Background Progress in identifying genetic factors protective against alcohol dependence (AlcD) requires a paradigm shift in psychiatric epidemiology. Aims To integrate analysis of research into the genetics of alcoholism. Method Data from prospective questionnaire and interview surveys of the Australian twin panel, and from a subsample who underwent alcohol challenge, were analysed. Results In men, effects of alcohol dehydrogenase ADH2*1/*2 genotype or high alcohol sensitivity (risk-decreasing), and of history of childhood conduct disorder, or having monozygotic co-twin or twin sister with AlcD (risk-increasing) were significant and comparable in magnitude. Religious affiliation (Anglican versus other) was associated with the ADH2 genotype, but did not explain the associations with AlcD symptoms. No protective effect of the ADH2*1/*2 genotype was observed in women. Conclusions The early onset and strong familial aggregation of AlcD, and opportunity for within-family tests of genetic association to avoid confounding effects, make epidemiological family studies of adolescents and young adults and their families a priority.

Substance misuse in individuals with schizophrenia is very common, especially in young men, in communities where use is frequent and in people receiving inpatient treatment. Problematic use occurs at very low intake levels, so that most affected people are not physically dependent (with the exception of nicotine). People with schizophrenia and substance misuse have poorer symptomatic and functional outcomes than those with schizophrenia alone. Unless there is routine screening, substance misuse is often missed in assessments. Service systems tend to be separated, with poor inter-communication, and affected patients are often excluded from services because of their comorbidity. However, effective management of these disorders requires a fully integrated approach because of the close inter-relationship of the disorders. Use of atypical antipsychotics may be especially important in this population because of growing evidence (especially on clozapine and risperidone) that nicotine smoking, alcohol misuse and possibly some other substance misuse is reduced. Several pharmacotherapies for substance misuse can be used safely in people with schizophrenia, but the evidence base is small and guidelines for their use are necessarily derived from experience in the general population.

Zoonotic visceral leishmaniasis (ZVL) is a serious public health problem in several Brazilian cities. Although the proximity of chicken houses is often cited as a risk factor in studies of urban ZVL, the role chickens play in the epidemiology of the disease has not been defined. Chickens attract both male and female sand flies (Lutzomyia longripalpis) but are unable to sustain Leishmania infections, and their presence may exert a zooprophylactic effect. We discuss environmental, physiologic, socioeconomic, and cultural factors related to chicken raising that could influence Le. infantum transmission in Brazilian cities and evaluate whether this practice significantly affects the risk of acquiring ZVL.

The incidence of surgically confirmed cystic echinococcosis in eastern Libya was estimated to be at least 4.2 cases/100,000, with significantly more female cases than male. The prevalences of infection with Echinococcus granulosus among 1087 sheep, 881 goats, 428 camels and 614 cattle from the same region, determined postmortem in abattoirs, were 20%, 3.4%, 13.6% and 11%, respectively. Infection in the livestock was age-dependent and, generally, the female animals were more often infected than the male. The measurements of rostellar hooks on protoscoleces collected from sheep and cattle were similar but significantly different from the corresponding measurements of parasites of human or camel origin. However, when a portion of the cytochrome c-oxidase subunit I (coxl) gene from each of 30 protoscolex samples from Libya (12 from cattle, three from humans, five from camels and 10 from sheep) was sequenced, the sequences were all found to be identical to that published for the common sheep strain of E. granulosus.

The authors identify key issues that researchers, funding bodies, ethics committees and ethicists might consider in contemplating research subject payment ethics. They argue that what is missing from the broader debate is due consideration of ethics committee decision processes; research subject reasons for participation; and current research practices. The authors explore these issues and how they relate to existing guidelines on voluntary consent, and arguments that have been proposed for and against research subject payments. Bovine Respiratory Disease (BRD) results from a complex, multifactorial interaction of stressors, animal susceptibility, and respiratory pathogens. The infectious agents associated with BRD are ubiquitous among cattle populations. Typically, one or a combination of stressors are necessary to initiate BRD. Prevention of BRD should, therefore, address management procedures to minimise stressors. Administration of vaccines against BRD agents may help reduce the incidence of BRD but is unlikely to eliminate the condition. The effectiveness of antimicrobials in the treatment of BIRD depends primarily on early recognition and treatment. The use of antioxidant vitamins, minerals or other agents in the prevention and treatment of BRD warrants further research.

A anemia de Fanconi é um raro distúrbio autossômico recessivo caracterizado por malformações congênitas, aplasia da medula óssea e instabilidade genômica, com predisposição ao desenvolvimento de neoplasias malignas, em especial as leucemias e os tumores do trato aerodigestivo alto. Em razão de características inerentes à síndrome em questão, o tratamento de tais neoplasias é particularmente difícil. Relata-se o caso de anemia de Fanconi uma jovem de 24 anos, que desenvolveu carcinoma de células escamosas da hipofaringe, na ausência de fatores de risco como o tabagismo e o alcoolismo, e faz-se uma revisão sumária da literatura a respeito do tema.

Foi feito um levantamento da ocorrência de anemia ferropriva no Vale do Ribeira, nas localidades de Iguape, Apiaí, Ribeira, Barra do Chapéu e Pontal do Ribeira, através de dosagens, no sangue, de Hemoglobina, Hematócrito, Ferro sérico e Capacidade de Ligação de Ferro. Na mesma amostra populacional, pelo estudo da composição dos alimentos consumidos, foi obtida a ingestão de ferro, média, diária, "per capita" nas seis localidades referidas. Os resultados da adequação de consumo foram: em Iguape, 91%; Pontal do Ribeira, 63%; Icapara, 81%; Apiaí, 122%; Ribeira, 99% e em Barra do Chapéu, 125%. Através dos índices aplicados, evidenciou-se a existência de anemia como problema de Saúde Pública na grande maioria das áreas estudadas.

O objetivo proposto foi verificar a prevalência de deficiência de ferro, num estrato de parturientes e recém-nascidos do município de São Paulo, de características sócio-econômicas bem definidas. O levantamento bioquímico constituiu-se de dosagens de concentração de hemoglobina, hematócrito, ferro sérico e capacidade de ligação de ferro. Os resultados desse levantamento mostraram uma prevalência de 52,3% de anemia em parturientes e de 21% de anemia em recém-nascidos. Concluiu-se que a ministração de sais de ferro, em serviços especializados de atendimento às gestantes pertencentes a classe sócio-econômica semelhante à essa estudada, seria uma medida profilática que poderia diminuir essas porcentagens de anemia.

Com o objetivo de estudar deficiências de vitamina A e de ferro em mulheres grávidas, a nível de comunidade, estudou-se 60 gestantes, residentes na zona urbana de Apiaí e Ribeira (Estado de São Paulo), através de inquérito bioquímico, realizando-se dosagens séricas de beta-caroteno, vitamina A, hemoglobina, ferro sérico e capacidade de ligação de ferro, bem como determinação do hematócrito. Os resultados foram comentados e apontaram não haver hipovitaminose A nesta amostra, apesar de haver grande proporção de resultados de caroteno sérico considerados não-normais. Houve maior prevalência de anemia entre gestantes de Ribeira. A comparação dos resultados obtidos entre as gestantes com respectivos grupos-testemunha, apontou freqüência maior de casos considerados não-normais entre as primeiras, em todos os coeficientes levantados.
