Depressed patients have reduced glucocorticoid receptor (GR) function, as demonstrated by resistance to the suppressive effects of the synthetic glucocorticoid hormone, and GR agonist, dexamethasone. We have developed a suppressive test with prednisolone, a synthetic glucocorticoid that is similar to cortisol in its pharmacodynamics and pharmacokinetics, and binds to both the GR and the mineralocorticoid receptor (MR). We have found that depressed patients suppress normally to prednisolone, unless they are particularly non-responsive to treatment. In the present study, we evaluated 28 inpatients with treatment-resistant depression (TRD), and compared salivary cortisol secretion (at 0900 h, 1200 h and 1700 h) after placebo or after prednisolone (5 mg), before and after an inpatient treatment admission. Half of the patients (n = 14) reached treatment response. When comparing the assessment between admission and discharge, cortisol output after placebo fell (-26% of area under the curve; p = 0.024) while the output after prednisolone did not change. Moreover, there was no change in the response to prednisolone (percentage suppression) between admission at discharge, and this was not influenced by treatment response. Finally, we could confirm and extend our previously published data with prednisolone (5 mg), showing that depressed patients (n = 12) and controls (n = 12) suppressed equally to both 5 and 10 mg doses of prednisolone. This study suggests that the response to prednisolone is similar in depressed patients and controls at different doses of prednisolone, and does not change with symptomatic improvement. This is in contrast with findings, from us and others, using other measures of hypothalamic-pituitary-adrenal axis function, such as basal cortisol levels or the response to dexamethasone. Thus, we propose that the prednisolone suppression test may offer specific biological and clinical information, related to its action at both the GR and the MR. (C) 2010 Elsevier Ltd. The early identification of hearing impairment allows for an intervention still in the ""critical"" and ideal period of hearing and language stimulation. Pediatric ototoxicity is a very controversial topic. There have been variable percentages of ototoxicity cases in children with different aminoglycosides antibiotics. The main pediatric groups whom receive aminoglycosides are newborns with severe infections on the neonatal ICU. Aim: to check the functional aspect of the cochlear external hair cells and treatment regimens used to treat infections during the neonatal period. Study design: Experimental. Materials and Methods: we studied 26 albino guinea pigs, through distortion product otoacoustic emissions, before and after the use of gentamicin. Results: in all the assessments, the external hair cells functional status, studied by means of the distortion product otoacoustic emissions, proved preserved. Conclusion: In the present study, we did not notice changes in outer hair cell function in the albino guinea pigs treated with gentamicin in the doses of 4 mg/Kg/ day and 2.5 mg/Kg/day every 12 hours for 10 and 14 days.

Hypertension induces vascular alterations that are associated with up-regulation of matrix metalloproteinases (MMPs). While these alterations may be blunted by doxycycline, a non-selective MMPs inhibitor, no previous study has examined the effects of different doses of doxycycline on these alterations. This is important because doxycycline has been used at sub-antimicrobial doses, and the use of lower doses may prevent the emergence of antibiotic-resistant microorganisms. We studied the effects of doxycycline at 3, 10 and 30 mg/kg per day on the vascular alterations found in the rat two kidneyone clip (2K1C) hypertension (n = 20 rats/group). Systolic blood pressure (SBP) was monitored during 4 weeks of treatment. We assessed endothelium-dependent and independent relaxations. Quantitative morphometry of structural changes in the aortic wall was studied, and aortic MMP-2 levels/proteolytic activity were determined by gelatin and in situ zymography, respectively. All treatments attenuated the increases in SBP in hypertensive rats (195.4 +/- 3.9 versus 177.2 +/- 6.2, 176.3 +/- 4.5, and 173 +/- 5.1 mmHg in 2K1C hypertensive rats treated with vehicle, or doxycycline at 3, 10, 30 mg/kg per day, respectively (all p < 0.01). However, only the highest dose prevented 2K1C-induced reduction in endothelium-dependent vasorelaxation (p < 0.05), vascular hypertrophy and increases in MMP-2 levels (all p < 0.05). In conclusion, our results suggest that relatively lower doses of doxycycline do not attenuate the vascular alterations found in the 2K1C hypertension model, and only the highest dose of doxycycline affects MMPs and vascular structure. Our results support the idea that the effects of doxycycline on MMP-2 and vascular structure are pressure independent.

This study is the first in the literature to focus attention on the possible immunotoxic effect of integerrimine N-oxide content in the butanolic residue (BR) of Senecio brasiliensis, a poisonous hepatotoxic plant that contains pyrrolizidine alkaloids (PAs). PAs have been reported as a pasture and food contaminant and as herbal medicine used worldwide and are responsible for poisoning events in livestock and human beings. After the plant extraction, BR extracted from Senecio brasiliensis was found to contain approximately 70% integerrimine N-oxide by elemental and spectral analyses ((1)H and (13)C NMR), which was administered to adult male Wistar Hannover rats at doses of 3, 6 and 9 mg/kg for 28 days. Body weight gain, food consumption, lymphoid organs, neutrophil analysis, humoural immune response, cellular immune response and lymphocyte analysis were evaluated. Our study showed that integerrimine N-oxide could promote an impairment in the body weight gain, interference with blood cell counts and a reducing T cell proliferative activity in rats; however, no differences in the neutrophil activities, lymphocytes phenotyping and humoural and cellular immune responses were observed. It is concluded that doses of integerrimine N-oxide here employed did not produce marked immunotoxic effects. (C) 2011 Elsevier Ltd. All rights reserved.

Nude rats bearing melanomas on their hindlimbs were treated by isolated limb infusion (ILI) with increasing doses (7.5-400 mug/ml) of melphalan. The response of tumours to treatment at the end of the observation period was graded, according to diameter, as complete response (CR), partial response (PR), no change (NC) or progressive disease (PD). No linear relationship between the dose of melphalan and the tumour response was observed. All doses above a threshold of 15 mug/ml achieved a PR or CR. The achievement of CR was not related to increased dose. Two major implications arise from this work. Firstly, the typically two-to three-fold increase in cytotoxic drug concentration given in high dose chemotherapy compared with standard drug concentration may not be sufficient to produce the expected increase in tumour response and possibly survival, and the controversial results of high dose chemotherapy in different studies may thus be explained. Secondly, since an increase in melphalan dose above a certain threshold does not greatly increase tumour response, the use of combination therapies would seem to be more likely to be effective than increased chemotherapeutic drug doses in achieving better tumour responses.

We recently demonstrated that suppressed bone remodeling allows microdamage to accumulate and causes reductions in some mechanical properties. However, in our previous study, I year treatment with high-dose etidronate (EHDP) did not increase microdamage accumulation in most skeletal sites of dogs in spite of complete remodeling suppression and the occurrence of spontaneous fractures of ribs and/or thoracic spinous processes. This study evaluates the effects of EHDP on microdamage accumulation and biomechanical properties before fractures occur. Thirty-six female beagles, 1-2 years old, were treated daily for 7 months with subcutaneous injections of saline vehicle (CNT) or EHDP at 0.5 (E-low) or 5 mg/kg per day (E-high). After killing, bone mineral measurement, histomorphometry, microdamage analysis, and biomechanical testing were performed. EHDP treatment suppressed intracortical and trabecular remodeling by 60%-75% at the lower dose, and by 100% at the higher dose. Osteoid accumulation caused by a mineralization deficit occurred only in the E-high group, and this led to a reduction of mineralized bone mass. Microdamage accumulation increased significantly by two- to fivefold in the rib, lumbar vertebra, ilium, and thoracic spinous process in E-low, and by twofold in the lumbar vertebra and ilium in E-high. However, no significant increase in damage accumulation was observed in ribs or thoracic spinous processes in E-high where fractures occur following 12 months of treatment. Mechanical properties of lumbar vertebrae and thoracic spinous processes were reduced significantly in both E-low and E-high. These findings suggest that suppression of bone remodeling by EHDP allows microdamage accumulation, but that osteoid accumulation reduces production of microdamage. Objectives Low doses of ACTH [1-24] (0.1, 0.5 and 1.0 mug per 1.73 m(2)) may provide a more physiological level of adrenal stimulation than the standard 250 mug test, but not all studies have concluded that the 1.0 fig is a more sensitive screening test for central hypoadrenalism. Eight-hour infusions of high dose ACTH [1-24] have also been suggested as a means of assessing the adrenals' capacity for sustained cortisol secretion. In this study, we compared the diagnostic accuracy of three low dose ACTH tests (LDTs) and the 8-h infusion with the standard 250 jig test (HDT) and the insulin hypoglycaemia test (IHT) in patients with hypothalamic-pituitary disease. Subjects and design Three groups of subjects were studied. A healthy control group (group 1, n=9) and 33 patients with known hypothalamic or pituitary disease who were divided into group 2 (n=12, underwent IHT) and group 3 (n=21, IHT contraindicated). Six different tests were performed: a standard IHT (0.15 U/kg soluble insulin); a 60-minute 250 mug HDT; three different LDTs using 0.1 mug, 0.5 mug and 1.0 mug (all per 1.73 m(2)); and an 8-h infusion test (250 mug ACTH [1-24] at a constant rate over 8 h). Results Nine out of the 12 patients in group 2 failed the IHT. Three out of 12 patients from group 2 who clearly passed the IHT, also passed all the ACTH [1-24] stimulation tests. Objective To compare the effects of transferring from low-dose transdermal estrogen to raloxifene (RLX), with a phase of alternate-day RLX therapy with or without low-dose transdermal estrogen, on serum lipids and fibrinogen in postmenopausal women previously administered estrogen plus progestogen therapy. Methods Sixty postmenopausal women (mean age 55 years) were randomized to one of two treatment groups: RLX + low-dose transdermal estrogen (RLX + E) or RLX + placebo. The study consisted of four 8-week phases: phase I (all subjects low-dose transdermal estrogen 25 mug/day), phase II (double-blind RLX 60 mg every 2nd day in combination with either low-dose transdermal estrogen or placebo), phase III (all subjects RLX 60 mg every 2nd day + placebo) and phase IV (all subjects RLX 60 mg/day + placebo). Results No significant differences existed between groups for baseline measurements prior to phase I. In phase I, for all subjects combined, total cholesterol and low-density lipoprotem cholesterol both showed a significant increase (median increase of 0.2 mmol/l, p = 0.008 and 0.4 mmol/l, p < 0.001, respectively), while triglycerides decreased significantly (median decrease of 0.2 mmol/l, p < 0.001). For the primary analysis (phase II to phase IV), the mean change from baseline observations showed no significant differences between the therapy groups for serum lipids, fibrinogen, vital signs or weight. In the comparison phase (phase II), changes in serum lipids, fibrinogen, vital signs and weight were not significantly different between groups. Conclusion Gradual conversion to RLX from low-dose transdermal estrogen, with a phase of alternate-day RLX therapy with or without low-dose transdermal estrogen, does not have any effect on the serum lipid profile or fibrinogen level.

Orlikowski e Barley (2001) apontam a importância da pesquisa em TI sob a ótica dos Estudos Organizacionais, principalmente quando é utilizada a análise neoinstitucional, porque dessa forma há uma compreensão mais sistêmica sobre como a tecnologia está entrelaçada nas complexas redes interdependentes, sociais, econômicas e políticas. O estudo de caso ocorreu em um órgão do judiciário, onde foram obtidos dados por meio de entrevistas, que foram triangulados com uma pesquisa documental. A dissertação busca elucidar como pressões isomórficas podem influenciar estruturas e processos de TI recomendados pela legislação em vigor, de forma a contribuir para gestores de organizações públicas durante a definição de prioridades para a TI. Para analisar o estágio institucional utilizou-se a classificação proposta por Tolbert e Zucker (1996) e como resultado, apesar das pressões isomórficas coercitivas, obteve-se que poucas estruturas e processos se institucionalizaram totalmente e, aparentemente, só aquelas que traziam com si outros tipos de pressões isomórficas é que parecem ter maior chance de chegar ao último estágio institucional. Foi constatada que a organização, durante o processo de institucionalização, pode incorrer em novas falhas, gerando mitos burocráticos e ritos cerimoniais que por exemplo, podem causar maior descolamento da preocupação por efetividade em seus resultados. Para determinados problemas, apontados durante as entrevistas, não foi possível encontrar influência de nenhuma pressão isomórfica. Tal fato sugere que a teoria neoinstitucional não conseguiu explicar todo o cenário da organização.

O presente artigo tem com objetivo analisar a atuação do Órgão de Solução de Controvérsias da Organização Mundial do Comércio, entre os anos de 1995 e 2007. Trata-se de um dos principais fóruns de solução internacional de conflitos, sobretudo em função da quantidade de contenciosos, de Estados envolvidos e de valores. O texto procura destacar o cumprimento dos procedimentos previstos; a originalidade dos mecanismos de estimulo à obediência das decisões: o ganho de legitimidade do sistema com o tempo e as principais sugestões de reforma em discussão.
Dose inseminante utilizada na fertilização artificial de ovócito de piracanjuba (Brycon orbignyanus)

A piracanjuba (Brycon orbignyanus Valenciennes, 1849) é uma espécie de peixe migratória, ameaçada de extinção. O objetivo do presente estudo foi determinar a dose inseminante na fertilização artificial de ovócitos de piracanjuba. Para isso, utilizou-se delineamento em blocos casualizados, com quatro tratamentos e três repetições. Três casais de piracanjuba, selecionados dos tanques de reprodutores da Estação Ambiental de Itutinga (EAI - CEMIG), no período de piracema 2006/2007, receberam aplicação de hormônio extrato bruto de hipófise de carpa (EBHC) para obtenção dos gametas. Adotaram-se quatro tratamentos diferentes para a fertilização de 0,1 grama de ovócitos: 10µL, 20µL, 30µL e 40µL de sêmen. As amostras foram ativadas com 5 mL de água do próprio tanque e, em seguida, levadas para incubadoras, dotadas de renovação constante de água, à temperatura de 28ºC. Após 8 e 16 horas, analisaram-se as taxas de fertilização (ovos viáveis) e de eclosão dos ovos, respectivamente. Os resultados obtidos foram comparados pelo teste de Tukey a 5%. As relações sêmen-ovócitos testadas não alteraram as taxas de fertilização e eclosão (P>0,05). O número de espermatozoides-ovócitos, variando de 10,4 x10(5) a 41,6 x10(5), foi eficiente para obtenção de boas taxas de fertilidade.

Os glicocorticóides são muito utilizados para fins antiinflamatórios e imunossupressores, porém apresentam importantes efeitos colaterais no fígado. O exame ultrassonográfico é um método não-invasivo que avalia a anatomia intra-hepática e possibilita a caracterização de alterações no parênquima decorrentes dos efeitos indesejáveis dos glicocorticóides. O histograma é uma mensuração ultrassonográfica que minimiza a subjetividade do exame, permitindo uma avaliação quantitativa da ecotextura e da ecogenicidade do órgão. Exames citopatológicos e histopatológicos podem auxiliar no diagnóstico de lesões hepáticas induzidas por corticóides. O objetivo deste trabalho foi avaliar os efeitos da terapia com prednisona no fígado de cães por meio de exames citopatológicos, histopatológicos e ultrassonográficos. Para isto, administrou-se em 11 cães adultos prednisona na dose de 2 mg/kg a cada 24 horas durante 14 dias, sendo feita análise ultrassonográfica pela técnica de histograma e coletadas amostras guiada por ultrassom para avaliação citopatológica e histopatológica no primeiro e último dia de administração do medicamento. A partir dessas mensurações pelo histograma, foi possível detectar um significativo aumento da ecogenicidade hepática ao final do protocolo experimental. As análises citopatológicas e histopatológicas demonstraram presença de vacuolização e granulação citoplasmática promovendo um aumento no tamanho dos hepatócitos. Verificou-se que mesmo em curtos períodos e em dosagens terapêuticas, a prednisona promove importantes efeitos colaterais sobre o parênquima hepático.
