BACKGROUND E-learning and blended learning approaches gain more and more popularity in emergency medicine curricula. So far, little data is available on the impact of such approaches on procedural learning and skill acquisition and their comparison with traditional approaches. OBJECTIVE This study investigated the impact of a blended learning approach, including Web-based virtual patients (VPs) and standard pediatric basic life support (PBLS) training, on procedural knowledge, objective performance, and self-assessment. METHODS A total of 57 medical students were randomly assigned to an intervention group (n=30) and a control group (n=27). Both groups received paper handouts in preparation of simulation-based PBLS training. The intervention group additionally completed two Web-based VPs with embedded video clips. Measurements were taken at randomization (t0), after the preparation period (t1), and after hands-on training (t2). Clinical decision-making skills and procedural knowledge were assessed at t0 and t1. PBLS performance was scored regarding adherence to the correct algorithm, conformance to temporal demands, and the quality of procedural steps at t1 and t2. Participants' self-assessments were recorded in all three measurements. RESULTS Procedural knowledge of the intervention group was significantly superior to that of the control group at t1. At t2, the intervention group showed significantly better adherence to the algorithm and temporal demands, and better procedural quality of PBLS in objective measures than did the control group. These aspects differed between the groups even at t1 (after VPs, prior to practical training). Self-assessments differed significantly only at t1 in favor of the intervention group. CONCLUSIONS Training with VPs combined with hands-on training improves PBLS performance as judged by objective measures.

This appraisal of David Scott FitzGerald and David Cook-Martín's Culling the Masses: The Democratic Origins of Racist Immigration Policy in the Americas argues that there is no ‘elective affinity’ between liberalism and racism, which is the core argument of the book. The notion of ‘elective affinity’, which the authors borrow from Max Weber, requires a structural homology between the ‘electively’ related elements that just does not exist in this case. The relationship between both is entirely contingent, ‘racism’ being a doctrine of inter-group relations while ‘liberalism’ is a doctrine of intra-group relations, with no consideration of how the boundaries of the group are constituted.

Introducción. Las enfermedades cardiovasculares (ECV) son la principal causa de muerte en nuestro país. Entre los factores independientes más importantes para el desarrollo de ECV se encuentran en primer lugar las alteraciones del perfil lipídico como el aumento del colesterol total (TC), las lipoproteínas de baja densidad (LDL) y los triglicéridos (TG) y/o la disminución de las lipoproteínas de alta densidad (HDL). Entre las diferentes formas de abordar el problema para prevenir y tratar estas patologías se encuentra la modificación de los hábitos de vida a través de un programa combinado de dieta y ejercicio. La evidencia confirma la efectividad de la primera variable, sin embargo, en el empleo del ejercicio se encuentran discrepancias acerca de cuál es el modo más eficaz para mejorar el perfil lipídico. Objetivo. Estudiar los cambios en las variables del perfil lipídico y los índices lipoproteicos comparando cuatro tipos de intervención que combinan dieta y diferentes modos de ejercicio, así como, analizar otras variables independientes (género, edad y genotipo ApoE) que pueden tener influencia sobre estos cambios. Diseño de la investigación. Los datos analizados en esta tesis forman parte del estudio “PROgramas de Nutrición y Actividad Física para el tratamiento de la obesidad” (PRONAF). Se trata de un estudio clínico desarrollado en España entre el 2008 y el 2011. La metodología del estudio nos permite comparar cuatro tipos de intervención para la pérdida de peso y evaluar su impacto sobre el perfil lipídico. El diseño fue experimental aleatorizado donde a todos los participantes se les sometió a un programa de dieta equilibrada hipocalórica junto a uno de los tres modos de ejercicio (grupo de entrenamiento de fuerza, grupo de entrenamiento de resistencia y grupo de entrenamiento combinado de los modos anteriores; los cuales fueron igualados en volumen e intensidad) o grupo de recomendaciones de actividad física. Las principales variables analizadas en los estudios que comprende esta tesis doctoral fueron: HDL, LDL, TG y TC, los índices derivados de estas y variables de la composición corporal y del entrenamiento. Conclusiones. Los cuatro tipos de intervención mostraron ser favorables para mejorar las variables del perfil lipídico y los índices lipoproteicos, sin diferencias significativas entre ellos. Tras la intervención, los varones mostraron una respuesta más favorable en los cambios del perfil lipídico. El genotipo ApoE2 obtuvo una reducción mayor en la concentración de TG y TC que el genotipo ApoE3 y ApoE4. Por último, los índices lipoproteicos mejoraron tras un programa de pérdida de peso, obteniéndose mayores cambios en el grupo de dieta más entrenamiento aeróbico para los índices ApoB/ApoA-1, TG/HDL y LDL/ApoB. ABSTRACT Introduction. The main cause of death in our country is cardiovascular disease (CVD). The most important independent factors for the development of CVD are the lipid profile alterations: increased total cholesterol (TC), low density lipoprotein (LDL) and triglycerides (TG) and/or decreased high-density lipoprotein (HDL). Among the different approaches to prevent and treat these diseases is modifying the lifestyle combining a diet and exercise program. The evidence confirms the effectiveness of the first variable, however, there is still controversy about the most effective mode of exercise combined with diet to achieve improvements. Objective. To study changes in lipoprotein profile comparing four types of intervention combining diet with different modes of exercise, and to analyze the independent variables (gender, age, and ApoE genotype) that can influence these changes. Research design. The data analized in this thesis are part of the study Nutrition and Physical Activity Programs for Obesity Treatments (the PRONAF study according to its Spanish initials). This is a clinical research carried out in Spain between 2008 and 2011. The aim of this study was to compare four types of intervention to weight loss with diet combining exercise. The design was experimental randomized where all participants were subjected to follow a hypocaloric balanced diet along one of the three modes of exercise (strength training group, resistance training group and combined training group of the above modes, which were matched by volume and intensity) or physical activity recommendations group. The main variables under investigation in this thesis were: HDL, LDL, TG and TC, the lipoprotein ratios, body composition and training variables. Main outcomes. The four types of interventions shown to be favorable to improve the lipid profile and lipoprotein level, with no significant differences between intervention groups. After the intervention, the men showed a more favorable respond in lipid profile changes. The genotype ApoE2 obtained more positive changes in the concentration of TG and TC than ApoE3 and ApoE4 genotype. Last, the lipoprotein ratios improve after weight loss treatment with diet combined different modes exercise. Our results reflected greater changes for E group in apoB/ApoA1, TG/HDL and LDL/ApoB compared within different intervention groups.

O método adotado foi a pesquisa quanti- qualitativa numa abordagem de estudo de caso. O local do estudo foi às três (03) residências terapêuticas localizadas no município de Santo André. Todos os 25 (vinte e cinco) moradores foram sujeitos da pesquisa, utilizamos como instrumentos: entrevista semi-estruturada, livro de anotações diárias e os prontuários. Os resultados forma analisados segundo Bardin (1979) construindo categorias que demonstrassem com clareza: o perfil sócio demográfico dos egressos de hospitais psiquiátricos, e demonstrasse os indicadores de qualidade e autonomia. Procurando focar a implementação do processo de reabilitação psicossocial com estes moradores.

CONTEXTO: A nossa investigação estuda o stress no trabalho e a sua relação com a saúde mental. Descrevemos fatores específicos e de risco psicossocial no trabalho, particularmente no trabalho dos enfermeiros, e as suas implicações para a saúde mental e para o bem-estar biopsicossocial, tais como: Tipo de trabalho; Conteúdo do trabalho; Desempenho de papel; Relações interpessoais e grupais; Desenvolvimento da carreira; Novas tecnologias e Aspetos organizacionais. OBJETIVO(S): O objetivo fundamental foi estudar a influência de algumas variáveis pessoais e situacionais de risco biopsicossocial na saúde mental e no bem-estar dos profissionais de saúde, em contexto hospitalar METODOLOGIA: Trata-se de um estudo quantitativo, transversal e descritivo, do tipo correlacional. A recolha de informação obedeceu a um protocolo constituído por dados pessoais e as escalas: Satisfação geral do trabalho, Questionário geral de saúde, Questionário de saúde, Escala de fadiga crónica, Escala de ansiedade cognitiva-somática, Inventário de personalidade de Eysenck, Inventário clínico de autoconceito, Inventário de resolução de problemas, Questionário de vulnerabilidade ao stress e Questionário de stress ao trabalho. A amostra foi não probabilística intencional, constituída por 570 enfermeiros, a laborarem por turnos e em regime normal, no contexto hospitalar, perfazendo, no final, 360 enfermeiros. RESULTADOS: Os principais resultados apontam o seguinte: Os enfermeiros manifestam algum descontentamento com o ambiente de trabalho; A globalidade da amostra demonstra índices de saúde baixos; Regra geral, todos os enfermeiros estão vulneráveis ao stress; Em relação ao stress produzido pelas circunstâncias organizacionais, a totalidade da amostra revela elevados índices de stress e o seu bem-estar biopsicossocial, manifestamente afetado. CONCLUSÕES: Destes resultados fomos levados a concluir que quanto maior for a fadiga crónica, o neuroticíssimo e a ansiedade cognitiva, maior será a tendência dos enfermeiros para diminuírem a autorresponsabilização e o medo. Esta relação pode tornar-se circular e levar a comportamentos desajustados como, por exemplo, indiferença, desinteresse, relações interpessoais conflituosas, entre outros aspetos. As consequências de tais comportamentos poderão traduzir-se em absentismo, erros de desempenho ou vontade de abandonar a instituição.

This is the story of an extraordinary Aboriginal woman, Princy Carlo, and the identity of place she and her descendants fashioned within the confines of the Aboriginal settlement of Cherbourg (formerly Barambah), during the early twentieth century. The patch of Cherbourg that came to be known as 'Chinatown' has to date attracted cursory reference in historical commentary on the south-eastern Queensland Aboriginal settlement. Yet, hidden beneath what may appear as an inconsequential historical detail lies a fascinating illustration of the negotiation of place identity within a frame of triangulated group relations (Aboriginal-Chinese-White) in what remained, in essence, a colonial society. Incorporating primary written sources and oral accounts from descendants the study analyses the forging of the Chinatown identity of place through a process of 'spatial othering', eliciting features unique to this indigenous identity-construct. The study provides an insight into Aboriginal connection and kinship with land following forced removal to a government settlement, and contributes to the historical records of the Cherbourg Aboriginal community and the Eidsvold district in Queensland, Australia. We investigated whether juvenile freshwater stingrays (Potamotrygon motoro) can solve spatial tasks by constructing a cognitive map of their environment. Two experimental conditions were run: allocentric and ego-allocentric. Rays were trained to locate food within a four-arm maze placed in a room with visual spatial cues. The feeding location (goal) within the maze (room) remained constant while the starting position varied for the allocentrically but not for the ego-allocentrically trained group. After training, all rays solved the experimental tasks; however, different orientation strategies were used within and between groups. Allocentrically trained rays reached the goal via novel routes starting from unfamiliar locations, while ego-allocentrically trained rays primarily solved the task on the basis of an egocentric turn response. Our data suggest that P. motoro orients by constructing a visual cognitive map of its environment, but also uses egocentric and/or other orientation strategies alone or in combination for spatial orientation, a choice which may be governed by the complexity of the problem. We conclude that spatial memory functions are a general feature of the vertebrate brain.

People and their performance are key to an organization's effectiveness. This review describes an evidence-based framework of the links between some key organizational influences and staff performance, health and well-being. This preliminary framework integrates management and psychological approaches, with the aim of assisting future explanation, prediction and organizational change. Health care is taken as the focus of this review, as there are concerns internationally about health care effectiveness. The framework considers empirical evidence for links between the following organizational levels: 1. Context (organizational culture and inter-group relations; resources, including staffing; physical environment) 2. People management (HRM practices and strategies; job design, workload and teamwork; employee involvement and control over work; leadership and support) 3. Psychological consequences for employees (health and stress; satisfaction and commitment; knowledge, skills and motivation) 4. Employee behaviour (absenteeism and turnover; task and contextual performance; errors and near misses) 5. Organizational performance; patient care. This review contributes to an evidence base for policies and practices of people management and performance management. Its usefulness will depend on future empirical research, using appropriate research designs, sufficient study power and measures that are reliable and valid.

Interregionalism is deeply rooted in the foreign policies and external relations of the EU. Interregional cooperation today not only encompasses trade and aid but also political dialogue, cultural relations and even security cooperation. Although the EU’s official ambition has been to formalize and institutionalize its interregional relations with other regional bodies or organizations (so-called ‘pure interregionalism’), in practice there are a bewildering variety of interregional or group-to-group relations on display (Hänggi 2006; Baert et al. 2014). The EU is rapidly evolving as a global actor and while doing so it has been trying to export its own civilian and normative values. Interregionalism is an important tool in this process, contributing to the EU’s policy of fostering regionalism worldwide, not only in the triad (Europe, North America and East Asia) (De Lombaerde and Schulz 2009). Through interregionalism, the EU and its regional others enhance their presence, gain recognition, tighten institutional cohesion and define identities. Interregionalism, therefore, occupies a special position in the construction of regional actorness in global affairs (Wunderlich 2012). However, the link between interregionalism and regionalism is both complex and underexplored (Baert et al. 2014; Doidge 2007). Much depends on the type of interregional relations, and the balance of other forms of cooperation, which appears to play out differently in different regions. All this leads to a number of research questions that should be addressed by the academic literature, including is there a preference for interregional relations in EU’s foreign policy? If so, for what reason(s)? What are the consequences of such a preference? What is the role of interregionalism in the broader context of EU’s external policies? How are expressions of regionalism related to expressions of interregionalism? Does the sui generis character of the EU lead to a sui generis character of EU interregionalism? This chapter provides a general overview of the evolution of the field, the key conceptual and analytical debates, as well as the main research questions that drive the research agenda. Emphasis is also placed on identifying the main gaps in the field and suggesting directions.

The purpose of this study was to determine which of the two methods is more appropriate to teach pitch discrimination to Grade 6 choral students to improve sight-singing note accuracy. This study consisted of three phases: pre-testing, instruction and post-testing. During the four week study, the experimental group received training using the Kodaly method while the control group received training using the traditional method. The pre and post tests were evaluated by three trained musicians. The analysis of the data utilized an independent t-test and a paired t-test with the methods of teaching (experimental and control) as a factor. Quantitative results suggest that the experimental subjects, those receiving Kodaly instruction at post-treatment showed a significant improvement in the pitch accuracy than the control group. The specific change resulted in the Kodaly method to be more effective in producing accurate pitch in sight-singing.

Background: Cardiac Rehabilitation (CR) has effect on mortality in patients with heart failure (HF) chronic, and the exercise of the treatment of this patient. The most common exercise is ongoing training. Recently we have been studying the effects of interval training, but there is no consensus on the optimal dose of exercise. Objective: To evaluate the effects of interval aerobic training are superior to continuous aerobic training in patients with chronic HF. Methods: The clinical trial evaluated patients through cardiopulmonary test (CPX) and quality of life before and after the RC (3 times / 12 weeks). Patients were randomized into Group Interval Training (GTI - 85% of heart rate reserve - FCR), Continuous Training Group (GTC - 60% of HRR) and control group (CG) who received guidelines. Results: 18 patients were evaluated (mean age 44.7 ± 13.2 years and 35.2 ± 8.9% of left ventricular ejection fraction [LVEF]). Both groups were efficient to increase the peak VO2 and 15.1% (P = 0.02) in GTI and 16.1% (P = 0.01) GTC. As for the quality of life the GTI GTC showed improvement compared to the control group (P = 0.006). Hemodynamic mismatch events during the CPX were reduced after training in more GTC (patients 1 to 4) than in the GTI (5 to 3). Cardiac risk also decreased in the GTC (3 patients left the severe risk to take after training). Conclusion: Continuous training becomes more appropriate for improving fitness with little chance of developing cardiac event patients with chronic HF.
