Stapled haemorrhoidopexy vs. Milligan-Morgan haemorrhoidectomy for grade III haemorrhoids: a randomized clinical trial

The aim of this double blind randomized clinical trial was to compare the short-term and long-term outcomes of stapled haemorrhoidopexy (SH group) performed using a circular stapler with that of the Milligan-Morgan haemorrhoidectomy (MMH group). A total of 79 consecutive patients with grade III haemorrhoids were randomized into two groups treated with SH (n. 39) and MMH (n. 40). The outcomes of the procedures were evaluated postoperatively and over a follow-up period of minimum 2 years. Patients undergoing the SH procedure showed greater short term advantages than MMH group with reduced pain, shorter length of hospital stay, earlier return to work and high patient satisfaction. Long-term follow-up has indicated more favourable results in MMH group in terms of resumption of symptoms with absence of residual prolapse and risk of recurrence of prolapse. At two years follow-up recurrent prolapse was confirmed in six patients of SH group (13%) whereas in none of the MMH group. At six months follow-up there weren’t significant difference in the mean satisfaction score for the two groups. At two years the mean satisfaction score was higher in the MMH group vs SH group. Seven patients in the SH group needed a reoperation whereas none in MMH group. From January 2009, in our Surgery Unit the patients are always informed about a higher recurrence rate of SH and we perform this technique only when the patient choices to accept this risk to take advantage of the short-term benefits of this procedure.

“PROSPERING BECAUSE THAT’S ITS HISTORY”: BLACK RESILIENCE AND HONORS DEVELOPMENT IN HIGHER EDUCATION: MORGAN STATE UNIVERSITY AND THE STATE OF MARYLAND, 1867-1988

This study explores the origins and development of honors education at a Historically Black College and University (HBCU), Morgan State University, within the context of the Maryland higher education system. During the last decades, public and private institutions have invested in honors experiences for their high-ability students. These programs have become recruitment magnets while also raising institutional academic profiles, justifying additional campus resources. The history of higher education reveals simultaneous narratives such as the tension of post-desegregated Black colleges facing uncertain futures; and the progress of the rise and popularity of collegiate honors programs. Both accounts contribute to tracing seemingly parallel histories in higher education that speaks to the development of honors education at HBCUs. While the extant literature on honors development at Historically White Institutions (HWIs) of higher education has gradually emerged, our understanding of activity at HBCUs is spotty at best. One connection of these two phenomena is the development of honors programs at HBCUs. Using Morgan State University, I examine the role and purpose of honors education at a public HBCU through archival materials and oral histories. Major unexpected findings that constructed this historical narrative beyond its original scope were the impact of the 1935/6 Murray v Pearson, the first higher education desegregation case. Other emerging themes were Morgan’s decades-long efforts to resist state control of its governance, Maryland’s misuse of Morrill Act funds, and the border state’s resistance to desegregation. Also, the broader histories of Black education, racism, and Black citizenship from Dred Scott and Plessy, the 1863 Emancipation Proclamation to Brown, inform this study. As themes are threaded together, Critical Race Theory provides the framework for understanding the emerging themes. In the immediate wake of the post-desegregation era, HBCUs had to address future challenges such as purpose and mission. Competing with HWIs for high-achieving Black students was one of the unanticipated consequences of the Brown decision. Often marginalized from higher education research literature, this study will broaden the research repository of honors education by documenting HBCU contributions despite a challenging landscape.

Tidal Energy ”Round Robin” Tests Comparisons between towing tank and circulating tank results

One key step of the industrial development of a tidal energy device is the testing of scale prototype devices within a controlled laboratory environment. At present, there is no available experimental protocol which addresses in a quantitative manner the differences which can be expected between results obtained from the different types of facilities currently employed for this type of testing. As a consequence, where differences between results are found it has been difficult to confirm the extent to which these differences relate to the device performance or to the test facility type. In the present study, a comparative ”Round Robin” testing programme has been conducted as part of the EC FP VII MaRINET program in order to evaluate the impact of different experimental facilities on the test results. The aim of the trials was to test the same model tidal turbine in four different test facilities to explore the sensitivity of the results to the choice of facility. The facilities comprised two towing tanks, of very different size, and two circulating water channels. Performance assessments in terms of torque, drag and inflow speed showed very similar results in all facilities. However, expected differences between the different tank types (circulating and towing) were observed in the fluctuations of torque and drag measurements. The main facility parameters which can influence the behaviour of the turbine were identified; in particular the effect of blockage was shown to be significant in cases yielding for high thrust coefficients, even at relatively small blockage ratios.

Esta es probablemente la razón por la que muchos «villanos» han sido definidos como «bandidos sociales», término utilizado originalmente por Eric Hobsbawm para referirse a un forajido quizás mejor ejemplificado por el personaje de Robin Hood en la tradición inglesa. Este bandido social, admirado por todos y en todas partes, que roba a los ricos y reparte el botín entre los pobres, actúa al margen de la ley y personifica asimismo la resistencia a la opresión social y una serie de virtudes más personales. En este artículo, vamos a analizar la forma en que el bandido inglés se ha convertido en un héroe mítico más que legendario. De hecho, el personaje de Robin Hood ha trascendido el tiempo y la cultura, y se ha convertido en el arquetipo del proscrito benevolente, un héroe popular nacido de las frustraciones de una clase inferior que considera que no hay leyes ni reglas en una sociedad injusta, sino la voluntad y el capricho de quien detenta el poder. [en] Throughout history, villains -whether real or imaginary- have served useful functions, especially those of “safety valves” for society. This is probably why many “villains” have been defined as “social bandits”, a term originally used by Eric Hobsbawm to refer to a type best exemplified in the Anglo-Saxon tradition by Robin Hood. This true and widely admired social bandit who steals from the rich and gives to the poor operates outside the law but at the same time personifies both group resistance to oppression and a number of more personal virtues. In this paper, we shall analyse the way in which the English bandit has become a mythical rather than a legendary hero. Indeed, the Robin Hood figure has transcended time and culture, and has become the archetype of the benevolent outlaw, a folk hero born of the frustrations of an underclass that sees the law as the will and whim of the privileged classes above them.

Contested Play and Clean Water: McMillan Park, Race, and the Built Environment in Washington, D.C., 1900-1941

This study focuses on the intersection of the politics and culture of open public space with race relations in the United States from 1900 to 1941. The history of McMillan Park in Washington, D.C. serves as a lens to examine these themes. Ultimately, the park’s history, as documented in newspapers, interviews, reports, and photographs, reveals how white residents attempted to protect their dominance in a racial hierarchy through the control of both the physical and cultural elements of public recreation space. White use of discrimination through seemingly neutral desires to protect health, safety, and property values, establishes a congruence with their defense of residential property. Without similar access to legal methods, African Americans acted through direct action in gaps of governmental control. Their use of this space demonstrates how African-American residents of Washington and the United States contested their race, recreation, and spatial privileges in the pre-World War II era.

Results of a round-robin exercise on read-across

A round-robin exercise was conducted within the CALEIDOSLIFE project. The participants were invited to assess the hazard posed by a substance, applying in silico methods and read-across approaches. The exercise was based on three endpoints: mutagenicity, bioconcentration factor and fish acute toxicity. Nine chemicals were assigned for each endpoint and the participants were invited to complete a specific questionnaire communicating their conclusions.The interesting aspect of this exercise is the justification behind the answers more than the final prediction in itself. Which tools were used? How did the approach selected affect the final answer?

This pamphlet is a reprint (with extensive rev1s10n and added material) of a pamphlet prepared under the auspices of this department by interested workers in the area of physical education, and printed and distributed by the Iowa State Teachers Association.Mr. Louis E. Hutto, then Supervisor of Physical Education in the Des Moines school system, served as chairman of the original committee of three. Miss Monica R. Wild, Head of the Department of Physical Education for Women in Iowa State Teachers College, and Miss Doris E. White, of her staff, were the other members. These latter two, recently appointed as a Committee on Revision, have rewritten and enlarged the pamphlet, while retaining the plan of the earlier edition. Teachers of physical education in Iowa, this department, and all interested in the subject feel a debt of gratitude for the painstaking and efficient service rendered by these devoted workers. This debt I am most happy to acknowledge.
