The goal of this paper is to present an optimal resource allocation model for the regional allocation of public service inputs. Theproposed solution leads to maximise the relative public service availability in regions located below the best availability frontier, subject to exogenous budget restrictions and equality ofaccess for equal need criteria (equity-based notion of regional needs). The construction of non-parametric deficit indicators is proposed for public service availability by a novel application of Data Envelopment Analysis (DEA) models, whose results offer advantages for the evaluation and improvement of decentralised public resource allocation systems. The method introduced in this paper has relevance as a resource allocation guide for the majority of services centrally funded by the public sector in a given country, such as health care, basic and higher education, citizen safety, justice, transportation, environmental protection, leisure, culture, housing and city planning, etc.

The ATP-binding cassette (ABC) family of proteins comprise a group of membrane transporters involved in the transport of a wide variety of compounds, such as xenobiotics, vitamins, lipids, amino acids, and carbohydrates. Determining their regional expression patterns along the intestinal tract will further characterize their transport functions in the gut. The mRNA expression levels of murine ABC transporters in the duodenum, jejunum, ileum, and colon were examined using the Affymetrix MuU74v2 GeneChip set. Eight ABC transporters (Abcb2, Abcb3, Abcb9, Abcc3, Abcc6, Abcd1, Abcg5, and Abcg8) displayed significant differential gene expression along the intestinal tract, as determined by two statistical models (a global error assessment model and a classic ANOVA, both with a P < 0.01). Concordance with semiquantitative real-time PCR was high. Analyzing the promoters of the differentially expressed ABC transporters did not identify common transcriptional motifs between family members or with other genes; however, the expression profile for Abcb9 was highly correlated with fibulin-1, and both genes share a common complex promoter model involving the NFkappaB, zinc binding protein factor (ZBPF), GC-box factors SP1/GC (SP1F), and early growth response factor (EGRF) transcription binding motifs. The cellular location of another of the differentially expressed ABC transporters, Abcc3, was examined by immunohistochemistry. Staining revealed that the protein is consistently expressed in the basolateral compartment of enterocytes along the anterior-posterior axis of the intestine. Furthermore, the intensity of the staining pattern is concordant with the expression profile. This agrees with previous findings in which the mRNA, protein, and transport function of Abcc3 were increased in the rat distal intestine. These data reveal regional differences in gene expression profiles along the intestinal tract and demonstrate that a complete understanding of intestinal ABC transporter function can only be achieved by examining the physiologically distinct regions of the gut.

O presente estudo teve como objectivo verificar o nível de satisfação no trabalho, dos Agentes efectivos do Comando Regional da Praia, tendo como referencial a teoria dos dois factores de Frederick Herzberg. Dos 240 efectivos daquela organização, foram estudados 97, Agentes provenientes dos mais diversos Esquadras e funções, e os dados foram colectados mediante um questionário. Assim, o estudo foi dividido em cinco capítulos em que, no primeiro capítulo fez-se uma introdução geral, isto é, o enquadramento dos assuntos tratados, o segundo capítulo aborda sínteses dos resultados que foram obtidos por outros investigadores. Enquanto que o terceiro capítulo apresenta um estudo de caso na Polícia Nacional, com propósito de verificar o nível de satisfação no trabalho, dos Agentes efectivos do Comando Regional da Praia e no quarto capítulo, foram analisadas as dimensões do estudo, isto é, a variável trabalho, autonomia, relacionamento, autonomia e perspectiva. Dimensões estas foram também analisadas á luz do modelo teórico pré estabelecido (Teoria bifactorial de Frederick Herzberg). No último capítulo após a análise estatístico apropriado e mediante o teste estatístico Qui-Quadrado chegou-se à conclusão que há um elevado nível de satisfação dos Agentes do Comando Regional da Praia. Foram tecidas algumas recomendações que achamos pertinentes e limitações que este estudo acarreta consigo.

INTRODUCTION De nos jours, le divorce est devenu un événement courant de la vie conjugale. Un des sujets qui restent au coeur de la polémique dans le domaine du droit du divorce depuis toujours, est celui de la contribution d'entretien après divorce. En effet, il s'agit de savoir dans quelles circonstances l'ancien conjoint (épouse ou époux) peut prétendre au maintien de l'entretien auquel il avait droit durant le beau et bon temps du mariage de la part de son ex-partenaire. Pour se convaincre de la complexité des problèmes que la dissolution de l'union conjugale peut créer, il suffit de parcourir la masse des décisions des différents tribunaux en la matière. Quant à l'actualité de la question, on peut simplement observer le nombre de divorces qui va croissant chaque année. Le demandeur peut être aussi bien l'ex-épouse que l'ex-époux ; c'est cette constatation qui a fondé notre démarche et qui est le fil conducteur de ce travail. Mais, alors qu'initialement nous nous demandions si un homme pouvait réellement être dans la situation de l'époux demandeur, notre réflexion s'est peu à peu orientée dans une direction différente et une autre question s'est profilée : qu'en est-il dans les faits de l'égalité acceptée juridiquement et intégrée dans les textes de loi ? La présente thèse examine l'évolution opérée parallèlement dans trois domaines qui s'influencent réciproquement : le droit du mariage, le droit du divorce et le principe constitutionnel de l'égalité. En effet, la révision constitutionnelle qui a abouti à l'adoption en 1981 du principe explicite de l'égalité entre homme et femme, a conduit à certaines modifications législatives. C'est ainsi que ce principe constitutionnel a été, par exemple, concrétisé dans le domaine du droit du mariage en 1984. Il a, par la même occasion, contribué au bouleversement du petit monde traditionnel familial et du système légal qui le soutenait. Nous avons donc analysé les conséquences que cette réforme législative a entraînées au sein du couple, et plus précisément, lors de sa désunion. Le présent travail consacre une partie à chacune des étapes qui a permis de faire évoluer ce domaine du droit si controversé qu'est la contribution d'entretien après divorce. Ainsi, la première partie montre l'influence de la situation de la femme sur les règles régissant le mariage en 1984 : alors que son statut était imprégné des principes patriarcaux justifiant, en 1907, une distribution législative traditionnelle des rôles au sein de l'union conjugale, la femme a, en 1984, été placée devant le même rôle que son mari. Ce titre premier s'intéresse donc à la réforme du droit du mariage entrée en vigueur en 1988 et qui va servir de cadre au bouleversement ultérieur qu'est la révision du droit du divorce. Dans la deuxième partie, nous avons étudié le droit du divorce tel qu'il ressortait du Code civil adopté en 1907 jusqu'à sa révision en 1998. Notre étude a donc porté sur les conditions essentielles que le Code d'Eugen Huber imposait pour obtenir une prestation lors de la dissolution des liens du mariage, ainsi que sur l'articulation entre les deux dispositions clés dans ce domaine : les articles 151 et 152 CC 1907. Cette analyse est divisée en deux sous-chapitres qui illustrent l'influence de la révision du droit du mariage de 1984 sur le droit du divorce de 1907. Le premier sous-chapitre examine ainsi les règles régissant la dissolution de l'union conjugale au regard du droit du mariage de 1907 jusqu'à sa révision entrée en vigueur en 1988; le second sous-chapitre porte sur ces mêmes règles de 1988 à 2000 (date de l'entrée en vigueur du nouveau droit du divorce) mais sous un nouvel éclairage, puisque le nouveau droit du mariage et ses nouveaux principes s'appliquent en marge du droit du divorce de 1907. La question se pose de savoir quelle a été l'influence réelle de la réforme de 1984, elle-même marquée par la modification constitutionnelle de 1981, sur le droit du divorce qui date, lui, de 1907 et qui a alors été adopté dans un contexte différent et baigné d'anciens principes traditionnels. En effet, qu'en est-il du droit aux prestations des articles 151 et 152 CC 1907 justifiées par le modèle traditionnel de répartition des tâches, dans un système basé sur la libre attribution des rôles au sein du couple ? Quelles sont les conséquences juridiques de l'indifférenciation des rôles masculin et féminin au sein de l'entité familiale ? Enfin, la troisième partie est consacrée à l'étude du nouveau droit du divorce de 1998, entré en vigueur en l'an 2000. Cette réforme suit l'évolution qui a eu lieu dans le domaine du droit de la famille ; elle a été en quelque sorte imposée par les différents développements législatifs de notre société. Nous nous trouvons ainsi face à de nouvelles interrogations : qu'en est-il des traditions familiales actuelles dans la pratique ? Qu'en est-il de l'évolution du principe constitutionnel de l'égalité et de son application dans les nouveaux droits du mariage et du divorce sous l'angle des contributions d'entretien ?

This paper reports an analysis of income related health inequalities at the AutonomousCommunity level in Spain using the self assessed health measure in the 2001 edition of theEncuesta Nacional de Salud. We use recently developed methods in order to cardinalise andmodel self assessed health within a regression framework, decompose the sources ofinequality and explain the observed differences across regions. We find that the regions with the highest levels of mean health tend to enjoy the lowest degrees of income related health inequality and vice-versa. The main feature characterizing regions where income related health inequality is low is the absence of a positive gradient between income and health. In turn, the regions where income related health inequality is greater are characterized by a strong and significant positive gradient between health and income. These results suggest that policies aimed at eliminating the gradient between health and income can potentially lead to greate r reductions in socio-economic health inequalities than policies aimed at redistributing income.

This article aims to identify the key groups of regions with respectto farms oriented to fruit and citrus production.Twenty variables of fruit and citrus oriented farms corresponding toforty-one regions of the European Union were analyzed. Seven groupsemerged from cluster analysis. Only two of them showed good perspectives. Regions in the South of the Community need an important modernisation and restructuring process, which entails serious social consequences.

O presente trabalho de investigação propõe estudar a “Importância do Lúdico no Impacto Psicológico da Hospitalização Infantil”, tendo como objetivo perceber a importância do lúdico no contexto da hospitalização infantil. Sendo assim, questionamos: até que ponto o lúdico pode influenciar as repercussões psicológicas emergentes da hospitalização infantil? Na tentativa de responder a esta questão traçamos algumas hipóteses que se seguem:  As atividades lúdicas contribuem para a diminuição das emoções negativas provenientes da hospitalização;  O tempo de internamento e a diminuição das emoções negativas estão unicamente ligadas as caraterísticas da doença e não à prática das atividades lúdicas;  O tempo de recuperação está pouco relacionado à prática de atividades lúdicas no ambiente hospitalar; A recolha dos dados, para a análise, processou-se mediante a observação das crianças hospitalizadas na pediatria do Hospital Regional Santiago Norte, Santa Rita Viera, elaborados em concordância com os objetivos que norteiam o presente estudo. Enfim, conseguimos perceber que o lúdico é uma atividade de extrema importância no processo da hospitalização, constituindo um bem para as crianças, bem como para os seus acompanhantes e os profissionais de saúde, uma vez que através dela, a criança adquire uma outra elaboração sobre os serviços hospitalares; terá uma melhora significativa do estado de humor, diminuindo os medos e as angústias, contribuindo melhor e mais ativamente para a sua melhoria.

BACKGROUND: Although arrhythmogenic right ventricular dysplasia (ARVD) predominantly affects the right ventricle (RV), genetic/molecular and histological changes are biventricular. Regional left ventricular (LV) function has not been systematically studied in ARVD. METHODS AND RESULTS: The study population included 21 patients with suspected ARVD who underwent evaluation with MRI including tagging. Eleven healthy volunteers served as control subjects. Peak systolic regional circumferential strain (Ecc, %) was calculated by harmonic phase from tagged MRI based on the 16-segment model. Patients who met ARVD Task Force criteria were classified as definite ARVD, whereas patients with a positive family history who had 1 additional minor criterion and patients without a family history with 1 major or 2 minor criteria were classified as probable ARVD. Of the 21 ARVD subjects, 11 had definite ARVD and 10 had probable ARVD. Compared with control subjects, probable ARVD patients had similar RV ejection fraction (58.9+/-6.2% versus 53.5+/-7.6%, P=0.20), but definite ARVD patients had significantly reduced RV ejection fraction (58.9+/-6.2% versus 45.2+/-6.0%, P=0.001). LV ejection fraction was similar in all 3 groups. Compared with control subjects, peak systolic Ecc was significantly less negative in 6 of 16 (37.5%) segments in definite ARVD and 3 of 16 segments (18.7%) in probable ARVD (all P<0.05). CONCLUSIONS: ARVD is associated with regional LV dysfunction, which appears to parallel degree of RV dysfunction. Further large studies are needed to validate this finding and to better define implications of subclinical segmental LV dysfunction.

Repetition of environmental sounds, like their visual counterparts, can facilitate behavior and modulate neural responses, exemplifying plasticity in how auditory objects are represented or accessed. It remains controversial whether such repetition priming/suppression involves solely plasticity based on acoustic features and/or also access to semantic features. To evaluate contributions of physical and semantic features in eliciting repetition-induced plasticity, the present functional magnetic resonance imaging (fMRI) study repeated either identical or different exemplars of the initially presented object; reasoning that identical exemplars share both physical and semantic features, whereas different exemplars share only semantic features. Participants performed a living/man-made categorization task while being scanned at 3T. Repeated stimuli of both types significantly facilitated reaction times versus initial presentations, demonstrating perceptual and semantic repetition priming. There was also repetition suppression of fMRI activity within overlapping temporal, premotor, and prefrontal regions of the auditory "what" pathway. Importantly, the magnitude of suppression effects was equivalent for both physically identical and semantically related exemplars. That the degree of repetition suppression was irrespective of whether or not both perceptual and semantic information was repeated is suggestive of a degree of acoustically independent semantic analysis in how object representations are maintained and retrieved.

BACKGROUND: Coronary endothelial function is abnormal in patients with established coronary artery disease and was recently shown by MRI to relate to the severity of luminal stenosis. Recent advances in MRI now allow the noninvasive assessment of both anatomic and functional (endothelial function) changes that previously required invasive studies. We tested the hypothesis that abnormal coronary endothelial function is related to measures of early atherosclerosis such as increased coronary wall thickness. METHODS AND RESULTS: Seventeen arteries in 14 healthy adults and 17 arteries in 14 patients with nonobstructive coronary artery disease were studied. To measure endothelial function, coronary MRI was performed before and during isometric handgrip exercise, an endothelial-dependent stressor, and changes in coronary cross-sectional area and flow were measured. Black blood imaging was performed to quantify coronary wall thickness and indices of arterial remodeling. The mean stress-induced change in cross-sectional area was significantly higher in healthy adults (13.5%±12.8%, mean±SD, n=17) than in those with mildly diseased arteries (-2.2%±6.8%, P<0.0001, n=17). Mean coronary wall thickness was lower in healthy subjects (0.9±0.2 mm) than in patients with coronary artery disease (1.4±0.3 mm, P<0.0001). In contrast to healthy subjects, stress-induced changes in cross-sectional area, a measure of coronary endothelial function, correlated inversely with coronary wall thickness in patients with coronary artery disease (r=-0.73, P=0.0008). CONCLUSIONS: There is an inverse relationship between coronary endothelial function and local coronary wall thickness in patients with coronary artery disease but not in healthy adults. These findings demonstrate that local endothelial-dependent functional changes are related to the extent of early anatomic atherosclerosis in mildly diseased arteries. This combined MRI approach enables the anatomic and functional investigation of early coronary disease.
