The Ministry of Health's National Human Rabies Control Program advocates pre-exposure prophylaxis (PEP) for professionals involved with animals that are at risk of contracting rabies. We report an antemortem and postmortem diagnosis of rabies in a veterinarian who became infected when handling herbivores with rabies. The antemortem diagnosis was carried out with a saliva sample and a biopsy of hair follicles using molecular biology techniques, while the postmortem diagnosis used a brain sample and conventional techniques. The veterinarian had collected samples to diagnose rabies in suspect herbivores (bovines and caprines) that were subsequently confirmed to be positive in laboratory tests. After onset of classic rabies symptoms, saliva and hair follicles were collected and used for antemortem diagnostic tests and found to be positive by RT-PCR. Genetic sequencing showed that the infection was caused by variant 3 (Desmodus rotundus), a finding confirmed by tests on the brain sample. It is essential that professionals who are at risk of infection by the rabies virus undergo pre-exposure prophylaxis. This study also confirms that molecular biology techniques were used successfully for antemortem diagnosis and therefore not only allow therapeutic methods to be developed, but also enable the source of infection in human rabies cases to be identified accurately and quickly.

Background : The neonatal arterial switch operation (ASO) is now the standard of care for children born with transposition of the great arteries. Stenosis of the neopulmonary artery on long‑term follow up is a known complication. Methods : We performed a retrospective analysis of eleven patients who underwent a cardiac magnetic resonance imaging (MRI) due to echocardiographic evidence suggestive of stenosis of the neopulmonary artery or its branches (mean estimated Doppler gradient 48 mmHg, min 30 mmHg, max 70 mmHg). A comprehensive evaluation of anatomy and perfusion was done by cardiac MRI. Results : The branches of the neopulmonary artery (neo PA) showed decreased caliber in three patients unilaterally and in two patients, bilaterally. Magnetic resonance (MR) perfusion studies showed concomitant decreased flow, with discrepancy between the two lungs of 35/65% or worse, only in the three patients with unilateral obstruction, by two different MR perfusion methods. Conclusions : Cardiac MR can be used as a comprehensive non‑invasive imaging technique to diagnose stenosis of the branches of the neopulmonary after the ASO, allowing evaluation of anatomy and function of the neoPA, its branches, and the differential perfusion to each lung, thus facilitating clinical decision making.

INTRODUCTION: The symptoms of Brazilian borreliosis resemble the clinical manifestations of Lyme disease (LD). However, there are differences between the two in terms of epidemiological and laboratory findings. Primers usually employed to diagnose LD have failed to detect Borrelia strains in Brazil. OBJECTIVE: We aimed to identify the Brazilian Borrelia using a conserved gene that synthesizes the flagellar hook (flgE) of Borrelia burgdorferi sensu lato. METHOD: Three patients presenting with erythema migrans and positive epidemiological histories were recruited for the study. Blood samples were collected, and the DNA was extracted by commercial kits. RESULTS: The gene flgE was amplified from DNA of all selected patients. Upon sequencing, these positive samples revealed 99% homology to B. burgdorferi flgE. CONCLUSION: These results support the existence of borreliosis in Brazil. However, it is unclear whether this borreliosis is caused by a genetically modified B. burgdorferi sensu stricto or by a new species of Borrelia spp.

This study contributes to knowledge of Anopheles species, including vectors of Plasmodium from the western Brazilian Amazon in Porto Velho, Rondônia State. The sampling area has undergone substantial environmental changes as a consequence of agricultural and hydroelectric projects, which have caused intensive deforestation and favored habitats for some mosquito species. The purpose of this study was to diagnose the occurrence of anopheline species from collections in three locations along an electric-power transmission line. Each locality was sampled three times from 2010 to 2011. The principal adult mosquitoes captured in Shannon trap were Anopheles darlingi, An. triannulatus, An. nuneztovari l.s., An.gilesi and An. costai. In addition, larvae were collected in ground breeding sites for Anopheles braziliensis, An. triannulatus, An. darlingi, An. deaneorum, An. marajoara, An. peryassui, An. nuneztovari l.s. and An. oswaldoi-konderi. Anopheles darlingi was the most common mosquito in the region. We discuss Culicidae systematics, fauna distribution, and aspects of malaria in altered habitats of the western Amazon.

8A>C>86A G:EDGI: A patient diagnosed Wilson’s disease (WD) 22 years previously, successfully treated initially with zinc, developed neuropsychiatric disease after years of irregular therapy. Reassuming zinc therapy was successful. After a normal pregnancy, she had two therapeutic abortions for corpus callosum agenesis, and a missed abortion. We review the genetics, physiopathology, clinics and imagiologic response to zinc therapy, the problems of pregnancy in WD, advising to maintain therapy. A hypothetic cause for fetus brain anomaly would be hypocupremia due to zinc therapy, confronting with two other possibilities, one related to Wilson’s disease in itself, other due to a congenital syndrome of agenesis of the corpus callosum, impossible to diagnose by our available diagnostic methods.

Introduc¸ão: Os divertículos uretrais femininos (DUF) afetam de 0,6 a 4,7% das mulheres, causam frequentemente sintomas urinários persistentes e podem associar-se a complicac¸ões como litíase e transformac¸ão maligna. Existe ainda elevado subdiagnóstico de DUF, bem como atraso diagnóstico e terapêutico. O objetivo deste trabalho é analisar o estado de arte em relac¸ão à etiologia, diagnóstico e terapêutica desta patologia. Materiais e métodos: Revisão bibliográfica de artigos obtidos até janeiro/2014 na base de dados Medline utilizando as palavras-chave: «female urethral diverticula», «female urethral diverticulum», «urethral diverticula», «urethral diverticulum» e «female urethra»; e após pesquisa de referências bibliográficas relevantes nos artigos obtidos. Resultados: A maioria dos DUF parecem surgir secundariamente à infec¸ão das glândulas periuretrais e uretrais. Apesar da associac¸ão clássica à tríade de disúria, dispareunia e gotejo pós-miccional, as manifestac¸ões clínicas são diversas e inespecíficas. Mais de um terc¸o são palpáveis ao exame ginecológico. Os métodos imagiológicos disponíveis, nomeadamente a ressonância magnética e a ecografia, apresentam elevada capacidade diagnóstica e contribuem para o planeamento cirúrgico. Dependendo da localizac¸ão, conformac¸ão e sintomatologia associada, a terapêutica dos DUF poderá ser conservadora ou, na maioria dos casos, cirúrgica. Discussão: A avaliac¸ão clínica permanece essencial para o diagnóstico de DUF. A ressonância magnética é a modalidade com maior taxa de diagnóstico e de exclusão de complicac¸ões pré ou pós-operatórias. Foram descritas técnicas menos invasivas, porém a diverticulectomia uretral transvaginal permanece a terapêutica com maior cura sintomática. Conclusão: A sensibilizac¸ão da comunidade médica é a melhor arma no combate ao subdiagnóstico e atraso diagnóstico dos DUF.

A tecnologia está em constante evolução e os benefícios, em diversas áreas, que ela nos traz são imensos. Uma das áreas que tem vindo a usufruir desta evolução é a medicina. Os avanços na tecnologia médica têm permitido aos médicos diagnosticar e tratar melhor os seus pacientes. O seu objetivo não é substituir o médico mas sim aconselhá-los e ajudá-los a tomar a melhor decisão face ao caso clínico que possam ter em mãos. Os sistemas de informação estão já tão “entrelaçados” com as práticas médicas que a ideia de uma instituição de prestação de cuidados médicos não os possuir é impensável. Isto porque a informação que estes sistemas processam diariamente é imensa e variadíssima (indo desde relatórios clínicos, a exames efetuados entre outros) para cada utente. As doenças orais fazem parte do grupo de patologias que afetam o maior número de pessoas no mundo. As ações preventivas para estes sintomas devem fazer parte da higiene diária dos indivíduos logo desde os primeiros anos de vida. Assim a aplicação apresentada nesta tese teve como objetivo a sensibilização para uma prática de higiene oral cuidada e constante. Teve também como objetivo a implementação de funcionalidades para gestão de dados dos pacientes da clínica, nomeadamente para o histórico clínico, ficando este armazenado numa anamnese. Para a implementação do presente projeto procedeu-se a um estudo prévio do estado da arte e ao levantamento de requisitos. Estes foram definidos através de reuniões de trabalho onde se analisou as necessidades da clínica com o objetivo de encontrar as soluções que melhor se enquadravam a cada caso. Para garantir que as metas propostas foram alcançadas, foram realizados inquéritos verificando assim o sucesso da aplicação.

Este documento foi redigido no âmbito da dissertação do Mestrado em Engenharia Informática na área de Arquiteturas, Sistemas e Redes, do Departamento de Engenharia Informática, do ISEP, cujo tema é diagnóstico cardíaco a partir de dados acústicos e clínicos. O objetivo deste trabalho é produzir um método que permita diagnosticar automaticamente patologias cardíacas utilizando técnicas de classificação de data mining. Foram utilizados dois tipos de dados: sons cardíacos gravados em ambiente hospitalar e dados clínicos. Numa primeira fase, exploraram-se os sons cardíacos usando uma abordagem baseada em motifs. Numa segunda fase, utilizamos os dados clínicos anotados dos pacientes. Numa terceira fase, avaliamos a combinação das duas abordagens. Na avaliação experimental os modelos baseados em motifs obtiveram melhores resultados do que os construídos a partir dos dados clínicos. A combinação das abordagens mostrou poder ser vantajosa em situações pontuais.

In order to establish if neuropsychiatric systemic lupus erythematosus (NPSLE) can be identified by any characteristic other than those used to diagnose the neuropsychiatric (NP) disease itself, we retrospectively reviewed 98 systemic lupus erythematosus (SLE) patients followed over a mean period of 10 years. NPSLE was identified in 22 patients. Stroke and generalized seizures were the most frequent NP manifestations. The NPSLE and non-NPSLE groups were similar with regard to demographic characteristics, ACR criteria, serum autoantibodies, and frequency of hypertension and hypercholesterolemia. Of note, compared to the non-NPSLE group, NPSLE was associated with a higher frequency of smoking (78 versus 26%), organ damage (73 versus 34%), and cumulative mortality rate (14 versus 7%). The series of patients was further analysed according to the presence of antiphospholipid syndrome (APS). Significantly, the interval between the onset of NP disease and SLE diagnosis was shorter in the APS(-) (0.3 ± 1 years) than in the APS(+) (5 ± 7 years) groups. Recurrence and/or persistence of NP events were only documented in the APS(-) group. Overall cumulative mortality was highest in NPSLE and in APS(+) patients with inadequate anticoagulation control, identifying an aspect that requires improved vigilance and the development of novel therapeutic modalities.

A total of 1397 sera collected from 1095 cases of exanthematic disease notified as measles in ES and RJ states during July 1992 to December 1994 were investigated. These sera were first tested for measles and rubella specific IgM. When they proved negative, they were tested for B19 specific IgM by an enzyme immunoassay. B19 infection was confirmed in 27 (2.5%) of these cases. Sera from 194 negative cases for measles and rubella IgM received from other Brazilian states were also investigated and B19 infection was confirmed for 11 of them. Sera from these 38 IgM positive cases for B19, were tested for anti-B19 IgG by an enzyme immunoassay and for B19 DNA by dot blot hybridization. Anti-B19 IgG antibodies were detected in most of the acute sera. B19 DNA was detected in the acute serum of one patient that had been splenectomized before. As the exanthem caused by human parvovirus infection may be clinically diagnosed as rubella, it could be important to diagnose B19 infection in Brazil since it is becoming prevalent as the cause of rash in countries where rubella is controlled by vaccination.

Magnetic resonance imaging (MRI) is a method of image diagnose proven to be of undeniable importance when it comes to neuro and cardio related diseases. In fact, these diseases (such as: ischemic heart disease, stroke and acute myocardial infection) have high incidence in Portugal. For these reasons, the allocation of this medical technology should not be considered with light thoughts. In fact, making decision of resource allocation in health care can be a very complex and contested matter. The impacts of new technology allocation, such MRI, can be assessed in a variety of ways. However, a fundamental component should always be present: the use of evidence-based decision-making methods. One of these methods is Technology Assessment (TA). This paper aims to characterize the equity on access of the Portuguese population in general, to a specific medical device such as MRI, under the TA point of view. It is hoped to promote a bridge of scientific knowledge between the gap on research and policy-making through TA that can emerge as a tool to aid decision-makers in the organization of health systems. There are gaps in providing healthcare, due to geographical imbalances, with some areas unable to provide certain specialized services, as hospitals in the countryside do not provide all medical specialties. Portugal has also a large independent private sector that provides diagnostic and therapeutic services to NHS users under contracts called conventions. These medical contracts cover ambulatory health facilities for laboratory tests and examinations such as diagnostic tests and Radiology. However, there is no convention from the NHS when concerning the MRI exam. Therefore, this reality can be considered a limitation in the access of the general population to this kind of clinical exam. TA can play an useful and important role in helping the decision-makers to explore potential gains that might be achieved by introducing a more rational decision making into health care management, namely into the Radiology area, regarding the allocation of MRI equipment.
