Fibre distribution and orientation in a series of round panel specimens of ultra high performance fibre reinforced concrete (UHPFRC) was investigated using electrical resistivity measurements and confirmed by X-ray CT imaging. By pouring specimens in different ways, the orientation of steel fibres was influenced and the sensitivity of the electrical resistivity technique was investigated. The round panels were tested in flexure and the results are discussed in relation to the observed orientation of fibres in the panels. It was found that the fibres tended to align perpendicular to the direction of flow. As a result, panels poured from the centre were significantly stronger than panels poured by other methods because the alignment of fibres led to more fibres bridging the radial cracks formed during mechanical testing.

Bone defects in revision knee arthroplasty are often located in load-bearing regions. The goal of this study was to determine whether a physiologic load could be used as an in situ osteogenic signal to the scaffolds filling the bone defects. In order to answer this question, we proposed a novel translation procedure having four steps: (1) determining the mechanical stimulus using finite element method, (2) designing an animal study to measure bone formation spatially and temporally using micro-CT imaging in the scaffold subjected to the estimated mechanical stimulus, (3) identifying bone formation parameters for the loaded and non-loaded cases appearing in a recently developed mathematical model for bone formation in the scaffold and (4) estimating the stiffness and the bone formation in the bone-scaffold construct. With this procedure, we estimated that after 3 years mechanical stimulation increases the bone volume fraction and the stiffness of scaffold by 1.5- and 2.7-fold, respectively, compared to a non-loaded situation.

Study design: A retrospective study of image guided cervical implant placement precision. Objective: To describe a simple and precise classification of cervical critical screw placement. Summary of Background Data: "Critical" screw placement is defined as implant insertion into a bone corridor which is surrounded circumferentially by neurovascular structures. While the use of image guidance has improved accuracy, there is currently no classification which provides sufficient precision to assess the navigation success of critical cervical screw placement. Methods: Based on postoperative clinical evaluation and CT imaging, the orthogonal view evaluation method (OVEM) is used to classify screw accuracy into grade I (no cortical breach), grade la (screw thread cortical breach), grade II (internal diameter cortical breach) and grade III (major cortical breach causing neural or vascular injury). Grades II and III are considered to be navigation failures, after accounting for bone corridor / screw mismatch (minimal diameter of targeted bone corridor being smaller than an outer screw diameter). Results: A total of 276 screws from 91 patients were classified into grade I (64.9%), grade la (18.1%), and grade II (17.0%). No grade III screw was observed. The overall rate of navigation failure was 13%. Multiple logistic regression indicated that navigational failure was significantly associated with the level of instrumentation and the navigation system used. Navigational failure was rare (1.6%) when the margin around the screw in the bone corridor was larger than 1.5 mm. Conclusions: OVEM evaluation appears to be a useful tool to assess the precision of critical screw placement in the cervical spine. The OVEM validity and reliability need to be addressed. Further correlation with clinical outcomes will be addressed in future studies.

Ce mémoire présente l’étude de la faisabilité de la planification de traitements pour la curiethérapie interstitielle du sein en utilisant l’imagerie par résonance magnétique (IRM) seule au lieu de l’imagerie par tomodensitométrie (CT). L'imagerie CT étant la référence, la mesure des différences observables sur la distribution de doses provenant des deux types d’imagerie a été effectuée. Des fantômes de seins ont été fabriqués et utilisés, ainsi que l’imagerie de patients. La taille des fantômes en fonction du positionnement dans l’appareil d’IRM et la longueur reconstruite des cathéters ont été analysées. Les différences dans les distributions de doses de fantômes et de patients ont été calculées en s’assurant que la reconstruction des cathéters provenant des images CT et IRM est la seule variable. La différence dans les critères de doses à respecter est plus grande lorsque la taille du fantôme et/ou un déplacement latéral dans l’IRM sont plus grands. La longueur reconstruite des cathéters est comparable entre les deux techniques d’imagerie. Pour le petit fantôme des différences <2% ont été observées pour tous les critères de dose. Pour le grand fantôme et pour les patients, une valeur maximale de 5% est observée pour les critères sur la cible, mais peut atteindre 19% pour le critère Externe V150%/V100% pour le grand fantôme et 33% pour les patients. Par contre, le seuil clinique de ce critére est toujours respecté. Ceci nous indique que pour la plupart des patients, la zone à traiter serait bien couverte en utilisant les images IRM uniquement pour planifier.

Les artéfacts métalliques entraînent un épaississement artéfactuel de la paroi des tuteurs en tomodensitométrie (TDM) avec réduction apparente de leur lumière. Cette étude transversale prospective, devis mesures répétées et observateurs avec méthode en aveugle, chez 24 patients consécutifs/71 tuteurs coronariens a pour objectif de comparer l’épaisseur de paroi des tuteurs en TDM après reconstruction par un algorithme avec renforcement des bords et un algorithme standard. Une angiographie coronarienne par TDM 256 coupes a été réalisée, avec reconstruction par algorithmes avec renforcement des bords et standard. L’épaisseur de paroi des tuteurs était mesurée par méthodes orthogonale (diamètres) et circonférentielle (circonférences). La qualité d’image des tuteurs était évaluée par échelle ordinale, et les données analysées par modèles linéaire mixte et régression logistique des cotes proportionnelles. L’épaisseur de paroi des tuteurs était inférieure avec l’algorithme avec renforcement des bords comparé à l’algorithme standard, avec les méthodes orthogonale (0,97±0,02 vs 1,09±0,03 mm, respectivement; p<0,001) et circonférentielle (1,13±0,02 vs 1,21±0,02 mm, respectivement; p<0,001). Le premier causait moins de surestimation par rapport à l’épaisseur nominale comparé au second, avec méthodes orthogonale (0,89±0,19 vs 1,00±0,26 mm, respectivement; p<0,001) et circonférentielle (1,06±0,26 vs 1,13±0,31 mm, respectivement; p=0,005) et diminuait de 6 % la surestimation. Les scores de qualité étaient meilleurs avec l’algorithme avec renforcement des bords (OR 3,71; IC 95% 2,33–5,92; p<0,001). En conclusion, la reconstruction des images avec l’algorithme avec renforcement des bords génère des parois de tuteurs plus minces, moins de surestimation, et de meilleurs scores de qualité d’image que l’algorithme standard.

The calvarial bone (Cb) grafts have been reported to produce greater responses compared with other donor regions in maxillofacial reconstructions, but the scientific evidences for this are still lacking. The objectives of this study are (1) to study the morphological pattern of Cb onlay bone grafts and compare them with the biological events through immunohistochemical responses and (2) to establish the effects of perforations in maintaining the volume and bone density of the receptor bed. Material and methods Sixty New Zealand White rabbits were submitted to Cb onlay bone grafts on the mandible. In 30 rabbits, the receptor bed was perforated (perforated group), while for the remaining animals the bed was kept intact (non-perforated group). Six animals from each group were sacrificed at 5, 7, 10, 20 and 60 days after surgery. Histological sections from the grafted area were prepared for immunohistochemical and histological analyses. Immuno-labeling was found for proteins Osteoprotegerin (OPG), receptor activator of nuclear factor-kappa beta ligand (RANKL), alkaline phosphatase (ALP), osteopontin (OPN), vascular endothelial growth factor (VEGF), tartrate-resistant acid phosphatase (TRAP), Type I collagen (COL I) and osteocalcin (OC). The tomography examination [computerized tomography (CT) scan] was conducted just after surgery and at the sacrifice. Results The histological findings revealed that the perforations contributed to higher bone deposition during the initial stages at the graft-receptor bed interface, accelerating the graft incorporation process. The results of the CT scan showed lower resorption for the perforated group (P < 0.05), and both groups showed high bone density rates at 60 days. This set of evidences is corroborated by the immunohistochemical outcomes indicating that proteins associated with revascularization and osteogenesis (VEGF, OPN, TRAP and ALP) were found in higher levels in the perforated group. Conclusions These findings indicate that the bone volume of calvarial grafts is better maintained when the receptor bed is perforated, probably resulting from more effective graft revascularization and greater bone deposition. The process of bone resorption peaked between 20 and 60 days post-operatively in both groups although significantly less in the perforated group. Purpose: To estimate the metabolic activity of rectal cancers at 6 and 12 weeks after completion of chemoradiation therapy (CRT) by 2-[fluorine-18] fluoro-2-deoxy-D-glucose-labeled positron emission tomography/computed tomography ([18 FDG] PET/CT) imaging and correlate with response to CRT. Methods and Materials: Patients with cT2-4N0-2M0 distal rectal adenocarcinoma treated with long-course neoadjuvant CRT (54 Gy, 5-fluouracil-based) were prospectively studied (ClinicalTrials. org identifier NCT00254683). All patients underwent 3 PET/CT studies (at baseline and 6 and 12 weeks fromCRT completion). Clinical assessment was at 12 weeks. Maximal standard uptakevalue (SUVmax) of the primary tumor wasmeasured and recorded at eachPET/CTstudy after 1 h (early) and3 h (late) from 18 FDGinjection. Patientswith an increase in early SUVmax between 6 and 12 weeks were considered " bad" responders and the others as "good" responders. Results: Ninety-one patients were included; 46 patients (51%) were "bad" responders, whereas 45 (49%) patients were " good" responders. " Bad" responders were less likely to develop complete clinical response (6.5% vs. 37.8%, respectively; PZ. 001), less likely to develop significant histological tumor regression (complete or near-complete pathological response; 16% vs. 45%, respectively; PZ. 008) and exhibited greater final tumor dimension (4.3cmvs. 3.3cm; PZ. 03). Decrease between early (1 h) and late (3 h) SUVmax at 6-week PET/CTwas a significant predictor of " good" response (accuracy of 67%). Conclusions: Patients who developed an increase in SUVmax after 6 weeks were less likely to develop significant tumor downstaging. Early-late SUVmax variation at 6-week PET/CT may help identify these patients and allow tailored selection of CRT-surgery intervals for individual patients. (C) 2012 Elsevier Inc.

BACKGROUND: Neoadjuvant chemoradiation (CRT) therapy may result in significant tumor regression in patients with rectal cancer. Patients who develop complete tumor regression have been managed by treatment strategies that are alternatives to standard total mesorectal excision. Therefore, assessment of tumor response with positron emission tomography/computed tomography (PET/CT) after neoadjuvant treatment may offer relevant information for the selection of patients to receive alternative treatment strategies. METHODS: Patients with clinical T2 (cT2) through cT4NxM0 rectal adenocarcinoma were included prospectively. Neoadjuvant therapy consisted of 54 grays of radiation and 5-fluorouracil-based chemotherapy. Baseline PET/CT studies were obtained before CRT followed by PET/CT studies at 6 weeks and 12 weeks after the completion of CRT. Clinical assessment was performed at 12 weeks after CRT completion. PET/CT results were compared with clinical and pathologic data. RESULTS: In total, 99 patients were included in the study. Twenty-three patients were complete responders (16 had a complete clinical response, and 7 had a complete pathologic response). The PET/CT response evaluation at 12 weeks indicated that 18 patients had a complete response, and 81 patients had an incomplete response. There were 5 false-negative and 10 false-positive PET/CT results. PET/CT for the detection of residual cancer had 93% sensitivity, 53% specificity, a 73% negative predictive value, an 87% positive predictive value, and 85% accuracy. Clinical assessment alone resulted in an accuracy of 91%. PET/CT information may have detected misdiagnoses made by clinical assessment alone, improving overall accuracy to 96%. CONCLUSIONS: Assessment of tumor response at 12 weeks after CRT completion with PET/CT imaging may provide a useful additional tool with good overall accuracy for the selection of patients who may avoid unnecessary radical resection after achieving a complete clinical response. Objective: This study aimed to assess the presence of additional foramina and canals in the anterior palate region, through cone beam computed tomography (CBCT) images, describing their location, direction, and diameter. Materials & Methods: CBCT exams of 178 subjects displaying the anterior maxilla were included and the following parameters were registered: gender; age group; presence of additional foramina in the anterior palate (AFP) with at least 1 mm in diameter; location and diameter of AFP; and direction of bony canals associated with AFP. Results: Twenty-eight patients (15.7%) presented AFP and in total 34 additional foramina were registered. No statistical differences between patients with or without AFP were found for gender or age. The average diameter of AFP was 1.4 mm (range from 1 to 1.9 mm). Their location was variable, with most of the cases occurring in the alveolar process near the incisors or canines (n = 27). In 18 cases, AFP was associated with bony canals with upward or oblique direction toward the anterior nasal cavity floor. In 14 cases, the canal presented as a direct extension of the canalis sinuosus, in an upward direction laterally to the nasal cavity aperture. In two cases, the canal was observed adjacent to the incisive and joined the nasopalatine canal superiorly. Discussion: CBCT images have a crucial role in the recognition of anatomical variations by allowing detailed tridimensional evaluations. Additional foramina and canals in the anterior region of the upper jaw are relatively frequent. Practitioners should be aware and trained to identify these variations. Conclusions: Over 15% of the population studied had additional foramina in the anterior palate, between 1 mm and 1.9 mm wide, with variable locations. In most cases the canals associated with these foramina either presented as a direct extension of the canalis sinuosus, or coursed towards the nasal cavity floor.

Für eine erfolgreiche Behandlung bösartiger Tumore ist eine frühzeitige Diagnose, aber auch eine effektive und effiziente Therapie essentiell. In diesem Zusammenhang sind Nanomaterialien in den Fokus der Arzneimittelentwicklung gerückt, welche für Diagnostik und Therapie genutzt werden könnten.rnSystematische Studien zur Radiometallmarkierung von Nanopartikeln und deren Stabilität in vitro im Zusammenhang mit der Struktur des Linkers und dem Anteil an Chelator wurden anhand verschiedener HPMA-DOTA-Konjugate durchgeführt. Es konnte gezeigt werden, dass die Linkerstruktur und der Belegungsgrad sowohl die Markierung als auch die in vitro -Stabilität von radiometallmarkierten HPMA-rnDOTA-Konjugaten beeinflussen.rnFür die Markierung selbst stehen mehrere Generator-produzierte metallische Positronenemitter zur Verfügung. Infolge der gesetzlichen Bestimmungen muss das Eluat der Generatoren bestimmte Spezifikationen (Elutionsausbeute, Durchbruch des Mutternuklids, Gehalt an Fremdionen, pH-Wert etc.) erfüllen, um für die Darstellung von Radiopharmaka verwendet werden zu können.rnFür das bereits etablierte PET-Nuklid 68Ga konnte eine Ethanol-basierte Aufreinigung entwickelt werden, welche hohe Elutions- und Markierungsausbeuten sowie Radionuklidreinheit garantiert und damit einen wichtigen Schritt für die Entwicklung von Kit-Formulierungen repräsentiert. Ausserdem konnten zwei Methoden zur Qualitätskontrolle entwickelt werden, welche es ermöglichen die Radionuklidreinheit des initialen 68Ga-Eluats, aber auch des finalen 68Ga-Radiopharmakons innerhalb einer Stunde ohne γ–Spektroskopie zu bestimmen.rnWährend mit 68Ga die Pharmakokinetik markierter Derivate für einen Zeitraum von bis 3 Stunden zugänglich ist, deckt das Generator-produzierte 44Sc eine Periode von bis zu einem Tag ab. Damit lässt sich die Pharmakokinetik markierter polymerer Drug Carrier-Systeme – von der frühen Ausscheidungsphase bis hin zu organspezifischen Akkumulationen durch passives und aktives Targeting – gut beschreiben.rnFür 44Sc konnte anhand der Modellverbindung DOTATOC gezeigt werden, dass das aufgereinigte Generatoreluat für die Markierung mit hohen radiochemischen Ausbeuten geeignet ist und etablierte Markierungsmethoden übertragbar sind. In weiterführenden Studien zur molekularen Bildgebung könnte das Potential dieses PET-Nuklids für die Langzeitbildgebung gezeigt werden.
