Based on a viewpoint of an intricate system demanding high, this thesis advances a new concept that urban sustainable development stratagem is a high harmony and amalgamation among urban economy, geo-environment and tech-capital, and the optimum field of which lies in their mutual matching part, which quantitatively demarcates the optimum value field of urban sustainable development and establishes the academic foundation to describe and analyze sustainable development stratagem. And establishes a series of cause-effect model, a analysissitus model, flux model as well as its recognizing mode for urban system are established by the approach of System Dynamics, which can distinguish urban states by its polarity of entropy flows. At the same time, the matter flow, energy flow and information flow which exist in the course of urban development are analyzed based on the input/output (I/O) relationships of urban economy. And a new type of I/O relationships, namely new resources-environment account, are established, in which both resource and environment factors are considered. All above that settles a theoretic foundation for resource economy and environment economy as well as quantitative relationships of inter-function between urban development and geoenvironment, and gives a new approach to analyze natinal economy and urban sustainable development. Based on an analysis of the connection between resource-environmental construct of geoenvironment and urban economy development, the Geoenvironmental Carrying Capability (GeCC) is analyzed. Further more, a series of definitions and formula about the Gross Carrying Capability (GCC), Structure Carrying Capability (SCC) and Impulse Carrying Capability (ICC) is achieved, which can be applied to evaluate both the quality and capacity of geoenvironment and thereunder to determine the scale and velocity for urban development. A demonstrative study of the above is applied to Beihai city (Guangxi province, PRC), and the numerical value laws between the urban development and its geoenvironment is studied by the I/O relationship in the urban economy as following: · the relationships between the urban economic development and land use as well as consumption of underground water, metal mineral, mineral energy source, metalloid mineral and other geologic resources. · the relationships between urban economy and waste output such as industrial "3 waste", dust, rubbish and living polluted water as well as the restricting impact of both resource-environmental factors and tech-capital on the urban grow. · Optimization and control analysis on the reciprocity between urban economy and its geoenvironment are discussed, and sensitive factors and its order of the urban geoenvironmental resources, wastes and economic sections are fixed, which can be applied to determine the urban industrial structure, scale, grow rate matching with its geoenvironment and tech-capital. · a sustainable development stratagem for the city is suggested.

Guangxi Longtan Hydropower Station is not only a representative project of West Developing and Power Transmission from West to East in China, but also the second Hydropower Station to Three Gorges Project which is under construction in China. There are 770 X 104m3 creeping rock mass on the left bank slope in upper reaches, in which laid 9 water inlet tunnels and some underground plant buildings. Since the 435m high excavated slope threatens the security of the Dam, its deformation and stability is of great importance to the power station.Based on the Autodesk Map2004, Longtan Hydropower Station Monitoring Information System on Left Bank has been basically finished on the whole. Integrating the hydropower station monitoring information into Geographic Information System(GIS) environment, managers and engineers can dynamically gain the deformation information of the slop by query the symbols. By this means, designers can improve the correctness of analysis, and make a strategic and proper decision. Since the system is beneficial to effectively manage the monitoring-data, equitably save the cost of design and safe construction, and decrease the workload of the engineers, it is a successful application to the combination of hydropower station monitoring information management and computer information system technology.At the same time, on the basis of the geological analysis and rock mass toppling deformation and failure mechanism analysis of Longtan engineering left bank slope, the synthetic space-time analysis and influence factors analysis on the surface monitoring data and deep rock mass monitoring data of A-zone on left bank slope are carried on. It shows that the main intrinsic factor that effects the deformation of Zone A is the argillite limestone interbedding toppling structure, and its main external factors are rain and slope excavation. What's more, Degree of Reinforcement Demand(DRD) has been used to evaluate the slop reinforce effect of Zone A on left bank according to the Engineering Geomechanics-mate-Synthetics(EGMS). The result shows that the slop has been effective reinforced, and it is more stable after reinforce.At last, on the basis of contrasting with several forecast models, a synthetic forecast GRAV model has been presented and used to forecast the deformation of zone A on left bank in generating electricity period. The result indicates that GRAV model has good forecast precision, strong stability, and practical valuable reliability.

Purpose: To evaluate preoperative characteristics and follow-up in rural China after trabeculectomy, the primary treatment for glaucoma there. Methods: Patients undergoing trabeculectomy at 14 rural hospitals in Guangdong and Guangxi Provinces and their doctors completed questionnaires concerning clinical and sociodemographic information, transportation, and knowledge and attitudes about glaucoma. Follow-up after surgery was assessed as cumulative score (1 week: 10 points, 2 weeks: 7 points, 1 month: 5 points). Results Among 212 eligible patients, mean preoperative presenting acuity in the operative eye was 6/120, with 61.3% (n=130) blind (≤6/60). Follow-up rates were 60.8% (129/212), 75.9% (161/212) and 26.9% (57/212) at 1 week, 2 weeks and 1 month, respectively. Patient predictors of poor follow-up included elementary education or less (OR=0.37, 95% CI 0.20 to 0.70, p=0.002), believing follow-up was not important (OR=0.62, 95% CI 0.41 to 0.94, p=0.02), lack of an accompanying person (OR=0.14, 95% CI 0.07 to 0.29, p<0.001), family annual income <US$800 (OR=0.28, 95% CI 0.11 to 0.72, p=0.008) and not requiring removal of scleral flap sutures postoperatively (OR=0.11, 95% CI 0.06 to 0.22, p<0.001). Age, sex, employment, travel distance/time/costs, patient preoperative clinical factors and physician factors were unassociated with follow-up. Conclusions: Follow-up after 2 weeks was inadequate to provide optimal clinical care, and surgery is being applied too late to avoid blindness in the majority of patients. Earlier surgery, support for return visits and better explanations of the importance of follow-up are needed. Directing all patients to return for possible scleral flap suture removal may be a valid strategy to improve follow-up.

L’histoire de la médicalisation de la maternité en Chine reste encore mal connue et ce mémoire constitue une amorce pour tenter de défricher ce riche et vaste terrain. Il examine dans quel cadre et dans quelle mesure la prise en charge de la maternité des femmes chinoises a évolué au sein des postes médicaux consulaires français du sud de la Chine (Guangdong, Guangxi, Yunnan), de l’arrivée des premiers médecins en 1898, jusqu’à la veille de la Seconde Guerre mondiale en 1938. Il démontre comment a pu se traduire l’œuvre médicale française en matière de prise en charge de la grossesse, de l’accouchement et des soins à donner au nouveau-né dans les établissements de santé consulaires, et tente de voir jusqu’à quel point, pourquoi et dans quels domaines précisément l’offre de soins à l’occidentale proposée par les Français dans ces régions a pu atteindre les futures et nouvelles mères chinoises.

Los hospitales son el pilar fundamental para la atención de las víctimas de situaciones de emergencia o desastre. Las instituciones de salud son consideradas indispensables para la población, por lo que deben estar preparadas para funcionar no solo en condiciones normales sino en situaciones de alerta, como suele suceder en desastres de origen natural como los sismos. La relevancia de la problemática, radica en que de acuerdo con la amenaza sísmica de Colombia, Bogotá se encuentra en una zona de amenaza sísmica intermedia, los actuales eventos naturales como el terremoto de Haití con un saldo de 300.000 muertos y más de 700.000 mil heridos, y el de Chile, nos hace pensar en la importancia de la preparación hospitalaria ante un evento con múltiples víctimas como lo es un sismo. El Objetivo general del estudio es identificar la capacidad de respuesta hospitalaria distrital en Bogotá ante un evento con múltiples víctimas (terremoto). Además se identificaran las oportunidades de mejora para optimizar la respuesta hospitalaria de acuerdo a su nivel de atención. La Investigación se realizó por medio de un estudio de corte transversal, en donde se tomó una muestra de la red hospitalaria Distrital por conveniencia, bajo la aplicación de una encuesta dirigida. Los resultados fueron recopilados en una base de datos de Excel 2013, y fueron analizados bajo un software estadístico, STATA 12.0, donde se evaluaron variables, categóricas, nominales y cuantitativas. Como resultados se encontraron un porcentaje de ocupación de más del 100% en el 25% de los hospitales. Los 16 hospitales encuestados cuentan con comité hospitalario de emergencias, así como también con la elaboración de planes de emergencia y la revisión e implementación de estos. El 50% de los hospitales contemplan dentro su estructura de plan de emergencias, el sistema comando de incidentes; Sólo el 18.8% de los hospitales cuentan con reforzamiento estructural, y el 81,2% de los hospitales refieren tener una cooperación con organizaciones locales o externas. Solo 4 de los 16 hospitales cuentan con protocolos de diagnóstico y tratamiento médico en desastres. El plan de contingencia para un Sismo, es el plan bandera de todos los hospitales dado el esfuerzo por parte de la secretaria de salud y de FOPAE en información y capacitación en todo el Distrito, es por eso que el 93,85 de todos los hospitales cuentan con este plan. Al realizar el análisis general, la red hospitalaria no está en capacidad de una adecuada respuesta en caso de un evento con múltiples víctimas, en el escenario de un sismo de gran magnitud, teniendo en cuenta el porcentaje de ocupación actual donde el 25% de la red hospitalaria distrital cuenta con sobrecupo y el 50% se encuentra a tope de su capacidad instalada. En cuanto a la capacidad de respuesta, no se cuenta con protocolos de atención; Haciendo una evaluación según los niveles de atención, solo los hospitales de III nivel estarían medianamente preparados y con capacidad de respuesta ante un evento con víctimas en masa.

Given the background of serious urban pollution in Hong Kong, the intake fraction (iF) of carbon monoxide due to mobile vehicles in urban area of Hong Kong is investigated and estimated to be 600 per million, much higher than those in US urban areas, Helsinki and even Beijing, indicating the high exposure level to urban pollutants in Hong Kong. The dependence of iF to the metrological factors is also discussed. Easterly and northerly winds contribute most to the total iF value. A new method of predicting ventilation rate for a city based on iF concept is proposed. City ventilation rates for different cities are calculated and compared. It is found that Hong Kong has to face the fact that it has the lowest ventilation rate and ACH.

Triassic turbidites of the Nanpanjiang basin of south China represent the most expansive and voluminous siliciclastic turbidite accumulation in south China. The Nanpanjiang basin occurs at a critical junction between the southern margin of the south China plate and the Indochina, Siamo and Sibumasu plates to the south and southwest. The Triassic Yangtze carbonate shelf and isolated carbonated platforms in the basin have been extensively studied, but silicilastic turbidites in the basin have received relatively little attention. Deciphering the facies, paleocurrent indicators and provenance of the Triassic turbidites is important for several reasons: it promises to help resolve the timing of plate collisions along suture zones bordering the basin to the south and southwest, it will enable evaluation of which suture zones and Precambrian massifs were source areas, and it will allow an evaluation of the impact of the siliciclastic flux on carbonate platform evolution within the basin. Turbidites in the basin include the Early Triassic Shipao Formation and the Middle-Late Triassic Baifeng, Xinyuan, Lanmu Bianyang and Laishike formations. Each ranges upward of 700 m and the thickest is nearly 3 km. The turbidites contain very-fine sand in the northern part of the basin whereas the central and southern parts of the basin also commonly contain fine and rarely medium sand size. Coarser sand sizes occur where paleocurrents are from the south, and in this area some turbidites exhibit complete bouma sequences with graded A divisions. Successions contain numerous alternations between mud-rich and sand-rich intervals with thickness trends corresponding to proximal/ distal fan components. Spectacularly preserved sedimentary structures enable robust evaluation of turbidite systems and paleocurrent analyses. Analysis of paleocurrent measurements indicates two major directions of sediment fill. The northern part of the basin was sourced primarily by the Jiangnan massif in the northeast, and the central and southern parts of the basin were sourced primarily from suture zones and the Yunkai massif to the south and southeast respectively. Sandstones of the Lower Triassic Shipao Fm. have volcaniclastic composition including embayed quartz and glass shards. Middle Triassic sandstones are moderately mature, matrix-rich, lithic wackes. The average QFL ratio from all point count samples is 54.1/18.1/27.8% and the QmFLt ratio is 37.8/ 18.1/ 44.1%. Lithic fragments are dominantly claystone and siltstone clasts and metasedimentary clasts such as quartz mica tectonite. Volcanic lithics are rare. Most samples fall in the recycled orogen field of QmFLt plots, indicating a relatively quartz and lithic rich composition consistent with derivation from Precambrian massifs such as the Jiangnan, and Yunkai. A few samples from the southwest part of the basin fall into the dissected arc field, indicating a somewhat more lithic and feldspar-rich composition consistent with derivation from a suture zone Analysis of detrial zircon populations from 17 samples collected across the basin indicate: (1) Several samples contain zircons with concordant ages greater than 3000 Ma, (2) there are widespread peaks across the basin at 1800 Ma and 2500, (3) a widespread 900 Ma population, (3) a widespread population of zircons at 440 Ma, and (5) a larger population of younger zircons about 250 Ma in the southwestern part which is replaced to the north and northwest by a somewhat older population around 260-290 Ma. The 900 Ma provenance fits derivation from the Jiangnan Massif, the 2500, 1800, and 440 Ma provenance fits the Yunkai massif, and the 250 Ma is consistent with convergence and arc development in suture zones bordering the basin on the south or southwest. Early siliciclastic turbidite flux, proximal to source areas impacted carbonate platform evolution by infilling the basin, reducing accommodation space, stabilizing carbonate platform margins and promoting margin progradation. Late arrival, in areas far from source areas caused margin aggradation over a starved basin, development of high relief aggradational escarpments and unstable scalloped margins.

Importance:
Follow-up after trabeculectomy surgery is important to surgical success, but little is known about the effect of interventions on improving follow-up in low-resource areas.
Objective:
To examine whether text message reminders and free eye medications improve follow-up after trabeculectomy in rural southern China.
Design, Setting, and Participants:
This randomized clinical trial studied 222 consecutive patients undergoing trabeculectomy from October 1, 2014, through November 31, 2015, at 4 rural hospitals in Guangdong and Guangxi Provinces, China. Data from the intention-to-treat population were analyzed.
Interventions:
Patients undergoing trabeculectomy were randomized (1:1) to receive text message reminders 3 days before appointments at 1 and 2 weeks and 1 month after surgery and free topical corticosteroid medication (US$5.30) at each visit or to standard follow-up without reminders or free medication.
Main Outcomes and Measure:
Follow-up at 1 month postoperatively.
Results:
Among 222 eligible patients, 13 (5.9%) refused and 209 (94.1%) were enrolled, with 106 (50.7%) randomized to the intervention group (mean [SD] age, 64.4 [12.7] years; 56 women [52.8%]) and 103 (49.3%) to the control group (mean [SD] age, 63.0 [12.7] years; 53 women [51.5%]). A total of 6 patients (2.9%) were unavailable for follow-up. Attendance at 1 month for the intervention group (59 of 102 [57.8%]) was significantly higher than for the control group (34 of 101 [33.7%]) (unadjusted relative risk [RR], 1.72; 95% CI, 1.13-2.63; P = .01). Factors associated with 1-month attendance in multiple regression models included intervention group membership (RR, 1.65; 95% CI, 1.08-2.53; P = .02) and being told to return for suture removal (RR, 1.80; 95% CI, 1.06-3.06; P = .03). One-month attendance among controls not told about suture removal was 3 of 31 (9.7%), whereas it was 44 of 68 (64.7%) among the intervention group with suture removal (unadjusted RR, 6.69; 95% CI, 2.08-21.6; P = .001).
Conclusions and Relevance:
In this setting, low-cost interventions may significantly improve postoperative follow-up after glaucoma surgery, a potential opportunity for interventions known to improve surgical success.
