Iowa’s investment in the Chicago to Iowa City passenger rail service will produce more ongoing benefits than costs for Iowa residents and taxpayers over the next 30 years. An Iowa investment of $20.6 million will match a federal investment of $86.8 million for the capital costs of the Iowa segment of the Chicago to Iowa City service. Iowa’s share of the expected gap between revenues and operating and maintenance expense is estimated at $3 million annually. The economic analysis included in this document demonstrates the cost-effectiveness of the Iowa investments in the Chicago to Iowa City service and details the benefits that Iowa can expect from Iowa’s expenditures.

This article analyzes whether and to what extent the policy environment of civil servants has an impact on their level of Public Service Motivation (PSM). It hypothesizes that public employees working in different policy domains and stages of the policy cycle are diversely motivated by four PSM orientations (Compassion, Commitment to the public interest, Self-sacrifice and Attraction to politics). The empirical results are based on a survey of 6885 Swiss civil servants. They show that those in charge of Welfare State policies are inclined to have higher levels of 'Compassion', whereas those performing core state functions report lower levels. Furthermore, employees whose main tasks are related to policy formulation display high levels of the 'Attraction to politics' dimension of PSM. This study questions the generalization of previous findings on PSM that are based on heterogeneous survey populations.

La variabilité interindividuelle au sein de la population générale se manifeste à plusieurs niveaux aussi bien externe pour des différences physiologiques qu'interne pour des différences toxicocinétiques et toxicodynamiques . Face à une agression chimique par exemple, nous ne réagissons en effet pas tous de manière identique. Entre le moment de l'exposition à un composé toxique et la maladie, une suite d'évènements biologiques peut survenir. La façon dont cette suite d'évènements se développera chez une personne dépend de sa susceptibilité individuelle aux risques chimiques. Celle-ci peut être déterminée par des facteurs génétiques, physiopathologiques (âge, sexe, grossesse, hormones, maladies, alimentation, etc.) et environnementaux. [Auteur]

INTRODUCTION: Intraosseous access is increasingly recognised as an effective alternative vascular access to peripheral venous access. We aimed to prospectively study the patients receiving prehospital intraosseous access with the EZ-IO(®), and to compare our results with those of the available literature. METHODS: Every patient who required an intraosseous access with the EZ-IO from January 1st, 2009 to December 31st, 2011 was included. The main data collected were: age, sex, indication for intraosseous access, localisation of insertion, success rate, drugs and fluids administered, and complications. All published studies concerning the EZ-IO device were systematically searched and reviewed for comparison. RESULTS: Fifty-eight patients representing 60 EZ-IO procedures were included. Mean age was 47 years (range 0.5-91), and the success rate was 90%. The main indications were cardiorespiratory arrest (74%), major trauma (12%), and shock (5%). The anterior tibia was the main route. The main drugs administered were adrenaline (epinephrine), atropine and amiodarone. No complications were reported. We identified 30 heterogeneous studies representing 1603 EZ-IO insertions. The patients' characteristics and success rate were similar to our study. Complications were reported in 13 cases (1.3%). CONCLUSION: The EZ-IO provides an effective way to achieve vascular access in the pre-hospital setting. Our results were similar to the cumulative results of all studies involving the use of the EZ-IO, and that can be used for comparison for further studies.

1.1 Objectifs Le syndrome de bas débit cardiaque est une appréciation clinique (Hoffman et al.) dont les signes sont peu sensibles, peu spécifiques, souvent tardifs et dont la mesure chez l'enfant est rendue impossible en clinique pour des raisons techniques et de fiabilités des systèmes de mesures. Aucun score n'est applicable à l'heure actuelle chez l'enfant. C'est pourquoi, ces 10 dernières années, de nombreuses équipes se sont penchée activement sur ce problème afin de définir des marqueurs performants prédisant la survenue du LCOS après chirurgie cardiaque chez l'enfant. Cette étude s'est attachée à réunir ses marqueurs cardiaques, de les combiner et de les inclure dans un score de bas débit cardiaque. 1.2 Méthode Enfants et nouveau-nés ayant subit une chirurgie cardiaque au CHUV, après malformation cardiaque congénitale, entre janvier 2010 et octobre 2011 (N=48). Age : 8 jours à 13 ans (médiane : 16.3 mois). Deux scores développés. Soumission à l'aveugle de la liste des patients à un comité d'expert pour identifier les patients en LCOS à 48h post-chirurgie, puis comparaison avec le résultat du score. Les paramètres du premier score (SCORE 1), sont agendées de manière ordinales, alors que dans le deuxième score (SCORE 2) elles le sont de manière dichotomiques. Valeurs cut-off supérieures et inférieures des scores choisies selon une recherche extensive dans la littérature. Les valeurs cut-off intermédiaires (SCORE 1) ont été choisies au hasard. 1.3 Résultats La régression logistique multivariée pour la prédiction d'un LCOS à 48h, démontre que seul le score d'amine durant les 24 premières heures et un prédicteur indépendant de LCOS (OR 16.6 [2.6- 105.5] p<0.0001). Ce paramètre est bien corrélé avec le résultat des experts avec un coefficient de corrélation r=0.57 (p<0.0001). Les spécificités des deux scores (AUC=0.78 (p<0.0001) respectivement AUC=0.81 (p<0.0001)) sont de 71% respectivement 93.5%, les sensibilités de 70.6% respectivement 41.2 %, VPP de 57.1% respectivement 77.8%, VPN de 81.5 % respectivement 74.4%. Les tests du khi2 valent 7.7 (p=0.006) respectivement 8.69 (p=003), rejettent l'hypothèse nulle d'indépendance entre le résultat des experts et celui prédit par le score. 1.4 Conclusions Les scores développés dans le cadre de cette étude ne montrent pas une corrélation significative avec l'apparition d'un bas débit cardiaque. Même si le choix des paramètres permettant de quantifier l'apparition d'un bas débit cardiaque à 48h fût réalisé selon une recherche extensive dans la littérature, le design rétrospectif de l'étude, n'a pas permit de vérifier efficacement la relation entre l'apparition d'un bas débit cardiaque et le score de bas débit cardiaque.

In June 2009, the Iowa Department of Human Service (IDHS) developed a Child and Family Service Plan (CFSP) that sets forth the Department’s vision and goals to be accomplished for FYs 2010 through 2014. The purpose of the CFSP is to strengthen the States’ overall child welfare system and to facilitate the state’s integration of the programs that serve children and families into a comprehensive and continuum array of child welfare services from prevention and protection through permanency. These programs include title IV-B, subparts 1 and 2 of the Act, the Child Abuse Prevention and Treatment Act (CAPTA), the Chafee Foster Care Independence Program (CFCIP), and the Education and Training Vouchers (ETV) programs for older and/or former foster care youth. IDHS administers the IV-B, CAPTA, CFCIP and the ETV programs described within Iowa’s CFSP. Iowa’s Annual Progress and Services Report (APSR) provides an annual update on the progress made toward accomplishing the goals and objectives identified in the state’s CFSP for the previous fiscal year (2009-2010) and the planned activities for next fiscal year (2010-2011) .
