INTRODUÇÃO: Descrever os achados fundoscópicos em pacientes com AIDS e neurotoxoplasmose em fase ativa. MÉTODOS: Foi desenvolvido estudo prospectivo tipo série de casos incluindo 70 pacientes, de ambos os sexos, com idade variando de 20 a 63 anos, internados nas enfermarias de três hospitais públicos da Cidade do Recife, Pernambuco, com diagnósticos de AIDS e neurotoxoplasmose firmados segundo os critérios do Centers for Disease Control and Prevention (1992), no período de janeiro a outubro de 2008. Os pacientes se caracterizavam por: primeiro episódio de neurotoxoplasmose (65; 92,9%) ou recidiva (5; 7,1%); desconhecimento de ter AIDS (23; 32,9%), contagem média de linfócitos T CD4 de 139,8 ± 3,04 células/mm3 e carga viral média igual a 137.080 ± 39.380 cópias/mL. Todos foram submetidos a exame oftalmológico, consistindo de: inspeção ocular; aferição da acuidade visual; investigação da função muscular extrínseca ocular e fundoscopia, empregando oftalmoscópio indireto binocular (modelo OHN 3.5 (Eyetec®) e lente externa de 20 dioptrias (Volk®). RESULTADOS: Os achados consistiram em: exsudatos algodonosos retinianos (8,6%), constricção arteriolar difusa leve (8,6%); lesões de retinocoroidite cicatricial, características de toxoplasmose ocular (5,7%), atrofia do epitélio pigmentar retiniano (2,9%), descolamento da retina (2,9%), aumento de escavação papilar (1,4%), degeneração periférica retiniana (1,4%), macroaneurisma (1,4%), papiledema bilateral (1,4%), tração vítreo-retiniana (1,4%). CONCLUSÕES: Pacientes com AIDS e neurotoxoplasmose podem apresentar alterações fundoscópicas características da toxoplasmose ocular, na forma ativa ou cicatricial, relacionadas ao HIV ou, ainda, a outras doenças oportunistas ou sistêmicas, podendo ser de grande auxílio num tratamento integral do paciente por uma equipe multiprofissional.

A Retinopatia Diabética (RD) é uma patologia ocular, consequência da Diabetes Mellitus, e que pode levar à perda de visão. Dado que nos primeiros estágios da doença, não ocorre manifestação sintomática, é de máxima importância a existência de programas estruturados de rastreio. Para diminuir a carga laboral que estes programas acarretam, pode-se recorrer à utilização de sistemas de deteção e classificação automática de RD que têm vindo a ser desenvolvidos. No entanto, estes demonstram algumas falhas, sendo que na origem destas pode estar a existência de imagens com qualidade não adequada. De maneira a reduzir as falhas destes sistemas, potenciando a sua utilização em programas de rastreio e automatizando assim todo o processo, foi conduzido este estudo, no qual se teve diretamente em consideração a qualidade das imagens, avaliada também de forma automática, utilizando a sua informação como parâmetro de entrada nos classificadores automáticos de RD. A avaliação automática da qualidade é constituída por 4 classificadores das características desta: Cor, Focagem, Contraste e Iluminação, sendo que a combinação destes dá origem a um último classificador, denominado Qualidade Geral. Desta forma, foi efetuada a fusão destes classificadores com os algoritmos de classificação automática de RD para todas as imagens, numa primeira abordagem, e desconsiderando as imagens marcadas pelo classificador Qualidade Geral como inadequadas, numa última abordagem. Os resultados obtidos mostraram uma melhoria no conjunto dos indicadores sensibilidade e especificidade dos métodos de classificação automática de RD quando se procedeu à combinação destes com a Qualidade Geral. Mostraram ainda uma melhoria mais acentuada quando se usou informação de cada um dos classificadores das características que contribuem para a avaliação da Qualidade, em vez de uma informação única sobre qualidade. A presença de imagens de má qualidade mostrou ser prejudicial para o desempenho dos classificadores automáticos de RD, não tendo tanta influência no classificador resultante da fusão que produziu os melhores resultados do conjunto sensibilidade e especificidade.

The potential of human adenovirus vectors as vehicles for gene transfer with clinical applications in vaccination, cancer treatment and in many monogenic and acquired diseases has been demonstrated in several studies and clinical trials. However, the clinical use of these vectors can be limited by pre-existing humoral and cellular anti-capsid immunity. One way to circumvent this bottleneck while keeping the advantages of using adenovirus vectors is using non-human viruses such as Canine Adenovirus type 2 (CAV-2). Moreover, CAV-2 vectors present attractive features to develop potential treatment of neurodegenerative and ocular disorders. While the interest in CAV-2 vectors increases, scalable and robust production processes are required to meet the need for preclinical and possibly clinical uses.(...)

INTRODUCTION: Toxoplasmosis is usually a benign infection, except in the event of ocular, central nervous system (CNS), or congenital disease and particularly when the patient is immunocompromised. Treatment consists of drugs that frequently cause adverse effects; thus, newer, more effective drugs are needed. In this study, the possible activity of artesunate, a drug successfully being used for the treatment of malaria, on Toxoplasma gondii growth in cell culture is evaluated and compared with the action of drugs that are already being used against this parasite. METHODS: LLC-MK2 cells were cultivated in RPMI medium, kept in disposable plastic bottles, and incubated at 36ºC with 5% CO2. Tachyzoites of the RH strain were used. The following drugs were tested: artesunate, cotrimoxazole, pentamidine, pyrimethamine, quinine, and trimethoprim. The effects of these drugs on tachyzoites and LLC-MK2 cells were analyzed using nonlinear regression analysis with Prism 3.0 software. RESULTS: Artesunate showed a mean tachyzoite inhibitory concentration (IC50) of 0.075µM and an LLC MK2 toxicity of 2.003µM. Pyrimethamine was effective at an IC50 of 0.482µM and a toxicity of 11.178µM. Trimethoprim alone was effective against the in vitro parasite. Cotrimoxazole also was effective against the parasite but at higher concentrations than those observed for artesunate and pyrimethamine. Pentamidine and quinine had no inhibitory effect over tachyzoites. CONCLUSIONS: Artesunate is proven in vitro to be a useful alternative for the treatment of toxoplasmosis, implying a subsequent in vivo effect and suggesting the mechanism of this drug against the parasite.

Introduction: There are more than 300,000 extractors using the babaçu coconut as a source of income in the States of Maranhão, Pará, Tocantins and Piauí, and this activity is associated with fungal infections. The objective of this study was to examine the occurrence of emergent fungi in the conjunctiva, nails and surface and subcutaneous injuries of female coconut breakers in Esperantinópolis, Maranhão. Additionally, soil samples and palm structures were collected. Methods: The obtained samples were cultured in Petri dishes containing potato-dextrose-agar and chloramphenicol. The etiological agent was confirmed by a direct mycological exam and growth in culture. Results: In total, 150 domiciles were visited, and samples were collected from 80 patients. From the ground, the most frequently isolated fungus was Aspergillus niger (53. 8%). the most frequently detected fungus in babaçu coconut was Aspergillus niger (66.7%). Conjunctival fungal growth occurred in 76.3% of the women. The ocular fungal microbiota consisted of filamentous fungi (80.6%), and yeasts were present in 19.4% of cases. Onychomycosis was diagnosed in 44% (11/25) of the women. Conclusions: The identification of the genera Neosartorya, Rhizopus and Curvularia in onychomycoses shows that emergent filamentous fungi can be isolated. Aspergillus sp., Penicillium sp. and Scedosporium sp. were the predominant genera found in the babaçu coconut. From ocular conjunctiva, Candida spp. were the most prevalent species isolated, and Fusarium sp. was present only in one woman. The nearly permanent exposure of coconut breakers to the external environment and to the soil is most likely the reason for the existence of a mycotic flora and fungal infections, varying according to the individual's practices and occupation.

The obligate intracellular bacterium Chlamydia trachomatis is a human pathogen of major public health significance. Strains can be classified into 15 main serovars (A to L3) that preferentially cause ocular infections (A-C), genital infections (D-K) or lymphogranuloma venereum (LGV) (L1-L3), but the molecular basis behind their distinct tropism, ecological success and pathogenicity is not welldefined. Most chlamydial research demands culture in eukaryotic cell lines, but it is not known if stains become laboratory adapted. By essentially using genomics and transcriptomics, we aimed to investigate the evolutionary patterns underlying the adaptation of C. trachomatis to the different human tissues, given emphasis to the identification of molecular patterns of genes encoding hypothetical proteins, and to understand the adaptive process behind the C. trachomatis in vivo to in vitro transition. Our results highlight a positive selection-driven evolution of C. trachomatis towards nichespecific adaptation, essentially targeting host-interacting proteins, namely effectors and inclusion membrane proteins, where some of them also displayed niche-specific expression patterns. We also identified potential "ocular-specific" pseudogenes, and pointed out the major gene targets of adaptive mutations associated with LGV infections. We further observed that the in vivo-derived genetic makeup of C. trachomatis is not significantly compromised by its long-term laboratory propagation. In opposition, its introduction in vitro has the potential to affect the phenotype, likely yielding virulence attenuation. In fact, we observed a "genital-specific" rampant inactivation of the virulence gene CT135, which may impact the interpretation of data derived from studies requiring culture. Globally, the findings presented in this Ph.D. thesis contribute for the understanding of C.trachomatis adaptive evolution and provides new insights into the biological role of C. trachomatishypothetical proteins. They also launch research questions for future functional studies aiming toclarify the determinants of tissue tropism, virulence or pathogenic dissimilarities among C. trachomatisstrains.

Pouchitis is the most frequent complication of ileal pouch-anal anastomosis for treatment of ulcerative colitis. There are several possible explanations. Among them, we focus on the one that considers pouchitis as an extracolonic manifestation of ulcerative colitis. The aim of this study was to investigate the association between pouchitis and extra-intestinal manifestations (EIM), which are frequent in these patients. Sixty patients underwent restorative proctocolectomy with an ileal J pouch (IPAA) from September 1984 to December 1998. Pouchitis was defined by clinical, endoscopic, and histologic criteria. The following extra-intestinal manifestations were studied: articular, cutaneous, hepatobiliary, ocular, genitourinary, and growth failure. Thirteen patients, of which 10 were female (76.9%), developed one or more episodes of pouchitis. Twelve patients of this group (92.3%) presented some kind of extra-intestinal manifestation, 4 pre-operatively (exclusively), 2 post-operatively (exclusively), and 6 both pre- and post-operatively (1.7 per patient). Twenty patients (42.7%) of the 47 without pouchitis did not present extra-intestinal manifestations; 10/35 (28.5%) of females had pouchitis, compared to 3/35 (12.0%) of men. Pouchitis was more frequent among females, though not statistically significant. EIM increases the risk of pouchitis. Pouchitis is related to EIM in 92.3 % of cases, corroborating the hypothesis that it could be an extracolonic manifestation of ulcerative colitis.

OBJECTIVE: To identify social characteristics and expectations of individuals seen during a community project for the treatment of senile cataracts. Expected results from their eye surgery and its consequences to their quality of life were studied as well. METHODOLOGY: Cataract patients (visual acuity equal to or lower than 0.2 in the more superior eye) aged 50 years or over, were surveyed by means of interviews held during their visit at the Cataract Project in São Paulo city, State of São Paulo, Brazil, in 1999. RESULTS: The sample was composed of 331 subjects of low socioeconomic level ranging in age from 50 to 97 years (average = 71.8 years). Expectation of total recovery from the cataract condition by means of surgery was declared by 80.0% of the respondents, with no significant differences between male and female subjects (P < 0.1723). Hope to resume manual activities was expressed by 59.8%. CONCLUSION: A predominance of expectations of resuming normal activity and achieving a better quality of life after cataract surgery were identified.

The cerebellum floccular complex lobes (FCLs) are housed in the FCL fossa of the periotic complex. There is experimental evidence indicating that the FCLs integrate visual and vestibular information, responsible for the vestibulo-ocular reflex, vestibulo-collic reflex, smooth pursuit and gaze holding. Thus, the behavior of extinct animals has been correlated with FCLs dimension in multiple paleoneuroanatomy studies. Here I analyzed braincase endocasts of a representative sample of Mammalia (48 species) and Aves (59 species) rendered using tomography and image segmentation and tested statistical correlations between the floccular complex volume, ecological and behavioral traits to assess various previously formulated paleobiological speculations. My results demonstrate: 1) there is no significant correlation between relative FCL volume and body mass; 2) there is no significant correlation between relative FCL and optic lobes size in birds; 3) average relative FCL size is larger in diurnal than in nocturnal birds but there is no statistically significant difference in mammals; 4) feeding strategies are related with different FCL size patterns in birds, but not in mammals; 5) locomotion type is not related with relative FCL size in mammals; 6) agility is not significantly correlated with FCL size in mammals. I conclude that, despite the apparent relation between FCL size and ecology in birds, the cerebellum of tetrapods is a highly plastic structure and may be adapted to control different functions across different taxonomic levels. For example, the european mole (Talpa europaea) which is fossorial and practically blind, has a FCL fossae relative size larger than those of bats, which are highly maneuverable. Therefore, variation in FCL size may be better explained by a combination of multiple factors with relation to anatomical and phylogenetic evolutionary constraints.

INTRODUÇÃO: As complicações oculares do VIH/SIDA são comuns e podem afetar qualquer tecido do olho. Na era da terapêutica antirretroviral combinada (TARc), houve uma redução drástica nas infeções oportunistas oculares. O impacto da terapêutica em outras alterações na visão de doentes VIH+ sem retinopatia infecciosa, como alterações na sensibilidade ao contraste (SC), visão cromática (VC) e nos campos visuais, sinal de uma disfunção microvascular retiniana, não está totalmente esclarecido. OBJETIVOS: Determinar os efeitos da TARc em parâmetros e patologia oculares de doentes VIH+ que vão iniciar ou reiniciar TARc MÉTODOS: Foi realizado um estudo observacional, longitudinal, transversal e prospetivo com a inclusão de 31 doentes VIH+ sem infeções oculares oportunistas, que iam iniciar ou reiniciar TARc. Após serem recolhidos alguns dados da história clínica, foi feita uma avaliação oftalmológica completa que incluiu: biomicroscopia do segmento anterior, acuidade visual com a escala de ETDRS, pressão intra-ocular com a tonometria de aplanação de Goldmann, SC com o CSV-1000E, VC com o Farnsworth- Munsell 100 e perimetria com o programa 24-2 do Octopus® 900. Após midríase farmacológica foram realizadas fotografias do segmento posterior, avaliação da espessura da camada de fibras nervosas (CFN) e macular com o OCT Stratus™ e avaliação da densidade do cristalino e do ângulo irido-corneano pelo Pentacam®. Cerca de 9 meses após o início da TARc, foi realizada uma segunda observação oftalmológica usando os mesmos métodos. Procuraram-se associações estatísticas entre vários parâmetros da infeção pelo VIH e a avaliação oftalmológica. RESULTADOS: Na primeira observação, encontraram-se 15 olhos (24%) com complicações anteriores do VIH, 10 olhos (16%) com retinopatia do VIH, 15 (24%) com alterações vasculares retinianas e 6 (10%) com defeitos na CFN; nenhuma destas alterações estava relacionada com o nível de linfócitos CD4+ ou carga viral. A SC, VC e perimetria estavam alteradas em 45%, 68% e 76% dos olhos, respetivamente. Encontrou-se uma correlação positiva entre valores mais elevados de linfócitos T CD4+ e melhor SC, sobretudo nos 6 e 12 ciclos por grau e no valor soma da SC (p<0,01). A avaliação pelo OCT revelou que os doentes com CFN fina tinham tendencialmente infeção mais antiga, níveis mais baixos de linfócitos CD4+ (p<0,05) e pior SC (p<0,05). Foi possível realizar segunda observação em 16 doentes (52%). Após 9 meses de TARc as alterações encontradas na primeira avaliação mantiveram-se, com exceção da retinopatia do VIH que regrediu. As alterações na SC, VC, perimetria e na espessura da CFN mantiveram-se; a densidade do cristalino não se alterou com a reconstituição imunitária após TARc. CONCLUSÕES: Foram detetadas alterações na avaliação oftalmológica de doentes VIH+ sem retinopatia infecciosa e que parecem estar relacionadas com a existência de microvasculopatia e disfunção neurorretiniana associada. A reconstituição imunitária com a TARc não parece levar a uma melhoria desta disfunção apesar da recuperação da imunidade para valores normais.

Analisou-se o valor nutricional e energético de alguns frutos e sementes das florestas de várzea e igapó, próximas à região de Manaus-AM, importantes na alimentação do tambaqui (Colossoma macropomum Cuvier, 1818). Para o levantamento das espécies e os tipos de frutos consumidos por esta espécie foram utilizadas informações da literatura, complementadas com os resultados de análises de conteúdo dos tratos digestivos coletados através de capturas de animais e no principal porto de desembarque e comercialização de peixes em Manaus, a feira da Panair. Foram realizadas, semanalmente, na feira, coleta de 15 tratos digestivos, durante nove meses. Após a evisceração, os tratos digestivos foram acondionados em sacos plásticos, armazenados em isopor com gelo e transportados até o laboratório onde permaneceram em freezer (-20ºC). As análises de conteúdo dos tratos digestivos consistiram na coleta dos frutos e sementes para identificação, após serem preservados em álcool a 70% constituindo uma coleção de referência. Após identificação, as espécies de frutos e sementes foram coletadas nas áreas de ocorrência na várzea e igapó para as análises bromatológicas, seguindo metodologia da A.O.A.C (1995) e a energia bruta (kJ/g) através de bomba calorimétrica. Identificou-se 46 espécies vegetais, distribuídas em 21 famílias e classificados nove tipos de frutos. De acordo com os dados atuais, a alimentação do tambaqui engloba, pelo menos, 133 espécies, entre frutos e sementes, distribuídas em 38 famílias e 15 tipos de frutos. O consumo destes itens variou entre inteiros e/ou triturados. A composição bromatológica de 14 espécies de sementes e 40 de frutos demostrou que estes itens são mais energéticos do que protéicos.

Purpose. To analyze dry eye disease (DED) tests and their consistency in similar nonsymptomatic population samples living in two geographic locations with different climates (Continental vs. Atlantic). Methods. This is a pilot study including 14 nonsymptomatic residents from Valladolid (Continental climate, Spain) and 14 sex-matched and similarly aged residents from Braga (Atlantic climate, Portugal); they were assessed during the same season (spring) of two consecutive years. Phenol red thread test, conjunctival hyperemia, fluorescein tear breakup time, corneal and conjunctival staining, and Schirmer test were evaluated on three different consecutive visits. Reliability was assessed using the intraclass correlation coefficient and weighted kappa (J) coefficient for quantitative and ordinal variables, respectively. Results. Fourteen subjects were recruited in each city with a mean (TSD) age of 63.0 (T1.7) and 59.1 (T0.9) years (p = 0.08) in Valladolid and Braga, respectively. Intraclass correlation coefficient and J values of the tests performed were below 0.69 and 0.61, respectively, for both samples, thus showing moderate to poor reliability. Subsequently, comparisons were made between the results corresponding to the middle and higher outdoor relative humidity (RH) visit in each location as there were no differences in mean temperature (p Q 0.75) despite RH values significantly differing (p e 0.005). Significant (p e 0.05) differences were observed between Valladolid and Braga samples on tear breakup time (middle RH visit, 2.76 T 0.60 vs. 5.26 T 0.64 seconds; higher RH visit, 2.61 T 0.32 vs. 5.78 T 0.88 seconds) and corneal (middle RH, 0.64 T 0.17 vs. 0.14 T 0.10; higher RH, 0.60 T 0.22 vs. 0.0 T 0.0) and conjunctival staining (middle RH, 0.61 T 0.17 vs. 0.14 T 0.08; higher RH, 0.57 T 0.15 vs. 0.18 T 0.09). Conclusions. This pilot study provides initial evidence to support that DED test outcomes assessing the ocular surface integrity and tear stability are climate dependent. Future large-sample studies should support these outcomes also in DED patients. This knowledge is fundamental for multicenter clinical trials. Lack of consistency in diagnostic clinical tests for DED was also corroborated. (Optom Vis Sci 2015;92:e284Ye289)

Purpose: To study the relationship among the variables intensity ofthe end-of-day (EOD) dryness, corneal sensitivity and blink rate in soft contact lens (CL) wearers. Methods: Thirty-eight soft CL wearers (25 women and 13 men; mean age 27.1 ± 7.2 years) were enrolled. EOD dryness was assessed using a scale of 0–5 (0, none to 5, very intense). Mechanical and thermal (heat and cold) sensitivity were measured using a Belmonte’s gas esthesiometer. The blink rate was recorded using a video camera while subjects were wearing a hydrogel CL and watching a film for 90 min in a controlled environmental chamber. Results: A significant inverse correlation was found between EOD dryness and mechanical sensitivity (r: −0.39; p = 0.02); however, there were no significant correlations between EOD dryness and thermal sensitivity. A significant (r: 0.56; p < 0.001) correlation also was observed between EOD dryness and blink rate, but no correlations were found between blink rate and mechanical or thermal sensitivity. Conclusions: CL wearers with higher corneal sensitivity to mechanical stimulation reported more EOD dryness with habitual CL wear. Moreover, subjects reporting more EOD dryness had an increased blink rates during wear of a standard CL type. The increased blink rate could act to improve the ocular surface environment and relieve symptoms

Purpose: Higher myopic refractive errors are associated with serious ocular complications that can put visual function at risk. There is respective interest in slowing and if possible stopping myopia progression before it reaches a level associated with increased risk of secondary pathology. The purpose of this report was to review our understanding of the rationale(s) and success of contact lenses (CLs) used to reduce myopia progression. Methods: A review commenced by searching the PubMed database. The inclusion criteria stipulated publications of clinical trials evaluating the efficacy of CLs in regulating myopia progression based on the primary endpoint of changes in axial length measurements and published in peerreviewed journals. Other publications from conference proceedings or patents were exceptionally considered when no peer-review articles were available. Results: The mechanisms that presently support myopia regulation with CLs are based on the change of relative peripheral defocus and changing the foveal image quality signal to potentially interfere with the accommodative system. Ten clinical trials addressing myopia regulation with CLs were reviewed, including corneal refractive therapy (orthokeratology), peripheral gradient lenses, and bifocal (dual-focus) and multifocal lenses. Conclusions: CLs were reported to be well accepted, consistent, and safe methods to address myopia regulation in children. Corneal refractive therapy (orthokeratology) is so far the method with the largest demonstrated efficacy in myopia regulation across different ethnic groups. However, factors such as patient convenience, the degree of initial myopia, and non-CL treatments may also be considered. The combination of different strategies (i.e., central defocus, peripheral defocus, spectral filters, pharmaceutical delivery, and active lens-borne illumination) in a single device will present further testable hypotheses exploring how different mechanisms can reinforce or compete with each other to improve or reduce myopia regulation with CLs.
