Sound is a key sensory modality for Hawaiian spinner dolphins. Like many other marine animals, these dolphins rely on sound and their acoustic environment for many aspects of their daily lives, making it is essential to understand soundscape in areas that are critical to their survival. Hawaiian spinner dolphins rest during the day in shallow coastal areas and forage offshore at night. In my dissertation I focus on the soundscape of the bays where Hawaiian spinner dolphins rest taking a soundscape ecology approach. I primarily relied on passive acoustic monitoring using four DSG-Ocean acoustic loggers in four Hawaiian spinner dolphin resting bays on the Kona Coast of Hawai‛i Island. 30-second recordings were made every four minutes in each of the bays for 20 to 27 months between January 8, 2011 and March 30, 2013. I also utilized concomitant vessel-based visual surveys in the four bays to provide context for these recordings. In my first chapter I used the contributions of the dolphins to the soundscape to monitor presence in the bays and found the degree of presence varied greatly from less than 40% to nearly 90% of days monitored with dolphins present. Having established these bays as important to the animals, in my second chapter I explored the many components of their resting bay soundscape and evaluated the influence of natural and human events on the soundscape. I characterized the overall soundscape in each of the four bays, used the tsunami event of March 2011 to approximate a natural soundscape and identified all loud daytime outliers. Overall, sound levels were consistently louder at night and quieter during the daytime due to the sounds from snapping shrimp. In fact, peak Hawaiian spinner dolphin resting time co-occurs with the quietest part of the day. However, I also found that humans drastically alter this daytime soundscape with sound from offshore aquaculture, vessel sound and military mid-frequency active sonar. During one recorded mid-frequency active sonar event in August 2011, sound pressure levels in the 3.15 kHz 1/3rd-octave band were as high as 45.8 dB above median ambient noise levels. Human activity both inside (vessels) and outside (sonar and aquaculture) the bays significantly altered the resting bay soundscape. Inside the bays there are high levels of human activity including vessel-based tourism directly targeting the dolphins. The interactions between humans and dolphins in their resting bays are of concern; therefore, my third chapter aimed to assess the acoustic response of the dolphins to human activity. Using days where acoustic recordings overlapped with visual surveys I found the greatest response in a bay with dolphin-centric activities, not in the bay with the most vessel activity, indicating that it is not the magnitude that elicits a response but the focus of the activity. In my fourth chapter I summarize the key results from my first three chapters to illustrate the power of multiple site design to prioritize action to protect Hawaiian spinner dolphins in their resting bays, a chapter I hope will be useful for managers should they take further action to protect the dolphins.

Aanleiding voor het onderzoek Deze scriptie sluit aan bij de afstudeerkring “effectieve verandermanagers”. Aanleiding voor dit onderzoek was de brede vraag “Wat onderscheidt een effectieve veranderingsmanager op het middenniveau in organisaties van een minder effectieve bij het implementeren van veranderingen?” Binnen deze brede vraag past de centrale vraagstelling in dit onderzoek: “Welke neerwaartse activiteiten oefenen middenmanagers uit in een sensemakingproces tijdens een verandering?“ Middenmanager Allereerst definiëren we het begrip ‘middenmanager’. De middenmanager bevindt zich op een kruispunt: In de verticale lijn tussen het topmanagement en de werkvloer, op de horizontale lijn tussen de diverse afdelingen (Nonaka,1994). Ten behoeve van mijn onderzoek sluit ik mij aan bij de positionering van Huy (2001). Hij plaatst de middenmanager 2 lagen onder de CEO en 1 laag boven lijnmedewerkers en professionals. In een veranderingsproces heeft de middenmanager verschillende rollen te vervullen. Deze verschillende rollen zijn, onder anderen, beschreven door Huy (2001) en Quinn (1998). Zij noemen: entrepreneur, communicator en therapeut, stimulator, mentor, innovator, bemiddelaar, producent, bestuurder, coördinator, controleur. De onderzoekers zijn het er over eens dat de middenmanager moet balanceren en doseren in de verschillende rollen om effectief te kunnen zijn in een veranderingsproces. Sensemakingproces Dit onderzoek probeert meer kennis te genereren over welke neerwaartse activiteiten middenmanagers uitoefenen gedurende een sensemakingproces tijdens een verandering. We definiëren het begrip ‘sensemakingproces’. Het sensemakingproces vindt plaats als mensen geconfronteerd worden met boodschappen of gebeurtenissen die nieuw zijn. Door met deze nieuwe informatie te kijken naar de bestaande werkelijkheid, is een bewustwording op gang geholpen die kan leiden tot verandering. Er is nog weinig onderzoek gedaan naar de activiteiten (wat doen middenmanagers op dagelijkse basis) van middenmanagers in sensemakingprocessen. Rouleau & Balogun (2007) hebben dat wel onderzocht en onderscheiden de volgende vier sensegivingactiviteiten: het uitrollen van netwerken; het distribueren van gedachten door het voeren van gesprekken; het identificeren van sociale en culturele systemen. Relevantie onderzoek De externe validiteit van het hierboven genoemde onderzoek van Rouleau & Balogun (2007) is echter beperkt, doordat deze is uitgevoerd onder een kleine populatie in Canada en het Verenigd Koninkrijk. Mijn onderzoek wil een bijdrage leveren aan de toename van de externe validiteit door te verifiëren of de neerwaartse activiteiten van de middenmanager die gevonden zijn in het onderzoek van Rouleau & Balogun (2007) middels een andere onderzoeksmethode en met een andere onderzoekspopulatie (de Nederlandse zorginstelling Tactus). Wat betreft de praktische relevantie kan gezegd worden dat mijn onderzoek meer inzicht geeft aan de middenmanagers in welke activiteiten zij uitvoeren of zouden kunnen uitvoeren. Zij kunnen zich bewust worden van hun mogelijkheden om invloed uit te oefenen op het sensemakingproces van hun medewerkers. Ik verwacht daarmee hun effectiviteit te vergroten. Centrale vraag en deelvragen Bij de centrale vraag kunnen de volgende deelvragen geformuleerd worden: 1. Wat is de rol van middenmanagers in een sensemakingproces tijdens een verandering? 2. Zijn de neerwaartse sensemakingactiviteiten van middenmanagers (uit de studie van Rouleau & Balogun, 2007) ook te identificeren zijn binnen één Nederlandse GGZ-stichting? 3. Zijn er andere neerwaartse sensemakingactiviteiten van middenmanagers te identificeren? Conceptuele model Het model dat de basis is voor de beantwoording van de tweede en derde deelvraag, is gebaseerd op de literatuur. In dit model worden de sensemakingactiviteiten van de middenmanager als een onafhankelijke variabele gezien die direct invloed heeft op het sensemakingproces binnen het team. Daarbij kunnen we enkele mediërende en modererende factoren onderscheiden. Als modererende factor noemen we, ten eerste, de mate waarin voor het teamlid het persoonlijke triggerpoint bereikt is. Hoe dichter genaderd, hoe meer kans op verandering bij dit teamlid (Kloosterboer, 2008). Ten tweede noemen we het sneeuwbaleffect van de positieve feedback (Kelman, 2005): hoe verder in het proces, hoe meer kans op verandering. Tenslotte noemen we als modererende factor het emotiemanagement van de middenmanager. Hoe meer de middenmanager in staat is de balans te vinden, hoe meer kans op verandering (Huy, 2002). Als mediërende factor kunnen we de horizontale sensegiving noemen, ofwel de sensegiving door peers (Balogun & Johnson, 2005). Deze sensegiving kan, los van de sensemakingactiviteiten van de manager een geheel eigen verandering op gang brengen. Het kan dus gezien worden als (een deel van) de verklaring voor het sensemakingproces binnen het team. Dit onderzoek beperkt zich tot het bestuderen van de sensemakingactiviteiten van de middenmanager en hun effect op het sensemakingproces binnen het team. In het onderzoek worden de modererende factoren en de mediërende factor dus niet meegenomen. Onderzoeksopzet Het onderzoek is opgesteld als multiple-case design, toetsend onderzoek. Toetsend, omdat de externe validering van de neerwaartse sensemakingactiviteiten van de middenmanager nog laag is. Het multiple-case design biedt de mogelijkheid om cases met elkaar te vergelijken en bredere steun te vinden voor de geformuleerde proposities. De voorgenomen bronnen waren een (één op één) interview met acht middenmanagers, een groepsinterview met meerdere medewerkers per middenmanager en documenten. De focus in alle interviews was de middenmanager en diens sensemakingactiviteiten. De opgevraagde en aangeleverde documenten zijn echter geen adequate bron van informatie gebleken. Resultaten De eerste deelvraag is beantwoord middels literatuurstudie. De onderzoekers zijn het er over eens dat de middenmanager moet balanceren en doseren in de verschillende rollen om effectief te kunnen zijn in een veranderingsproces. De tweede en derde deelvraag werden beantwoord middels een multiple-case design onderzoek. De resultaten uit dit onderzoek laten zien dat de neerwaartse sensemakingactiviteiten uit de studie van Rouleau & Balogun (2007), inderdaad gevonden werden binnen de genoemde GGZ-stichting. De hypothese is daarmee geverifieerd. De derde conclusie is dat er geen andere neerwaartse sensemakingactiviteiten van middenmanagers zijn geïdentificeerd. Door de verkregen informatie over de vier sensemakingactiviteiten van middenmanager terug te brengen naar handelingsniveau, is zichtbaar geworden dat er verschillen zijn tussen de middenmanagers. De handelingen in gezamenlijkheid leidden wel tot de vier sensemakingactiviteiten. Discussie De casestudie heeft geen nieuwe activiteiten aangetoond. Dat maakt de definiëring van het begrip steviger. Door de vier sensemakingactiviteiten (Rouleau & Balogun, 2007) op een derde niveau op te delen in kleinere eenheden, draagt deze casestudie tevens bij aan een nadere definiëring van het begrip ‘neerwaartse sensemakingactiviteiten van de middenmanager’. Door de verduidelijking van het begrip is kwantitatief onderzoek beter mogelijk, waardoor nu ook causale verbanden tussen de neerwaartse sensemakingactiviteiten van de middenmanager en de sensemaking bij de medewerkers gemaakt kunnen worden. Aanbevelingen In een vervolgonderzoek zou het conceptuele model deels of geheel onderzocht kunnen worden. Met een dergelijk onderzoek, of meerdere onderzoeken zou meer duidelijkheid kunnen komen over de mate van invloed van de mediërende factoren en modererende factor op de sensemaking. Aanbeveling voor verder onderzoek is om de onderzoeksmethode te variëren of uit te breiden met bijvoorbeeld participerende observatie. Dan zou waarneming kunnen plaatsvinden vanuit het perspectief van een betrokkene (Yin, 2003). Een andere methode om de activiteiten beter te inventariseren is het laten bijhouden door managers van hun sensemakingactiviteiten. Een bijkomende propositie die dit onderzoek oplevert, is dat de relatie tussen al dan niet hebben van onbewuste kennis en het beschikken over ervaring als leidinggevende niet gelegd kan worden. Deze propositie zou verder onderzocht kunnen worden.

This thesis reports on the development of quantitative measurement using micromachined scanning thermal microscopy (SThM) probes. These thermal probes employ a resistive element at their end, which can be used in passive or active modes. With the help of a review of SThM, the current issues and potentials associated with this technique are revealed. As a consequence of this understanding, several experimental and theoretical methods are discussed, which expand our understanding of these probes. The whole thesis can be summarized into three parts, one focusing on the thermal probe, one on probe-sample thermal interactions, and the third on heat transfer within the sample. In the first part, a series of experiments are demonstrated, aimed at characterizing the probe in its electrical and thermal properties, benefiting advanced probe design, and laying a fundamental base for quantifying the temperature of the probe. The second part focuses on two artifacts observed during the thermal scans – one induced by topography and the other by air conduction. Correspondingly, two devices, probing these artifacts, are developed. A topography-free sample, utilizing a pattern transfer technique, minimises topography-related artifacts that limited the reliability of SThM data; a controlled temperature ‘Johnson noise device’, with multiple-heater design, offers a uniform, accurate, temperature distribution. Analyzing results of scan from these samples provides data for studying the thermal interactions within the probe and the tip-sample interface. In the final part, the observation is presented that quantification of measurements depends not only on an accurate measurement tool, but also on a deep understanding of the heat transfer within the sample resulting from the nanoscopic contact. It is believed that work in this thesis contributes to SThM gaining wider application in the scientific community.

A repulsion algorithm that invokes the Nelder-Mead (N-M) local search method and uses a penalty-type merit function based on the error function, known as 'erf', is presented. In the N-M algorithm context, different strategies are proposed to enhance the quality of the solutions and improve the overall efficiency. The main goal of this paper is to use a two-level factorial design of experiments to analyze the statistical significance of the observed differences in selected performance criteria produced when testing different strategies in the N-M based repulsion algorithm. Lipid nanoballoons integrating multiple emulsions of the type water-in-oil-in-water enclose, at least in theory, a biomimetic aqueous-core suitable for housing hydrophilic biomolecules such as proteins, peptides and bacteriophage particles. The research effort entertained in this paper reports a full statistical 23x31 factorial design study (three variables at two levels and one variable at three levels) to optimize biomimetic aqueous-core lipid nanoballoons for housing hydrophilic protein entities. The concentrations of protein, lipophilic and hydrophilic emulsifiers, and homogenization speed were set as the four independent variables, whereas the mean particle hydrodynamic size (HS), zeta potential (ZP) and polydispersity index (PI) were set as the dependent variables. The V23x31 factorial design constructed led to optimization of the higher (+1) and lower (-1) levels, with triplicate testing for the central (0) level, thus producing thirty three experiments and leading to selection of the optimized processing parameters as 0.015% (w/w) protein entity, 0.75% (w/w) lipophilic emulsifier (soybean lecithin) and 0.50% (w/w) hydrophilic emulsifier (poloxamer 188). In the present research effort, statistical optimization and production of protein derivatives encompassing full stabilization of their three-dimensional structure, has been attempted via housing said molecular entities within biomimetic aqueous-core lipid nanoballoons integrating a multiple (W/O/W) emulsion.

BACKGROUND AND PURPOSE: Stroke registries are valuable tools for obtaining information about stroke epidemiology and management. The Acute STroke Registry and Analysis of Lausanne (ASTRAL) prospectively collects epidemiological, clinical, laboratory and multimodal brain imaging data of acute ischemic stroke patients in the Centre Hospitalier Universitaire Vaudois (CHUV). Here, we provide design and methods used to create ASTRAL and present baseline data of our patients (2003 to 2008). METHODS: All consecutive patients admitted to CHUV between January 1, 2003 and December 31, 2008 with acute ischemic stroke within 24 hours of symptom onset were included in ASTRAL. Patients arriving beyond 24 hours, with transient ischemic attack, intracerebral hemorrhage, subarachnoidal hemorrhage, or cerebral sinus venous thrombosis, were excluded. Recurrent ischemic strokes were registered as new events. RESULTS: Between 2003 and 2008, 1633 patients and 1742 events were registered in ASTRAL. There was a preponderance of males, even in the elderly. Cardioembolic stroke was the most frequent type of stroke. Most strokes were of minor severity (National Institute of Health Stroke Scale [NIHSS] score ≤ 4 in 40.8% of patients). Cardioembolic stroke and dissections presented with the most severe clinical picture. There was a significant number of patients with unknown onset stroke, including wake-up stroke (n=568, 33.1%). Median time from last-well time to hospital arrival was 142 minutes for known onset and 759 minutes for unknown-onset stroke. The rate of intravenous or intraarterial thrombolysis between 2003 and 2008 increased from 10.8% to 20.8% in patients admitted within 24 hours of last-well time. Acute brain imaging was performed in 1695 patients (97.3%) within 24 hours. In 1358 patients (78%) who underwent acute computed tomography angiography, 717 patients (52.8%) had significant abnormalities. Of the 1068 supratentorial stroke patients who underwent acute perfusion computed tomography (61.3%), focal hypoperfusion was demonstrated in 786 patients (73.6%). CONCLUSIONS: This hospital-based prospective registry of consecutive acute ischemic strokes incorporates demographic, clinical, metabolic, acute perfusion, and arterial imaging. It is characterized by a high proportion of minor and unknown-onset strokes, short onset-to-admission time for known-onset patients, rapidly increasing thrombolysis rates, and significant vascular and perfusion imaging abnormalities in the majority of patients.

BACKGROUND: Multiple electrode aggregometry (MEA) is a point-of-care test evaluating platelet function and the efficacy of platelet inhibitors. In MEA, electrical impedance of whole blood is measured after addition of a platelet activator. Reduced impedance implies platelet dysfunction or the presence of platelet inhibitors. MEA plays an increasingly important role in the management of perioperative platelet dysfunction. In vitro, midazolam, propofol, lidocaine and magnesium have known antiplatelet effects and these may interfere with MEA interpretation. OBJECTIVE: To evaluate the extent to which MEA is modified in the presence of these drugs. DESIGN: An in-vitro study using blood collected from healthy volunteers. SETTING: Centre Hospitalier Universitaire Vaudois, Lausanne, Switzerland, 2010 to 2011. PATIENTS: Twenty healthy volunteers. INTERVENTION: Measurement of baseline MEA was using four activators: arachidonic acid, ADP, TRAP-6 and collagen. The study drugs were then added in three increasing, clinically relevant concentrations. MAIN OUTCOME MEASURE: MEA was compared with baseline for each study drug. RESULTS: Midazolam, propofol and lidocaine showed no effect on MEA at any concentration. Magnesium at 2.5 mmol l had a significant effect on the ADP and TRAP tests (31 ± 13 and 96 ± 39 AU, versus 73 ± 21 and 133 ± 28 AU at baseline, respectively), and a less pronounced effect at 1 mmol l on the ADP test (39 ± 0 AU). CONCLUSION: Midazolam, propofol and lidocaine do not interfere with MEA measurement. In patients treated with high to normal doses of magnesium, MEA results for ADP and TRAP-tests should be interpreted with caution. TRIAL REGISTRATION: Clinicaltrials.gov (no. NCT01454427).

A method of making a multiple matched filter which allows the recognition of different characters in successive planes in simple conditions is proposed. The generation of the filter is based on recording on the same plate the Fourier transforms of the different patterns to be recognized, each of which is affected by different spherical phase factors because the patterns have been placed at different distances from the lens. This is proved by means of experiments with a triple filter which allows satisfactory recognition of three characters.

OBJECTIVE: Intervention during the pre-psychotic period of illness holds the potential of delaying or even preventing the onset of a full-threshold disorder, or at least of reducing the impact of such a disorder if it does develop. The first step in realizing this aim was achieved more than 10 years ago with the development and validation of criteria for the identification of young people at ultra-high risk (UHR) of psychosis. Results of three clinical trials have been published that provide mixed support for the effectiveness of psychological and pharmacological interventions in preventing the onset of psychotic disorder. METHOD: The present paper describes a fourth study that has now been undertaken in which young people who met UHR criteria were randomized to one of three treatment groups: cognitive therapy plus risperidone (CogTher + Risp: n = 43); cognitive therapy plus placebo (CogTher + Placebo: n = 44); and supportive counselling + placebo (Supp + Placebo; n = 28). A fourth group of young people who did not agree to randomization were also followed up (monitoring: n = 78). Baseline characteristics of participants are provided. RESULTS AND CONCLUSION: The present study improves on the previous studies because treatment was provided for 12 months and the independent contributions of psychological and pharmacological treatments in preventing transition to psychosis in the UHR cohort and on levels of psychopathology and functioning can be directly compared. Issues associated with recruitment and randomization are discussed.

OBJECTIVE: To compare the effects of sodium bicarbonate and lactate for continuous veno-venous hemodiafiltration (CVVHDF) in critically ill patients. DESIGN AND SETTINGS: Prospective crossed-over controlled trial in the surgical and medical ICUs of a university hospital. PATIENTS: Eight patients with multiple organ dysfunction syndrome (MODS) requiring CVVHDF. INTERVENTION: Each patient received the two buffers in a randomized sequence over two consecutive days. MEASUREMENTS AND RESULTS: The following variables were determined: acid-base parameters, lactate production and utilization ((13)C lactate infusion), glucose turnover (6,6(2)H(2)-glucose), gas exchange (indirect calorimetry). No side effect was observed during lactate administration. Baseline arterial acid-base variables were equal with the two buffers. Arterial lactate (2.9 versus 1.5 mmol/l), glycemia (+18%) and glucose turnover (+23%) were higher in the lactate period. Bicarbonate and glucose losses in CVVHDF were substantial, but not lactate elimination. Infusing (13)C lactate increased plasma lactate levels equally with the two buffers. Lactate clearance (7.8+/-0.8 vs 7.5+/-0.8 ml/kg per min in the bicarbonate and lactate periods) and endogenous production rates (14.0+/-2.6 vs 13.6+/-2.6 mmol/kg per min) were similar. (13)C lactate was used as a metabolic substrate, as shown by (13)CO(2) excretion. Glycemia and metabolic rate increased significantly and similarly during the two periods during lactate infusion. CONCLUSION: Lactate was rapidly cleared from the blood of critically ill patients without acute liver failure requiring CVVHDF, being transformed into glucose or oxidized. Lactate did not exert undesirable effects, except moderate hyperglycemia, and achieved comparable effects on acid-base balance to bicarbonate.

Quantitative information from magnetic resonance imaging (MRI) may substantiate clinical findings and provide additional insight into the mechanism of clinical interventions in therapeutic stroke trials. The PERFORM study is exploring the efficacy of terutroban versus aspirin for secondary prevention in patients with a history of ischemic stroke. We report on the design of an exploratory longitudinal MRI follow-up study that was performed in a subgroup of the PERFORM trial. An international multi-centre longitudinal follow-up MRI study was designed for different MR systems employing safety and efficacy readouts: new T2 lesions, new DWI lesions, whole brain volume change, hippocampal volume change, changes in tissue microstructure as depicted by mean diffusivity and fractional anisotropy, vessel patency on MR angiography, and the presence of and development of new microbleeds. A total of 1,056 patients (men and women ≥ 55 years) were included. The data analysis included 3D reformation, image registration of different contrasts, tissue segmentation, and automated lesion detection. This large international multi-centre study demonstrates how new MRI readouts can be used to provide key information on the evolution of cerebral tissue lesions and within the macrovasculature after atherothrombotic stroke in a large sample of patients.

Culverts are common means to convey flow through the roadway system for small streams. In general, larger flows and road embankment heights entail the use of multibarrel culverts (a.k.a. multi-box) culverts. Box culverts are generally designed to handle events with a 50-year return period, and therefore convey considerably lower flows much of the time. While there are no issues with conveying high flows, many multi-box culverts in Iowa pose a significant problem related to sedimentation. The highly erosive Iowa soils can easily lead to the situation that some of the barrels can silt-in early after their construction, becoming partially filled with sediment in few years. Silting can reduce considerably the capacity of the culvert to handle larger flow events. Phase I of this Iowa Highway Research Board project (TR-545) led to an innovative solution for preventing sedimentation. The solution was comprehensively investigated through laboratory experiments and numerical modeling aimed at screening design alternatives and testing their hydraulic and sediment conveyance performance. Following this study phase, the Technical Advisory Committee suggested to implement the recommended sediment mitigation design to a field site. The site selected for implementation was a 3-box culvert crossing Willow Creek on IA Hwy 1W in Iowa City. The culvert was constructed in 1981 and the first cleanup was needed in 2000. Phase II of the TR 545 entailed the monitoring of the site with and without the selfcleaning sedimentation structure in place (similarly with the study conducted in laboratory). The first monitoring stage (Sept 2010 to December 2012) was aimed at providing a baseline for the operation of the as-designed culvert. In order to support Phase II research, a cleanup of the IA Hwy 1W culvert was conducted in September 2011. Subsequently, a monitoring program was initiated to document the sedimentation produced by individual and multiple storms propagating through the culvert. The first two years of monitoring showed inception of the sedimentation in the first spring following the cleanup. Sedimentation continued to increase throughout the monitoring program following the depositional patterns observed in the laboratory tests and those documented in the pre-cleaning surveys. The second part of Phase II of the study was aimed at monitoring the constructed self-cleaning structure. Since its construction in December 2012, the culvert site was continuously monitored through systematic observations. The evidence garnered in this phase of the study demonstrates the good performance of the self-cleaning structure in mitigating the sediment deposition at culverts. Besides their beneficial role in sediment mitigation, the designed self-cleaning structures maintain a clean and clear area upstream the culvert, keep a healthy flow through the central barrel offering hydraulic and aquatic habitat similar with that in the undisturbed stream reaches upstream and downstream the culvert. It can be concluded that the proposed self-cleaning structural solution “streamlines” the area upstream the culvert in a way that secures the safety of the culvert structure at high flows while producing much less disturbance in the stream behavior compared with the current constructive approaches.
