Anticoagulants are a mainstay of cardiovascular therapy, and parenteral anticoagulants have widespread use in cardiology, especially in acute situations. Parenteral anticoagulants include unfractionated heparin, low-molecular-weight heparins, the synthetic pentasaccharides fondaparinux, idraparinux and idrabiotaparinux, and parenteral direct thrombin inhibitors. The several shortcomings of unfractionated heparin and of low-molecular-weight heparins have prompted the development of the other newer agents. Here we review the mechanisms of action, pharmacological properties and side effects of parenteral anticoagulants used in the management of coronary heart disease treated with or without percutaneous coronary interventions, cardioversion for atrial fibrillation, and prosthetic heart valves and valve repair. Using an evidence-based approach, we describe the results of completed clinical trials, highlight ongoing research with currently available agents, and recommend therapeutic options for specific heart diseases.

O objetivo deste trabalho foi avaliar o uso de marcadores bioquímicos e fisiológicos, na caracterização do estresse hídrico em calos de cultivares de cana-de-açúcar (Sacharum sp.), RB 72 454 (sensível) e SP 813250 (resistente), contrastantes quanto à resistência à seca em campo. O delineamento experimental foi o inteiramente casualizado, em esquema fatorial de 2x5 [cultivar x doses de polietilenoglicol (PEG)], com dez tratamentos e três repetições. Os calos foram submetidos a concentrações de PEG correspondentes aos potenciais osmóticos de 0, -0,3, -0,6, -0,9 e -1,2 MPa, por 120 horas. A variação no conteúdo relativo de água e na umidade não foi significativamente diferente entre as cultivares. Entretanto, foi observada a tendência de aumento no vazamento de eletrólitos, em conseqüência da diminuição do potencial osmótico na cultivar tolerante. Na cultivar sensível, observou-se tendência de aumento de prolina e, na resistente, diminuição, embora os níveis não tenham sido afetados pelo deficit hídrico. As concentrações de aminoácidos livres foram maiores na sensível. Houve queda nas concentrações de amônia, em ambas cultivares. Os níveis de proteínas não foram afetados pelo PEG. O perfil protéico por SDS-PAGE não mostrou aumento induzido por PEG, na intensidade das bandas correspondentes aos peptídeos entre 14 e 66 kDa. Os marcadores bioquímicos e fisiológicos não foram relacionados ao grau diferencial de resistência observado nas cultivares em condições de campo.

Newer antiepileptic drugs (AEDs) are increasingly prescribed and seem to have a comparable efficacy as the classical AEDs; however, their impact on status epilepticus (SE) prognosis has received little attention. In our prospective SE database (2006-2010), we assessed the use of older versus newer AEDs (levetiracetam, pregabalin, topiramate, lacosamide) over time and its relationship to outcome (return to clinical baseline conditions, new handicap, or death). Newer AEDs were used more often toward the end of the study period (42% of episodes versus 30%). After adjustment for SE etiology, SE severity score, and number of compounds needed to terminate SE, newer AEDs were independently related to a reduced likelihood of return to baseline (p<0.001) but not to increased mortality. These findings seem in line with recent findings on refractory epilepsy. Also, in view of the higher price of the newer AEDs, well-designed, prospective assessments analyzing the impact of newer AEDs on efficacy and tolerability in patients with SE appear mandatory.

O objetivo deste trabalho foi investigar o desempenho fotossintético de plantas jovens de andiroba (Carapa guianensis), submetidas à deficiência hídrica e à reidratação. As plantas foram irrigadas diariamente, em casa de vegetação, durante 15 dias. Após aclimatação, as plantas foram separadas em dois tratamentos: plantas irrigadas e não irrigadas. Quando a resposta fotossintética das plantas não irrigadas alcançou valores próximos a zero, as plantas foram reidratadas. A cada sete dias, durante 21 dias, foram realizadas as determinações das trocas gasosas e da fluorescência da clorofila a. O potencial hídrico foliar foi determinado no início e no fim do experimento. As taxas de fotossíntese líquida, condutância estomática e transpiração se reduziram em 88, 89 e 89%, respectivamente, após 21 dias de supressão da irrigação. Quanto às variáveis da fluorescência da clorofila a, observaram-se reduções de 27 a 58%. O potencial hídrico foliar das plantas foi reduzido em mais de quatro vezes, após 21 dias de deficiência hídrica. De quatro a oito dias após a reidratação, as plantas recuperaram as características fotossintéticas e o potencial hídrico foliar, o que indica que plantas jovens de andiroba apresentaram alta plasticidade fisiológica em relação ao estresse hídrico.

Objectives: To study the dental status and treatment needs of institutionalized older adults with chronic mental illness compared to a non-psychiatric control sample. Study Design: The sample size was 100, in which 50 were psychogeriatric patients (study group; SG) classified according to DSM-IV, with a mean age of 69.6 ± 6.7 years, and 50 non-psychiatric patients (control group; CG), with a mean age of 68.3 ± 6.9 years. Clinical oral health examinations were conducted and caries were recorded clinically using the Decayed, Missing and Filled Teeth Index (DMFT). Results were analyzed statistically using the Student"s t-test or analysis of variance. Results: Caries prevalence was 58% and 62% in SG and CG, respectively. DMFT index was 28.3 ± 6.6 in SG and 21.4 ± 6.07 in CG (p < 0.01). Mean number of decayed teeth was higher in SG (3.1) compared to CG (1.8) (p=0.047). Mean number of missing teeth were 25.2 and 16.4 in SG and CG respectively (p<0.05). DMFT scores were higher in SG in all the age groups (p < 0.01). Mean number of teeth per person needing treatment was 3.4 in SG and 1.9 in CG (p= 0.037). The need for restorative dental care was significantly lower in the SG (0.8 teeth per person) than in the CG (1.7 teeth per person) (p = 0.043). Conclusions: Institutionalized psychiatric patients have significantly worse dental status and more dental treatment needs than non-psychiatric patients.

Medulloblastoma is the most frequent malignant paediatric brain tumour. The activation of the Wnt/beta-catenin pathway occurs in 10-15% of medulloblastomas and has been recently described as a marker for favourable patient outcome. We report a series of 72 paediatric medulloblastomas evaluated for beta-catenin protein expression, CTNNB1 mutations, and comparative genomic hybridization. Gene expression profiles were also available in a subset of 40 cases. Immunostaining of beta-catenin showed extensive nuclear staining (>50% of the tumour cells) in six cases and focal nuclear staining (<10% of cells) in three cases. The other cases either exhibited a signal strictly limited to the cytoplasm (58 cases) or were negative (five cases). CTNNB1 mutations were detected in all beta-catenin extensively nucleopositive cases. The expression profiles of these cases documented strong activation of the Wnt/beta-catenin pathway. Remarkably, five out of these six tumours showed a complete loss of chromosome 6. In contrast, cases with focal nuclear beta-catenin staining, as well as tumours with negative or cytoplasmic staining, never demonstrated CTNNB1 mutation, Wnt/beta-catenin pathway activation or chromosome 6 loss. Patients with extensive nuclear staining were significantly older at diagnosis and were in continuous complete remission after a mean follow-up of 75.7 months (range 27.5-121.2 months) from diagnosis. All three patients with focal nuclear staining of beta-catenin died within 36 months from diagnosis. Altogether, these data confirm and extend previous observations that CTNNB1-mutated tumours represent a distinct molecular subgroup of medulloblastomas with favourable outcome, indicating that therapy de-escalation should be considered. International consensus on the definition criteria of this distinct medulloblastoma subgroup should be achieved.
