Bacterial vaginosis (BV) is the most prevalent vaginal infection worldwide and is characterized by depletion of the indigenous lactobacilli. Antimicrobial therapy is often ineffective. We hypothesized that probiotic Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14 might provide an adjunct to antimicrobial treatment and improve cure rates. Sixty-four Brazilian women diagnosed with BV were randomly assigned to receive a single dose of tinidazole (2 g) supplemented with either 2 placebo capsules or 2 capsules containing L. rhamnosus GR-1 and L. reuteri RC-14 every morning for the following 4 weeks. At the end of treatment (day 28), the probiotic group had a significantly higher cure rate of BV (87.5%) than the placebo group (50.0%) (p = 0.001). In addition, according to the Gram-stain Nugent score, more women were assessed with ""normal`` vaginal microbiota in the probiotic group (75.0% vs. 34.4% in the placebo group; p = 0.011). This study shows that probiotic lactobacilli can provide benefits to women being treated with antibiotics for an infectious condition.

Observar la incidencia de las variables sexo, edad y permanencia en los centros de reforma en el manejo de la agresión del delincuente joven y compararlo con una muestra de sujetos normales. 120 chicos delincuentes, 40 para cada grupo de edad y escogidos de cuatro centros diferentes. 36 chicas delincuentes escogidas de otros centros. 205 varones y 138 mujeres de los mismos grupos de edad de la población normal. Describe las aportaciones de diversas teorías y modelos sobre la agresión: modelo etológico, psicoanálisis, cohesión social así como una introducción a la psicología del delincuente. Lleva a cabo un estudio empírico sobre una muestra de sujetos delincuentes y otra de sujetos normales para determinar si existen diferencias entre ellos respecto al manejo de la agresión y en función de las variables: sexo, edad y tiempo de permanencia en centros de reclusión. Plantea unas hipótesis de trabajo que posteriormente reformula en función del instrumento de medida elegido para la obtención de los datos. Utiliza un contraste de medias y representaciones gráficas para analizar los datos y extrae conclusiones. Test de frustración de Rosenzweig. Contraste de medias y representaciones gráficas. Edad: los jóvenes delincuentes muestran menos conformidad a la norma que los normales, dan menos respuestas extrapunitivas y más intrapunitivas. Sexo: los delincuentes varones dan el mismo número de respuestas impunitivas que los normales y más respuestas intrapunitivas de defensa ante el super-yo. Lo mismo sucede con las chicas. Chicos y chicas delincuentes de la misma edad no difieren en cuanto a las respuestas ante la conformidad. Los varones dan más respuestas intrapunitivas que las mujeres pero no difieren en el número de respuestas impunitivas. Tiempo de reclusión: no hay diferencias significativas en ningún tipo de respuestas. Los jóvenes delincuentes dirigen la agresión hacia sí mismos, mientras que las chicas lo dirigen hacia el exterior, ya que se sienten maltratadas por el entorno. Destaca como posible explicación, que las chicas tienen un mejor manejo verbalizado de la agresión mientras que los chicos están en un momento más primario en la evolución hacia el metabolismo mental de los conflictos. Además, la sanción social negativa y persecutoria hacia la delincuencia explica la culpa inconsciente de los chicos. Por otro lado, el delito más común entre las chicas es la prostitución y con ello parece que expíen la culpabilidad por la agresión hacia fuera.

PURPOSE Our main objective was to prospectively determine the prognostic value of [(18)F]fluorodeoxyglucose positron emission tomography/computed tomography (PET/CT) after two cycles of rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone given every 14 days (R-CHOP-14) under standardized treatment and PET evaluation criteria. PATIENTS AND METHODS Patients with any stage of diffuse large B-cell lymphoma were treated with six cycles of R-CHOP-14 followed by two cycles of rituximab. PET/CT examinations were performed at baseline, after two cycles (and after four cycles if the patient was PET-positive after two cycles), and at the end of treatment. PET/CT examinations were evaluated locally and by central review. The primary end point was event-free survival at 2 years (2-year EFS). RESULTS Median age of the 138 evaluable patients was 58.5 years with a WHO performance status of 0, 1, or 2 in 56%, 36%, or 8% of the patients, respectively. By local assessment, 83 PET/CT scans (60%) were reported as positive and 55 (40%) as negative after two cycles of R-CHOP-14. Two-year EFS was significantly shorter for PET-positive compared with PET-negative patients (48% v 74%; P = .004). Overall survival at 2 years was not significantly different, with 88% for PET-positive versus 91% for PET-negative patients (P = .46). By using central review and the Deauville criteria, 2-year EFS was 41% versus 76% (P < .001) for patients who had interim PET/CT scans after two cycles of R-CHOP-14 and 24% versus 72% (P < .001) for patients who had PET/CT scans at the end of treatment. CONCLUSION Our results confirmed that an interim PET/CT scan has limited prognostic value in patients with diffuse large B-cell lymphoma homogeneously treated with six cycles of R-CHOP-14 in a large prospective trial. At this point, interim PET/CT scanning is not ready for clinical use to guide treatment decisions in individual patients.

Carbon isotopically based estimates of CO2 levels have been generated from a record of the photosynthetic fractionation of 13C (epsilon p) in a central equatorial Pacific sediment core that spans the last ~255 ka. Contents of 13C in phytoplanktonic biomass were determined by analysis of C37 alkadienones. These compounds are exclusive products of Prymnesiophyte algae which at present grow most abundantly at depths of 70-90 m in the central equatorial Pacific. A record of the isotopic compostion of dissolved CO2 was constructed from isotopic analyses of the planktonic foraminifera Neogloboquadrina dutertrei, which calcifies at 70-90 m in the same region. Values of epsilon p, derived by comparison of the organic and inorganic delta values, were transformed to yield concentrations of dissolved CO2 (c e) based on a new, site-specific calibration of the relationship between epsilon p and c e. The calibration was based on reassessment of existing epsilon p versus c e data, which support a physiologically based model in which epsilon p is inversely related to c e. Values of PCO2, the partial pressure of CO2 that would be in equilibrium with the estimated concentrations of dissolved CO2, were calculated using Henry's law and the temperature determined from the alkenone-unsaturation index UK 37. Uncertainties in these values arise mainly from uncertainties about the appropriateness (particularly over time) of the site-specific relationship between epsilon p and 1/c e. These are discussed in detail and it is concluded that the observed record of epsilon p most probably reflects significant variations in Delta pCO2, the ocean-atmosphere disequilibrium, which appears to have ranged from ~110 µatm during glacial intervals (ocean > atmosphere) to ~60 µatm during interglacials. Fluxes of CO2 to the atmosphere would thus have been significantly larger during glacial intervals. If this were characteristic of large areas of the equatorial Pacific, then greater glacial sinks for the equatorially evaded CO2 must have existed elsewhere. Statistical analysis of air-sea pCO2 differences and other parameters revealed significant (p < 0.01) inverse correlations of Delta pCO2 with sea surface temperature and with the mass accumulation rate of opal. The former suggests response to the strength of upwelling, the latter may indicate either drawdown of CO2 by siliceous phytoplankton or variation of [CO2]/[Si(OH)4] ratios in upwelling waters.
