Objectives. Verify the influence of different filler distributions on the subcritical crack growth (SCG) susceptibility, Weibull parameters (m and sigma(0)) and longevity estimated by the strength-probability-time (SPT) diagram of experimental resin composites. Methods. Four composites were prepared, each one containing 59 vol% of glass powder with different filler sizes (d(50) = 0.5; 0.9; 1.2 and 1.9 mu m) and distributions. Granulometric analyses of glass powders were done by a laser diffraction particle size analyzer (Sald-7001, Shimadzu, USA). SCG parameters (n and sigma(f0)) were determined by dynamic fatigue (10(-2) to 10(2) MPa/s) using a biaxial flexural device (12 x 1.2 mm; n = 10). Twenty extra specimens of each composite were tested at 10(0) MPa/s to determine m and sigma(0). Specimens were stored in water at 37 degrees C for 24 h. Fracture surfaces were analyzed under SEM. Results. In general, the composites with broader filler distribution (C0.5 and C1.9) presented better results in terms of SCG susceptibility and longevity. C0.5 and C1.9 presented higher n values (respectively, 31.2 +/- 6.2(a) and 34.7 +/- 7.4(a)). C1.2 (166.42 +/- 0.01(a)) showed the highest and C0.5 (158.40 +/- 0.02(d)) the lowest sigma(f0) value (in MPa). Weibull parameters did not vary significantly (m: 6.6 to 10.6 and sigma(0): 170.6 to 176.4 MPa). Predicted reductions in failure stress (P-f = 5%) for a lifetime of 10 years were approximately 45% for C0.5 and C1.9 and 65% for C0.9 and C1.2. Crack propagation occurred through the polymeric matrix around the fillers and all the fracture surfaces showed brittle fracture features. Significance. Composites with broader granulometric distribution showed higher resistance to SCG and, consequently, higher longevity in vitro. (C) 2012 Academy of Dental Materials. Published by Elsevier Ltd. OBJECTIVE: We present a prospective study of a microemulsion of cyclosporin to treat idiopathic nephrotic syndrome in ten children with normal renal function who presented cyclosporin trough levels between 50 and 150 ng/ml and achieved complete remission with cyclosporin. To compare the pharmacokinetic parameters of cyclosporin in idiopathic nephrotic syndrome during remission and relapse of the nephrotic state. METHOD: The pharmacokinetic profile of cyclosporin was evaluated with the 12-hour area under the time-concentration curve (auc0-12) using seven time-point samples. This procedure was performed on each patient during remission and relapse with the same cyclosporin dose in mg/kg/day. The 12-hour area under the time-concentration curve was calculated using the trapezoidal rule. All of the pharmacokinetic parameters and the resumed 4-hour area under the time-concentration curve were correlated with the 12-hour area under the time-concentration curve. RESULTS: There were no significant differences in any parameters of the pharmacokinetic of cyclosporin during remission and relapse, even when the data were normalized by dose. The best correlation with the 12-hour area under the time-concentration curve was the 4-hour area under the time-concentration curve on remission and relapse of the disease, followed by the 2-hour level after cyclosporin (c2) dosing in both disease states. CONCLUSIONS: These data indicate that the same parameters used for cyclosporin therapeutic monitoring estimated during the nephrotic state can also be used during remission. Larger controlled studies are needed to confirm these findings.

Aim. The aim of this study was to evaluate whether an association of elastic stockings and walking for a short period in the late afternoon reduces leg edema. Methods. Volume changes of the legs of sixteen patients (32 limbs), who walked on a treadmill for 30 minutes using elastic compression stockings, were analyzed in a quantitative, cross-over randomized (in order of arrival at the clinic) study. They were submitted to volumetry using the water displacement technique and subsequently required to put on 20/30 made-to-measure compression stockings (Sigvaris). The patients walked on a treadmill for 30 minutes and after removing the stockings volumetry of the legs was again performed. Legs were assessed using the CEAP classification and divided into groups. Analysis of variance was used for statistical analysis with an alpha error of 5% being considered acceptable. Results. When participants walked wearing compression stockings, there was a reduction in leg volume. When the CEAP classification was evaluated, it was noted that there was a statistically significant difference for the CEAP C0, C1 and C2 categories of legs using stockings compared to those that did not use. Conclusion. Compression stockings have a synergistic effect with walking in the late afternoon thus reducing edema of the lower limbs. [Int Angiol 2012;31:490-3]

A resiliência física de solos é proveniente de processos regenerativos que incluem ciclos de umedecimento e secamento, congelamento e descongelamento, assim como as atividades biológicas. Este estudo testou a hipótese de que as propriedades físicas do solo, como a permeabilidade do solo ao ar, densidade do solo, porosidade de aeração e porosidade total, são indicadores físicos eficientes para quantificar a resiliência de solos de diferentes texturas submetidos ao estresse mecânico (compactação) e após subsequentes ciclos de umedecimento e secamento. O objetivo foi avaliar o comportamento e a resiliência do solo por meio de propriedades físicas de dois Latossolos Vermelhos. Foram retiradas 25 amostras indeformadas (0,00-0,05 m) de dois solos: solo I, com textura argilosa, e solo II, com textura franco-argilo-arenosa, realizando as determinações das propriedades físicas nos tratamentos: antes da compactação (A), depois da compactação (C0) e após ciclos de umedecimento e secamento (C1, C2, C3 e C4). As propriedades densidade do solo e porosidade total não apresentaram recuperação da condição inicial após a compactação nos solos I e II; as propriedades conteúdo volumétrico de água e porosidade de aeração mostraram recuperação parcial apenas no solo I; e a permeabilidade do solo ao ar foi a propriedade que apresentou a melhor recuperação e a maior resiliência. Em relação ao distinto comportamento dos dois solos, observou-se que o solo I foi mais resiliente que o solo II nas propriedades que apresentaram recuperação.

Gegenstand und Ziel der vorliegenden Arbeit war die Synthese und Charakterisierung einer Hydrogelmatrix, welche für die Herstellung eines vielseitig verwendbaren Sensors, der mehrere Analyten (Proteine, DNA etc.) in hoher Verdünnung (c0 < 50 fM) aus kleinsten Probenmengen (Volumina <20 nl) schnell (t < 1 min) und parallel nachweisen kann, Verwendung finden soll. Der Fokus der Arbeit lag dabei in der Synthese und Charakterisierung von Copolymeren als Hydrogelmatrix, welche geeignetes temperaturabhängiges Verhalten zeigen. Die Copolymere wurden in eine dreidimensionale Netzwerkstruktur überführt und auf einer Goldoberfläche kovalent angebunden, um Delamination zu vermeiden und die Untersuchung mittels Oberflächenplasmonenresonanz-Spektroskopie (SPR) und Optischer Wellenleiter-Spektroskopie (OWS) zu erlauben. Weiterhin wurde das temperaturabhängige Verhalten der Polymernetzwerke in Wasser mittels optischen Messungen (SPR/OWS) untersucht, um Erkenntnisse über die Quell- und Kollabiereigenschaften des Hydrogels zu gewinnen. Um temperaturschaltbare Hydrogele herzustellen, wurden auf N-Isopropylacrylamid (NIPAAm) basierende Polymere synthetisiert. Es wurde sowohl die für Hydrogele übliche Methode der freien radikalischen Vernetzungspolymerisation in Wasser, wie eine neue, auf Benzophenoneinheiten basierende Syntheseroute, welche die freie radikalische Polymerisation in organischem Medium nutzt, verwendet. Die synthetisierten Polymere sind Copolymere aus N‑Isopropylacrylamid (NIPAAm) und 4-Methacryloyloxybenzophenon (MABP). NIPAAm ist dabei für das temperaturschaltbare Verhalten der Gele verantwortlich und MABP dient als Photovernetzer. Weitere Copolymere, die neben den genannten Monomeren noch andere Funktionen, wie z.B. ionische Gruppen oder Aktivesterfunktionen enthalten, wurden ebenfalls synthetisiert. Das temperaturabhängige Quellverhalten in Bezug auf die chemische Zusammensetzung wurde mit der Oberflächenplasmonenresonanz-Spektroskopie (SPR) und Optischen Wellenleiter-Spektroskopie (OWS) untersucht. Es zeigte sich, dass die Anwesenheit von Salz im Hydrogel (Natriumacrylat als Monomer, P4S) Inhomogenität, in Form eines Brechungsindexgradienten senkrecht zur Substratoberfläche, hervorruft. Dies ist nicht der Fall, wenn statt des Salzes die Säure (Methacrylsäure als Monomer, P4A) verwendet wird. Durch die Inhomogenität lassen sich die Filme mit dem Natriummethacrylat nicht mehr mit dem, üblicherweise zur Auswertung genutzten, Kastenmodell beschreiben. Die Anwendung der Wentzel-Kramers-Brillouin-Näherung (WKB) auf die Messdaten führt hingegen zu dem gewünschten Ergebnis. Man findet ein kastenähnliches Brechungsindexprofil für das Hydrogel mit der Säure (P4A) und ein Gradientenprofil für das Gel mit dem Salz (P4S). Letzteres ist nicht nur hydrophiler und insgesamt stärker gequollen, sondern ragt auch weiter in die überstehende Wasserphase hinein. Anhand eines säurehaltigen Hydrogels (P8A) konnte der quellungshemmende Einfluss von hohen Salzkonzentrationen gezeigt werden. Weiterhin wurde während des Quellvorgangs eine gewisse Anisotropie gefunden, die aber im vollständig gequollenen und vollständig kollabierten Zustand nicht mehr vorliegt. Anhand eines Hydrogels ohne ionisierbare Gruppen (P9) wurde die Reversibilität des Quell- und Kollabiervorgangs gezeigt. Bei einem Vergleich zwischen einem säurehaltigen Hydrogel (P8A, Quellgrad von 7,3) und einem ohne ionisierbare Gruppen (P9, Quellgrad von 6,1), hat die Anwesenheit der 8 mol% Säuregruppen eine leichte Verstärkung der Quellung um den Faktor 1,2 bewirkt. Rasterkraftmikroskopische Untersuchungen (AFM) an diesen beiden Hydrogelen im getrockneten Zustand, haben gezeigt, dass nach dem Quellen, Kollabieren und Trocknen bei beiden Gelen Porenstrukturen sehr unterschiedlicher Ausmaße vorliegen.

Microemulsions are thermodynamically stable, macroscopically homogeneous but microscopically heterogeneous, mixtures of water and oil stabilised by surfactant molecules. They have unique properties like ultralow interfacial tension, large interfacial area and the ability to solubilise other immiscible liquids. Depending on the temperature and concentration, non-ionic surfactants self assemble to micelles, flat lamellar, hexagonal and sponge like bicontinuous morphologies. Microemulsions have three different macroscopic phases (a) 1phase- microemulsion (isotropic), (b) 2phase-microemulsion coexisting with either expelled water or oil and (c) 3phase- microemulsion coexisting with expelled water and oil.rnrnOne of the most important fundamental questions in this field is the relation between the properties of the surfactant monolayer at water-oil interface and those of microemulsion. This monolayer forms an extended interface whose local curvature determines the structure of the microemulsion. The main part of my thesis deals with the quantitative measurements of the temperature induced phase transitions of water-oil-nonionic microemulsions and their interpretation using the temperature dependent spontaneous curvature [c0(T)] of the surfactant monolayer. In a 1phase- region, conservation of the components determines the droplet (domain) size (R) whereas in 2phase-region, it is determined by the temperature dependence of c0(T). Individuare e conoscere la natura degli inquinanti atmosferici e disporre dei dati delle emissioni sono azioni fondamentali per formulare politiche ambientali incentrate sul miglioramento della qualità dell'aria e monitorarne l'efficacia. Sorge l'esigenza di un controllo costante della qualità dell'aria, processo che avviene utilizzando delle centraline di monitoraggio fisse sparse nelle vie delle maggiori città o nei pressi dei principali insediamenti industriali. Lo scopo di questo progetto è quello di realizzare una stazione di monitoraggio mobile al fine di aumentare la superficie di controllo, realizzando un oggetto dinamico capace di acquisire dati sull'inquinamento. Questo è stato fatto applicando ad un drone un sistema di sensori capaci di rilevare le variazioni dei livelli di concentrazione degli agenti inquinanti. Ciò permette di eliminare le stazioni di monitoraggio fisse, le quali rappresentano una spesa ingente. Inoltre, attraverso l'utilizzo di un drone, è possibile monitorare siti più vasti, permettendo un monitoraggio costante e ripetuto nel tempo. La prima parte dell'elaborato analizza il sistema Embedded utilizzato per l'acquisizione dei dati, concentrando l'attenzione prevalentemente sui moduli utilizzati. La seconda descrive quali sono i primi passi per cominciare ad utilizzare i sensori posti sulla Gases Board 2.0 e risponde ai dubbi più comuni su quali parametri di configurazione adottare per un avere una risposta adeguata, quale processo di calibrazione seguire o come trasformare i dati acquisiti, espressi in tensioni, in valori di concentrazione di gas. La terza parte illustra i test effettuati per verificare il corretto funzionamento del sistema completo, con l’esposizione delle problematiche individuate, e una presentazione delle alternative più valide per superarle.

Although the influence of cytochrome P450 inhibitory drugs on the area under the curve (AUC) of cyclosporine (CsA) has been described, data concerning the impact of these substances on the shape of the blood concentration curve are scarce. By assessment of CsA blood levels before and 1, 2, and 4 hr after oral intake (C0, C1, C2, and C4, respectively) CsA profiling examinations were performed in 20 lung transplant recipients taking 400 mg, 200 mg, and no itraconazole, respectively. The three groups showed comparable results for C0, C2, and AUC(0-12). Greater values were found for Cmax, Cmax-C0, peak-trough fluctuation and rise to Cmax in favor of the non-itraconazole group. Additionally, tmax was shorter in the non-itraconazole group. Comedication with the metabolic inhibitor itraconazole is associated with a flattening of the CsA blood concentration profile in lung transplant recipients. These changes cannot be assessed by isolated C0, C2, or AUC(0-12) values alone.

STUDY DESIGN: Case report and review of the literature. OBJECTIVE: We describe the first case of a synovial cyst arising from pseudarthrosis of a previous dens fracture. The literature is reviewed and etiological, diagnostic, and therapeutic options of atlantoaxial cysts are discussed. SUMMARY OF BACKGROUND DATA: Symptomatic synovial cysts of the atlantoaxial joint are rare. To the authors' knowledge only 24 cases have been reported.A 60-year-old patient presented with bilateral hand numbness, quadrihyperreflexia, and gait deterioration. Magnetic resonance imaging of the cervical spine disclosed a cystic mass located at the transverse ligament of dens axis causing bulbomedullary compression. METHODS: Surgery was performed via transoral image guided approach. The ventral atlas arch, dens, transverse ligament, tectorial membrane, and the compressing cyst were removed, followed by a C0-C3 fusion. RESULTS: Two months postsurgery the patient recovered completely from the cervical myelopathy with transient remnant dysparesthesia of the finger tips. CONCLUSION: Magnetic resonance imaging findings are not specific enough to establish a preoperative diagnosis. Radical resection via image-guided transoral route followed by posterior fusion allows complete resection of the cystic lesion and results in excellent long-term decompression.

BACKGROUND After heart transplantation (HTx), the interindividual pharmacokinetic variability of immunosuppressive drugs represents a major therapeutic challenge due to the narrow therapeutic window between over-immunosuppression causing toxicity and under-immunosuppression leading to graft rejection. Although genetic polymorphisms have been shown to influence pharmacokinetics of immunosuppressants, data in the context of HTx are scarce. We thus assessed the role of genetic variation in CYP3A4, CYP3A5, POR, NR1I2, and ABCB1 acting jointly in immunosuppressive drug pathways in tacrolimus (TAC) and ciclosporin (CSA) dose requirement in HTx recipients. METHODS Associations between 7 functional genetic variants and blood dose-adjusted trough (C0) concentrations of TAC and CSA at 1, 3, 6, and 12 months after HTx were evaluated in cohorts of 52 and 45 patients, respectively. RESULTS Compared with CYP3A5 nonexpressors (*3/*3 genotype), CYP3A5 expressors (*1/*3 or *1/*1 genotype) required around 2.2- to 2.6-fold higher daily TAC doses to reach the targeted C0 concentration at all studied time points (P ≤ 0.003). Additionally, the POR*28 variant carriers showed higher dose-adjusted TAC-C0 concentrations at all time points resulting in significant differences at 3 (P = 0.025) and 6 months (P = 0.047) after HTx. No significant associations were observed between the genetic variants and the CSA dose requirement. CONCLUSIONS The CYP3A5*3 variant has a major influence on the required TAC dose in HTx recipients, whereas the POR*28 may additionally contribute to the observed variability. These results support the importance of genetic markers in TAC dose optimization after HTx.

BACKGROUND Everolimus (ERL) has become an alternative to calcineurin inhibitors (CNIs) due to its renal-sparing properties, especially in heart transplant (HTx) recipients with kidney dysfunction. However, ERL dosing is challenging due to its narrow therapeutic window combined with high inter-individual pharmacokinetic variability. Our aim was to evaluate the effect of clinical and genetic factors on ERL dosing in a pilot cohort of 37 HTx recipients. METHODS Variants in CYP3A5, CYP3A4, CYP2C8, POR, NR1I2, and ABCB1 were genotyped and clinical data were retrieved from patient charts. RESULTS While ERL trough concentration (C0 ) was within the targeted range for most patients, over 30-fold variability in the dose-adjusted ERL C0 was observed. Regression analysis revealed a significant effect of the non-functional CYP3A5*3 variant on the dose-adjusted ERL C0 (P = 0.031). ERL dose requirement was 0.02 mg/kg/day higher in patients with CYP3A5*1/*3 genotype compared to patients with CYP3A5*3/*3 to reach the targeted C0 (P = 0.041). ERL therapy substantially improved estimated glomerular filtration rate (28.6 ± 6.6 ml/min/1.73m(2) ) in patients with baseline kidney dysfunction. CONCLUSION ERL pharmacokinetics in HTx recipients is highly variable. Our preliminary data on patients on a CNI-free therapy regimen suggest that CYP3A5 genetic variation may contribute to this variability. This article is protected by copyright. All rights reserved.

About one third of the anthropogenic carbon dioxide (CO2) released into the atmosphere in the past two centuries has been taken up by the ocean. As CO2 invades the surface ocean, carbonate ion concentrations and pH are lowered. Laboratory studies indicate that this reduces the calcification rates of marine calcifying organisms, including planktic foraminifera. Such a reduction in calcification resulting from anthropogenic CO2 emissions has not been observed, or quantified in the field yet. Here we present the findings of a study in the Western Arabian Sea that uses shells of the surface water dwelling planktic foraminifer Globigerinoides ruber in order to test the hypothesis that anthropogenically induced acidification has reduced shell calcification of this species. We found that light, thin-walled shells from the surface sediment are younger (based on 14C and d13C measurements) than the heavier, thicker-walled shells. Shells in the upper, bioturbated, sediment layer were significantly lighter compared to shells found below this layer. These observations are consistent with a scenario where anthropogenically induced ocean acidification reduced the rate at which foraminifera calcify, resulting in lighter shells. On the other hand, we show that seasonal upwelling in the area also influences their calcification and the stable isotope (d13C and d18O) signatures recorded by the foraminifera shells. Plankton tow and sediment trap data show that lighter shells were produced during upwelling and heavier ones during non-upwelling periods. Seasonality alone, however, cannot explain the 14C results, or the increase in shell weight below the bioturbated sediment layer. We therefore must conclude that probably both the processes of acidification and seasonal upwelling are responsible for the presence of light shells in the top of the sediment and the age difference between thick and thin specimens.
