienes se les aplicó un cuestionario integral adaptado incluyendo variables sociodemográficos, laborales, hábitos, información nutricional, antropométricas y de salud. Resultados: Entrevistados 113 trabajadores, edad promedio 39 años, entre 21 a 59 años de edad, 69% vinculación por contratista, 31% directo por la empresa, los cargos fueron dividos: producción 58,4%, administrativo 23%, oficios varios 18,6%. En antecedentes familiares, la hipertensión arterial e infarto agudo de miocardio fue 19,5%, antecedentes personales como estrés laboral 76.4%, consumo de alcohol 55.8%, sedentarismo 54,5%, el índice de masa corporal (IMC > 24.9) 54%, mientras los de menor prevalencia fue IAM y accidente cerebro vascular 0,9% seguidos de tabaquismo 6.3%. Conclusiones: Existe una prevalencia importante en ciertos factores modificables, los cuales encontramos principalmente estrés laboral, sedentarismo, consumo de alcohol, sobrepeso y obesidad, susceptibles a modificación mediante planes de promoción y prevención específicos.

RESUMEN El ausentismo laboral genera un gran impacto económico en las empresas y a la sociedad en general. Es un problema difícil de manejar ya que es multifactorial, porque a pesar de que en su gran mayoría es generado por enfermedad general, al analizarlo se puede encontrar otros factores que conlleven a la ausencia del trabajador y con ello producir alteración al normal funcionamiento de la empresa, por lo que resulta indispensable estudiar este tema. Objetivo Caracterizar las principales causas de ausentismo laboral en los médicos generales de una IPS que presta servicios de consulta externa de medicina general a nivel nacional durante el año 2014. Materiales y Métodos: es un estudio de corte transversal sobre datos secundarios correspondientes al registro de incapacidades que presento la IPS durante el año 2014. Los criterios de inclusión fueron los médicos generales con los que contaba la IPS que presta servicios de salud a nivel nacional durante el año 2014 y los criterios de exclusión fueron las licencias de maternidad y paternidad. El tamaño de la muestra final fue de 202 médicos y el número de incapacidades que se presentó durante el año 2014 fue 313. Se realizó análisis de distribución de frecuencias, porcentaje y prevalencia de las incapacidades. Resultados: durante el año 2014 se presentaron 313 incapacidades, en una población de 202 médicos generales con prevalencia en las mujeres. El diagnóstico más frecuente de las incapacidades fue la categoría diagnostica “otros” en el cual se encuentra migraña, vértigo, alteraciones de la mama con 59 incapacidades, seguida por enfermedades gastrointestinales con 25 incapacidades. Conclusiones y recomendaciones: Las incapacidades fueron más frecuentes en mujeres que en hombres. El diagnóstico de las incapacidades más frecuente fue “enfermedad genérica o ausencia de diagnóstico”. La incapacidad más frecuente de un día que se presentaron 46 registros. El médico que mayor número de incapacidades presento fue de 18 para el año 2014. Se recomienda a la empresa tener un seguimiento de las incapacidades repetitivas, ya que estas podrían tener relación con enfermedad laboral que aún no ha sido calificada. Se recomienda complementar la base de datos con información como el antecedente de enfermedad crónica y el sedentarismo, lo que puede permitir realizar nuevos estudios respecto al riesgo cardiovascular de esta población.

A importância da actividade física é muitas vezes subestimada. A compreensão da fisiologia do exercício e a sua integração em programas de exercício adequados às necessidades de cada indivíduo ajudam a prevenir algumas doenças e facilitam a reabilitação, enquanto promovem a saúde em geral. O exercício físico regular envolve múltiplos benefícios para todas as pessoas, desde que apropriado às suas características. A actividade física melhora o desempenho, o metabolismo e a constituição física, tanto no indivíduo são como na doença (cardiovascular, metabólica, cancro), e permite minimizar ou retardar a perda progressiva de funções relacionadas com o envelhecimento. A inactividade dever ser assim contrariada através da promoção da actividade física quer nos mais jovens quer nos adultos, para que se adoptem estilos de vida saudáveis que minimizem os factores de risco. O presente trabalho visa rever os fundamentos da fisiologia do exercício e chamar a atenção para os pontos fulcrais que devem motivar o combate ao sedentarismo e a procura de mais e melhor qualidade de vida através da actividade física.

Este trabalho teve como objetivo principal analisar os estilos de vida de alunos de Educação Física, e relaciona-los com a perceção de competência, orientação de objetivos para a mestria, performance aproach, performance avoidance, perceção de clima motivacional para a mestria, performance aproach, performance avoidance, atitude gosto e importância face a disciplina de Educação Física. Neste sentido foi aplicado um Questionário (Papaioannou et al. 2007, versão adaptada por Martins, 2010), em quatro escolas da rede de estágio pedagógico em Educação Física de 3º ciclo e Secundário, de modo a avaliar as perceções de 199 alunos com idades compreendidas entre os 14-20 anos. Os resultados revelaram diferenças significativas entre alunos ativos e sedentários ao nível da perceção de competência (p=0,000), orientação de objetivos para a mestria (p=0,000), orientação de objetivos performance avoidance (p=0,028), perceção de clima motivacional para a mestria (p=0,010), atitude gosto pela educação física e suas matérias (p=0,001), e a atitude e importância atribuída à EF (p=0,002). A análise das correlações demonstrou a existência de relação entre o estilo de vida sedentário relativamente à perceção de competência (p=0,004), e orientação de objetivos performance avoidance esta ultima no sentido inverso (p=0,010). Em relação aos alunos ativos apenas se verificou a existência de correlação para a orientação de objetivos performance aproach (p=0,048).

A obesidade infantil apresenta-se como uma epidemia que afeta milhares de crianças em todo o Mundo, e a publicidade tem sido apontada por inúmeros autores como sendo a principal causa por este problema global. Ao longo deste estudo, tivemos como objetivo analisar se existe algum efeito da publicidade na obesidade infantil, tendo em conta o estímulo ao consumo de grande variedade de alimentos com alta intensidade calórica, por via das estratégias de marketing. Para enquadrar este estudo, procuramos identificar o papel da publicidade na vida das crianças, nas atividades desenvolvidas nos tempos livres, a influência do estilo de vida nas atividades físicas, e por fim tentar perceber o papel que a publicidade, os tempos livres e o estilo de vida têm na obesidade infantil. A investigação apresenta-se como um estudo de caso exploratório qualitativo, onde foram utilizadas entrevistas estruturadas e não estruturadas individuais, bem como de grupo utilizando a técnica de Focus Grupo num infantário privado no concelho de Cascais. A amostra analisada contou com 12 crianças com idades compreendidas entre os 2 anos e os 5 anos, 10 encarregados de educação, entre eles 3 homens e 7 mulheres com idades compreendidas entre os 27 anos e os 45 anos. Foram incluídas ainda na amostra uma educadora de infância e a directora do infantário, o que permitiu estabelecer uma relação entre as diversas variáveis em estudo. Desta forma, foi possível chegar a resultados que nos indicaram em primeiro lugar, que as crianças nesta faixa etária estão expostas a um número muito elevado de mensagens publicitárias, e que a publicidade já faz parte integrante da sua vida. Percebemos ainda que alguns pais tentam controlar o número de horas de exposição à publicidade, na televisão, bem como os canais visualizados pelos filhos. No entanto não o conseguem, por eles estarem em contato com a publicidade através dos mais variados meios. Verificou-se deste modo que as crianças já conhecem muitas marcas, embalagens e as respectivas mascotes e que por isso solicitam a sua compra. A embalagem reforçou neste estudo a sua importância, uma vez que se apresenta em alguns casos como a grande atração no ponto de venda independentemente do tipo de produto. Relativamente aos tempos livres, existe um padrão de comportamentos onde as crianças praticam mais actividades no verão e aos fins-de-semana devido à disponibilidade dos pais, sendo que no restante período o sedentarismo é muito grande e que a exposição à televisão é muito acentuada. Os factores económicos, sociais e tecnológicos apresentam-se como os principais influenciadores relativamente ao estilo de vida. Como conclusão, consideramos que a obesidade nas crianças é multifacetada e que a publicidade não é o seu principal influenciador, é apenas mais um factor que poderá agravar a situação de muitas crianças quando não têm o devido acompanhamento familiar.

The endocannabinoid system (ECS) was only 'discovered' in the 1990s. Since then, many new ligands have been identified, as well as many new intracellular targets--ranging from the PPARs, to mitochondria, to lipid rafts. It was thought that blocking the CB-1 receptor might reverse obesity and the metabolic syndrome. This was based on the idea that the ECS was dysfunctional in these conditions. This has met with limited success. The reason may be that the ECS is a homeostatic system, which integrates energy seeking and storage behaviour with resistance to oxidative stress. It could be viewed as having thrifty actions. Thriftiness is an innate property of life, which is programmed to a set point by both environment and genetics, resulting in an epigenotype perfectly adapted to its environment. This thrifty set point can be modulated by hormetic stimuli, such as exercise, cold and plant micronutrients. We have proposed that the physiological and protective insulin resistance that underlies thriftiness encapsulates something called 'redox thriftiness', whereby insulin resistance is determined by the ability to resist oxidative stress. Modern man has removed most hormetic stimuli and replaced them with a calorific sedentary lifestyle, leading to increased risk of metabolic inflexibility. We suggest that there is a tipping point where lipotoxicity in adipose and hepatic cells induces mild inflammation, which switches thrifty insulin resistance to inflammation-driven insulin resistance. To understand this, we propose that the metabolic syndrome could be seen from the viewpoint of the ECS, the mitochondrion and the FOXO group of transcription factors. FOXO has many thrifty actions, including increasing insulin resistance and appetite, suppressing oxidative stress and shifting the organism towards using fatty acids. In concert with factors such as PGC-1, they also modify mitochondrial function and biogenesis. Hence, the ECS and FOXO may interact at many points; one of which may be via intracellular redox signalling. As cannabinoids have been shown to modulate reactive oxygen species production, it is possible that they can upregulate anti-oxidant defences. This suggests they may have an 'endohormetic' signalling function. The tipping point into the metabolic syndrome may be the result of a chronic lack of hormetic stimuli (in particular, physical activity), and thus, stimulus for PGC-1, with a resultant reduction in mitochondrial function and a reduced lipid capacitance. This, in the context of a positive calorie environment, will result in increased visceral adipose tissue volume, abnormal ectopic fat content and systemic inflammation. This would worsen the inflammatory-driven pathological insulin resistance and inability to deal with lipids. The resultant oxidative stress may therefore drive a compensatory anti-oxidative response epitomised by the ECS and FOXO. Thus, although blocking the ECS (e.g. via rimonabant) may induce temporary weight loss, it may compromise long-term stress resistance. Clues about how to modulate the system more safely are emerging from observations that some polyphenols, such as resveratrol and possibly, some phytocannabinoids, can modulate mitochondrial function and might improve resistance to a modern lifestyle.

The Brazilian Osteoporosis Study (BRAZOS) is the first epidemiological study carried out in a representative sample of Brazilian men and women aged 40 years or older. The prevalence of fragility fractures is about 15.1% in the women and 12.8% in the men. Moreover, advanced age, sedentarism, family history of hip fracture, current smoking, recurrent falls, diabetes mellitus and poor quality of life are the main clinical risk factors associated with fragility fractures. The Brazilian Osteoporosis Study (BRAZOS) is the first epidemiological study carried out in a representative sample of Brazilian men and women aged 40 years or older with the purpose of identifying the prevalence and the main clinical risk factors (CRF) associated with osteoporotic fracture in our population. A total of 2,420 individuals (women, 70%) from 150 different cities in the five geographic regions in Brazil, and all different socio-economical classes were selected to participate in the present survey. Anthropometrical data as well as life habits, fracture history, food intake, physical activity, falls and quality of life were determined by individual quantitative interviews. The representative sampling was based on Brazilian National data provided by the 2000 and 2003 census. Low trauma fracture was defined as that resulting of a fall from standing height or less in individuals 50 years or older at specific skeletal sites: forearm, femur, ribs, vertebra and humerus. Sampling error was 2.2% with 95% confidence intervals. Logistic regression analysis models were designed having the fragility fracture as the dependent variable and all other parameters as the independent variable. Significance level was set as p < 0.05. The average of age, height and weight for men and women were 58.4 +/- 12.8 and 60.1 +/- 13.7 years, 1.67 +/- 0.08 and 1.56 +/- 0.07 m and 73.3 +/- 14.7 and 64.7 +/- 13.7 kg, respectively. About 15.1% of the women and 12.8% of the men reported fragility fractures. In the women, the main CRF associated with fractures were advanced age (OR = 1.6; 95% CI 1.06-2.4), family history of hip fracture (OR = 1.7; 95% CI 1.1-2.8), early menopause (OR = 1.7; 95% CI 1.02-2.9), sedentary lifestyle (OR = 1.6; 95% CI 1.02-2.7), poor quality of life (OR = 1.9; 95% CI 1.2-2.9), higher intake of phosphorus (OR = 1.9; 95% CI 1.2-2.9), diabetes mellitus (OR = 2.8; 95% CI 1.01-8.2), use of benzodiazepine drugs (OR = 2.0; 95% CI 1.1-3.6) and recurrent falls (OR = 2.4; 95% CI 1.2-5.0). In the men, the main CRF were poor quality of life (OR = 3.2; 95% CI 1.7-6.1), current smoking (OR = 3.5; 95% CI 1.28-9.77), diabetes mellitus (OR = 4.2; 95% CI 1.27-13.7) and sedentary lifestyle (OR = 6.3; 95% CI 1.1-36.1). Our findings suggest that CRF may contribute as an important tool to identify men and women with higher risk of osteoporotic fractures and that interventions aiming at specific risk factors (quit smoking, regular physical activity, prevention of falls) may help to manage patients to reduce their risk of fracture.

This article presents findings from a study that evaluated the utility of Protection Motivation Theory to explain cardiovascular health behaviors among people with schizophrenia (n = 83) and depression (n = 70). Results indicated that the prevalence of overweight, cigarette smoking and a sedentary lifestyle were greater among people with a mental illness compared to individuals without a mental illness. Major predictors were high levels of fear of cardiovascular disease, lack of knowledge of correct dietary principles, lower self-efficacy, limited social support and psychiatric symptoms. Implications of these results are discussed in designing education and preventive health programs for individuals with schizophrenia and Mental Depressive Disorder (MDD).

Can schools solve the problem? The author examines the effort to strengthen physical education in Australian schools and considers why the subject remains marginal in the curriculum.

This paper presents findings from two studies. Study 1 explored differences between people with psychiatric illness (PPI) (N=144) and the general population (N=151) in levels of low-fat diet, exercise and smoking. Study 2 investigated barriers and health care needs of PPI (N=60). The prevalence of overweight, cigarette smoking and sedentary lifestyle were significantly greater among PPI than the general population. Major predictors were limited social support, knowledge of correct dietary principles, lower self-efficacy, psychiatric symptomatology and various psychotropic drugs. The findings demonstrated that PPI over-used medical services but under-used preventive services due to inaccessibility, lower satisfaction and knowledge of services.

Recent research in Australia has found that people with a mental illness experience higher mortality rates from preventable illnesses, such as cardiovascular disease, respiratory disease and diabetes compared to the general population. Lifestyle and other behavioural factors contribute significantly to these illnesses. Lifestyle behaviours that affect these illnesses include lack of physical activity, consumption of a poor diet and cigarette smoking. Research on the influence of these factors has been mainly directed towards the mainstream population in Australia. Consequently, there remains limited understanding of health behaviours among individuals with psychiatric disabilities, their health needs, or factors influencing their participation in protective health behaviours. This thesis presents findings from two studies. Study 1 evaluated the utility of the main components of Roger’s (1983) Protection Motivation Theory (PMT) to explain health behaviours among people with a mental illness. A clinical population of individuals with schizophrenia (N=83), Major Depressive Disorder (MDD) (N=70) and individuals without a mental illness (N=147) participated in the study. Respondents provided information on intentions and self-reported behaviour of engaging in physical activity, following a low-fat diet, and stopping smoking. Study 2 investigated the health care service needs of people with psychiatric disabilities (N=20). Results indicated that the prevalence of overweight, cigarette smoking and a sedentary lifestyle were significantly greater among people with a mental illness compared to that reported for individuals without a mental illness. Major predictors of the lack of intentions to adopt health behaviours among individuals with schizophrenia and MDD were high levels of fear of cardiovascular disease, lack of knowledge of correct dietary principles, lower self-efficacy, a limited social support network and a high level of psychiatric symptoms. In addition, findings demonstrated that psychiatric patients are disproportionately higher users of medical services, but they are under-users of preventive medical care services. These differences are primarily due to a lack of focus on preventive health, feelings of disempowerment and lower satisfaction of patient-doctor relationships. Implications of these results are discussed in terms of designing education and preventive programs for individuals with schizophrenia and MDD.

Objective To investigate the short-term efficacy of a multicomponent intervention to reduce office workers' sitting time. Methods Allocation for this non-randomized controlled trial (n = 43 participants; 56% women; 26–62 years; Melbourne, Australia) was by office floor, with data collected during July–September 2011. The 4-week intervention emphasized three key messages: “Stand Up, Sit Less, Move More” and comprised organizational, environmental, and individual elements. Changes in minutes/day at the workplace spent sitting (primary outcome), in prolonged sitting (sitting time accumulated in bouts ≥ 30 min), standing, and moving were objectively measured (activPAL3). Results Relative to the controls, the intervention group significantly reduced workplace sitting time (mean change [95%CI]: − 125 [− 161, − 89] min/8-h workday), with changes primarily driven by a reduction in prolonged sitting time (− 73 [− 108, − 40] min/8-h workday). Workplace sitting was almost exclusively replaced by standing (+ 127 [+ 92, + 162] min/8-h workday) with non-significant changes to stepping time (− 2 [− 7, + 4] min/8-h workday) and number of steps (− 70 [− 350, 210]). Conclusions This multicomponent workplace intervention demonstrated that substantial reductions in sitting time are achievable in an office setting. Larger studies with longer timeframes are needed to assess sustainability of these changes, as well as their potential longer-term impacts on health and work-related outcomes.
