Liquid chromatography coupled with mass spectrometry is one of the most powerful tools in the toxicologist’s arsenal to detect a wide variety of compounds from many different matrices. However, the huge number of potentially abused substances and new substances especially designed as intoxicants poses a problem in a forensic toxicology setting. Most methods are targeted and designed to cover a very specific drug or group of drugs while many other substances remain undetected. High resolution mass spectrometry, more specifically time-of-flight mass spectrometry, represents an extremely powerful tool in analysing a multitude of compounds not only simultaneously but also retroactively. The data obtained through the time-of-flight instrument contains all compounds made available from sample extraction and chromatography, which can be processed at a later time with an improved library to detect previously unrecognised compounds without having to analyse the respective sample again. The aim of this project was to determine the utility and limitations of time-of-flight mass spectrometry as a general and easily expandable screening method. The resolution of time-of-flight mass spectrometry allows for the separation of compounds with the same nominal mass but distinct exact masses without the need to separate them chromatographically. To simulate the wide variety of potentially encountered drugs in such a general screening method, seven drugs (morphine, cocaine, zolpidem, diazepam, amphetamine, MDEA and THC) were chosen to represent this variety in terms of mass, properties and functional groups. Consequently, several liquid-liquid and solid phase extractions were applied to urine samples to determine the most general suitable and unspecific extraction. Chromatography was optimised by investigating the parameters pH, concentration, organic solvent and gradient of the mobile phase to improve data obtained by the time-of-flight instrument. The resulting method was validated as a qualitative confirmation/identification method. Data processing was automated using the software TargetAnalysis, which provides excellent analyte recognition according to retention time, exact mass and isotope pattern. The recognition of isotope patterns allows excellent recognition of analytes even in interference rich mass spectra and proved to be a good positive indicator. Finally, the validated method was applied to samples received from the A& E Department of Glasgow Royal Infirmary in suspected drug abuse cases and samples received from the Scottish Prison Service, which we received from their own prevalence study targeting drugs of abuse in the prison population. Abstract Background: The current obesity epidemic is thought to be partly driven by over-consumption of sugar-sweetened diets and soft drinks. Loss-of-control over eating and addiction to drugs of abuse share overlapping brain mechanisms including changes in motivational drive, such that stimuli that are often no longer ‘liked’ are still intensely ‘wanted’ [7,8]. The neurokinin 1 (NK1) receptor system has been implicated in both learned appetitive behaviors and addiction to alcohol and opioids; however, its role in natural reward seeking remains unknown. Methodology/Principal Findings: We sought to determine whether the NK1-receptor system plays a role in the reinforcing properties of sucrose using a novel selective and clinically safe NK1-receptor antagonist, ezlopitant (CJ-11,974), in three animal models of sucrose consumption and seeking. Furthermore, we compared the effect of ezlopitant on ethanol consumption and seeking in rodents. The NK1-receptor antagonist, ezlopitant decreased appetitive responding for sucrose more potently than for ethanol using an operant self-administration protocol without affecting general locomotor activity. To further evaluate the selectivity of the NK1-receptor antagonist in decreasing consumption of sweetened solutions, we compared the effects of ezlopitant on water, saccharin-, and sodium chloride (NaCl) solution consumption. Ezlopitant decreased intake of saccharin but had no effect on water or salty solution consumption. Conclusions/Significance: The present study indicates that the NK1-receptor may be a part of a common pathway regulating the self-administration, motivational and reinforcing aspects of sweetened solutions, regardless of caloric value, and those of substances of abuse. Additionally, these results indicate that the NK1-receptor system may serve as a therapeutic target for obesity induced by over-consumption of natural reinforcers.

Rationale: Anabolic steroids are drugs of abuse. However, the potential for addiction remains unclear. Testosterone induces conditioned place preference in rats and oral self-administration in hamsters. Objectives: To determine if male rats and hamsters consume testosterone by intravenous (IV) or intracerebroventricular (ICV) self- administration. Methods: With each nose-poke in the active hole during daily 4-h tests in an operant condi- tioning chamber, gonad-intact adult rats and hamsters received 50 mg testosterone in an aqueous solution of b-cyclodextrin via jugular cannula. The inactive nose- poke hole served as a control. Additional hamsters received vehicle infusions. Results: Rats (n=7) expressed a significant preference for the active nose-poke hole (10.0€2.8 responses/4 h) over the inactive hole (4.7€1.2 responses/4 h). Similarly, during 16 days of testosterone self-administration IV, hamsters (n=9) averaged 11.7€2.9 responses/4 h and 6.3€1.1 responses/4 h in the active and inactive nose-poke holes, respectively. By contrast, vehicle controls (n=8) failed to develop a preference for the active nose-poke hole (6.5€0.5 and 6.4€0.3 responses/4 h). Hamsters (n=8) also self-administered 1 mg testosterone ICV (active hole:39.8€6.0 nose-pokes/ 4 h; inactive hole: 22.6€7.1 nose-pokes/4 h). When testosterone was replaced with vehicle, nose-poking in the active hole declined from 31.1€7.6 to 11.9€3.2 responses/ 4 h within 6 days. Likewise, reversing active and inactive holes increased nose-poking in the previously inactive hole from 9.1€1.9 to 25.6€5.4 responses/4 h. However, reducing the testosterone dose from 1 mg to 0.2 mg per 1 ml injection did not change nose-poking. Conclu- sions: Compared with other drugs of abuse, testosterone reinforcement is modest. Nonetheless, these data support the hypothesis that testosterone is reinforcing.

Delta opioid receptors are implicated in a variety of psychiatric and neurological disorders. These receptors play a key role in the reinforcing properties of drugs of abuse, and polymorphisms in OPRD1 (the gene encoding delta opioid receptors) are associated with drug addiction. Delta opioid receptors are also involved in protecting neurons against hypoxic and ischemic stress. Here, we first examined a large sample of 738 elderly participants with neuroimaging and genetic data from the Alzheimer's Disease Neuroimaging Initiative. We hypothesized that common variants in OPRD1 would be associated with differences in brain structure, particularly in regions relevant to addictive and neurodegenerative disorders. One very common variant (rs678849) predicted differences in regional brain volumes. We replicated the association of this single-nucleotide polymorphism with regional tissue volumes in a large sample of young participants in the Queensland Twin Imaging study. Although the same allele was associated with reduced volumes in both cohorts, the brain regions affected differed between the two samples. In healthy elderly, exploratory analyses suggested that the genotype associated with reduced brain volumes in both cohorts may also predict cerebrospinal fluid levels of neurodegenerative biomarkers, but this requires confirmation. If opiate receptor genetic variants are related to individual differences in brain structure, genotyping of these variants may be helpful when designing clinical trials targeting delta opioid receptors to treat neurological disorders.

Alcohol addiction is a debilitating disorder producing maladaptive changes in the brain, leading drinkers to become more sensitive to stress and anxiety. These changes are key factors contributing to alcohol craving and maintaining a persistent vulnerability to relapse. Serotonin (5-Hydroxytryptamine, 5-HT) is a monoamine neurotransmitter widely expressed in the central nervous system where it plays an important role in the regulation of mood. The serotonin system has been extensively implicated in the regulation of stress and anxiety, as well as the reinforcing properties of all of the major classes of drugs of abuse, including alcohol. Dysregulation within the 5-HT system has been postulated to underlie the negative mood states associated with alcohol use disorders. This review will describe the serotonergic (5-HTergic) neuroplastic changes observed in animal models throughout the alcohol addiction cycle, from prenatal to adulthood exposure. The first section will focus on alcohol-induced 5-HTergic neuroadaptations in offspring prenatally exposed to alcohol and the consequences on the regulation of stress/anxiety. The second section will compare alterations in 5-HT signalling induced by acute or chronic alcohol exposure during adulthood and following alcohol withdrawal, highlighting the impact on the regulation of stress/anxiety signalling pathways. The third section will outline 5-HTergic neuroadaptations observed in various genetically-selected ethanol preferring rat lines. Finally, we will discuss the pharmacological manipulation of the 5-HTergic system on ethanol- and anxiety/stress-related behaviours demonstrated by clinical trials, with an emphasis on current and potential treatments.

Väärinkäytettyjen aineiden seulontaan käytetyn menetelmän tulee olla herkkä, selektiivinen, yksinkertainen, nopea ja toistettava. Työn tavoitteena oli kehittää yksinkertainen, mutta herkkä, esikäsittelymenetelmä bentsodiatsepiinien ja amfetamiinijohdannaisten kvalitatiiviseen seulomiseen virtsasta mikropilarisähkösumutussirun (μPESI) avulla, mikä tarjoaisi vaihtoehdon seulonnassa käytetyille immunologisille menetelmille, joiden herkkyys ja selektiivisyys ovat puutteellisia. Tavoitteena oli samalla tarkastella mikropilarisähkösumutussirun toimivuutta biologisten näytteiden analyysissa. Esikäsittely optimoitiin erikseen bentsodiatsepiineille ja amfetamiinijohdannaisille. Käytettyjä esikäsittelymenetelmiä olivat neste-nesteuutto, kiinteäfaasiuutto Oasis HLB-patruunalla ja ZipTip®-pipetinkärjellä sekä laimennus ja suodatus ilman uuttoa. Mittausten perusteella keskityttiin optimoimaan ZipTip®-uuttoa. Optimoinnissa tutkittavia yhdisteitä spiikattiin 0-virtsaan niiden ennaltamääritetyn raja-arvon verran, bentsodiatsepiineja 200 ng/ml ja amfetamiinijohdannaisia 300 ng/ml. Bentsodiatsepiinien kohdalla optimoitiin kutakin uuton vaihetta ja optimoinnin tuloksena näytteen pH säädettiin arvoon 5, faasi kunnostettiin asetonitriililla, tasapainotettiin ja pestiin veden (pH 5) ja asetonitriilin (10 % v/v) seoksella ja eluoitiin asetonitriilin, muurahaishapon ja veden (95:1:4 v/v/v) seoksella. Amfetamiinijohdannaisten uutossa optimoitiin näytteen ja liuottimien pH-arvoja ja tuloksena näytteen pH säädettiin arvoon 10, faasi kunnostettiin veden ja ammoniumvetykarbonaatin(pH 10, 1:1 v/v) seoksella, tasapainotettiin ja pestiin asetonitriilin ja veden (1:5 v/v) seoksella ja eluoitiin metanolilla. Optimoituja uuttoja testattiin Yhtyneet Medix Laboratorioista toimitetuilla autenttisilla virtsanäytteillä ja saatuja tuloksia verrattiin kvantitatiivisen GC/MS-analyysin tuloksiin. Bentsodiatsepiininäytteet hydrolysoitiin ennen uuttoa herkkyyden parantamiseksi. Autenttiset näytteet analysoitiin Q-TOF-laitteella Viikissä. Lisäksi hydrolysoidut bentsodiatsepiininäytteet mitattiin Yhtyneet Medix Laboratorioiden TOF-laitteella. Kehitetty menetelmä vaatii tulosten perusteella lisää optimointia toimiakseen. Ongelmana oli etenkin toistoissa ilmennyt tulosten hajonta. Manuaalista näytteensyöttöä tulisi kehittää toistettavammaksi. Autenttisten bentsodiatsepiininäytteiden analyysissa ongelmana olivat virheelliset negatiiviset tulokset ja amfetamiinijohdannaisten analyysissa virheelliset positiiviset tulokset. Virheellisiä negatiivisia tuloksia selittää menetelmän herkkyyden puute ja virheellisiä positiivisia tuloksia mittalaitteen, sirujen tai liuottimien likaantuminen.

Use of adverse drug combinations, abuse of medicinal drugs and substance abuse are considerable social problems that are difficult to study. Prescription database studies might fail to incorporate factors like use of over-the-counter drugs and patient compliance, and spontaneous reporting databases suffer from underreporting. Substance abuse and smoking studies might be impeded by poor participation activity and reliability. The Forensic Toxicology Unit at the University of Helsinki is the only laboratory in Finland that performs forensic toxicology related to cause-of-death investigations comprising the analysis of over 6000 medico-legal cases yearly. The analysis repertoire covers most commonly used drugs and drugs of abuse, and the ensuing database contains also background information and information extracted from the final death certificate. In this thesis, the data stored in this comprehensive post-mortem toxicology database was combined with additional metabolite and genotype analyses that were performed to complete the profile of selected cases. The incidence of drug combinations possessing serious adverse drug interactions was generally low (0.71%), but it was notable for the two individually studied drugs, a common anticoagulant warfarin (33%) and a new generation antidepressant venlafaxine (46%). Serotonin toxicity and adverse cardiovascular effects were the most prominent possible adverse outcomes. However, the specific role of the suspected adverse drug combinations was rarely recognized in the death certificates. The frequency of bleeds was observed to be elevated when paracetamol and warfarin were used concomitantly. Pharmacogenetic factors did not play a major role in fatalities related to venlafaxine, but the presence of interacting drugs was more common in cases showing high venlafaxine concentrations. Nicotine findings in deceased young adults were roughly three times more prevalent than the smoking frequency estimation of living population. Contrary to previous studies, no difference in the proportion of suicides was observed between nicotine users and non-nicotine users. However, findings of abused substances, including abused prescription drugs, were more common in the nicotine users group than in the non-nicotine users group. The results of the thesis are important for forensic and clinical medicine, as well as for public health. The possibility of drug interactions and pharmacogenetic issues should be taken into account in cause-of-death investigations, especially in unclear cases, medical malpractice suspicions and cases where toxicological findings are scarce. Post-mortem toxicological epidemiology is a new field of research that can help to reveal problems in drug use and prescription practises.

Uma questão particularmente relevante é o fato de exposições precoces a drogas de abuso durante o desenvolvimento potencialmente aumentarem a susceptibilidade a estas drogas posteriormente durante o desenvolvimento. No presente estudo utilizando estudos comportamentais e eletrofisiológicos, investigamos efeitos tardios da exposição de camundongos à fumaça de cigarro, à nicotina e ao etanol durante o período que corresponde à gestação em humanos. Para tal, esta tese foi dividida em 2 estudos. No Estudo 1, submetemos camundongos durante o período que corresponde à gestação de humanos à fumaça de cigarro e/ou etanol visando investigar se a estas drogas de abuso, separadamente ou quando combinadas, programam maior susceptibilidade aos efeitos da nicotina durante a adolescência (PN30) ou idade adulta (PN90). Para avaliar a susceptibilidade, utilizamos 3 testes: campo aberto (CA), preferencia pela nicotina (PPN) e preferencia condicionada por lugar (CPP). No Estudo 2, os animais foram expostos a nicotina durante o período gestacional e, no período que corresponde à infância (PN9 a PN20), fatias de cérebro contendo o núcleo tegumental laterodorsal (LDT) foram expostas a etanol. Este núcleo foi escolhido uma vez que estudos recentes indicam sua participação em mecanismos de toxicodependência. Foram realizados registros eletrofisiológicos de uma única célula. No Estudo 1, identificamos maior sensibilidade para os efeitos da reexposição à nicotina na adolescência quando comparada com a idade adulta . Em animais testados no CA durante a adolescência, a nicotina foi capaz de causar aumento da atividade locomotora nos animais controle, previamente expostos à fumaça de cigarro e ao etanol. Contudo, em animais expostos à fumaça combinada com etanol, não houve aumento da locomoção. Na idade adulta, a nicotina causou um aumento da atividade locomotora no CA somente nos animais expostos à fumaça de cigarro. Quanto ao CPP, a exposição prévia à fumaça de cigarro e ao etanol causaram aumento da resposta condicionada à nicotina em fêmeas adolescentes. Nos animais previamente expostos à combinação entre fumaça de cigarro e etanol, a resposta condicionada à nicotina não atingiu significância estatística. Não houve alterações na idade adulta. A exposição a fumaça de cigarro e/ou etanol não afetou a PPN. No Estudo 2, os dados eletrofisiológicos mostraram que a exposição pré-natal à nicotina foi capaz de alterar as correntes de despolarização basais e o potencial de repouso de células do LDT A nicotina também foi capaz de alterar as respostas deste núcleo ao etanol reduzindo as correntes de despolarização e aumentando, embora que não de forma significativa, as correntes inibitórias. De acordo com estes dados, injurias causadas pela exposição à fumaça do cigarro, à nicotina isoladamente, e ao etanol durante o desenvolvimento são capazes de perdurar por um logo tempo na vida do individuo, alterando as respostas a comportamentais e celulares a uma exposição tardia à nicotina e ao etanol.

Le système dopaminergique (DA) méso-corticolimbique du cerveau, qui prend son origine dans l'aire tegmentaire ventrale (ATV), est fortement impliqué dans les comportements motivés et la toxicomanie. Les drogues d'abus activent ce système et y induisent une plasticité synaptique de longue durée. Les neurones DA de l'ATV reçoivent sur leur arborisation dendritique une grande densité de terminaisons glutamatergiques. Les drogues d'abus induisent une potentialisation à long terme (PLT) de ces contacts glutamatergiques. La PLT est une augmentation prolongée de la transmission synaptique, qui semble sous-tendre la mémoire et l'apprentissage. Les endocannabinoïdes (ECs) sont des neurotransmetteurs qui agissent de façon rétrograde sur des récepteurs présynaptiques (CB1) pour diminuer la libération des neurotransmetteurs comme le glutamate. Les neurones libèrent les ECs à partir de leur compartiment somatodendritique suite à une stimulation des afférences et la dépolarisation membranaire qui s’ensuit. La neurotensine (NT) est un neuropeptide retrouvé de façon abondante dans le système DA du cerveau. Il a été découvert que la NT peut induire la libération des ECs dans le striatum. En faisant appel à une combinaison d’approches immunohistochimique, électrophysiologique et pharmacologique chez la souris, nous avons confirmé dans la première étude de cette thèse la présence des récepteurs CB1 sur les terminaisons glutamatergiques des neurones DA de l'ATV, et avons montré que leur activation induit une diminution de la libération de glutamate. Par ailleurs, nous avons montré que des trains de stimulation peuvent induire la libération des ECs. Nous avons découvert qu'en présence d'un antagoniste des récepteurs CB1, il y a facilitation de l’induction de la PLT. Cette observation suggère que les ECs ont un effet inhibiteur sur l’induction de la PLT, plutôt que sur son expression. Nous avons déterminé que le 2-arachidonoylglycerol (2-AG) est l’EC qui est principalement responsable de cette action inhibitrice. Finalement, la PLT induite en présence d’un antagoniste CB1 est aussi dépendante d'une activation des récepteurs NMDA du glutamate. Les travaux réalisés dans la deuxième étude de cette thèse ont montré que la NT est présente dans une sous-population de terminaisons axonales glutamatergiques dans l’ATV. Une application exogène de NT induit une diminution prolongée de l'amplitude des courants postsynaptiques excitateurs (CPSEs). Cette diminution est bloquée en présence d'un antagoniste non-sélectif des récepteurs à la NT, ainsi qu'en présence d'un antagoniste sélectif pour le récepteur de NT de type 1 (NTS1). Confirmant l’implication d’une production d’ECs, la baisse des CPSEs par la NT a été bloquée en présence d’un antagoniste des récepteurs CB1 ou d’un bloqueur de la synthèse de 2-AG. La chélation du calcium intracellulaire n'empêchait pas l’effet inhibiteur de la NT sur les CPSEs, cependant, l'inhibition des protéines G ou de la phospholipase C a complètement bloqué la dépression synaptique induite par la NT. Par ailleurs, nos travaux ont montré que la nature prolongée de la dépression synaptique induite par la NT exogène s’explique par une libération soutenue des ECs, et non pas à une activation prolongée des NTR. Finalement, notre observation qu’un antagoniste des récepteurs de la NT ne facilite pas l’induction de la PLT, comme le fait un antagoniste du récepteur CB1, suggère que la stimulation répétitive des afférences glutamatergiques nécessaire à l’induction de la PLT n’induit pas de libération des ECs via la libération de NT, nous permettant ainsi de conclure que la sécrétion de NT n'agit pas dans ces conditions comme un facteur de régulation négative de la PLT.

Beaucoup de personnes consomment des drogues d’abus de façon récréative ou expérimentale dans leur vie, mais peu d’entre elles développent une toxicomanie. Nous avons exploré, chez le rat, deux facteurs impliqués dans la transition vers la toxicomanie, soit la vitesse à laquelle la drogue parvient au cerveau et le fait d’être sous traitement antipsychotique. Dans une première étude, notre objectif était de déterminer si augmenter la vitesse de livraison de la cocaïne (0.5 mg/kg) par auto-administration intraveineuse (i.v.; livrée en 5 secondes dans un groupe versus 90 secondes dans l’autre) mènerait à une plus grande consommation de celle-ci lors d’un accès prolongé (6 h/j versus 1 h/j), et à une plus grande motivation à obtenir la drogue telle que mesurée sous un ratio de renforcement progressif à une vitesse différente (10 secondes). Nous avons trouvé que le groupe 5 s consommait plus de cocaïne que le groupe 90 s en accès prolongé, mais aussi en accès limité. Cependant, la motivation des deux groupes était la même à la vitesse de 10 s, ainsi qu’à leurs vitesses initiales. Nous pensons que ceci peut être dû à une forme de plasticité du système méso-cortico-limbique survenue suite à l’auto-administration en accès prolongé en conjonction avec l’augmentation de consommation, chez les deux groupes, rendant impossible une distinction de leur motivation. Dans une deuxième série d’études nous avons émis l’hypothèse que l’antipsychotique typique, halopéridol (HAL, 0.5 mg/kg/j), et non l’atypique, aripiprazole (ARI, 1 mg/kg/j), un modulateur dopaminergique, induirait une augmentation de la poursuite de récompense conditionnée (RC) et de la locomotion (LOCO) en réponse à l’amphétamine (AMPH). Cependant, nous avons trouvé une augmentation chez le groupe HAL, mais non ARI, de la réponse RC, trois semaines, mais non une semaine post traitement, ainsi qu’une augmentation de la LOCO, chez le groupe HAL, mais non ARI, une semaine mais non trois semaines post traitement. L’incohérence des résultats entre les deux tests (RC et LOCO) rend leur interprétation difficile. Ces études restent à être explorées d’avantage afin de pouvoir en tirer des conclusions plus éclairées quant à l’impact de la vitesse d’administration de la cocaïne et du traitement antipsychotique sur le développement d’une toxicomanie.

Les drogues, les conditionnements et les voies d’administration qui mènent à l’augmentation rapide du taux drogue dans le cerveau favorisent cette évolution. La raison est méconnue. Nous avons émis l’hypothèse que l’injection rapide de drogue promeut des changements dans le cerveau qui mènent à l’augmentation de la consommation et de la motivation à obtenir la drogue. Nous avons comparé la consommation lors de conditions à ratio fixe (FR) et à ratio progressif (PR) chez des rats s’auto-administrant la cocaïne administrée par voie intraveineuse (i.v.) en 5 ou 90 secondes (s). Tous les rats ont été entrainés à peser sur un levier afin de s’auto administrer des injections de cocaïne de 0.25 ou 0.5 mg/kg par voie intraveineuse injectée en 5 s sous FR avant d’être divisés en groupes s’auto administrant la cocaïne injectée en 5 ou 90 s pendant 1 heure (h)/session. Pour étudier les différences potentielles en consommation, l’accès à la cocaïne à été augmenté à 6 h/session. Les différences en motivation ont été détectées par l’auto administration de la cocaïne sous PR en fonction de la dose et de la vitesse d’infusion. L’accès à la drogue pendant 1 h/session n’a pas influencé la consommation. Lorsque l’accès a été prolongé à 6 h, tous les animaux ont augmenté leur consommation, mais l’augmentation était plus prononcée chez les rats s’injectant la cocaïne en 5 s. De plus, la vitesse d’injection a influencé la motivation pour obtenir la drogue. Lors de conditions à PR, la courbe dose-réponse pour le nombre d’infusions prises a été déplacée vers le haut pour les rats s’auto administrant des injections de cocaïne en 5 s versus 90 s. De plus, des différences qualitatives on été observées en PR. La consommation de cocaïne des rats s’injectant des infusions en 5 s était dépendante de la dose, tandis que les rats s’auto administrant la drogue en 90 s ont pris la même quantité de drogue, peu importe la dose. Finalement, les rats s’auto administrant des infusions de cocaïne 0.5 mg/kg en 5 s ont consommé plus de cocaïne que les rats prenant des infusions en 90 s, peu importe si elle était injectée en 5 ou 90 s le jour du test. Ainsi, nos résultats montrent que l’injection rapide de drogue dans le cerveau mène à l’augmentation de la consommation et de la motivation pour obtenir la cocaïne, deux symptômes qui caractérisent la toxicomanie.
