Giardia duodenalis is considered the most common protozoan infecting humans worldwide. Molecular characterization of G. duodenalis isolates has revealed the existence of eight groups (assemblages A to H) which differ in their host distribution. A cross-sectional study was conducted in 639 children from La Habana between January and December 2013. Two assemblage-specific PCRs were carried out for the molecular characterization. The overall prevalence of Giardia infection was 11.9%. DNA from 63 of 76 (82.9%) samples was successfully amplified by PCR-tpi, while 58 from 76 (76.3%) were detected by PCRE1-HF. Similar results by both PCRs were obtained in 54 from 76 samples (71%). According to these analyses, assemblage B and mixed assemblages A + B account for most of the Giardia infections in the cohort of children tested. Our current study identified assemblage B as predominant genotype in children infected with Giardia. Univariate analysis indicated that omission of washing hands before eating and keeping dogs at home were significant risk factors for a Giardia infection. In the future, novel molecular tools for a better discrimination of assemblages at the subassemblages level are needed to verify possible correlations between Giardia genotypes and symptomatology of giardiasis.

This paper focuses on the fiscal decentralization in the Philippines after the 1991 Local Government Code. It first examines the intergovernmental fiscal relationship between central and local governments by using fiscal decentralization indicators, and then investigates its impact on local finance. After fiscal decentralization, the local expenditure responsibility is expanded while the local fiscal capacity is not strengthened in the Philippines. Local governments consequently comes to depend heavily on fiscal transfers from the central government, internal revenue allotments (IRAs), which has a substantial influence on local finance. The heavy dependence on IRAs makes local finance unpredictable and unstable. The distribution of IRAs also affects the horizontal balance between provincial governments.

We review the current status of our knowledge of cosmic velocity fields, on both small and large scales. A new statistic is described that characterizes the incoherent, thermal component of the velocity field on scales less than 2h−1 Mpc (h is H0/100 km·s−1·Mpc−1, where H0 is the Hubble constant and 1 Mpc = 3.09 × 1022 m) and smaller. The derived velocity is found to be quite stable across different catalogs and is of remarkably low amplitude, consistent with an effective Ω ∼ 0.15 on this scale. We advocate the use of this statistic as a standard diagnostic of the small-scale kinetic energy of the galaxy distribution. The analysis of large-scale flows probes the velocity field on scales of 10–60 h−1 Mpc and should be adequately described by linear perturbation theory. Recent work has focused on the comparison of gravity or density fields derived from whole-sky redshift surveys of galaxies [e.g., the Infrared Astronomical Satellite (IRAS)] with velocity fields derived from a variety of sources. All the algorithms that directly compare the gravity and velocity fields suggest low values of the density parameter, while the POTENT analysis, using the same data but comparing the derived IRAS galaxy density field with the Mark-III derived matter density field, leads to much higher estimates of the inferred density. Since the IRAS and Mark-III fields are not fully consistent with each other, the present discrepancies might result from the very different weighting applied to the data in the competing methods.

Introdução: Infecções relacionadas à assistência de saúde (IRAS) representam hoje um dos principais desafios da qualidade do cuidado do paciente, principalmente em pacientes submetido a transplante de células tronco e hematopoiéticas (TCTH) O banho diário com a clorexidina (CHG) degermante a 2% tem sido proposto principalmente em unidades de terapia intensivas (UTIs) para diminuir a colonização bacteriana do paciente e assim diminuir IRAS. O objetivo deste estudo foi avaliar o impacto do banho com CHG degermante a 2% em unidade de internação de TCTH na incidência de infecção e colonização por patógenos multirresistentes e ainda avaliar seu impacto na sensibilidade das bactérias ao antisséptico. Métodos: Foi realizado um estudo quasi-experimental, com duração de 9 anos, com início em janeiro/2005 até dezembro/2013. A intervenção foi iniciada em agosto de 2009, sendo que os períodos pré e pós-intervenção tiveram duração de 4,5 anos. As taxas de IRAS, infecção por gram-negativos multirresistentes e infecção e colonização por enterococo resistente a vancomicina (VRE) foram avaliadas através de série temporal, para estudar o impacto da intervenção. As concentrações inibitórias mínimas (CIM) das bactérias para a CHG com e sem o inibidor de bomba de efluxo (CCCP) foram avaliadas nos dois períodos. Os genes de resistência a CHG foram estudados por meio da PCR e a clonalidade dos isolados por eletroforese em campo pulsátil. Resultados: Foi observada redução significativa na incidência de infecção e colonização de VRE na unidade no período pós-intervenção (p: 0,001). Essa taxa permaneceu estável em outras UTIs clínicas do hospital. Contudo as taxas de infecção por Gram negativos multirresistentes aumentou nos últimos anos na unidade. Não ocorreu diminuição na taxa de IRAS na unidade. As CIMs testadas de CHG aumentaram nas amostras de VRE e K. pneumoniae após o período de exposição ao antisséptico, com queda importante da CIM após o uso do CCCP, revelando ser a bomba de efluxo, um importante mecanismo de resistência à CHG. As amostras de A. baumannii e P. aeruginosa não apresentaram aumento da CIM após período de exposição à clorexidina. As bombas de efluxo Ade A, B e C estiveram presentes na maioria dos A. baumannii do grupo controle (66%). A bomba cepA foi encontrada em 67% de todas as K. pneumoniae testadas e em 44,5% das P. aeruginosas do grupo pré intervenção. Observamos uma relação positiva entre a presença da CepA nas amostras de K. pneumoniae e a resposta ao CCCP: de todas as 49 amostras CepA positivas 67,3% obtiveram redução do seu MIC em 4 diluições após adição do CCCP. A avaliação de clonalidade demonstrou padrão policlonal das amostras de VRE, K. pneumoniae e A. baumannii avaliadas. Em relação às amostras de P. aeruginosa foi observado que no período pós-intervenção ocorreu predominância de um clone com > 80% semelhança em 10 das 22 amostras avaliadas pelo dendrograma. Conclusões: O banho de clorexidina teve impacto na redução da incidência de infecção e colonização por VRE na unidade de TCTH, e não teve o mesmo impacto nas bactérias gram-negativas. Os mecanismos moleculares de resistência à clorexidina estão intimamente ligados à presença de bomba de efluxo, sendo provavelmente o principal mecanismo de resistência e tolerância das bactérias ao antisséptico

Na mesma proporção que fontes de água potável são reduzidas, a competição por elas tem aumentado, conseqüência imediata da expansão de áreas urbanas e de novas demandas agro-industriais. Os conflitos gerados por esta competição tendem a comprometer o desenvolvimento sustentável e o progresso de uma forma geral, uma vez que resultam em perda de eficácia para vultuosos investimentos públicos e privados e em prejuízos para os usuários menos estruturados. Considerando os fatos acima, o objeto deste trabalho é o estabelecimento de cenários de alocação das disponibilidades hídricas entre usuários de água da bacia do Sapucaí-Mirim/Grande, visando determinar a convivência harmoniosa das várias finalidades de uso provenientes do mesmo recurso hídrico. Tal meta poderá ser alcançada através da análise dos indicadores de desempenho - confiabilidade, vulnerabilidade e resiliência - do sistema os quais serão obtidos com o auxílio do modelo de simulação IRAS (Interactive River Aquifer Simulation).

Advances in neonatology resulted in reducing the mortality rate and the consequent increase in survival of newborn pre terms (PTN). On the other hand, there was also a considerable increase in the risk of developing health care-related infection (HAI) in its most invasive, especially for bloodstream. This situation is worrying, and prevent the occurrence of it is a challenge and becomes one of the priorities in the Neonatal Intensive Care Unit (NICU). Sepsis is the main cause of death in critical neonates and affects more than one million newborns each year, representing 40% of all deaths in neonates. The incidence of late sepsis can reach 50% in NICUs. Currently the major responsible for the occurrence of sepsis in developed countries is the coagulase negative Staphylococcus (CoNS), followed by S. aureus. The cases of HAIs caused by resistant isolates for major classes of antimicrobial agents have been increasingly frequent in the NICU. Therefore, vancomycin has to be prescribed more frequently, and, today, the first option in the treatment of bloodstream infections by resistant Staphylococcus. The objectives of this study were to assess the impact on late sepsis in epidemiology III NICU after the change of the use of antimicrobials protocol; check the frequency of multiresistant microorganisms; assess the number of neonates who came to death. This study was conducted in NICU Level III HC-UFU. three study groups were formed based on the use of the proposed late sepsis treatment protocol, with 216 belonging to the period A, 207 B and 209 to the C. The work was divided into three stages: Period A: data collected from neonates admitted to the unit between September 2010 to August 2011. was using treatment of late sepsis: with oxacillin and gentamicin, oxacillin and amikacin, oxacillin and cefotaxime. Period B: data were collected from March 2012 to February 2013. Data collection was started six months after protocol change. Due to the higher prevalence of CoNS, the initial protocol was changed to vancomycin and cefotaxime. Period C: data were collected from newborns inteerne in the unit from September 2013 to August 2014. Data collection was started six months after the protocol change, which occurred in March 2013. From the 632 neonates included in this study, 511 (80,8%) came from the gynecology and obstetrics department of the HC-UFU. The mean gestational age was 33 weeks and the prevailing sex was male (55,7%). Seventy-nine percent of the studied neonates were hospitalized at the NICU HC-UFU III because of complications related to the respiratory system. Suspicion of sepsis took to hospitalization in the unit of 1,9% of newborns. In general, the infection rate was 34,5%, and the most frequent infectious sepsis syndrome 81,2%. There was a tendency to reduce the number of neonates who died between periods A 11 and C (p = 0,053). From the 176 cases of late sepsis, 73 were clinical sepsis and 103 had laboratory confirmation, with greater representation of Gram positive bacteria, which corresponded to 67.2% of the isolates and CoNS the most frequent micro-organism (91,5%). There was a statistically significant difference in the reduction of isolation of Gram positive microorganisms between periods A and C (p = 0,0365) as well as in reducing multidrug-resistant CoNS (A and B period p = 0,0462 and A and C period, p = 0,158). This study concluded that: the CoNS was the main microorganism responsible for the occurrence of late sepsis in neonates in the NICU of HC-UFU; the main risk factors for the occurrence of late sepsis were: birth weight <1500 g, use of PICC and CUV, need for mechanical ventilation and parenteral nutrition, SNAPPE> 24 and length of stay more than seven days; the new empirical treatment protocol late sepsis, based on the use of vancomycin associated cefepime, it was effective, since promoted a reduction in insulation CoNS blood cultures between the pre and post implementation of the Protocol (A and C, respectively); just as there was a reduction in the number of newborns who evolved to death between periods A and C.

We present the stellar calibrator sample and the conversion from instrumental to physical units for the 24 μm channel of the Multiband Imaging Photometer for Spitzer (MIPS). The primary calibrators are A stars, and the calibration factor based on those stars is 4.54 × 10^-2 MJy sr^–1 (DN/s)^–1, with a nominal uncertainty of 2%. We discuss the data reduction procedures required to attain this accuracy; without these procedures, the calibration factor obtained using the automated pipeline at the Spitzer Science Center is 1.6% ± 0.6% lower. We extend this work to predict 24 μm flux densities for a sample of 238 stars that covers a larger range of flux densities and spectral types. We present a total of 348 measurements of 141 stars at 24 μm. This sample covers a factor of ~460 in 24 μm flux density, from 8.6 mJy up to 4.0 Jy. We show that the calibration is linear over that range with respect to target flux and background level. The calibration is based on observations made using 3 s exposures; a preliminary analysis shows that the calibration factor may be 1% and 2% lower for 10 and 30 s exposures, respectively. We also demonstrate that the calibration is very stable: over the course of the mission, repeated measurements of our routine calibrator, HD 159330, show a rms scatter of only 0.4%. Finally, we show that the point-spread function (PSF) is well measured and allows us to calibrate extended sources accurately; Infrared Astronomy Satellite (IRAS) and MIPS measurements of a sample of nearby galaxies are identical within the uncertainties.

Las infecciones relacionadas con la asistencia sanitaria (IRAS) suponen una complicación potencialmente grave que afecta a los pacientes hospitalizados. Además, se asocian con unas tasas de resistencia más altas en comparación con las infecciones adquiridas en la comunidad, y se observa una prevalencia preocupante de enterobacterias productoras de Betalactamasas de espectro extendido (BLEE). Los pacientes ingresados en el Servicio de Urología presentan riesgos específicos para el desarrollo de IRAS, tales como la frecuente necesidad de cateterismo de la vía urinaria y la realización de un procedimiento quirúrgico durante el ingreso. Aunque el correcto conocimiento de los factores de riesgo y de las características microbiológicas permite optimizar los resultados en el manejo de las IRAS, se han realizado pocos estudios para las IRAS en los pacientes ingresados en Unidades de Urología. La Odontología Pediátrica Contemporánea al igual que la Odontología de Mínima Intervención, comprende la máxima conservación de tejido dental subyacente con la finalidad de evitar la invasión a tejidos pulpares que derivan en tratamientos radicales tales como pulpotomías y pulpectomías; las técnicas de operatoria dental actuales comprenden en tratar el complejo dentino-pulpar lo más conservadoramente posible, con la finalidad de prevenir la extensión. Una de las propuestas del concepto de Mínima Intervención consiste en el uso de agentes químicos para la remoción de tejido cariado, auxiliado por instrumentos manuales que permitan remover solo la dentina infectada, sin necesidad de piezas rotatorias, reduciendo tiempos operatorios y costos. Objetivo: Evaluar cuál es el método más eficaz para eliminar la caries dental entre métodos químico-mecánicos para así establecer un protocolo de atención no invasivo para pacientes pediátricos y poder llevarlo a cabo en las campañas de brigadas Odontológicas, para que un mayor número de población sea beneficiada con éstas. Materiales y Métodos: Se evaluaron 10 piezas posteriores deciduas, extraídas de niños de 6 a 8 años que cumplían con los criterios de inclusión de la Maestría en Ciencias Odontológicas en el área de Odontopediatría de la UANL, Monterrey, Nuevo León, México. Las piezas a evaluar fueron seccionadas y tratadas con Carisolv y Papacárie, ambos productos de remoción químico mecánicos, mientras se medía el tiempo de trabajo y la calidad de dentina remanente tras la remoción de caries para posteriormente describir los hallazgos observados.
