This study analyzes an accident in which two maintenance workers suffered severe burns while replacing a circuit breaker panel in a steel mill, following model of analysis and prevention of accidents (MAPA) developed with the objective of enlarging the perimeter of interventions and contributing to deconstruction of blame attribution practices. The study was based on materials produced by a health service team in an in-depth analysis of the accident. The analysis shows that decisions related to system modernization were taken without considering their implications in maintenance scheduling and creating conflicts of priorities and of interests between production and safety; and also reveals that the lack of a systemic perspective in safety management was its principal failure. To explain the accident as merely non-fulfillment of idealized formal safety rules feeds practices of blame attribution supported by alibi norms and inhibits possible prevention. In contrast, accident analyses undertaken in worker health surveillance services show potential to reveal origins of these events incubated in the history of the system ignored in practices guided by the traditional paradigm.

Os acidentes são fenômenos complexos, cuja determinação situa-se na organização do trabalho, dimensão invisível aos agentes de vigilância. O objetivo deste artigo é analisar e comparar o alcance das intervenções realizadas em uma empresa frigorífica, em 1997, baseada na checagem de normas de saúde e segurança, e em 2008, quando se incorpora a ergonomia da atividade. Foi realizado estudo de caso com análise documental referente à intervenção de 1997 e análise ergonômica do trabalho adotada em 2008. Em 1997 as ações de vigilância incidiram principalmente sobre os fatores de risco visíveis. Mesmo cumprindo as exigências, a empresa continuava, em 2008, com proporção de incidência anual de 26% de acidentes, o que motivou a busca da nova abordagem. Verificou-se, em 2008, que a gênese dos acidentes era provocada por um circulo vicioso: trabalho intenso; inadequação de meios técnicos; absenteísmo e rotatividade (84% ao ano) com recrutamento de inexperientes. Esse quadro é agravado por práticas autoritárias. A ergonomia da atividade contribuiu para compreender as causas organizacionais ultrapassando os aspectos normativos da vigilância tradicional, o que indica sua importância para tornar mais efetivas as ações de vigilância para a prevenção.

OBJETIVO: Identificar os acidentes de trabalho com exposição à material biológico ocorridos em um hospital universitário, discutindo os resultados com o processo de implementação das medidas de segurança e saúde dos trabalhadores, exigidas pela Norma Regulamentadora NR-32. MÉTODOS: Estudo exploratório de abordagem quantitativa dos dados. Foram realizados levantamento dos acidentes de trabalho, as entrevistas com o coordenador do Serviço de Segurança e Medicina do Trabalho e a análise de dados documentais do Programa de Prevenção de Riscos Ambientais e do Programa de Controle Médico de Saúde Ocupacional. RESULTADOS: O percentual de acidentes de trabalho reduziu ao longo do período, no qual várias exigências dessa norma foram sendo adotadas. Acidentes com material perfurocortante foram os mais frequentes, não havendo em todos os setores do hospital o oferecimento dos dispositivos de segurança exigidos pela NR-32. CONCLUSÃO: Houve redução de acidentes de trabalho com material biológico no hospital estudado entre 2007 e 2009. Contudo, não é quantitativamente significativa, apesar da implantação de várias diretrizes da NR-32 ao longo dos anos. É necessária a colaboração entre gestores, serviços de segurança e trabalhadores na promoção da saúde no trabalho.

La tesi si occupa, con originalità e spunti di innovazione, del tema degli infortuni sul lavoro e tecnopatie, attraverso un'ottica del tutto nuova perchè attenta sia agli aspetti previdenziali (indennizzo Inail) e sia agli aspetti risarcitori (risarcimento dei danni differenziali e complementari). Lo studio, non scevro da analisi critiche, indaga in particolare sulla risarcibiità del danno c.d. differenziale e sperimenta le nuove poste risarcitorie applicabili, alla luce delle più recenti evoluzioni legislative e giurisprudenziali. L'Autore non trascura, infine, i complessi profili processuali a completamento delle tutele e le responsabilità sia del datore di lavoro (obbligato principale) e sia dei terzi.

La tesi affronta il concetto di esposizione al rischio occupazionale e il suo scopo è quello di indagare l’ambiente di lavoro e il comportamento dei lavoratori, con l'obiettivo di ridurre il tasso di incidenza degli infortuni sul lavoro ed eseguire la riduzione dei rischi. In primo luogo, è proposta una nuova metodologia denominata MIMOSA (Methodology for the Implementation and Monitoring of Occupational SAfety), che quantifica il livello di "salute e sicurezza" di una qualsiasi impresa. Al fine di raggiungere l’obiettivo si è reso necessario un approccio multidisciplinare in cui concetti d’ingegneria e di psicologia sono stati combinati per sviluppare una metodologia di previsione degli incidenti e di miglioramento della sicurezza sul lavoro. I risultati della sperimentazione di MIMOSA hanno spinto all'uso della Logica Fuzzy nel settore della sicurezza occupazionale per migliorare la metodologia stessa e per superare i problemi riscontrati nell’incertezza della raccolta dei dati. La letteratura mostra che i fattori umani, la percezione del rischio e il comportamento dei lavoratori in relazione al rischio percepito, hanno un ruolo molto importante nella comparsa degli incidenti. Questa considerazione ha portato ad un nuovo approccio e ad una seconda metodologia che consiste nella prevenzione di incidenti, non solo sulla base dell'analisi delle loro dinamiche passate. Infatti la metodologia considera la valutazione di un indice basato sui comportamenti proattivi dei lavoratori e sui danni potenziali degli eventi incidentali evitati. L'innovazione consiste nell'applicazione della Logica Fuzzy per tener conto dell’"indeterminatezza" del comportamento umano e del suo linguaggio naturale. In particolare l’applicazione è incentrata sulla proattività dei lavoratori e si prefigge di impedire l'evento "infortunio", grazie alla generazione di una sorta d’indicatore di anticipo. Questa procedura è stata testata su un’azienda petrolchimica italiana.

OBJECTIVE The repair of cartilaginous lesions within synovial joints is still an unresolved and weighty clinical problem. Although research activity in this area has been indefatigably sustained, no significant progress has been made during the past decade. The aim of this educational review is to heighten the awareness amongst students and scientists of the basic issues that must be tackled and resolved before we can hope to escape from the whirlpool of stagnation into which we have fallen: cartilage repair redivivus! DESIGN Articular-cartilage lesions may be induced traumatically (e.g., by sports injuries and occupational accidents) or pathologically during the course of a degenerative disease (e.g., osteoarthritis). This review addresses the biological basis of cartilage repair and surveys current trends in treatment strategies, focussing on those that are most widely adopted by orthopaedic surgeons [viz., abrasive chondroplasty, microfracturing/microdrilling, osteochondral grafting and autologous-chondrocyte implantation (ACI)]. Also described are current research activities in the field of cartilage-tissue engineering, which, as a therapeutic principle, holds more promise for success than any other experimental approach. RESULTS AND CONCLUSIONS Tissue engineering aims to reconstitute a tissue both structurally and functionally. This process can be conducted entirely in vitro, initially in vitro and then in vivo (in situ), or entirely in vivo. Three key constituents usually form the building blocks of such an approach: a matrix scaffold, cells, and signalling molecules. Of the proposed approaches, none have yet advanced beyond the phase of experimental development to the level of clinical induction. The hurdles that need to be surmounted for ultimate success are discussed.

La necesidad de mejorar la eficacia preventiva del sector de la construcción en España exige profundizar en el análisis del modelo regulatorio vigente a la vez que evaluar los resultados del mismo. Considerando que la regulación española emana de la Directiva 92/57/CEE, es preciso partir de un estudio específico de dicha referencia normativa para, seguidamente, identificar tanto las diferencias y particularidades introducidas en nuestra transposición nacional como, en su caso, la coherencia de las mismas con la regulación comunitaria. Para todo ello, la memoria realizada analiza de manera discriminada las diferencias que, cada modelo regulatorio de la UE-15, introduce en las figuras y herramientas de gestión preventiva instauradas por la citada Directiva. Adicionalmente, la presente investigación evalúa los resultados que, en términos de siniestralidad laboral, costes económicos y responsabilidades legales, han arrojado los vigentes modelos de regulación del sector a nivel comunitario (UE-15) y español. Dicho análisis permite identificar, además, aquellos estados miembros de la UE-15 que, en términos comparativos, han logrado una mayor eficacia preventiva específica en el sector de la construcción y establecer, en su caso, la correspondiente correlación con el tipo de modelo regulatorio. Por otra parte, se analiza la vinculación existente entre los accidentes de trabajo en la construcción y las decisiones tomadas en la etapa de diseño (incluyendo un estudio experimental) así como, con carácter específico, las técnicas desarrolladas para integrar los aspectos preventivos en dicha etapa. Dichas técnicas, englobadas a nivel internacional bajo la denominación de Prevention Through Design (PtD), son analizadas en detalle a la vez que se prospecciona su aplicación al modelo regulatorio español. Por último, se realiza un análisis comparativo del modelo regulatorio español frente a aquellos sistemas que, a nivel internacional, han logrado mejores resultados relativos para, en base al mismo, efectuar una propuesta justificada y pormenorizada de modificación del actual modelo regulatorio de la seguridad y salud en la construcción en España. The need to improve risk prevention efficacy in construction sites in Spain requires a deeper analysis of the current regulatory model while evaluating its results. Whereas the Spanish regulation stems from Directive 92/57/EEC, it is necessary to start from a specific study of such reference, and then pass to identify both the differences and particularities introduced in national transposition and, where appropriate, its consistency with the European regulation system. Above from that, this research analyzes how each country of the EU15 has adapted the provisions in Directive 92/57/EEC and their respective regulation of the preventive management tools introduced by it. Additionally, this research evaluates the results, in terms of workplace accidents, economic costs and liabilities, produced by the current models of sector regulation both at EU-15 and Spanish level. This analysis allows to identify also those member states of the EU-15 that, in comparison, have achieved greater efficiency in specific preventive construction sector and establish, where appropriate, the corresponding correlation with the type of regulatory model. Moreover, we analyze the link between occupational accidents in the construction and the decisions taken at the design stage (including a pilot study) and, with specificity, the techniques developed to integrate the preventive aspects in the design works. Such techniques, included internationally under the name of Prevention through Design (PtD), are analyzed in detail while collating their application routes to the Spanish regulatory model. Finally, we develop a comparative analysis of the Spanish regulatory model against those systems that, worldwide, have achieved better results relating to make a justified and detailed proposal to amend the current regulatory model and health and safety in construction in Spain.

Objetivos: Analizar las desigualdades de género en las condiciones de empleo, trabajo, conciliación de la vida laboral y familiar, y en los problemas de salud relacionados con el trabajo en una muestra de la población ocupada en España en el año 2007 teniendo en cuenta la clase social y el sector de actividad. Métodos: Las desigualdades de género se analizaron mediante 25 indicadores en los 11.054 trabajadores entrevistados en la VI Encuesta Nacional de Condiciones de Trabajo. Se calcularon las odds ratio (OR) y sus intervalos de confianza del 95% (IC95%) mediante modelos de regresión logística multivariados, estratificando por clase social ocupacional y sector de actividad. Resultados: Más mujeres que hombres trabajaban sin contrato (OR = 1,83; IC95%: 1,51-2,21), con alto esfuerzo o baja recompensa (1,14:1,05-1,25) y sufriendo acoso sexual (2,85:1,75-4,62), discriminación (1,60:1,26-2,03) y más dolores osteomusculares (1,38:1,19-1,59). Más hombres que mujeres trabajaban a turnos (0,86:0,79-0,94), con altos niveles de ruido (0,34:0,30-0,40), altas exigencias físicas (0,58:0,54-0,63) y sufriendo más lesiones por accidentes de trabajo (0,67:0,59-0,76). Las trabajadoras no manuales mostraron trabajar con un contrato temporal (1,34:1,09-1,63), expuestas a más riesgos psicosociales y sufriendo mayor discriminación (2,47:1,49-4,09) y enfermedades profesionales (1,91:1,28-2,83). En el sector de la industria las desigualdades de género fueron más marcadas. Conclusiones: En España existen importantes desigualdades de género en las condiciones de empleo, trabajo y en los problemas de salud relacionados con el trabajo, que se ven influenciadas por la clase social y el sector de actividad, y que sería necesario tener en consideración en las políticas públicas de salud laboral.

Fundamentos: La relación entre inmigración, trabajo y salud constituye uno de los retos más importantes en salud laboral y más concretamente en España por el importante incremento de proporción inmigrante acontecido en el mercado laboral. El objetivo de esta investigación es conocer la relación entre las condiciones de trabajo y sus efectos en la salud de trabajadores inmigrantes en España. Métodos: Revisión bibliográfica de artículos científicos originales en español e inglés Medline y Medes (1998-2012). Se revisaron los textos completos de los artículos incluidos. Resultados: Se incluyeron 20 estudios, 13 con metodología de investigación cuantitativa y 7 cualitativa. Los temas tratados abordaban problemas específicos de salud relacionados con el trabajo (principalmente lesiones por accidente de trabajo), incapacidad laboral y diferencias en condiciones de trabajo y empleo. Los hallazgos de los estudios mostraron mayor incidencia de lesiones por accidentes de trabajo, menores tasas de incapacidad laboral, mayor prevalencia de presentismo laboral, exposición a factores psicosociales y precariedad laboral en la población inmigrante. Conclusiones: A pesar de la singularidad del proceso demográfico migratorio, los problemas de salud y determinantes identificados no difieren de los referenciados en otros países, en otros contextos y en otros momentos.

Introdução – Os acidentes de trabalho envolvendo material biológico entre profissionais de saúde são frequentes e constituem um problema de saúde pública, pois representam uma preocupação constante das instituições e dos profissionais de saúde. Os profissionais do pré hospitalar tornam-se mais expostos pela tensão emocional vivida, pela natureza e circunstâncias do seu exercício profissional. Objetivos – Identificar a ocorrência de acidentes ocupacionais com materiais perfurocortantes nos profissionais de saúde do SAMU Maceió; identificar os acidentes mais frequentes, os períodos em que ocorrem e avaliar as condutas tomadas após o acidente. Métodos – Estudo descritivo com orientação analítico-descritiva desenvolvido numa amostra de 40 profissionais de saúde, Enfermeiro socorrista, Técnico Enfermagem e Médicos Socorristas. A recolha de informação foi suportada num questionário elaborado para o efeito. Resultados – A nossa amostra apresentou uma média de idades de 39,2 anos, o sexo feminino foi maioritário com 70,0% e 50,0% trabalha entre 6 a 10 anos na unidade. A maioria dos acidentes perfuro cortantes foram provocados por agulhas (75,0%) e dos quais não resultou incapacidade em 75%. A maioria dos acidentes ocorreram no verão e no turno da noite. Grande número de profissionais (70%) não cumpriu com as normas de procedimentos após o acidente e 80,0% dos profissionais não realizaram a quimioprofilaxia, não cumprindo com o protocolo do Ministérios da Saúde do Brasil. O paciente-fonte foi identificado em 87,5% dos casos mas verificamos uma subnotificação dos acidentes pois 45,0% dos profissionais não o fizeram. Nenhum profissional frequentou formação específica sobre acidentes de trabalho Conclusões – Verificamos uma alta incidência de acidentes perfurocortantes entre os profisionais do SAMU. Existe uma subnotificação do acidente e um déficite nas condutas apropriadas após o acidente. A população estudada necessita de uma maior sensibilização/formação quanto às medidas preventivas de segurança no ambiente de trabalho e motivação para o controle e prevenção dos acidentes ocupacionais no exercício de sua profissão. Health and safety policies may be regarded as the cornerstone for positive prevention of occupational accidents and diseases. The Health and Safety at Work, etc Act 1974 makes it a legal duty for employers to prepare and revise a written statement of a general policy with respect to the health and safety at work of employees as well as the organisation and arrangements for carrying out that policy. Despite their importance and the legal equipment to prepare them, health and safety policies have been found, in a large number of plastics processing companies (particularly small companies), to be poorly prepared, inadequately implemented and monitored. An important cause of these inadequacies is the lack of necessary health and safety knowledge and expertise to prepare, implement and monitor policies. One possible way of remedying this problem is to investigate the feasibility of using computers to develop expert system programs to simulate the health and safety (HS) experts' task of preparing the policies and assisting companies implement and monitor them. Such programs use artificial intelligence (AI) techniques to solve this sort of problems which are heuristic in nature and require symbolic reasoning. Expert systems have been used successfully in a variety of fields such as medicine and engineering. An important phase in the feasibility of development of such systems is the engineering of knowledge which consists of identifying the knowledge required, eliciting, structuring and representing it in an appropriate computer programming language.

The increasing prevalence of Repetitive Strain Injury / Work-Related Musculoskeletal Disorders (RSI / MSDs) has been explained by changes in the work that has been characterized by setting goals and productivity. This fact does not take into account the psycho-physiological characteristics of workers who become ill as a result of professional activities engaged in and also the adverse conditions in which his work was done. This study aimed to analyze the RSI / MSDs reported in the state of Rio Grande do Norte, Brazil, 2010-2014 compared to the profile of the population, epidemiological aspects and features of these diseases. It is a descriptive epidemiological cross-sectional study using secondary data obtained in the Notifiable Diseases Information System (SINAN) in May 2015 was used as the area covered by the Rio Grande do Norte. Data were collected regarding the grievances of work-related RSI / MSDs, assigned by the Worker's Health Reference Center (CEREST) of the State Health Department. Data were processed with the help of Microsoft Excel® 2013 and presented in tables in absolute frequency (n) and relative frequency (%). The results showed a total of 403 notifications of RSI / MSDs, where the vast majority 72% (290) was recorded in Natal. Regarding the profile of notified employees, 88.59% (357) have aged between 25 and 54 years, 62.78% (253) were female, and in relation to education, 31.51% (127) had high school complete. The most affected occupations were seamstresses (the clothing industry) with 24.97% (97), followed by masons, construction with 3.23% (13) and cashiers with 2.99% (12). About the work situation, 75.93% (306) had a formal contract, however, were sent to Communications Occupational accidents (CAT) only 67% (270) of the cases. Regarding the signs and symptoms, pain and limitation of movement were the most mentioned respectively with 98.01% (395) and 95.04% (383) of notifications. Notifications, 94.29% (380) showed exposure to repetitive movements in their workplace. The most frequent specific diagnoses were synovitis and tenosynovitis (CID F 65), with 30.02% (121) of notifications, followed by back pain (CID F 54) to 19.35% (78) and shoulder injuries (ICD M 75) with 15.88% (64). They were away from work 81.64% (329) of workers reported with RSI / MSDs. Evolution more prevalent among cases was temporary disability with 75.68% (305). It was concluded that the SINAN is a database of potential to characterize the profile of RSI / MSDs, requiring, however, an improvement in the coverage of records and data quality. In addition, this study reflects the need to implement protection strategies to workers by companies, signaling promotion, prevention and rehabilitation aimed at reducing these injuries and the improvement of occupational health indicators in Rio Grande do Norte.

Diseases and disorders related to work sets up an important public health problem in Brazil and worldwide. However, the reality of these diseases still constitutes a gap with regard to its characterization and epidemiological situation, especially in Brazil. In this context, this study aims to analyze the magnitude of morbidity related to work from the injuries and illnesses reported by Health the Diseases Notifiable of Health of the State Public River Health Department worker Reference Center Information System services Grande do Norte from 2007 to 2014. It is ecological study, quantitative cross-sectional study in which the analysis unit of the municipalities of Rio Grande do Norte. Data were collected from the state base of Diseases Notifiable Information System Centre of the Secretariat of State Workers' Health Reference Public Health of Rio Grande do Norte, between March and June 2015, after the approval of the Committee of Ethics in Research of the Federal University of Rio Grande do Norte, Opinion 014/2014. The population was represented by the universe of cases of diseases and disorders related to work that were reported and shut down the system from 2007 to 2014. Data were analyzed using descriptive and inferential statistics, presented in tables, graphs, charts and figures. For this, we used the Microsoft Excel 2007 and SPSS version 20.0. To check the significance level we opted for the application of the chi-square or Fisher tests. We adopted the significance level of p <0.05. Of the 10,161 cases of diseases related to the reported work, the biological work accidents had the highest percentage (52.84%) followed by serious occupational accidents (37.49%). For diseases, the highlights were musculoskeletal (4.82%), mental disorders (2.19%) and exogenous intoxication (1.97%). Among men, there was a predominance of major accidents (91.80%), mental disorders (70.00%) and exogenous poisoning (52.84%). Women were most affected by biological accidents (77.50%) and musculoskeletal diseases (64.10%). Among workers who have suffered injuries predominated mulatto (%), mean age of 35.86 years, low education (%) and workers in the formal sector (%). Among the accidents, biological (n = 5,369) accounted for 52.84% of cases occurred predominantly among nursing professionals (48.31%). The percutaneous exposure was the most frequent (73.05%) and the occurrence of circumstances was improper disposal of sharps (45.28%), the needle the most common agent (66.62%) and the organic material was blood (72.99%). Most injured workers were vaccinated against hepatitis B (68.13%), but no information as to the assessment of the vaccine response. In the course of the disease predominated ignored the situation with loss of monitoring of clinical follow-up (55.62%). There was also an increase in the notification of serious industrial accidents predominantly male (91.80%) workers aged 25-44 years (54.3%) and typical accidents (76.3%). The temporary disability was the most common outcome (55.53%) and hand the most affected part (33.00%); the mining and construction industry had the highest number of cases (25.1%) in registered employee (34.2%). The findings of this study show a positive result in relation to increased mandatory reporting of injuries and illnesses related to work together to health services that meet victimized workers, towards the occurrence of knowledge of these accidents for decision making in public plans and policies of health. However, the information system still needs improvement in both the coverage and the quality of the data to demonstrate with greater reliability the magnitude of events to support the planning of workers' health into shares in the state.
