Background: This study aimed to examine factors associated with treatment adherence in first-episode psychosis (FEP) patients followed up over 8 years, especially involuntary first admission and stopping cannabis use. Methods: This prospective, longitudinal study of FEP patients collected data on symptoms, adherence, functioning,and substance use. Adherence to treatment was the main outcome variable and was categorized as ‘good’ or ‘bad’. Cannabis use during follow-up was stratified as continued use, stopped use, and never used. Bivariate and logistic regression models identified factors significantly associated with adherence and changes in adherence over the 8-year follow-up period. Results: Of the 98 FEP patients analyzed at baseline, 57.1% had involuntary first admission, 74.4% bad adherence,and 52% cannabis use. Good adherence at baseline was associated with Global Assessment of Functioning score (p = 0.019), Hamilton Depression Rating Scale score (p = 0.017) and voluntary admission (p < 0.001). Adherence patterns over 8 years included: 43.4% patients always bad, 26.1% always good, 25% improved from bad to good. Among the improved adherence group, 95.7% had involuntary first admission and 38.9% stopped cannabis use. In the subgroup of patients with bad adherence at baseline, involuntary first admission and quitting cannabis use during follow up were associated with improved adherence. Conclusions: The long-term association between treatment adherence and type of first admission and cannabis use in FEP patients suggest targets for intervention to improve clinical outcomes.

Mozambique tilapia (Oreochromis mossambicus) is an indigenous tilapia species in southern Africa, until now the majority of genetic research has been carried out on Asian species of tilapia but this project aims to look at this African species. Those most suited to further development in aquaculture in southern Africa have now been identified. The genetic characterisation of strains has been completed. This information has aided the choice of strains for use in small scale aquaculture and for genetically male tilapia (GMT) production. They will form the basis of future strategies for further genetic improvement, and management of genetic diversity of Mozambique tilapia. The information will also contribute towards responsible management and development of genetic resources, particularly with regard to indigenous species of tilapia. Good progress has been made with the adaptation and implementation of producing the supermale fish required to produce all male offspring, resulting in faster growing populations of tilapia. The presence of the project and its associated activity has been a catalyst for a surge in interest in tilapia culture throughout southern Africa. [PDF contains 183 pages]

This is the report from the Central Area Fisheries Advisory Committee meeting, which was held on the 1st April, 1981. It covers information on Rivers Leven and Kent fish distribution and water quality surveys 1980, authority works on Headwater Beck of the River Hodder and the River Douglas, the annual report on match fishing at Skerton and Mitton fisheries and the report by the area fisheries officer on fisheries activities. This comments on river conditions and fishing, migratory fish movement,an update on Middleton hatchery, salmon cages in Dalton-in-Furness, and Langcliffe hatchery. Finally the report adds about stockings by the authority, fish mortalities, fish disease and the monthy catches of salmon and sea trout for the 1980 season. The Fisheries Advisory Committee was part of the Regional Water Authorities, in this case the North West Water Authority. This preceded the Environment Agency which came into existence in 1996.

The report contains the minutes of previous meetings, draft netting rights on the River Eden, rod an line licence duties, a pipeline project by British Gas Corporation, fisheries byelaw on baiting, and information on a survey of goosanders and mergansers. It also contains the report by the area fisheries officer which looks at river conditions and fishing, general comments on fish stocks at Holmwrangle hatchery, a fish mortality on the River Wiza, salmon spawning and monthly salmon and sea trout catches for the 1979 season. The Fisheries Advisory Committee was part of the Regional Water Authorities, in this case the North West Water Authority. This preceded the Environment Agency which came into existence in 1996.

Fundamentos: Apesar dos conhecimentos adquiridos sobre marcadores preditores de mortalidade na síndrome coronariana aguda (SCA), a capacidade de avaliação a longo prazo permanece desconhecida. O peptídeo natriurético tipo B (BNP) tem sido extensamente utilizado, porém as evidências existentes se restringem ao seguimento de curto e médio prazos. Objetivos: Determinar se o BNP é um preditor independente de mortalidade por todas as causas a longo prazo em pacientes com síndrome coronariana aguda sem supradesnível do segmento ST (SCASSST). Métodos: No período de 1o de Janeiro de 2002 a 31 de Dezembro de 2003, foram selecionados 224 pacientes consecutivos atendidos na sala de emergência com SCASSST. A dosagem do BNP à admissão foi incorporada no protocolo diagnóstico, tendo o seu valor sido correlacionado com a mortalidade ao final do seguimento. Resultados: Os pacientes foram acompanhados por 9,34 anos (mediana), tinham 71,5 anos (intervalo IQ=60,5;79,0) e com predomínio do gênero masculino (62,9%). A hipertensão arterial esteve presente em 82,1% e o diabetes em 23,7%. A angina instável (AI) foi diagnosticada em 52,2% e o infarto agudo do miocárdio sem supradesnível do segmento ST (IAMSSST) em 47,8%. O BNP mediano foi de 81,9 pg/ml (intervalo IQ 22,2; 225). A mortalidade se correlacionou com os quartis crescentes do BNP: 14,3; 16,1; 48,2; e 73,2% (p<0,0001). A curva ROC determinou o BNP=100 pg/ml como o melhor ponto de corte, tendo apresentado área sobre a curva (AUC) de 0,79 (IC 95%=0,72-0,85) e sendo preditor de mortalidade ao final do seguimento: 17,3% vs. 65,0%, p<0,001, RR=3,76 (IC 95%=2,49-5,63). O BNP teve poder prognóstico tanto nos pacientes com (26,7 vs. 71,2%, p<0,001) como nos sem (12,9 vs. 56,8%, p<0,001) alteração da função ventricular, e também conforme o diagnóstico de AI (18,7 vs. 48,6%, p=0,001) e IAMSSST (14,9 vs. 75,0%, p<0,001). Na análise de regressão logística, a idade>72 anos (OR=3,79, IC 95%=1,62-8,86, p=0,002), o BNP≥100 pg/ml (OR=6,24, IC 95%=2,95-13,23, p<0,001) e a taxa de filtração glomerular estimada (TFGE)(OR=0,98, IC 95%=0,97-0,99, p=0.049) foram preditores independentes de mortalidade. Conclusões: O BNP dosado à admissão dos pacientes com SCASSST é um forte e independente preditor de mortalidade a longo prazo.
