The eighth Report of the Confidential Enquiries into Maternal Deaths in the UK investigates the deaths of 261 women who died in the triennium 2006-08, from causes directly or indirectly related to pregnancy. The full Report is available for purchase or download from the Centre for Maternal and Child Enquiries (CMACE; www.cmace.org.uk). Although every maternal death is a tragedy, particularly where avoidable factors were identified by the Enquiry process, the overall picture is encouraging. The maternal death rate in the UK continues to decline despite increasing pressures on maternity services and a changing maternal population. For the first time there has been a reduction in the inequalities gap between women living in different socio-economic circumstances, and timely production of guidelines and tools appears to have helped clinical staff to deliver improved clinical care.

The Bamford review of mental health and learning disabilities identified the need for research to help with service and policy development in a number of areas. We worked with key stakeholders, gaining significant input from service users and carers along with professionals and researchers, to agree five top priorities. Research reviews were funded by HSC R&D Division, Public Health Agency (PHA) to set out current knowledge about policies and care services relevant to Children and Young People; Patient Outcomes; Intellectual Disability; Psychological Therapies and Primary Care. The reviews which can be accessed below will serve as accessible, high quality sources of up-to-date knowledge for commissioners, policy-makers, academics and providers of health or social care services as well as service users. We hope that the reviews will help to inform future development and delivery of Mental Health and Intellectual Disability services and so achieve the best outcomes for service users and their families. The reviews have also identified a number of important areas for further research.A Call for research proposalsto these areas is announced today. Further information on this Call can be found by clicking hereA further Rapid review in personality disorders has been commissioned in conjunction with HSCB and DHSSPS and is now available to download here.

The Northern Ireland Abdominal Aortic Aneurysm (AAA) Screening Programme began in June 2012 and offers AAA screening to all men in their 65th year in Northern Ireland. The aim of the AAA screening programme is to reduce AAA-related mortality by providing systematic, population-based screening. There is evidence of a significant reduction (45%) in mortality from AAA in those men aged between 65 and 79 years who undergo ultrasound screening. Men older than 65 years will be able to opt into the programme and request screening through the central screening office. There is also evidence of the long-term cost-effectiveness of AAA screening in men and further evidence that the early mortality benefit from screening is maintained. The items available for download here were in the professional information pack that was sent out to all GPs, GP practice managers and pharmacies prior to the launch of the programme. The invitation leaflet is sent out to all eligible men with the letter inviting them to screening. The results leaflets are for men diagnosed with a small, medium or large AAA. The relevant result leaflet is given to men directly after their scan. The poster was sent out to all GPs, GP practice managers and pharmacies in the run-up to the launch of the programme as a means of raising awareness. The information sheet outlines the structure of the programme, the screening process and the primary care that follows for those men diagnosed with an AAA. It also highlights the risk factors and has stats on AAA prevalence. The frequently asked questions address issues relating to all aspects of the programme: what is an AAA, roll-out of the programme, the screening process, the scan itself, the possible results, the available treatment, and how personal information is used.

Testing for high-risk human papillomavirus (HR-HPV) as triage and test of cure was introduced into the Northern Ireland Cervical Screening Programme on Monday 28 January 2013. This policy change will significantly alter the screening pathway for women with a mild dyskaryosis or borderline smear result. The link between HR-HPV infection and the development of cervical cancer has now been clearly established, with almost 100% of cervical cancers containing HPV DNA. Women with no evidence of HR-HPV infection are extremely unlikely to develop cervical cancer in the short to medium term. HPV triage is the process whereby HR-HPV testing is used to manage women with low grade cervical abnormalities. Only 15-20% of women with a borderline or mild smear result have a significant abnormality that needs treatment. HR-HPV testing is effective in identifying which women may need treatment and allows colposcopy resources to be allocated more effectively. The test of cure process is being introduced because it is now known that women with a normal or low grade smear test, and who are HR-HPV negative at six months after treatment, are at very low risk of residual disease. These women do not need to be recalled for another screening appointment for three years. The test of cure process means all post-treatment smears (at six months) that are reported as normal, borderline or mild dyskaryosis will be tested for HR-HPV. Those women who are HR-HPV positive will remain at colposcopy. HR-HPV negative women can be safely returned to recall in three years. It is estimated that the HR-HPV test of cure will allow approximately 80% of women who have been through treatment to avoid undergoing annual smear tests. This flowchart poster outlines the new triage and test of cure process. It was distributed to all GPs in Northern Ireland and is available to download as a PDF from this website.
