A TTM (Malária Transmitida por Transfusão) ocorre através de qualquer componente de sangue que contenha eritrócitos que possam abrigar parasitas viáveis, porém, casos envolvendo plaquetas, leucócitos e plasma congelado têm sido reportado. As principais causas de relatos de TTM é a presença de baixa parasitemias em pacientes assintomáticos. O objetivo deste trabalho foi verificar a eficiência do ELISA-Malaria Antigen Test, que faz a captura do antígeno pLDH (Lactato Desidrogenase Plasmodial), como método de triagem de malária em doadores de sangue. Para a realização deste estudo selecionamos 1670 amostras das Unidades de Coleta e Transfusão (UCTs) das cidades do interior Estado do Pará: Altamira, Castanhal, Marabá, Santarém e Tucuruí e da capital Belém, sendo que desta tivemos amostras de um outro grupo, os de doadores que estiveram em zonas endêmicas 30 dias antes da doação. As amostras foram coletadas em duas estações (seca e chuva). Observou-se que o ELISA-Malaria Antigen Test é um teste sensível e prático, porém ajustes ainda devem ser feitos no ponto de corte estabelecido pelo fabricante, pois de acordo com a sua faixa só foi possível a detecção de 0,42% de amostras positivas e quando o ponto de corte foi ajustado com amostras do grupo controle, esse número passou para 4,37%. Não foi encontrada diferença sazonal no número de casos positivos e negativos, mas quando analisamos os ΔDO/Cutoff das amostras de todas as cidades, observamos diferenças significativas, o que demonstrou que a cidade de Belém possuía a menor possibilidade de encontro de casos de malária do que nas amostras das cidades de Altamira, Santarém e nas da Zona Endêmica. Os casos de TTM, embora não sejam comuns dentro de áreas não endêmicas, podem ocorrer, pois os doadores implicados em TTM invariavelmente apresentam baixas parasitemia, o que dificulta a detecção dos parasitas pelos métodos disponíveis. Assim, para uma boa triagem de doadores, estratégias efetivas devem ser criadas, incluindo triagens clínico-epidemiológicas, associadas a ferramentas sorológicas eficazes para assegurar a qualidade do sangue disposto à doação.

Hidrocarbonetos policíclicos aromáticos (HPAs) são compostos orgânicos originários de fontes naturais e antrópicas e, por apresentarem potencial carcinogênico e mutagênico, são considerados poluentes prioritários por agências ambientais. Desta forma, métodos analíticos para investigação de tais compostos que sejam rápidos e de baixo custo são de relevância considerável para o monitoramento ambiental. O presente trabalho teve como objetivos otimizar um método analítico para HPAs utilizando cromatografia líquida com detecção por arranjo de diodos (HPLC-DAD) e aplicar em testemunho sedimentar de região estuarina. Para otimização e avaliação do método, uma coluna sedimentar de 46 cm de comprimento foi coletada na foz do Rio Tucunduba (Belém, Pará) e seccionada em porções de 2 cm (subamostras). Após secagem, 30 g de cada porção foram extraídos com mistura de diclorometano em acetona (1:1) em ultrassom por 40 min. Os extratos obtidos foram centrifugados, purificados em sílica-gel, adaptação em funil necessária principalmente para reter partículas finas, e em seguida concentrados em rotaevaporador à vácuo e, por fim, filtrados com membrana de nylon 0,22 μm antes da injeção no HPLC. Amostras fortificadas com padrões analíticos de 16 HPAs e brancos também foram processados da mesma maneira. Um conjunto de parâmetros para validação do método foi investigado e observou-se: (1) boa linearidade: as curvas de calibração (analíticas) apresentaram coeficientes de correlação elevadas; (2) precisão adequada: obteve-se desvio padrão relativo dentro do aceitável, sendo o mínimo de 2,1% para acenaftileno e máximo de 19,7% para o fluoranteno; (3) limites de detecção baixos: entre 0,004 a 1,085 ng g g-1, viabilizando análises em concentrações reais in situ; (4) recuperação adequada para traços: sendo a mínima de 40,0% para o acenaftileno e máxima de 103,1% para o benzo(k)fluoranteno. As concentrações de HPAs totais variaram nas seções do testemunho sedimentar entre 60,77 - 783,3 ng g-1 de sedimento seco. O método otimizado mostrou-se vantajoso com relação aos tradicionais que utilizam extrator soxhlet e colunas de adsorventes para purificação de extratos por minimizar o tempo de extração e reduzir custos com uso de volumes menores de solventes para purificação do extrato. A limitação do método, porém, foi a coeluição do criseno e do benzo(a)antraceno e a sobreposição do fluoreno e acenafteno, além da quantificação benzo(g,h,i)perileno. Essa limitação provavelmente está associada à eficiência da coluna cromatográfica disponível para a análise, que é para aplicação geral. O método mostrou-se aplicável a amostras estuarinas complexas e ricas em silte e argila. Razões diagnósticas de HPAs parentais indicam fontes petrogênicas a profundidades de 24 – 26 cm, 28 – 30 cm; e fontes pirolíticas a profundidades de 6 – 8 cm, 10 – 12 cm e 14 – 16 cm respectivamente.

The water resources scarceness in world become more evident and worried everyday, because the occurrence of water waste, without consider context of potable water deficit, period of long dryness, resource distribution and population density, urban concentration, industrial and rural areas, pollution process, contamination, beyond aspect related to sanitation and public health affect an entire population. At Itu city, this problem is already real, the population face the lack of water risk in dryness period, damaging their life quality and the city economic development, because Itu is a touristic city that has a mixed economy with activities in business dealing, agriculture and industrial department, all conect to water consumption.

Lapa clas Boleiras rockshelter, a Paleoamerican archaeological site in Central Brazil, was intensely occupied by humans from 10,000 C-14 BP (11.8 cal kyr BP), until approximately 7500 C-14 BP (8.4 cal kyr BP). In this paper we present some interpretations about the formation processes operating on the site. One of our main conclusions is that the bulk of accumulated sediments at the site is of anthropogenic origin: they are constituted by volumetrically significant quantities of plant ash remains, what is somewhat unexpectedly in view of prevailing models about the lifestyle of Paleoamerican hunter-gatherers in South America. The evidence we discuss below is also consistent with paleoenvironmental data that suggest a dry period during the mid-Holocene in the region, probably leading to a decrease in human occupation [Araujo, A.G.M., Neves, W., Pilo, L.B., Atui, J.P., 2005. Holocene dryness and human Occupation in Brazil during the `Archaic Gap`. Quaternary Research 64, 298-307]. We surmise that both factors underpin a surprising reduction in sediment accumulation at the site during the mid-Holocene. (C) 2008 Published by Elsevier Ltd.

OBJECTIVE: The aim of this study was to evaluate the effectiveness of clinical criteria for the diagnosis of hyposalivation in hospitalized patients. MATERIAL AND METHODS: A clinical study was carried out on 145 subjects (48 males; 97 females; aged 20 to 90 years). Each subject was clinically examined, in the morning and in the afternoon, along 1 day. A focused anamnesis allowed identifying symptoms of hyposalivation, like xerostomia complaints (considered as a reference symptom), chewing difficulty, dysphagia and increased frequency of liquid intake. Afterwards, dryness of the mucosa of the cheecks and floor of the mouth, as well as salivary secretion during parotid gland stimulation were assessed during oral examination. RESULTS: Results obtained with Chi-square tests showed that 71 patients (48.9%) presented xerostomia complaints, with a significant correlation with all hyposalivation symptoms (p <0.05). Furthermore, xerostomia was also significantly correlated with all data obtained during oral examination in both periods of evaluation (p<0.05). CONCLUSION: Clinical diagnosis of hyposalivation in hospitalized patients is feasible and can provide an immediate and appropriate therapy avoiding further problems and improving their quality of life.

The quality of astronomical sites is the first step to be considered to have the best performances from the telescopes. In particular, the efficiency of large telescopes in UV, IR, radio etc. is critically dependent on atmospheric transparency. It is well known that the random optical effects induced on the light propagation by turbulent atmosphere also limit telescope’s performances. Nowadays, clear appears the importance to correlate the main atmospheric physical parameters with the optical quality reachable by large aperture telescopes. The sky quality evaluation improved with the introduction of new techniques, new instrumentations and with the understanding of the link between the meteorological (or synoptical parameters and the observational conditions thanks to the application of the theories of electromagnetic waves propagation in turbulent medias: what we actually call astroclimatology. At the present the site campaigns are evolved and are performed using the classical scheme of optical seeing properties, meteorological parameters, sky transparency, sky darkness and cloudiness. New concept are added and are related to the geophysical properties such as seismicity, microseismicity, local variability of the climate, atmospheric conditions related to the ground optical turbulence and ground wind regimes, aerosol presence, use of satellite data. The purpose of this project is to provide reliable methods to analyze the atmospheric properties that affect ground-based optical astronomical observations and to correlate them with the main atmospheric parameters generating turbulence and affecting the photometric accuracy. The first part of the research concerns the analysis and interpretation of longand short-time scale meteorological data at two of the most important astronomical sites located in very different environments: the Paranal Observatory in the Atacama Desert (Chile), and the Observatorio del Roque de Los Muchachos(ORM) located in La Palma (Canary Islands, Spain). The optical properties of airborne dust at ORM have been investigated collecting outdoor data using a ground-based dust monitor. Because of its dryness, Paranal is a suitable observatory for near-IR observations, thus the extinction properties in the spectral range 1.00-2.30 um have been investigated using an empirical method. Furthermore, this PhD research has been developed using several turbulence profilers in the selection of the site for the European Extremely Large Telescope(E-ELT). During the campaigns the properties of the turbulence at different heights at Paranal and in the sites located in northern Chile and Argentina have been studied. This given the possibility to characterize the surface layer turbulence at Paranal and its connection with local meteorological conditions.

A modified uvulopalatopharyngoplasty (UPPP) was carried out between January 1992 and December 2003 at the ENT Department of the Inselspital in Bern in 146 patients with habitual or complicated rhonchopathy. The operation consisted of a classical tonsillectomy or residual tonsil resection and additional shortening of the uvula. The natural mucosal fold between the uvula and the upper pole of the tonsils was carefully preserved. A wide opening to the rhinopharynx was created by asymmetric suturing of the glossopalantine and pharyngopalatine arches. A retrospective questionnaire with regard to rhonchopathy, phases of apnea, daytime drowsiness, obstruction of nasal breathing, long-term complications and patient satisfaction was used to evaluate the short-term and long-term effectiveness of the modified UPPP as well as the incidence of adverse side effects. Complete postoperative courses were evaluated in 116 patients. Surgical complications were restricted to one case with postoperative hemorrhage. A velum insufficiency or postoperative rhinopharyngeal stenosis did not occur. Eighty-three patients (72%) confirmed a persistent suppression or substantial improvement of the rhonchopathy. Disappearance or decrease of sleep apnea was confirmed in 12 (63%) out of 19 postoperative polysomnographic follow-up investigations. Long-term complications occurred in a total of 27 (23%) of 116 patients. They were confined to minor problems such as dryness of the mouth (n = 12), slight difficulty in swallowing (n = 7), discrete speech disturbances (n = 1), and slight pharyngeal dysesthesias (n = 7) with feeling of a lump in the throat and compulsive clearing of the throat. Eighty-five patients (73%) reported that they were satisfied with the postoperative result even several years after the operation. Looking back, 31 patients (27%) would no longer have the operation performed. The inadequate result of the rhonchopathy was specified as the reason by 21 patients. Ten patients had unpleasant memories of the operation because of intensive postoperative pain. Snoring and apneic phases are suppressed or improved by non-traumatic UPPP in the majority of patients. This effect persisted even years after the operation.

Acidic or EDTA-containing oral hygiene products and acidic medicines have the potential to soften dental hard tissues. The low pH of oral care products increases the chemical stability of some fluoride compounds, favors the incorporation of fluoride ions in the lattice of hydroxyapatite and the precipitation of calcium fluoride on the tooth surface. This layer has some protective effect against an erosive attack. However, when the pH is too low or when no fluoride is present these protecting effects are replaced by direct softening of the tooth surface. Xerostomia or oral dryness can occur as a consequence of medication such as tranquilizers, anti-histamines, anti-emetics and anti-parkinsonian medicaments or of salivary gland dysfunction e.g. due to radiotherapy of the oral cavity and the head and neck region. Above all, these patients should be aware of the potential demineralization effects of oral hygiene products with low pH and high titratable acids. Acetyl salicylic acid taken regularly in the form of multiple chewable tablets or in the form of headache powder as well chewing hydrochloric acids tablets for treatment of stomach disorders can cause erosion. There is most probably no direct association between asthmatic drugs and erosion on the population level. Consumers, patients and health professionals should be aware of the potential of tooth damage not only by oral hygiene products and salivary substitutes but also by chewable and effervescent tablets. Additionally, it can be assumed that patients suffering from xerostomia should be aware of the potential effects of oral hygiene products with low pH and high titratable acids.

GOALS OF WORK: To investigate the self-reported symptoms related to endocrine therapy in women with early or advanced breast cancer and the impact of these symptoms on quality of life (QL) indicators. MATERIALS AND METHODS: Symptom occurrence was assessed by the Checklist for Patients on Endocrine Therapy (C-PET) and symptom intensity was assessed by linear analogue self-assessment (LASA) indicators. Patients also responded to global LASA indicators for physical well-being, mood, coping effort and treatment burden. Associations between symptoms and these indicators were analysed by linear regression models. MAIN RESULTS: Among 373 women, the distribution of symptom intensity showed considerable variation in patients reporting a symptom as present. Even though patients recorded a symptom as absent, some patients reported having experienced that symptom when responding to symptom intensity, as seen for decreased sex drive, tiredness and vaginal dryness. Six of 13 symptoms and lower age had a detrimental impact on the global indicators, particularly tiredness and irritability. CONCLUSIONS: Patients' experience of endocrine symptoms needs to be considered both in patient care and research, when interpreting the association between symptoms and QL.

Infectious keratoconjunctivitis (IKC) caused by Mycoplasma conjunctivae is a widespread ocular affection of free-ranging Caprinae in the Alpine arc. Along with host and pathogen characteristics, it has been hypothesized that environmental factors such as UV light are involved in the onset and course of the disease. This study aimed at evaluating the role of topographic features as predisposing or aggravating factors for IKC in Alpine chamois (Rupicapra rupicapra rupicapra) and Alpine ibex (Capra ibex ibex). Geospatial analysis was performed to assess the effect of aspect (northness) and elevation on the severity of the disease as well as on the mycoplasmal load in the eyes of affected animals, using data from 723 ibex and chamois (583 healthy animals, 105 IKC-affected animals, and 35 asymptomatic carriers of M. conjunctivae), all sampled in the Swiss Alps between 2008 and 2010. An influence of northness was not found, except that ibex with moderate and severe signs of IKC seem to prefer more north-oriented slopes than individuals without corneal lesions, possibly hinting at a sunlight sensitivity consequent to the disease. In contrast, results suggest that elevation influences the disease course in both ibex and chamois, which could be due to altitude-associated environmental conditions such as UV radiation, cold, and dryness. The results of this study support the hypothesis that environmental factors may play a role in the pathogenesis of IKC.

BACKGROUND Skin and mucosal manifestations such as skin thickening, pruritus, reduced microvascular circulation, digital lesions, appearance-related changes, and dryness of the eyes and mucosa are common in systemic sclerosis (SSc). A specific skin and mucosa care education programme for patients and their family caregivers should increase their self-efficacy and improve coping strategies. AIMS The aims of this qualitative study were to explore the participants' experiences of both everyday life with skin and mucosal manifestations and the programme itself, while identifying unmet needs for programme development. METHODS Narrative interviews were conducted with eight SSc patients and two family caregivers of individuals with SSc. Using qualitative content analysis techniques, the transcribed interviews were systematically summarized and categories inductively developed. RESULTS The findings illustrated participants' experiences of skin and mucosal symptoms and revealed them to be experts in finding the right therapy mix alone (before diagnosis) and also in collaboration with health professionals (after diagnosis). Participants emphasized that the programme gave them useful education on skin and mucosa care. They described how they had to cope alone with the lack of information on pathophysiology, people's reactions, and the impact on their family and working lives. Nevertheless, participants said that they maintained a positive attitude by not dwelling on future disabilities. CONCLUSIONS Patients and family caregivers benefited from the individualized and SSc-specific education on skin and mucosa care. Future improvements to the programme should focus on imparting understandable information on SSc pathophysiology, dealing with disfigurement and seeking reliable disease information, as well as facilitating peer support.

Consumers and health professionals should be aware of the potential of tooth damage not only by oral hygiene products and salivary substitutes but also by chewable and effervescent tablets. Several paediatric medications show a direct erosive potential in vitro. Clinical proof of the occurrence of erosion after use of these medicaments is still lacking. However, regular and prolonged use of these medicaments might bear the risk of causing erosion. Additionally, it can be assumed that patients suffering from xerostomia should be aware of the potential effects of oral hygiene products with low pH and high titratable acidity.
