Potilaan hoitamisessa korostuvat mm. triagen tekeminen, potilaan voinnin seuranta ja hoitoa koskevien päätösten tekeminen nopeasti potilaan voinnin mukaan sekä potilaan jatkohoidon turvaaminen. Tämä kaksivaiheinen koulutustutkimus kohdistui päivystyshoitotyön osaamiseen. Tutkimuksen ensimmäisessä vaiheessa määriteltiin päivystyshoitotyön osaaminen ja toisessa vaiheessa arvioitiin valmistuvien sairaanhoitajaopiskelijoiden päivystyshoitotyön osaamista ja osaamiseen yhteydessä olevia tekijöitä. Osaamisen arvioinnin suorittivat opiskelijat itse ja vertailuperustana opiskelijoiden päivystyshoitotyön osaamiselle käytettiin ammatissa toimivien sairaanhoitajien päivystyshoitotyön osaamista. Tutkimuksen tavoitteena oli arvioinnin perusteella määrittää päivystyshoitotyön osaamisen nykytaso ja tehdä tarvittavat ehdotukset päivystyshoitotyön osaamisen kehittämiseen. Tutkimuksen ensimmäisessä vaiheessa (2006–2012) tiedonkeruumenetelminä oli kirjallisuuskatsaus ja asiantuntija-arviointi hyödyntäen delphi-menetelmää. Kirjallisuuskatsauksen perusteella muodostettiin päivystyshoitotyön osaamista kuvaavat pääkategoriat, yläkategoriat ja alakategoriat.Alakategoriat (n=61) annettiin asiantuntijoille (sairaanhoitajat, opettajat, ylihoitajat) arvioitavaksi.Kaksivaiheisen asiantuntija-arvioinnin perusteella muodostui 92 päivystyshoitotyön osaamista kuvaavaa alakategoriaa. Tutkimuksen toisessa vaiheessa (2007–2012) valmistuvien suomalaisten sairaanhoitaja-opiskelijoiden (N=382, n=208, vastausprosentti 55 %) päivystyshoitotyön osaamista arvioitiin tätä tutkimusta varten kehitetyllä mittarilla (Päivystyshoitotyön osaaminen -mittari). Mittari perustui tutkimuksen ensimmäisessä vaiheessa muodostettuun määrittelyyn päivystyshoitotyön osaamisesta. Osaamista mitattiin VAS-janalla (asteikko 0–100) arvon 100 ollessa optimaalinen taso, johon pyrittiin. Sairaanhoitajaopiskelijoiden tavoiteltavaksi osaamisen tasoksi asetettiin 80 olettaen opiskelijoiden osaamisen vielä kehittyvän työkokemuksen myötä. Ammatissa toimivien sairaanhoitajien (N=586, n=280, vastausprosentti 48 %) itsearvioitua osaamista käytettiin vertailuperustana opiskelijoiden osaamiselle. Aineisto analysoitiin tilastollisin menetelmin. Valmistuvien sairaanhoitajaopiskelijoiden itsearvioitu päivystyshoitotyön osaaminen oli alle tavoiteltavan osaamisen tason. Opiskelijoilla oli mielestään eniten eettistä osaamista sekä vuorovaikutus- ja yhteistyöosaamista ja vähiten päätöksenteko-osaamista ja kliinistä osaamista. Myös ammatissa toimivilla sairaanhoitajilla oli mielestään eniten vuorovaikutus- ja yhteistyöosaamista. Vähiten heillä oli ohjausosaamista ja päätöksenteko-osaamista. Sairaanhoitajilla oli tilastollisesti merkitsevästi enemmän päivystyshoitotyön osaamista kuin opiskelijoilla. Opiskelijoiden päivystyshoitotyön osaamista selitti eniten aikaisempi terveysalan tutkinto. Päivystyshoitotyön osaamisen kehittämisehdotukset kohdistuvat ammatillisen peruskoulutuksen ja täydennyskoulutuksen opetuksen sisältöihin ja määrään, opetus- ja opiskelumenetelmiin, osaamisen arviointiin sekä urasuunnitteluun. Jatkotutkimusehdotukset kohdistuvat päivystyshoitotyön osaamisen määrittelyn ja osaamista arvioivan mittarin edelleen kehittämiseen, erilaisten arviointimenetelmien kehittämiseen sekä osaamiseen yhteydessä olevien tekijöiden edelleen tutkimiseen.

La collaboration est une compétence essentielle que les futurs médecins doivent développer. La détermination des niveaux de compétence est cruciale dans la planification de cet apprentissage. Les échelles descriptives suscitent un intérêt croissant, car elles décrivent en termes qualitatifs les performances attendues. Nous inspirant de la méthodologie mixte de Blais, Laurier, & Rousseau (2009), nous avons construit en cinq étapes une échelle de niveau de compétence de collaboration: 1) formulation d’une liste d’indicateurs situés à quatre niveaux de la formation médicale (préclinique, externat, résidence junior et sénior) par les chercheurs (n= 3) et un groupe d’éducateurs (n=7), leaders pédagogiques possédant une expertise pour la compétence de collaboration; 2) sondage en ligne comprenant quatre questionnaires portant sur les niveaux de 118 indicateurs, auprès d’enseignants cliniciens représentant les différentes spécialités (n=277); 3) analyse, avec le modèle partial credit de Rasch, des réponses aux questionnaires appariés par calibration concurrente; 4) détermination des niveaux des indicateurs par les éducateurs et les chercheurs; et 5) rédaction de l’échelle à partir des indicateurs de chaque niveau. L’analyse itérative des réponses montre une adéquation au modèle de Rasch et répartit les indicateurs sur l’échelle linéaire aux quatre niveaux. Les éducateurs déterminent le niveau des 111 indicateurs retenus en tenant compte des résultats du sondage et de la cohérence avec le curriculum. L’échelle comporte un paragraphe descriptif par niveau, selon trois capacités : 1) participer au fonctionnement d’une équipe; 2) prévenir et gérer les conflits; et 3) planifier, coordonner et dispenser les soins en équipe. Cette échelle rend explicites les comportements collaboratifs attendus à la fin de chaque niveau et est utile à la planification de l’apprentissage et de l’évaluation de cette compétence. La discordance entre les niveaux choisis par les éducateurs et ceux issus de l’analyse des réponses des enseignants cliniciens est principalement due au faible choix de réponse du niveau préclinique par les enseignants et aux problèmes d’adéquation pour les indicateurs décrivant la gestion des conflits. Cette recherche marque une avan- cée dans la compréhension de la compétence de collaboration et démontre l’efficacité de la méthodologie de Blais (2009) dans un contexte de compétence transversale, en sciences de la santé. Cette méthodologie pourrait aider à approfondir les trajectoires de développement d’autres compétences.

Introducción: A los pacientes con Insuficiencia Cardíaca (IC) estadio D candidatos a trasplante cardiaco se les realiza la determinación de las presiones de la arteria pulmonar por CCD considerada como prueba de oro. Se decidió conocer si la medición de las presión de la arteria pulmonar obtenidas por ECO TT tenían concordancia con las obtenidas por el CCDen estos paciente que fueron evaluados para trasplante cardiaco. Metodología: Se realizó una recolección retrospectiva de los datos consignados en las historias clínicas de todos los pacientes que fueron receptores de un trasplante cardiaco en la FCI-IC desde septiembre del 2005 hasta Mayo del 2013 y se determinó la concordancia entre la presión sistólica pulmonar evaluado por ECO TT y CCD. Resultados: Se incluyeron 46 pacientes. El 76,1% son hombres y con edad promedio de 46,4 ± 13,1. La PSAP estimada por CCD fue 48,3 ± 13,1 mmHg Vs 45,1 ± 12,1 mmHg por ECO TT. La fracción de eyección fue 15,1 ± 4,06% (IC: 13,95-16,36). El 75,9% de los pacientes tenían HTP moderada y severa. La concordancia entre la PSAP determinada por ambos métodos fue 0,475 (I,C: 0.256 - 0.694), y el coeficiente de correlación intraclase fue de 0,090, indicando una baja concordancia entre los dos métodos. Discusión: La determinación de las presiones pulmonares determinada por ECO TT tiene mala concordancia con las obtenidas por CCD. No se debe usar este estudio para excluir los pacientes candidatos a trasplante cardiaco ni para evaluar las variaciones de las misma en la evolución de la enfermedad.

Para tal efecto, el docente universitario requiere del dominio conceptual y metodológico de las distintas técnicas de evaluación formativa. Este artículo da cuenta de las principales técnicas de evaluación por competencias en escenarios clínicos, concibiéndola como una competencia fundamental para la docencia universitaria en Salud.

Background: There is increased interest in developing training in cognitive behaviour therapy (CBT) with children and young people. However, the assessment of clinical competence has relied upon the use of measures such as the Cognitive Therapy Scale-Revised (CTSR: Blackburn et al., 2001) which has been validated to assess competence with adults. The appropriateness of this measure to assess competence when working with children and young people has been questioned. Aim: This paper describes the development and initial evaluation of the Cognitive Behaviour Therapy Scale for Children and Young People (CBTSCYP) developed specifically to assess competence in CBT with children and young people. Method: A cross section of child CBT practitioners (n = 61) were consulted to establish face validity. Internal reliability, convergent validity and discriminative ability were assessed in two studies. In the first, 12 assessors independently rated a single video using both the Cognitive Behaviour Therapy Scale for Children and Young People (CBTS-CYP) and Cognitive Therapy Scale-Revised (CTS-Revised: Blackburn et al., 2001). In the second, 48 different recordings of CBT undertaken with children and young people were rated on both the CBTS-CYP and CTS-R. Results: Face validity and internal reliability of the CBTS-CYP were high, and convergent validity with the CTS-R was good. The CBTS-CYP compared well with the CTSR in discriminative ability. Conclusion: The CBTS-CYP provides an appropriate way of assessing competence in using CBT with children and young people. Further work is required to assess robustness with younger children and the impact of group training in reducing interrater variations.

Inequalities within dentistry are common and are reflected in wide differences in the levels of oral health and the standard of care available both within and between countries and communities. Furthermore there are patients, particularly those with special treatment needs, who do not have the same access to dental services as the general public. The dental school should aim to recruit students from varied backgrounds into all areas covered by the oral healthcare team and to train students to treat the full spectrum of patients including those with special needs. It is essential, however, that the dental student achieves a high standard of clinical competence and this cannot be gained by treating only those patients with low expectations for care. Balancing these aspects of clinical education is difficult. Research is an important stimulus to better teaching and better clinical care. It is recognized that dental school staff should be active in research, teaching, clinical work and frequently administration. Maintaining a balance between the commitments to clinical care, teaching and research while also taking account of underserved areas in each of these categories is a difficult challenge but one that has to be met to a high degree in a successful, modern dental school.

The objective of this work was to assess the knowledge about orthodontic tooth movement and dental trauma held by a group of orthodontists in specific areas of Brazil. For this purpose, 166 questionnaires with 15 objective questions about this subject were distributed. One hundred and five questionnaires were properly filled and collected after 30 days. It was concluded that, except for avulsion, the knowledge on dental injuries held by the professionals interviewed was considered unsatisfactory, and about 40% of them were not acquainted with the recommendations for the orthodontic movement of traumatized teeth.

Due to shortage of time and limited availability of faculty surgeons to teach basic surgical skills during medical graduation, the search for alternative ways of simulated training with feedback is needed. The purpose of this study was to compare the simulated teaching of suture skills to novice medical students by senior medical students and by experienced faculty surgeons. Forty-eight novice medical students were randomly assigned to three practice conditions on bench model (n = 16): self-directed suture training (control), senior medical student-directed suture skills' training, or experienced faculty surgeon-directed suture skills' training. Pre- and post-tests were applied. Global Rating Scale with blinded evaluation and self-perceived confidence based on Likert scale were used to assess all suture performances in pre- and post-training. Effect size was also calculated. The analysis made after training showed that the students who received feedback from the instructors had better performance based on the Global Rating Scale (all p < 0.0000) and felt more confident to carry out sutures (all p < 0.0000) when compared to the control. There was no significant difference (all p > 0.05) between the student-directed teaching and faculty-directed teaching groups. The magnitude of the effect (instructor-directed training suture) was considered large (>0.80) in all measurements. The acquisition of suture skills after student-directed training was similar to the training supervised by faculty surgeon, and the increase in suture performances of trainees that received instructor administered training was superior to self-directed learning. © 2013 Springer-Verlag Italia.

Einleitung In unserer Institution sind die Anforderungen an Schauspielpatienten (SPs) seit 2010 stetig angestiegen(Guttormsen et al., 2013). Zudem stieg auch die Zahl der benötigten SPs insgesamt, so dass wir uns entschieden ein effektives Aufnahmeverfahren gemäss professioneller Qualitätskriterien (Wallace, 2007) zu gestalten. Methoden Seit 2012 organisieren wir 2 x jährlich eine Informations-Veranstaltung für interessierte Bewerber. In einem ersten Teil wird über die Einsätze von SPs informiert und die Rahmenbedingungen dargelegt. In einem zweiten Teil werden Improvisationsübungen in Gruppen, zu zweit oder einzeln durchgeführt. Die SP-Trainer beobachten das Spiel. Zusätzlich wird die Performance gefilmt. Alle Interessierten müssen sich nach dieser 2-stündigen Veranstaltung für 2 weitere Einzeltermine einschreiben: 1. Spielen eines OSCE-Falles (mit SP-Trainer) 2. Körperliche Untersuchung (mit Arzt). Anschliessend wird im Team entschieden, ob alle Voraussetzungen in Bezug auf Teamfähigkeit/Zuverlässigkeit, schauspielerische Fertigkeiten, körperliche Merkmale für eine Anstellung erfüllt sind. Ergebnisse Mit der Einführung dieses strukturierten Auswahlverfahrens haben wir folgendes erreicht: die ausgewählten SPs passen sehr gut in das Team und erfordern weniger organisatorischen Aufwand. Die Rollen können exakter auf das Profil der jeweiligen SPs abgestimmt werden (insbes. durch die körperliche Untersuchung).  Mit der Informationsveranstaltung für die Gruppe wird der zeitliche Aufwand für die Information der SPs gegenüber reinen Einzelinterviews gesenkt (50%), trotzdem können durch die Einzeltermine detaillierte Informationen erhoben werden. Diskussion Seit der Neustrukturierung des Auswahlverfahrens entsprechen die gewählten SPs deutlich besser unserem Anforderungsprofil. In Zukunft planen wir das Anforderungsprofil schriftlich zu dokumentieren und zu veröffentlichen, um das Verfahren transparent zu machen. Literatur: [1] Guttormsen, S., C. Beyeler, R. Bonvin, S. OBJECTIVE: To pilot test if Orthopaedic Surgery residents could self-assess their performance using newly created milestones, as defined by the Accreditation Council on Graduate Medical Education. METHODS: In June 2012, an email was sent to Program Directors and administrative coordinators of the 154 accredited Orthopaedic Surgery Programs, asking them to send their residents a link to an online survey. The survey was adapted from the Orthopaedic Surgery Milestone Project. Completed surveys were aggregated in an anonymous, confidential database. SAS 9.3 was used to perform the analyses. RESULTS: Responses from 71 residents were analyzed. First and second year residents indicated through self-assessment that they had substantially achieved Level 1 and Level 2 milestones. Third year residents reported they had substantially achieved 30/41, and fourth year residents, all Level 3 milestones. Fifth year, graduating residents, reported they had substantially achieved 17 Level 4 milestones, and were extremely close on another 15. No milestone was rated at Level 5, the maximum possible. Earlier in training, Patient Care and Medical Knowledge milestones were rated lower than the milestones reflecting the other four competencies of Practice Based Learning and Improvement, Systems Based Practice, Professionalism, and Interpersonal Communication. The gap was closed by the fourth year. CONCLUSIONS: Residents were able to successfully self-assess using the 41 Orthopaedic Surgery milestones. Respondents' rate improved proficiency over time. Graduating residents report they have substantially, or close to substantially, achieved all Level 4 milestones. Milestone self-assessment may be a useful tool as one component of a program's overall performance assessment strategy.

BACKGROUND: Child maltreatment is underreported in the United States and in North Carolina. In North Carolina and other states, mandatory reporting laws require various professionals to make reports, thereby helping to reduce underreporting of child maltreatment. This study aims to understand why emergency medical services (EMS) professionals may fail to report suspicions of maltreatment despite mandatory reporting policies. METHODS: A web-based, anonymous, voluntary survey of EMS professionals in North Carolina was used to assess knowledge of their agency's written protocols and potential reasons for underreporting suspicion of maltreatment (n=444). Results were based on descriptive statistics. Responses of line staff and leadership personnel were compared using chi-square analysis. RESULTS: Thirty-eight percent of respondents were unaware of their agency's written protocols regarding reporting of child maltreatment. Additionally, 25% of EMS professionals who knew of their agency's protocol incorrectly believed that the report should be filed by someone other than the person with firsthand knowledge of the suspected maltreatment. Leadership personnel generally understood reporting requirements better than did line staff. Respondents indicated that peers may fail to report maltreatment for several reasons: they believe another authority would file the report, including the hospital (52.3%) or law enforcement (27.7%); they are uncertain whether they had witnessed abuse (47.7%); and they are uncertain about what should be reported (41.4%). LIMITATIONS: This survey may not generalize to all EMS professionals in North Carolina. CONCLUSIONS: Training opportunities for EMS professionals that address proper identification and reporting of child maltreatment, as well as cross-agency information sharing, are warranted.
