This study is about the users' experiences of the services offered by League Against Câncer that are in treatment of breast cancer, focusing on how the health/illness process develops. It is distinguished, in this context, the National Politics of Health, approaching the sprouting, implantation and consolidation process of the Unified Health System and its shocks on the installment of health services to the low-income population as a legit right from Citizen Constitution. It has as an objective to analyze how the social-economic extract of these women intervenes with such process, the aspects who involve the cancer while a pathology, approaching the signals, symptoms, forms of prevention, diagnosis, among other aspects that configure themselves as important points to the understanding of these experiences, since the diagnosis, treatment and control phases. The research was carried out with 25 women, between the months of January and February of year of 2005. The used technique was the scriptstructured interview, whose universe was defined through intentional sample. The following distinguishes as a result of the research: The majority of the women has its origins on the interior of the state, possesses a familiar income from one to three minimum wages and didn't make any kind of prevention before diagnosis, currently make treatment, beyond other aspects. It becomes necessary posterior studies on this social problematic, with respect to the personal, professional, familiar and social daily behavior of these women. It is worth mentioning that the role of the social assistant in the developed work with these women, understanding all the impediments associated with such experiences, as well as giving information about the rights of the patients with cancer, conquered through the years

Background Venous ulcer is the most serious consequence of chronic venous insufficiency and is responsible for almost 70% of chronic leg ulcers. The main purpose of this research was to describe social, demographic and clinical characteristics of patients with venous ulcers and to identify some professional repercussions of this pathology.Subjects and methods We evaluated patients with a clinical picture compatible with venous ulcer. The sociodemographic characteristics of the patients, and the clinical characteristics and professional repercussions of the pathology were studied. We used the ankle-brachial index to identify associated arterial disease. The body mass index (BMI) was used for the classification of the nutritional condition of the patients.Results A total of 120 patients were included in the study (90 females and 30 males; 80.8% White; 44.2% > 60 years old). Ninety-one per cent of the patients had been in education for less than 4 years, and 89.7% were on the poverty line. Thirty-five per cent were retired, 2.5% were receiving government help due to disease and 4.2% were unemployed. About 16% of patients were out of work due to the disease, and 49.2% presented some degree of disability in terms of work tasks. The ulcer area was less than 30 cm(2) in 69.1% of patients. The first ulcer episode occurred in 13.4 years on average, and 64.2% of patients had recurrent episodes. Females presented on average 5.65 pregnancies, and 75.4% of patients were overweight.Conclusion Venous ulcer occurred mainly in the low-income population, who presented with little formal education. Generally, the ulcers were present for a long time and were recurrent, with repercussions for the capabilities of patients to work.

The rapid growth of the Brazilian cities in the latest years has brought a series of problems regarding housing policies and, therefore, the provision of dwellings for the low-income class people. Following the pattern of other capital cities, Natal has repeated the pattern of urbanization practiced in the country, which concentrates the low-income class inhabitants in specific zones of the city known as peripheries or even in a dwelling place for less-favored classes such as Felipe Camarão, which is one of ten boroughs belonging to the western administrative zone, a region which has historically received less attention from the public administrators towards infrastructure investments. Based on those aspects, the general objective of this work is to investigate which main social-environmental alterations have resulted from the processes of urbanization and field occupation in that area. The specific objectives are concerned with verifying 1) the process of urbanization and the process of dividing urban soil from the 1960s; 2) the current configuration of the public spaces in the borough; 3) the process of the deprivation of the original landscape characteristics of Environmental Protection Zones; 4) the dynamics of land occupation which are predominant in dune areas; 5) the dynamics of land occupation which are predominant in mangrove areas; 6) and the destruction of green covering on the land with its consequent alteration of the local floristic pattern. The methodology consisted of in-loco visits; the application of questionnaires as community research; a survey of bibliography published by the organisms and institutions in charge of carrying out the city hall s environmental and housing policies; and descriptive statistics of the collected data. Concerning the pattern of occupation which is predominant in the borough, the treatment of space dispensed by the local dwellers has culminated in the emergence and consolidation of environmental alterations which are clearly different from common occurrence in both the building area represented by housing complexes and in the green areas represented by dunes and mangroves. The data show that there was the predominance of the irregular land occupation process over the official housing policy during the population settlement which contributed with a series of frequent and foreseeable problems in the dynamics of urbanization of poverty such as invasions and appropriations of land parcels, the beginning of irregular arrangement of streets, the formation of villages and slums, which are full of self-constructed housing units, and the occupation and degradation of susceptibly fragile environmental areas such as the dune slopes and the mangroves
Social support and infant malnutrition: a case-control study in an urban area of Southeastern Brazil

The relationship between malnutrition and social support was first suggested in the mid-1990s. Despite its plausibility, no empirical studies aimed at obtaining evidence of this association could be located. The goal of the present study was to investigate such evidence. A case-control study was carried out including 101 malnourished children (weight-for-age National Center for Health Statistics/WHO 5th percentile) aged 12-23 months, who were compared with 200 well-nourished children with regard to exposure to a series of factors related to their social support system. Univariate and multiple logistic regressions were carried out, odds ratios being adjusted for per capita family income, mother's schooling, and number of children. The presence of an interaction between income and social support variables was also tested. Absence of a partner living with the mother increased risk of malnutrition (odds ratio 2.4 (95 % CI 1.19, 4.89)), even after adjustment for per capita family income, mother's schooling, and number of children. The lack of economic support during adverse situations accounted for a very high risk of malnutrition (odds ratio 10.1 (95 % CI 3.48, 29.13)) among low-income children, but had no effect on children of higher-income families. Results indicate that receiving economic support is an efficient risk modulator for malnutrition among low-income children. In addition, it was shown that the absence of a partner living with the mother is an important risk factor for malnutrition, with an effect independent from per capita family income, mother's schooling, and number of children.

This dissertation is about low-income rental housing vilas in the South Zone in the city of Natal. Vilas are a string of houses, built privately in an urban plot of land, usually a backyard, facing a narrow street beco. This type of housing is originated in the 19th century in the Brazilian scenario. In Natal, this has become more popular since the late 1960s. This study is aimed at analyzing the urban integration of the vilas, considering that these house a low income population, vis à vis the speedy development of Natal s most economically dynamic area. The study starts with a discussion of the production of space, taking into consideration categories such as use value, exchange value, commodity and land rent. Field work was undertaken through structured interviews with dwellers of vilas in Nova Descoberta and Ponta Negra neighbourhoods. Owners of vilas were also interviewed. Data collected indicated worrisome housing conditions. Vilas, in the areas researched, are increasingly becoming unwelcome, constituting guetos in a place where landscape is changing rapidly. Gentrification is fast under way, first because of rise in rent and taxes; second because of general rise in living costs

A leptospirose é uma grave zoonose associada às áreas de baixa renda dos centros urbanos. Embora roedores urbanos sejam considerados como principal reservatório para a leptospirose, o cão também pode desenvolver a doença e se tornar carreador assintomático. O objetivo do presente trabalho foi utilizar a metodologia estatística baseada na teoria de processos pontuais espaciais, buscando identificar a forma como se distribuem os cães sororreagentes para a leptospirose e seus determinantes de risco em uma vila na cidade de Curitiba. A análise do modelo possibilitou identificar as regiões de sobre-risco, onde o risco de soropositividade canina à leptospirose é significativamente maior. A relação significativa do efeito espacial no desenvolvimento da doença, além das variáveis estudadas, revela que não apenas um, mas a ação conjunta dos fatores relacionados ao animal, ao proprietário e ao ambiente influencia o risco maior da doença nos locais de maior efeito espacial. O resultado da análise indica claramente os territórios em maior risco na região da Vila Pantanal, possibilitando o planejamento de ações mais específicas e dirigidas a essas áreas em um contexto de vigilância da saúde.

Bivalve filter feeders are sessile animals that live in constant contact with water and its pollutants. Their gill is an organ highly exposed to these conditions due to its large surface and its involvement in gas exchanges and feeding. The bivalve Mytella falcata is found in estuaries of Latin America, on the Atlantic as well as the Pacific Coast. It is commonly consumed, and sometimes is the only source of protein of low-income communities. In this study, gill filaments of M. falcata were characterized using histology, histochemistry and transmission electron microscopy for future comparative studies among animals exposed to environmental pollutants. Gill filaments may be divided into abfrontal, intermediate and frontal zones. Filaments are interconnected by ciliary discs. In the center of filaments, haemocytes circulate through a haemolymph vessel internally lined by an endothelium and supported by an acellular connective tissue rich in polysaccharides and collagen. The abfrontal zone contains cuboidal cells, while the intermediate zone consists of a simple squamous epithelium. The frontal zone is composed of five columnar cell types: one absorptive, mainly characterized by the presence of pinocytic vesicles in the apical region of the cell; one secretory, rarely observed and three ciliated with abundant mitochondria. All cells lining the filament exhibit numerous microvilli and seem to absorb substances from the environment. PAS staining was observed in mucous cells in the frontal and abfrontal zones. Bromophenol blue allowed the distinction of haemocytes and detection of a glycoprotein secretion in the secretory cells of the frontal region. The characteristics of M. falcata gill filaments observed in this study were very similar to those of other bivalves, especially other Mytilidae, and are suitable for histopathological studies on the effect of water-soluble pollutants. O objetivo do estudo foi identificar e medir a presença de associação entre cuidado psicossocial e desnutrição. Realizou-se estudo caso-controle incluindo 101 crianças desnutridas (peso/idade < percentil 5 do padrão NCHS/OMS), com idades entre 12 e 23 meses, que foram comparadas a 200 controles eutróficos (peso/ idade > percentil 25) em termos de sua exposição a uma série de comportamentos maternos indicadores da qualidade de seu cuidado psicossocial. Criou-se um escore de cuidado psicossocial, variando de 0 a 14, de acordo com o número de comportamentos maternos desejáveis não observados: quanto maior o escore, pior a qualidade do cuidado psicossocial. Mediante análise de regressão logística verificou-se maior risco de desnutrição para as crianças no 2º e 3º tercis do escore de cuidado psicossocial. Esta associação foi modificada pela renda per capita. Após ajustes para possíveis confundidores, nas crianças dos estratos superiores de renda não houve associação entre cuidado psicossocial e desnutrição. Para as crianças com nível mais baixo de renda, pior cuidado psicossocial dobrou o risco de desnutrição (OR = 7,26; IC95%: 2,42-21,82) em relação àquele associado apenas à baixa renda (OR = 3,08; IC95%: 1,28-7,42).

Estudou-se a mortalidade de crianças menores de um ano de idade residentes no Município de Botucatu, SP (Brasil), em 1987. Tentando estabelecer o perfil de risco desses óbitos, tendo como seu principal responsável a desigualdade social, calculou-se o risco adicional (RA) em função de algumas variáveis usando a metodologia de estudos de caso-controle. O resultado obtido foi um RA de óbito de 15,58 para gestação pré-termo, 11,63 para o baixo peso ao nascer, 8,50 para inexistência de água intradomiciliar e 4,04 para escolaridade materna insuficiente. Verificou-se existir importante desigualdade entre as famílias das crianças que morreram e as das que sobreviveram, sugerindo que a melhor estratégia para enfrentar o excesso de mortalidade infantil residiria na melhoria sócio-econômica, isto é, todas as famílias deveriam ter a mesma capacidade de consumir os bens e serviços em igual quantidade e qualidade.

Objetivou-se apreender as representações sociais sobre gravidez após os 35 anos a partir de mulheres com baixa renda que vivenciaram essa experiência. A abordagem qualitativa foi empregada com base na Teoria das Representações Sociais. Foram realizadas entrevistas semiestruturadas com 25 gestantes usuárias de um serviço público de referência do interior paulista. Os dados foram sistematizados pela técnica do Discurso do Sujeito Coletivo. Verificou-se que a opção pela gravidez tardia se atrela ao desejo da mulher de consolidar suas relações em novas uniões conjugais, à estabilidade financeira e à maturidade do casal. As mulheres representam esta experiência como positiva, se houver planejamento prévio, envolvimento do companheiro e se for bem aceita pela família, após sua constatação. Sem a satisfação destas condições, as representações revestem-se de sentimentos negativos ligados a dor, sofrimento e morte. As conclusões deste estudo enfatizam a importância de os serviços públicos de saúde considerarem estes aspectos.

O presente trabalho mostrou freqüências diferentes de parasitoses intestinais (giardíase e helmintíases) entre crianças de pré-escolas e de escolas de 1 e 2 Graus, públicas e privadas. Pesquisou-se 199 indivíduos, sendo 96 pertencentes às instituições públicas e 103 pertencentes às instituições particulares. Nas instituições públicas as freqüências de giardíase foram maiores que nas instituições privadas. Para as helmintíases, as freqüências foram semelhantes entre as creches, porém maiores na escola pública em relação à particular. Indicadores do nível sócio-econômico, de escolaridade, de saneamento básico, como determinantes de parasitose intestinal, evidenciaram que as maiores freqüências de parasitoses ocorreram tanto entre as crianças de baixa renda quanto entre as que tinham pais com nível de escolaridade mais baixo. Também a origem das hortaliças foi um fator significativo para determinação destas parasitoses.

This paper examines the therapeutic possibilities offered by animal-based remedies in five Brazilian cities. Information was obtained through semi-structured questionnaires applied to 79 traders of medicinal animals at Sao Luis, Teresina, Joao Pessoa and Campina Grande (Northeastern) and Belem (Northern) Brazil. We recorded the use of 97 animal species as medicines, whose products were recommended for the treatment of 82 illnesses. The most frequently quoted treatments concerned the respiratory system (58 species; 407 use-citations), the osteomuscular system and conjunctive tissue (46 species; 384 use-citations), and the circulatory system (34 species; 124 use-citations). Mammals (27 species), followed by reptiles (24) and fishes (16) represented the bulk of medicinal species. In relation to users, 53% of the interviewees informed that zootherapeuticals resources were sought after by people from all social classes, while 47% stated that low income people were the main buyers. The notable use and commercialization of medicinal animals to alleviate and cure health problems and ailments in cities highlights the resilience of that resource in the folk medicine. Most remedies quoted by interviewees depend on wild-caught animals, including some species under official protection. Among other aspects, the harvesting of threatened species confers zootherapy a role in the discussions about biodiversity conservation in Brazil. (C) 2007 Elsevier B.V.. All rights reserved.
