The purpose of this investigation was to compare the dentoalveolar and skeletal cephalometric changes produced by the Frankel (FR-2) and bionator appliances in persons with Class 11 malocclusion. Lateral cephalograms were available for 66 patients of both sexes, who were divided into 3 groups of 22. The control group included untreated Class 11 children, with an initial mean age of 8 years 7 months; they were followed without treatment for 13 months. The FR-2 appliance group had an initial mean age of 9 years; those children were treated for a mean period of 17 months. The bionator group initially had a mean age of 10 years 8 months; on average, they were treated for 16 months. The results demonstrated no significant changes in maxillary growth during the evaluation period. Both appliances showed statistically significant increases in mandibular growth and mandibular protrusion, with greater increases in patients treated in the bionator group. Both experimental groups showed an improvement in the maxillomandibular relationship. There were no significant changes in growth direction, while the bionator group had a greater increase in posterior facial height. Both appliances produced similar labial tipping and protrusion of the lower incisors, lingual inclination, retrusion of the upper incisors, and a significant increase in mandibular posterior dentoalveolar height. The major treatment effects of bionator and FR-2 appliances were dentoalveolar, with a smaller, but significant, skeletal effect.

Introduction: The force delivered during rapid maxillary expansion (RME) produces areas of compression on the periodontal ligament of the supporting teeth. The resulting alveolar bone resorption can lead to unwanted tooth movement in the same direction. The purpose of this study was to evaluate periodontal changes by means of computed tomography after RME with tooth-tissue-borne and tooth-borne expanders. Methods: The sample comprised 8 girls, 11 to 14 years old, with Class I or II malocclusions with unilateral or bilateral posterior crossbites Four girls were treated with tooth-tissue-borne Haas-type expanders, and 4 were treated with tooth-borne Hyrax expanders. The appliances were activated up to the full 7-mm capacity of the expansion screw. Spiral CT scans were taken before expansion and after the 3-month retention period when the expander was removed. One-millimeter thick axial sections were exposed parallel to the palatal plane, comprising the dentoalveolar area and the base of the maxilla up to the inferior third of the nasal cavity. Multiplanar reconstruction was used to measure buccal and lingual bone plate thickness and buccal alveolar bone crest level by means of the computerized method. Results and Conclusions: RME reduced the buccal bone plate thickness of supporting teeth 0.6 to 0.9 mm and increased the lingual bone plate thickness 0.8 to 1.3 mm. The increase in lingual bone plate thickness of the maxillary posterior teeth was greater in the tooth-borne expansion group than in the tooth-tissue-borne group. RME induced bone dehiscences on the anchorage teeth's buccal aspect (7.1 ± 4.6 mm at the first premolars and 3.8 ± 4.4 mm at the mesiobuccal area of the first molars), especially in subjects with thinner buccal bone plates. The tooth-borne expander produced greater reduction of first premolar buccal alveolar bone crest level than did the tooth-tissue-borne expander. © 2006 American Association of Orthodontists.

BACKGROUND AND OBJECTIVES: The laryngeal mask has been frequently used in Anesthesiology. Although the rate of complications with this technique is smaller than that of the endotracheal tube, it is not devoid of risks, especially in cases of difficult airways. The objective of this study was to report a case of unilateral lingual nerve damage after the use of the laryngeal mask airway. CASE REPORT: A female patient underwent a surgical procedure for removal of bilateral breast prosthesis under general, balanced anesthesia, with a size three laryngeal mask. The balloon was inflated with 30 mL of air. After the first postoperative hour, she developed decreased sensation and pain in the oropharynx and posterior two thirds of the tongue, which evolved for loss of taste in the next 24 hours. A tentative diagnosis of lingual nerve neuropraxis secondary to the use of the laryngeal mask was made. After three weeks, her symptoms subsided. CONCLUSION: Although complications after the use of the laryngeal mask airway are rare, they do occur, and neuropraxis of the lingual nerve is one of them. The diagnosis is clinical and it has a good outcome, with resolution of the symptoms within a few weeks or months. © Sociedade Brasileira de Anestesiologia, 2007.

This article describes an orthodontic system used to obtain active continuous torque with movement control of both active and reactive units; the system relies on principles of the segmented technique. A cantilever system with .017 x .025-in beta-titanium alloy wire was designed to provide the desirable moment on the active unit. A transpalatal bar or a lingual arch increases the anchorage and neutralizes the side effects on the reactive unit. This technique is an efficient approach for major corrections of buccolingual inclinations of certain teeth. © 2010 American Association of Orthodontists.

OBJETIVO: avaliar as alterações cefalométricas em pacientes com perda bilateral do primeiro molar inferior permanente. MÉTODOS: foram analisadas 68 telerradiografias laterais de pacientes de consultórios particulares. A amostra foi dividida em dois grupos pareados quanto ao sexo e idade - 34 indivíduos sem perdas (grupo controle) e 34 com perda bilateral do primeiro molar inferior permanente (grupo com perda). Foram excluídos da amostra pacientes que haviam perdido outros dentes que não o primeiro molar inferior, casos de agenesia e pacientes com menos de 16 anos de idade. Buscou-se avaliar somente indivíduos que tivessem relatado a perda há pelo menos 5 anos. RESULTADOS: demonstraram que a perda bilateral do primeiro molar inferior permanente leva ao suave fechamento do ângulo GnSN (P=0,05), um giro anti-horário do plano oclusal (P=0,0001), uma suave diminuição da altura facial anteroinferior (P=0,05), uma acentuada inclinação lingual (P=0,04) e retrusão dos incisivos inferiores (P=0,03). Por outro lado, a perda bilateral do primeiro molar inferior permanente não foi capaz de influenciar a relação maxilomandibular no sentido anteroposterior (P=0,21), a quantidade de mento (P=0,45), a inclinação dos incisivos superiores (P=0,12) e a posição anteroposterior dos incisivos superiores (P=0,46). CONCLUSÃO: a perda bilateral dos primeiros molares inferiores é capaz de produzir alterações marcantes no posicionamento dos incisivos inferiores e no plano oclusal, além de uma suave redução vertical da face.

INTRODUÇÃO: frequentemente, os pacientes ortodônticos apresentam restaurações de resina composta; no entanto, existem poucos estudos que avaliam a melhor forma de colagem ortodôntica nessa situação. OBJETIVO: o objetivo do presente trabalho foi avaliar a força adesiva de braquetes ortodônticos em restaurações resinosas com tratamento de superfície. MÉTODOS: foram utilizados 51 incisivos inferiores bovinos divididos aleatoriamente em três grupos. No grupo controle (GC), os braquetes foram colados em esmalte dentário; nos grupos experimentais com tratamento (GCT) e sem tratamento (GST), os braquetes foram colados em restauração de resina previamente realizada, diferenciando-se pelo tratamento de superfície com broca diamantada. Os dentes foram incluídos em tubos de PVC com resina acrílica autopolimerizável. O ensaio de cisalhamento foi executado em máquina universal de ensaios Emic. Os grupos foram submetidos à ANOVA com pós-teste de Tukey para verificação da diferença estatística entre os grupos (α = 0,05). RESULTADOS: GC (6,62MPa) e GCT (6,82MPa) apresentaram resultados semelhantes, enquanto o GST (5,07MPa) obteve resultados estatisticamente menores (p < 0,05). CONCLUSÃO: conclui-se que a melhor técnica de colagem de braquetes ortodônticos em restaurações de resina composta é a de realização de desgaste sobre a superfície.

Objective: Investigated the consequences of tongue piercing in individuals from the city of Araraquara, São Paulo, Brazil. Methods: In Araraquara there are five piercing shops, but only two agreed to help the research by providing the contact information of customers who had had their tongues pierced. These customers were contacted by telephone. By the end of the research, the sample consisted of 100 individuals from both genders aged 18 years or more. Not all of them still had their tongue piercing. A form with six objective questions was used to collect information. The information was then analyzed by descriptive statistics. Results: From the 100 participants, 77 had been using tongue piercing for more than 6 months. It took less than 4 months for the tongue to heal. Sixty–eight individuals used oral rinses and of these, 19 also used painkillers. Only 11 individuals did not present symptoms after the first weeks using the piercing. On the other hand, 56 individuals reported complications such as increased salivation, speaking difficulties, gingival or mucosal traumas, dental fractures and eating difficulties. Conclusion: All these tongue-piercing complications justify efforts to inform the youth about the use of piercing in the oral cavity.
