BACKGROUND: Sex differences in patients with patent foramen ovale (PFO) and cryptogenic stroke have not been systematically analyzed. We aimed to determine sex influences on demographics, vascular risk factors, clinical manifestations, stroke location, and clinical outcome. METHODS: One thousand two hundred eighty-eight consecutive patients with ischemic stroke or transient ischemic attack (TIA) were admitted to a single stroke center. All patients underwent a complete stroke workup including clinical examination, standard blood tests, cerebral and vascular imaging, transesophageal echocardiography, and 24-hour electrocardiography. In 500 patients, no definite etiology could be established (cryptogenic stroke/TIA). Of them, 167 patients (107 men and 60 women, mean age 52 +/- 13 years) had an PFO. RESULTS: The prevalence of PFO in patients with cryptogenic stroke or TIA was higher in men than in women (38% vs 28%, P = .014). Stroke severity and the prevalence of risk factors did not differ between the 2 sexes. There was an independent association between male sex and stroke location in the posterior cerebral circulation (OR 3.0, 95% CI 1.4-6.5, P = .006). Men and women did not differ in respect to PFO grade, prevalence of right-to-left shunt at rest, or coexistence of atrial septal aneurysm. Clinical outcome at 3 months was similar in both sexes. CONCLUSION: Patent foramen ovale was more prevalent in men than in women with cryptogenic stroke. There were no sex influences on age, risk factors, echocardiographic characteristics of PFO, or clinical outcome. Male sex was independently associated with stroke in the posterior cerebral circulation.

OBJECTIVES: Pulmonary valve insufficiency remains a leading cause for reoperations in congenital cardiac surgery. The current percutaneous approach is limited by the size of the access vessel and variable right ventricular outflow tract morphology. This study assesses the feasibility of transapical pulmonary valve replacement based on a new valved stent construction concept. METHODS: A new valved stent design was implanted off-pump under continuous intracardiac echocardiographic and fluoroscopic guidance into the native right ventricular outflow tract in 8 pigs (48.5 +/- 6.0 kg) through the right ventricular apex, and device function was studied by using invasive and noninvasive measures. RESULTS: Procedural success was 100% at the first attempt. Procedural time was 75 +/- 15 minutes. All devices were delivered at the target site with good acute valve function. No valved stents dislodged. No animal had significant regurgitation or paravalvular leaking on intracardiac echocardiographic analysis. All animals had a competent tricuspid valve and no signs of right ventricular dysfunction. The planimetric valve orifice was 2.85 +/- 0.32 cm(2). No damage to the pulmonary artery or structural defect of the valved stents was found at necropsy. CONCLUSIONS: This study confirms the feasibility of direct access valve replacement through the transapical procedure for replacement of the pulmonary valve, as well as validity of the new valved stent design concept. The transapical procedure is targeting a broader patient pool, including the very young and the adult patient. The device design might not be restricted to failing conduits only and could allow for implantation in a larger patient population, including those with native right ventricular outflow tract configurations.

Since the UsedSoft ruling of the CJEU in 2012, there has been the distinct feeling that – like the big bang - UsedSoft signals the start of a new beginning. As we enter this brave new world, the Copyright Directive will be read anew: misalignments in the treatment of physical and digital content will be resolved; accessibility and affordability for consumers will be heightened; and lock-in will be reduced as e-exhaustion takes hold. With UsedSoft as a precedent, the Court can do nothing but keep expanding its own ruling. For big bang theorists, it is only a matter of time until the digital first sale meteor strikes non-software downloads also. This paper looks at whether the UsedSoft ruling could indeed be the beginning of a wider doctrine of e-exhaustion, or if it is simply a one-shot comet restrained by provisions of the Computer Program Directive on which it was based. Fighting the latter corner, we have the strict word of the law; in the UsedSoft ruling, the Court appears to willingly bypass the international legal framework of the WCT. As far as expansion goes, the Copyright Directive was conceived specifically to implement the WCT, thus the legislative intent is clear. The Court would not, surely, invoke its modicum of creativity there also... With perhaps undue haste in a digital market of many unknowns, it seems this might well be the case. Provoking the big bang theory of e-exhaustion, the UsedSoft ruling can be read as distinctly purposive, but rather than having copyright norms in mind, the standard for the Court is the same free movement rules that underpin the exhaustion doctrine in the physical world. With an endowed sense of principled equivalence, the Court clearly wishes the tangible and intangible rules to be aligned. Against the backdrop of the European internal market, perhaps few legislative instruments would staunchly stand in its way. With firm objectives in mind, the UsedSoft ruling could be a rather disruptive meteor indeed.

Epithelial cells of different phenotypes derived from bovine corpus luteum have been studied intensively in our laboratory. In this study, specific lectin binding was examined for cells of type 1 and 3, which were defined as endothelial cells. In order to confirm differences in their glycocalyx at the light microscopic level, five biotinylated lectins were applied to postconfluent cultures which had been fixed with buffered paraformaldehyde or glutaraldehyde. Cells were not permeabilized with any detergent. Lectin binding was localized with a streptavidin-peroxidase complex which was visualized with two different techniques. The DAB technique detected peroxidase histochemically, while the immunogold technique used an anti-peroxidase gold complex together with silver amplification. Neither cell type 1 nor cell type 3 bound a particular lectin selectively, yet each cell type expressed a particular lectin binding pattern. With the DAB technique, diverse lectin binding patterns were seen, probably indicating either "outside" binding, i.e., a diffuse pattern, a lateral-cell-side pattern and a microvillus-like pattern, or "inside" binding, i.e., a diffuse pattern, and a granule-like pattern. With the immunogold technique, only "outside" binding was observed. In addition, the patterns of single cilia or of single circles were detected, the latter roughly representing 3-micron-sized binding sites for concanavalin A. When localizing them at the ultrastructural level, single circles corresponded with micron-sized discontinuities of the plasma membrane. Shedding vesicles were detected whose outer membrane was labelled with concanavalin A. Our results confirm the diversity of the two cell types under study. The "inside" lectin binding may be caused by way of transient plasma membrane openings and related to shedding of right-side out vesicles ("ectocytosis").

Humans are noted for their capacity to over-ride self-interest in favor of normatively valued goals. We examined the neural circuitry that is causally involved in normative, fairness-related decisions by generating a temporarily diminished capacity for costly normative behavior, a 'deviant' case, through non-invasive brain stimulation (repetitive transcranial magnetic stimulation) and compared normal subjects' functional magnetic resonance imaging signals with those of the deviant subjects. When fairness and economic self-interest were in conflict, normal subjects (who make costly normative decisions at a much higher frequency) displayed significantly higher activity in, and connectivity between, the right dorsolateral prefrontal cortex (DLPFC) and the posterior ventromedial prefrontal cortex (pVMPFC). In contrast, when there was no conflict between fairness and economic self-interest, both types of subjects displayed identical neural patterns and behaved identically. These findings suggest that a parsimonious prefrontal network, the activation of right DLPFC and pVMPFC, and the connectivity between them, facilitates subjects' willingness to incur the cost of normative decisions.

Introduction Assist in unison to the patient’s inspiratory neural effort and feedback-controlled limitation of lung distension with neurally adjusted ventilatory assist (NAVA) may reduce the negative effects of mechanical ventilation on right ventricular function. Methods Heart–lung interaction was evaluated in 10 intubated patients with impaired cardiac function using esophageal balloons, pulmonary artery catheters and echocardiography. Adequate NAVA level identified by a titration procedure to breathing pattern (NAVAal), 50% NAVAal, and 200% NAVAal and adequate pressure support (PSVal, defined clinically), 50% PSVal, and 150% PSVal were implemented at constant positive end-expiratory pressure for 20 minutes each. Results NAVAal was 3.1 ± 1.1cmH2O/μV and PSVal was 17 ± 2 cmH20. For all NAVA levels negative esophageal pressure deflections were observed during inspiration whereas this pattern was reversed during PSVal and PSVhigh. As compared to expiration, inspiratory right ventricular outflow tract velocity time integral (surrogating stroke volume) was 103 ± 4%, 109 ± 5%, and 100 ± 4% for NAVAlow, NAVAal, and NAVAhigh and 101 ± 3%, 89 ± 6%, and 83 ± 9% for PSVlow, PSVal, and PSVhigh, respectively (p < 0.001 level-mode interaction, ANOVA). Right ventricular systolic isovolumetric pressure increased from 11.0 ± 4.6 mmHg at PSVlow to 14.0 ± 4.6 mmHg at PSVhigh but remained unchanged (11.5 ± 4.7 mmHg (NAVAlow) and 10.8 ± 4.2 mmHg (NAVAhigh), level-mode interaction p = 0.005). Both indicate progressive right ventricular outflow impedance with increasing pressure support ventilation (PSV), but no change with increasing NAVA level. Conclusions Right ventricular performance is less impaired during NAVA compared to PSV as used in this study. Proposed mechanisms are preservation of cyclic intrathoracic pressure changes characteristic of spontaneous breathing and limitation of right-ventricular outflow impedance during inspiration, regardless of the NAVA level.

Background Chronic mountain sickness (CMS) is often associated with vascular dysfunction, but the underlying mechanism is unknown. Sleep disordered breathing (SDB) frequently occurs at high altitude. At low altitude SDB causes vascular dysfunction. Moreover, in SDB, transient elevations of right-sided cardiac pressure may cause right-to-left shunting in the presence of a patent foramen ovale (PFO) and, in turn, further aggravate hypoxemia and pulmonary hypertension. We speculated that compared to healthy high-altitude dwellers, in patients with CMS, SDB and nocturnal hypoxemia are more pronounced and related to vascular dysfunction. Methods We performed overnight sleep recordings, and measured systemic and pulmonary-artery pressure in 23 patients with CMS (mean±SD age 52.8±9.8 y) and 12 healthy controls (47.8±7.8 y) at 3600 m. In a subgroup of 15 subjects with SDB, we searched for PFO with transesophagal echocardiography. Results The major new findings were that in CMS patients, a) SDB and nocturnal hypoxemia was more severe (P<0.01) than in controls (apnea/hypopnea index, AHI, 38.9±25.5 vs. 14.3±7.8[nb/h]; SaO2, 80.2±3.6 vs. 86.8±1.7[%], CMS vs. controls), and b) AHI was directly correlated with systemic blood pressure (r=0.5216, P=0.001) and pulmonary-artery pressure (r=0.4497, P=0.024). PFO was associated with more severe SDB (AHI 48.8±24.7 vs. 14.8±7.3[nb/h], P=0.013, PFO vs. no PFO) and hypoxemia. Conclusion SDB and nocturnal hypoxemia are more severe in CMS patients than in controls and are associated with systemic and pulmonary vascular dysfunction. The presence of a PFO appeared to further aggravate SDB. Closure of PFO may improve SDB, hypoxemia and vascular dysfunction in CMS patients. Clinical Trials Gov Registration NCT01182792.

A 10-yr-old female Asian small-clawed otter (Aonyx cinerea) presented with a history of right forelimb lameness. Antebrachial radiographs revealed a Monteggia lesion, classified by cranial radial head luxation and distal diaphyseal ulnar fracture. Open reduction with placement of an ulnar-radial positional screw was performed. The lateral collateral ligament was reconstructed using suture anchored by a condylar screw and bone tunnel in the radius. Reduction and proper implant placement was confirmed on postoperative radiographs. The ulnar-radial positional screw was removed 6 wk postoperatively to allow proper supination and pronation. Limb function was greatly improved at this time; however, a mild lameness was still observed. At 7 mo postoperatively, the otter was ambulating lameness-free. Radiographs documented proper joint reduction and stable condylar screw. At 32 mo postoperatively, the otter continued to exhibit normal ambulation.

This appraisal of David Scott FitzGerald and David Cook-Martín's Culling the Masses: The Democratic Origins of Racist Immigration Policy in the Americas argues that there is no ‘elective affinity’ between liberalism and racism, which is the core argument of the book. The notion of ‘elective affinity’, which the authors borrow from Max Weber, requires a structural homology between the ‘electively’ related elements that just does not exist in this case. The relationship between both is entirely contingent, ‘racism’ being a doctrine of inter-group relations while ‘liberalism’ is a doctrine of intra-group relations, with no consideration of how the boundaries of the group are constituted.

Las respuestas que se generan a partir de cuestionamientos del origen del hombre nos permitirán especular hacia dónde evolucionará como especie. Los grandes saltos evolutivos que diferencian a los primates humanos de los no humanos, se podrían describir entre otras características por una eficiente memoria para el uso de herramientas, la dominancia para el uso de la mano junto al desarrollo de la oposición del pulgar y el lenguaje. Se ha descrito un gen, el HSR que expresa para la dominancia para el uso de la mano derecha, habilidades cognitivas relacionadas con el lenguaje y asimetría cerebral en humanos. Este es un gen imprintado, es decir, que se hereda su expresión según el origen parental y cuya expresión está regulada por factores epigenéticos. Estos factores, modifican la expresión del gen sin afectar la estructura primaria del ADN. Se ha estudiado la expresión fenotípica del gen HSR en una población de niños escolarizados de La Rioja, dividida en dos regiones (Región 1 y Región 2). Los resultados obtenidos, que muestran una alteración de las proporciones fenotípicas del gen en la Región 2, apoyan fuertemente la posibilidad de que un factor ambiental estaría condicionando el epigenotipo del gen HSR. Se piensa que el estudio de estos mecanismos regulatorios en estos genes recientemente adquiridos por la evolución y blanco de funciones también recientemente adquiridas, podría dar información de hacia dónde la evolución del hombre podría proyectarse en el futuro.

El presente trabajo se ocupa de la importancia que tiene el concepto de imago Dei presente en Santo Tomás de Aquino en función de una adecuada comprensión de la doctrina de los trascendentales. Esto implicará considerar el concepto de imagen de Dios en el hombre para luego entender cómo es que a la luz de este concepto Tomás lee al Estagirita, cuyos textos cita como argumento de autoridad en su consideración de los trascendentales, de una manera que renueva y expande la significación de los textos aristotélicos. Esto también permitirá percibir con más precisión el significado de las expresiones utilizadas por Tomás para designar a los trascendentales, en especial cuando se refiere a la verdad y al bien.

La concepción de la notitia intuitiva, tal como la entienden los filósofos del siglo XIV, constituye un modo de acceso a la realidad de cuño agustiniano que subsiste a pesar del progresivo avance de la teoría aristotélica de la abstracción. Tomando como punto de partida la notitia intuitiva, Ockham propone una construcción del conocimiento y de la ciencia que procura una interpretación más delimitada y precisa del pensamiento de Duns Escoto. Esta doctrina ockhamista va a ser puesta a prueba por un desarrollo paradojal vinculado al conocimiento de lo no-existente. La importancia del ejemplo ha sido clave a la hora de la valoración de su gnoseología. En particular, el tema es expuesto, con algunos matices de diferenciación, en tres obras: en la quaestio primera del Prólogo del Commentarium in Sententiis; en las quaestiones 12-13 de la Reportatio II y en las Quaestiones Quodlibetales V y VI, a las que también se suman precisiones puntualizadas en el marco de sus composiciones físicas, en particular, en las Quaestiones Physicorum. Constituye nuestro propósito el análisis de estas fuentes en orden a discernir la importancia que el controvertido paso asume en el marco de la gnoseología ockhamista. El ejemplo ha sido interpretado de modo contrastante: para algunos introduce claramente una concepción escéptica de la doctrina del conocimiento, en el sentido que, una vez admitida la posibilidad de una intuición de cosas que no existen, no tenemos ningún criterio para establecer cuándo nuestro conocimiento es objetivo o subjetivo. Para otros, en cambio, debe ser entendido como una hipótesis que no interfiere sobre el plano natural del conocimiento. El recurso a las fuentes nos permitirá comparar los planos de explicitación del ejemplo aludido y determinar su contexto y alcance en orden a un examen del problema en el doble ámbito de lo natural y de lo sobrenatural, aportando nuestras propias conclusiones al caso.

El artículo analiza los argumentos en torno de los cuales se plantea el conflicto entre causalidad natural y causalidad por libertad en la obra Crítica de la Razón Pura /Kritik der reinen Vernunft/ del filósofo alemán Immanuel Kant. Específicamente, interesa desarrollar la solución kantiana a uno de los conflictos que los resultados de la crítica han generado, a partir de la Dialéctica Trascendental y su presentación del problema como antinomia de la razón y de la Doctrina Trascendental del Método, donde la noción de libertad es anunciada como núcleo del pensamiento moral. Se trata de la dificultad que enfrenta la razón al compaginar la necesidad en la naturaleza (sin la cual no es posible la ciencia) con la libertad humana (sin la cual no es posible la moral). La libertad inquirida por el examen crítico de la razón en su uso teórico, es restaurada desde la perspectiva de la razón práctica. Gracias a la restricción crítica del conocimiento humano, Kant abre el camino para la libertad humana, no tanto en los términos negativos de encontrarse “libre de coacción" sino en el sentido positivo de iniciar algo nuevo o no previsto en decurso de la historia fenoménica.

La doctrina de los sentidos internos -y, en particular, la de la cogitativa-, esbozada en la antigüedad, desarrollada en el Medioevo, y redescubierta parcialmente por la psicología contemporánea, constituye una elaboración conceptual originalísima con importantes consecuencias prácticas para la psicología. El presente estudio procura una primera aproximación en este sentido a la noción de cogitativa, examinando sus posibles proyecciones en los campos de la psicoterapia y de la comprensión etiológica de las enfermedades psíquicas, e indagando en las semejanzas que dicho concepto podría guardar con desarrollos psicológicos contemporáneos.
