Os números são uma presença constante nas nossas vidas, por isso não lhes conseguimos ficar indiferentes. Mesmo que de forma inconsciente, procuramos naturalmente padrões numéricos no dia a dia e naqueles acontecimentos que nos marcaram ao longo da vida. (...) Mais curioso ainda é que, de entre os números naturais, parece haver uma clara preferência pelos números primos, que geram muitas vezes sentimentos controversos. (...) Neste artigo, dedicamos a nossa atenção ao 13 e ao azar que aparenta encerrar, pelo menos na cultura ocidental. (...) A partir de meados do século XIX, esta superstição entrou no imaginário coletivo ocidental, sobretudo americano, e eram difundidas regularmente histórias de indivíduos que se haviam sentado numa mesa com outras 12 pessoas e que haviam morrido tragicamente no espaço de um ano. Esta paranóia era alimentada pelos jornais da época, o que levou um grupo de nova-iorquinos a fundar um clube, “The Thirteen Club”, com o objetivo de combater uma superstição que consideravam nefasta para a sociedade. No décimo terceiro dia de cada mês, os membros deste clube juntavam-se para jantar em mesas de 13 lugares. A ideia era provar que, ao fim de um ano, todos continuariam vivos. (...) Lawson teve o dom de juntar no seu romance duas superstições na moda na época: a crença de que 13 é um número de azar e a crença de que sexta-feira é um dia de azar (com origem provavelmente no facto de Jesus ter sido crucificado numa sexta-feira). Desde então, o receio associado à sexta-feira 13 cresceu exponencialmente, derrubando em popularidade todas as outras superstições assocadas ao 13, incluindo a primeira de todas: a fatalidade das mesas com 13 pessoas! Na sua obra "Unpopular Essays", de 1950, o conhecido matemático e filósofo Bertrand Russel refere que o ser humano é um animal crédulo que precisa acreditar em algo e que, na ausência de uma boa crença, ele fica satisfeito com a má. Na cultura ocidental, o 13 é um dos números com mais impacto no universo das superstições e das crenças populares. Há mesmo quem leve muito a sério a suposta influência negativa deste número e que, por isso mesmo, o evite a todo o custo. (...) Algumas companhias aéreas, como a Air France e a Lufthansa, ainda omitem a fila 13 nos seus aviões. Em algumas partes do mundo, é raro conseguir encontrar um Hotel que não tenha renumerado o seu décimo terceiro andar (substituindo o 13 pelo 14 ou por 12A). É o caso, por exemplo, de Nova Iorque. O curioso é que ainda antes dos prédios terem 13 andares, já se saltava do 12 para o 14 na numeração dos quartos. Mas há quem vá mais longe, recusando-se a pernoitar, por exemplo, num quarto 454, por entender que esse número está a camuflar o 13 (note-se que 4+5+4=13). Stephen King, conhecido autor de contos de horror fantástico e de ficção, revelou que, quando está a ler um livro, nunca pára nas páginas 94, 193, 283 e em todas as outras em que a soma dos algarismos seja 13. Os jogadores de críquete da Austrália costumam chamar ao 87 "o número do diabo", já que 87=100-13. Há também quem evite morar numa casa com o número 13 ou que não queira dar um nome ao seu filho com exatamente 13 letras. Outro aspeto referido com frequência tem a ver com o facto do décimo terceiro Arcano Maior do Tarot ser a carta da morte (...) Alguns acontecimentos históricos ajudaram a alimentar a fobia ao 13. Lançada no dia 11 de abril de 1970, às 13h13, a Apollo 13 consistiu na terceira missão tripulada do Projeto Apollo com destino à Lua (note-se que, se adicionarmos os algarismos de 11/04/70, obtém-se 13). Devido a um acidente causado por uma explosão num dos módulos, não foi possível concluir a missão. Mesmo assim, os tripulantes conseguiram regressar com a nave à Terra, após seis dias no espaço. OBJECTIVE: To analyze drug use trends among college students in 1996, 2001 and 2009. METHODS: A cross-sectional epidemiological study with a multistage stratified cluster sample with 9,974 college students was conducted in the city of São Paulo, southeastern Brazil. An anonymous self-administered questionnaire was used to collect information on drug use assessed in lifetime, the preceding 12 months and the preceding 30 days. The Bonferroni correction was used for multiple comparisons of drug use rates between surveys. RESULTS: There were changes in the lifetime use of tobacco and some other drugs (hallucinogens [6.1% to 8.8%], amphetamines [4.6% to 8.7%], and tranquilizers [5.7% to 8.2%]) from 1996 to 2009. Differences in the use of other drugs over the 12 months preceding the survey were also seen: reduced use of inhalants [9.0% to 4.8%] and increased use of amphetamines [2.4% to 4.8%]. There was a reduction in alcohol [72.9% to 62.1%], tobacco [21.3% to 17.2%] and marijuana [15.0% to 11.5%] use and an increase in amphetamine use [1.9% to 3.3%] in the preceeding 30 days. CONCLUSIONS: Over the 13-year study period, there was an increase in lifetime use of tobacco, hallucinogens, amphetamines, and tranquilizers. There was an increase in amphetamine use and a reduction in alcohol use during the preceding 12 months. There was an increase in amphetamine use during the preceding 30 days.

The aim of this case series was to describe the clinical, laboratory and epidemiological characteristics and the presentation of bacillary angiomatosis cases (and/or parenchymal bacillary peliosis) that were identified in five public hospitals of Rio de Janeiro state between 1990 and 1997; these cases were compared with those previously described in the medical literature. Thirteen case-patients were enrolled in the study; the median age was 39 years and all patients were male. All patients were human immunodeficiency virus type 1 (HIV-1) infected and they had previous or concomitant HIV-associated opportunistic infections or malignancies diagnosed at the time bacillary angiomatosis was diagnosed. Median T4 helper lymphocyte counts of patients was 96 cells per mm³. Cutaneous involvement was the most common clinical manifestation of bacillary angiomatosis in this study. Clinical remission following appropriate treatment was more common in our case series than that reported in the medical literature, while the incidence of relapse was similar. The frequency of bacillary angiomatosis in HIV patients calculated from two of the hospitals included in our study was 1.42 cases per 1000 patients, similar to the frequencies reported in the medical literature. Bacillary angiomatosis is an unusual opportunistic pathogen in our setting.

INTRODUCTION: Transthoracic echocardiography is the method of choice for the diagnosis of cardiac myxomas, but the transesophageal approach provides a better definition of the location and characteristics of the tumor. The authors review their thirteen years' experience on the echocardiographic diagnosis of this pathology. METHODS: From 1994 to 2007, 41 cardiac tumors were diagnosed in our echocardiographic laboratory, of which 27 (65.85%) were cardiac myxomas. The exams and the patients' clinical files were retrospectively reviewed. RESULTS: Of the 27 patients, 22 (81.5%) were female, with a mean age of 62.1 +/- 13.6 years (25-84 years). The predominant clinical features were due to the obstruction caused by the tumor in more than two thirds of the patients, followed by constitutional symptoms in one third and embolic events in 30%. In the lab results, anemia was found in three patients and elevated sedimentation rate and CRP in two. In two patients the myxoma was found by chance. All the cases were of the sporadic type, although we found a prevalence of thyroid disease of 14% (4 patients). All patients underwent urgent surgical resection except one, in whom surgery was refused due to advanced age and comorbidities. The myxomas followed a typical distribution with 24 (88.8%) located in the left atrium, 18 of them attached to the atrial septum (AS) and two to the mitral valve. In one patient, the tumor involved both atria. The other two cases originated in the right atrium at the AS. Embolic phenomena were more frequent in small tumors (p = 0.027) and in those with a villous appearance (p = 0.032). Obstructive manifestations were associated with larger tumors (p = 0.046) and larger left atria (p = 0.048). In our series, there were no deaths during hospitalization or in the follow-up period of 5.2 +/- 3.7 years in 19 patients. There were two recurrences, both patients being successfully reoperated. CONCLUSION: Myxoma is the most common cardiac tumor. Transesophageal echocardiography provides excellent morphologic definition, aiding in diagnosis and follow-up. Most clinical manifestations are obstructive and are associated with larger tumors. Small tumors with a friable appearance have a higher chance of embolization. Surgical resection is usually curative and the long-term prognosis is excellent.

Neurological disorders caused by Cytomegalovirus (CMV) in patients with Acquired Immunodeficiency Syndrome (AIDS) are rarely reported in the Highly Active Antiretroviral Therapy (HAART) period. The objective of this study was to describe the main clinical and laboratory features of patients with CMV-related neurological complications in HIV-infected patients admitted to a referral center in São Paulo, Brazil. CMV disease requires the identification of the virus in the cerebrospinal fluid (CSF) using Polymerase Chain Reaction (PCR). Thirteen cases were identified between January, 2004 and December, 2008. The median age of patients was 38 years and nine (69%) were men. At admission all patients were aware of their HIV status and only four (31%) patients were on HAART. Patients who were not on antiretroviral therapy before admission received HAART while inpatients. CMV disease was the first AIDS-defining illness in eight (62%) patients. The neurologic syndromes identified were diffuse encephalitis (n = 7; 62%), polyradiculopathy (n = 7; 54%), focal encephalitis (rhombencephalitis) (n = 1; 8%), and ventriculo-encephalitis (n = 1; 8%). Seven (54%) patients presented extra-neural CMV disease and four (31%) had retinitis. The median of CD4+ T-cell count was 13 cells/µL (range: 1-124 cells/µL). Overall in-hospital mortality was 38%. Eight patients used ganciclovir or foscarnet (in-hospital mortality: 50%) and five patients used ganciclovir and foscarnet (in-hospital mortality: 20%). None of the patients fulfilled the diagnosis criteria of immune reconstitution inflammatory syndrome. Four patients were lost to follow-up, and three patients presented immune recovery and discontinued secondary prophylaxis. Although infrequent, distinct neurological syndromes caused by CMV continue to cause high mortality among AIDS patients. Survival depends upon the use of effective antiviral therapy against CMV and the early introduction of HAART.
