Tutkimuksessa selvitetään Suomen terveydenhuollon kehityslinjoja 1800-luvulta nykyaikaan pääasiassa institutionalismin, polkuriippuvuuden ja funktionalismin teorioita hyväksi käyttäen. Tutkimuksen keskiössä ovat instituutiot ja niiden vaikutus terveydenhuoltoon. Miten terveyspoliittiset periaatteet ja arvot ovat muuttuneet ja mitkä tahot ovat muutokset toteuttaneet? Huomiota on kiinnitetty myös politiikkaan ja maan taloudellisessa tilanteessa tapahtuneisiin muutoksiin. Tutkimuksen empiirisessä osassa terveydenhuolto ja siihen tehdyt muutokset on jaettu neljään ajanjaksoon. Kunnilla on velvollisuus järjestää terveydenhuolto ja mahdollisuus päättää laajasti sen toteutuksesta. Valtion rooli on ohjaava ja kuntien rahoitusta tukeva. Merkittävä muutos tapahtui 1960luvulla, kun terveydenhuolto jakaantui kunnalliseen terveydenhoitojärjestelmään sekä sairausvakuutukseen ja työterveyshuoltoon. Yksityisten ja julkisten terveyspalvelujen vuorovaikutusta ei ole pystytty ratkaisemaan. Talouden ohella terveydenhuoltoa ovat ohjanneet politiikka ja instituutiot. Vaihtoehtoiset terveydenhuollon järjestämismallit ovat jääneet vähälle huomiolle. Kuntien itsehallinnon korostaminen terveyspalvelujen toteutuksessa on johtanut hajautettuun, pirstoutuneeseen järjestelmään. Paikallisilla päätöksillä on ollut etusija, mikä on vaikeuttanut keskitettyä ohjausta.

The compound is derived from a novel form of the monopropellant oxalohydroxamic acid. The two exocyclic C-O bond lengths of 1.240 (3) and 1.228 (4)A indicate double bonds. The C-N bond lengths of 1.334 (4), 1.390 (4) and 1.359 (4) A are characteristic of the amide bond. The N atom covalently bonded to the two carbonyl C atoms acts as a proton donor in an intermolecular hydrogen bond to the ring O atom: N1...O3i = 2.854 ]k (i =x-- 1,y, z), H...O = 2.15 A, N-H...O = 159 °.

The protective effect of bacteriophage was assessed against experimental Staphylococcus aureus lethal bacteremia in streptozotocin (STZ) induced-diabetic and non-diabetic mice. Intraperitoneal administrations of S. aureus (RCS21) of 2 x 10(8) CFU caused lethal bacteremia in both diabetic and non-diabetic mice. A single administration of a newly isolated lytic phage strain (GRCS) significantly protected diabetic and nondiabetic mice from lethal bacteremia (survival rate 90% and 100% for diabetic and non-diabetic bacteremic groups versus 0% for saline-treated groups). Comparison of phage therapy to oxacillin treatment showed a significant decrease in RCS21 of 5 and 3 log units in diabetic and nondiabetic bacteremic mice, respectively. The same protection efficiency of phage GRCS was attained even when the treatment was delayed up to 4 h in both diabetic and non-diabetic bacteremic mice. Inoculation of mice with a high dose (10(10) PFU) of phage GRCS alone produced no adverse effects attributable to the phage per se. These results suggest that phages could constitute valuable prophylaxis against S. aureus infections, especially in immunocompromised patients. (C) 2010 Institut Pasteur. Diastereomers (SRu,Sc)-1a and (RRu,Sc)-1b, in a ratio of 85: 15 and formulated as [Ru(η-MeC6H4Pri-p)Cl(L*)], have been prepared by treating [{Ru(η-MeC6H4Pri-p)Cl2}2] with the sodium salt of (S)-α-methylbenzylsalicylaldimine (HL*) in tetrahydrofuran at –70 °C. The reaction of 1(1a+1b) with AgClO4 in acetone followed by an addition of PPh3 or 4-methylpyridine (4Me-py) leads to the formation of adducts [Ru(η-MeC6H4Pri-p)(PPh3)(L*)]ClO42[(SRu,Sc)2a, (FRu,Sc)2b] and [Ru(η-MeC6H4Pri-p)(4Me-py)(L*)]ClO43[(SRu,Sc)3a, (RRu,Sc)3b] in the diastereomeric ratios (SRu,Sc) : (RRu,Sc) of 2 : 98 and 76 : 24, respectively. Complex 1 crystallises with equal numbers of 1a and 1b molecules in an asymmetric unit of monoclinic space group P21 with a= 10.854(1), b= 17.090(1), c= 12.808(4)Å, β= 110.51(1)°, and Z= 4. The structure was refined to R= 0.0552 and R′= 0.0530 with 2893 reflections having I[gt-or-equal] 1.5σ(I). The absolute configurations of the chiral centres in the optically pure single crystal of the PPh3 adduct have been obtained from an X-ray study. Crystals of formulation [Ru(η-MeC6H4Pri-p)-(PPh3)(L*)]2[ClO4][PF6]·1.5 CHCl3, obtained in presence of both ClO4 and PF6 anions, belong to the non-centric triclinic space group P1 with a= 10.852(2), b= 14.028(1), c= 15.950(2)Å, α= 91.51(1), β= 105.97(1), γ= 106.11(1)°, and Z= 2. The final residuals were R= 0.0713, R′= 0.0752 with 7283 reflections having I[gt-or-equal] 2.5σ(I). The crystal structures of 1a,1b, and the PPh3 adduct (2b,2b′) consist of a ruthenium(II) centre bonded to a η-p-cymene, a bidentate chelating Schiff base, and a unidentate ligand (Cl or PPh3). The chirooptical properties of the complexes have been studied using 1H NMR and CD spectral data. The presence of a low-energy barrier for the intermediate involved in these reactions, showing both retention as well as inversion of the metal configuration, is discussed.

Large amplitude stationary Rossby wave trains with wavelength in the range 50 degrees to 60 degrees longitude have been identified in the upper troposphere during May, through the analysis of 200 hPa wind anomalies. The spatial phase of these waves has been shown to differ by about 20 degrees of longitude between the dry and wet Indian monsoon years. It has been shown empirically that the Rossby waves are induced by the heat sources in the ITCZ. These heat sources appear in the Bay of Bengal and adjoining regions in May just prior to the onset of the Indian summer monsoon. The inter-annual spatial phase shift of the Rossby waves has been shown to be related to the shift in the deep convection in the zonal direction.

Emerging high-dimensional data mining applications needs to find interesting clusters embeded in arbitrarily aligned subspaces of lower dimensionality. It is difficult to cluster high-dimensional data objects, when they are sparse and skewed. Updations are quite common in dynamic databases and they are usually processed in batch mode. In very large dynamic databases, it is necessary to perform incremental cluster analysis only to the updations. We present a incremental clustering algorithm for subspace clustering in very high dimensions, which handles both insertion and deletions of datapoints to the backend databases.

The generation of a 16 μm laser beam through cascading in a downstream‐mixing CO2 gasdynamic laser is studied. To simulate actual lasing action, a generalized, two‐dimensional, flow‐radiation‐coupled power extraction model for a gasdynamic laser is used. Also, to model the cascade process a new four‐mode CO2‐N2 vibrational kinetic model has been proposed. The steady‐state intensity obtained for an exclusive 9.4 μm transition is of the order of 5×107 W/m2. In the cascade mode of operation the steady‐state intensities for 9.4 and 16 μm transitions of the order of 5×107 W/m2 and 1.0×106 W/m2, respectively, have been obtained.

This article is concerned with subsurface material identification for the 2-D Helmholtz equation. The algorithm is iterative in nature. It assumes an initial guess for the unknown function and obtains corrections to the guessed value. It linearizes the otherwise nonlinear problem around the background field. The background field is the field variable generated using the guessed value of the unknown function at each iteration. Numerical results indicate that the algorithm can recover a close estimate of the unknown function based on the measurements collected at the boundary.

Mesoporous MnO2 samples with average pore-size in the range of 2-20 nm are synthesized in sonochemical method from KMnO4 by using a tri-block copolymer, namely, poly(ethylene glycol)-block-poly(propylene glycol)-block-poly(ethylene glycol) (P123) as a soft template as well as a reducing agent. The MnO2 samples are found to be poorly crystalline. On increasing the amplitude of sonication, a change in the morphology of MnO2 from nanoparticles to nanorods and also change in porosity are observed. A high BET surface area of 245 m(2) g(-1) is achieved for MnO2 sample. The MnO2 samples are subjected to electrochemical capacitance studies by cyclic voltammetry (CV) and galvanostatic charge-discharge cycling in 0.1 M aqueous Ca(NO3)(2) electrolyte. A maximum specific capacitance (SC) of 265 Fg(-1) is obtained for the MnO2 sample synthesized in sonochemical method using an amplitude of 30 mu m. The MnO2 samples also possess good electrochemical stability due to their favourable porous structure and high surface area. (C) 2012 Elsevier B.V. All rights reserved.

Poly{(N,N-(dimethylamino)ethyl methacrylate]-co-(methyl methacrylate)} copolymers of various compositions were synthesized by reversible addition-fragmentation chain transfer (RAFT) polymerization at 70 degrees C in N,N-dimethylformamide. The polymer molecular weights and molecular weight distributions were obtained from size exclusion chromatography, and they indicated the controlled nature of the RAFT polymerizations; the polydispersity indices are in the range 1.11.3. The reactivity ratios of N,N-(dimethylamino)ethyl methacrylate (DMAEMA) and methyl methacrylate (MMA) (rDMAEMA = 0.925 and rMMA = 0.854) were computed by the extended KelenTudos method at high conversions, using compositions obtained from 1H NMR. The pH- and temperature-sensitive behaviour were studied in aqueous solution to confirm dual responsiveness of these copolymers. The thermal properties of the copolymers with various compositions were investigated by differential scanning calorimetry and thermogravimetric analysis. The kinetics of thermal degradation were determined by Friedmann and Chang techniques to evaluate various parameters such as the activation energy, the order and the frequency factor. „Winter fishery“ on brown shrimp does not imply a special type of fishery. It merely means the continuation of the standard fishing procedure of parts of the fleet during wintertime, when the majority of the mostly smaller vessels stay in harbour due to generally unfavourable weather conditions for their activity. During 1990 to 1999 mean European wide landings in January and February summedup to 854 tonnes making up to only 4 % of the mean annual landings (21 805 t). While German vessels landed0.7 % (68.7 t) of their mean annual landings during that period, the other countries caught about 7 % of their individual, mean annual landings at the same time. The Netherlands and Denmark contributed highest tonnages of 580and 110 tonnes, respectively, to the total European landings, making up 81 % of them. As about 70 % of brown shrimp may carry eggs in January, the winter fishery took a mean total of about 2.15 x 1012brown shrimp eggs out of the stocks in that period annually. As there is no reliable assessment available concerning the brown shrimp stocks, it is despite of these high losses of eggs not possible to trace a negative effect of the winter fishery in scientific terms. However, precautional catch reductions in winter would be in favour of higher survival rates of eggs, which are the carrying source for the recruitment of brown shrimp stocks and catches in forthcoming summer and autumn seasons according to Dutch investigations.

Point-particle based direct numerical simulation (PPDNS) has been a productive research tool for studying both single-particle and particle-pair statistics of inertial particles suspended in a turbulent carrier flow. Here we focus on its use in addressing particle-pair statistics relevant to the quantification of turbulent collision rate of inertial particles. PPDNS is particularly useful as the interaction of particles with small-scale (dissipative) turbulent motion of the carrier flow is mostly relevant. Furthermore, since the particle size may be much smaller than the Kolmogorov length of the background fluid turbulence, a large number of particles are needed to accumulate meaningful pair statistics. Starting from the relative simple Lagrangian tracking of so-called ghost particles, PPDNS has significantly advanced our theoretical understanding of the kinematic formulation of the turbulent geometric collision kernel by providing essential data on dynamic collision kernel, radial relative velocity, and radial distribution function. A recent extension of PPDNS is a hybrid direct numerical simulation (HDNS) approach in which the effect of local hydrodynamic interactions of particles is considered, allowing quantitative assessment of the enhancement of collision efficiency by fluid turbulence. Limitations and open issues in PPDNS and HDNS are discussed. Finally, on-going studies of turbulent collision of inertial particles using large-eddy simulations and particle- resolved simulations are briefly discussed.
