Control Centre operators are essential to assure a good performance of Power Systems. Operators’ actions are critical in dealing with incidents, especially severe faults, like blackouts. In this paper we present an Intelligent Tutoring approach for training Portuguese Control Centre operators in incident analysis and diagnosis, and service restoration of Power Systems, offering context awareness and an easy integration in the working environment.

RESUMO: Objetivos – Determinar a sensibilidade e especificidade das ponderações Difusão (DWI) e T2 Fluid-Attenuated Inversion Recovery (FLAIR) na avaliação de lesões da substância branca (SB) e verificar em que medida se complementam, por forma a criar um conjunto de boas práticas na RM cranioencefálica de rotina. Metodologia – Recorrendo-se a uma metodologia quantitativa, efetuou-se uma análise retrospetiva da qual foram selecionados 30 pacientes, 10 sem patologia e 20 com patologia (2 com EM, 7 com Leucoencefalopatia, 6 com doença microangiopática e 5 com patologia da substância branca indefinida). Obteve-se uma amostra de 60 imagens, nomeadamente: 30 imagens ponderadas em DWI e 30 em T2 FLAIR. Recorrendo ao programa Viewdex®, três observadores avaliaram um conjunto de imagens segundo sete critérios: visibilidade, deteção, homogeneidade, localização, margens e dimensões da lesão e capacidade de diagnóstico. Com os resultados obtidos recorreu-se ao cálculo de sensibilidade e especificidade pelas Curvas ROC, bem como à análise estatística, nomeadamente, Teste-T, Índice de Concordância Kappa e coeficiente de correlação de Pearson entre as variáveis em estudo. Resultados – Os resultados de sensibilidade e de especificidade obtidos para a ponderação T2 FLAIR foram superiores (0,915 e 0,038, respetivamente) aos da ponderação DWI (0,08 e 0,100, respetivamente). Não se verificaram variâncias populacionais significativas. Obteve-se uma elevada correlação linear entre as variáveis com um valor r situado entre 0,8 e 0,99. Verificou-se também uma variabilidade considerável entre os observadores. Conclusões – Dados os baixos valores de sensibilidade e especificidade obtidos para a DWI, sugere-se que esta deva ser incluída no protocolo de rotina de crânio como auxiliar no diagnóstico diferencial com outras patologias.

Amorphous SiC tandem heterostructures are used to filter a specific band, in the visible range. Experimental and simulated results are compared to validate the use of SiC multilayered structures in applications where gain compensation is needed or to attenuate unwanted wavelengths. Spectral response data acquired under different frequencies, optical wavelength control and side irradiations are analyzed. Transfer function characteristics are discussed. Color pulsed communication channels are transmitted together and the output signal analyzed under different background conditions. Results show that under controlled wavelength backgrounds, the device sensitivity is enhanced in a precise wavelength range and quenched in the others, tuning or suppressing a specific band. Depending on the background wavelength and irradiation side, the device acts either as a long-, a short-, or a band-rejection pass filter. An optoelectronic model supports the experimental results and gives insight on the physics of the device.

Introdução: As teorias etiológicas recíprocas que relacionam as disfunções da articulação temporo mandibular (ATM) com as disfunções da coluna cervical, os seus critérios de diagnóstico e formas de tratamento, não reúnem consenso, constituindo, actualmente, uma temática de debate e investigação. Objectivo: Descrever a avaliação e intervenção em fisioterapia, e os seus resultados numa utente com disfunção do complexo crânio-cervico-mandibular. Métodos: Foi realizado um estudo observacional descritivo, do tipo estudo de caso, reportando-se a uma enfermeira, de 28 anos, com quadro sintomatológico compatível com disfunção crânio-cervico-mandibular, com antecedentes de condilectomia e artroplastia da ATM, aos 14 anos. Inicialmente a utente apresentava dor irradiada na região cervical (7/10) e limitação dos movimentos articulares da coluna cervical. Referia, também, dor na região do ptérion direito (6/10), limitação dos movimentos da mandíbula, desvio lateral na abertura da boca e dificuldades funcionais na mastigação. A avaliação inicial e final (após tratamento), foi efectuada recorrendo-se à utilização do Goniometro CROM e da Therabite Range of Motion Scale, para medição das amplitudes articulares da cervical e ATM, respectivamente; Estetoscópio, para avaliação dos sons articulares da ATM; Escala Visual Analógica para graduação da dor. A intervenção decorreu ao longo de 10 sessões, bissemanais. No tratamento, foram aplicadas técnicas sobre os trigers points da musculatura cervical e músculos da mastigação; Mobilização passiva da cervical e ATM; Manipulação dos segmentos vertebrais cervicais e torácicos; Streching e técnicas de energia muscular; Técnicas funcionais para a ATM; Técnicas miofasciais para a coluna cervical e ATM; Exercícios de controlo motor da coluna cervical. Resultados: No final do tratamento, as amplitudes dos movimentos cervicais estavam completas e sem dor (0/10), mantendo, sensibilidade dolorosa à palpação das espinhosas de C5-C6 (1/10). Relativamente à ATM, verifica-se a abolição da dor (0/10) e a ausência do desvio lateral da mandíbula na abertura da boca, bem como, o aumento das amplitudes de movimento na abertura da boca (33 para 36 mm), e no desvio lateral esquerdo (2 para 2,8 mm). Conclusão: os resultados sugerem que a intervenção, com recurso a técnicas de terapia manual, no caso em estudo, parecem surtir efeitos positivos no quadro sintomatológico e funcional da utente.

Throughout the world, epidemiological studies were established to examine the relationship between air pollution and mortality rates and adverse respiratory health effects. However, despite the years of discussion the correlation between adverse health effects and atmospheric pollution remains controversial, partly because these studies are frequently restricted to small and well-monitored areas. Monitoring air pollution is complex due to the large spatial and temporal variations of pollution phenomena, the high costs of recording instruments, and the low sampling density of a purely instrumental approach. Therefore, together with the traditional instrumental monitoring, bioindication techniques allow for the mapping of pollution effects over wide areas with a high sampling density. In this study, instrumental and biomonitoring techniques were integrated to support an epidemiological study that will be developed in an industrial area located in Gijon in the coastal of central Asturias, Spain. Three main objectives were proposed to (i) analyze temporal patterns of PM10 concentrations in order to apportion emissions sources, (ii) investigate spatial patterns of lichen conductivity to identify the impact of the studied industrial area in air quality, and (iii) establish relationships amongst lichen conductivity with some site-specific characteristics. Samples of the epiphytic lichen Parmelia sulcata were transplanted in a grid of 18 by 20 km with an industrial area in the center. Lichens were exposed for a 5-mo period starting in April 2010. After exposure, lichen samples were soaked in 18-MΩ water aimed at determination of water electrical conductivity and, consequently, lichen vitality and cell damage. A marked decreasing gradient of lichens conductivity relative to distance from the emitting sources was observed. Transplants from a sampling site proximal to the industrial area reached values 10-fold higher than levels far from it. This finding showed that lichens reacted physiologically in the polluted industrial area as evidenced by increased conductivity correlated to contamination level. The integration of temporal PM10 measurements and analysis of wind direction corroborated the importance of this industrialized region for air quality measurements and identified the relevance of traffic for the urban area.

PURPOSE: Fatty liver disease (FLD) is an increasing prevalent disease that can be reversed if detected early. Ultrasound is the safest and ubiquitous method for identifying FLD. Since expert sonographers are required to accurately interpret the liver ultrasound images, lack of the same will result in interobserver variability. For more objective interpretation, high accuracy, and quick second opinions, computer aided diagnostic (CAD) techniques may be exploited. The purpose of this work is to develop one such CAD technique for accurate classification of normal livers and abnormal livers affected by FLD. METHODS: In this paper, the authors present a CAD technique (called Symtosis) that uses a novel combination of significant features based on the texture, wavelet transform, and higher order spectra of the liver ultrasound images in various supervised learning-based classifiers in order to determine parameters that classify normal and FLD-affected abnormal livers. RESULTS: On evaluating the proposed technique on a database of 58 abnormal and 42 normal liver ultrasound images, the authors were able to achieve a high classification accuracy of 93.3% using the decision tree classifier. CONCLUSIONS: This high accuracy added to the completely automated classification procedure makes the authors' proposed technique highly suitable for clinical deployment and usage.

This work aims at investigating the impact of treating breast cancer using different radiation therapy (RT) techniques – forwardly-planned intensity-modulated, f-IMRT, inversely-planned IMRT and dynamic conformal arc (DCART) RT – and their effects on the whole-breast irradiation and in the undesirable irradiation of the surrounding healthy tissues. Two algorithms of iPlan BrainLAB treatment planning system were compared: Pencil Beam Convolution (PBC) and commercial Monte Carlo (iMC). Seven left-sided breast patients submitted to breast-conserving surgery were enrolled in the study. For each patient, four RT techniques – f-IMRT, IMRT using 2-fields and 5-fields (IMRT2 and IMRT5, respectively) and DCART – were applied. The dose distributions in the planned target volume (PTV) and the dose to the organs at risk (OAR) were compared analyzing dose–volume histograms; further statistical analysis was performed using IBM SPSS v20 software. For PBC, all techniques provided adequate coverage of the PTV. However, statistically significant dose differences were observed between the techniques, in the PTV, OAR and also in the pattern of dose distribution spreading into normal tissues. IMRT5 and DCART spread low doses into greater volumes of normal tissue, right breast, right lung and heart than tangential techniques. However, IMRT5 plans improved distributions for the PTV, exhibiting better conformity and homogeneity in target and reduced high dose percentages in ipsilateral OAR. DCART did not present advantages over any of the techniques investigated. Differences were also found comparing the calculation algorithms: PBC estimated higher doses for the PTV, ipsilateral lung and heart than the iMC algorithm predicted.

Aims: This paper aims to address some of the main possible applications of actual Nuclear Medicine Imaging techniques and methodologies in the specific context of Sports Medicine, namely in two critical systems: musculoskeletal and cardiovascular. Discussion: At the musculoskeletal level, bone scintigraphy techniques proved to be a mean of diagnosis of functional orientation and high sensibility compared with other morphological imaging techniques in the detection and temporal evaluation of pathological situations, for instance allowing the acquisition of information of great relevance in athletes with stress fractures. On the other hand, infection/inflammation studies might be of an important added value to characterize specific situations, early diagnose of potential critical issues – so giving opportunity to precise, complete and fast solutions – while allowing the evaluation and eventual optimization of training programs. At cardiovascular system level, Nuclear Medicine had proved to be crucial in differential diagnosis between cardiac hypertrophy secondary to physical activity (the so called "athlete's heart") and hypertrophic cardiomyopathy, in the diagnosis and prognosis of changes in cardiac function in athletes, as well as in direct - and non-invasive - in vivo visualization of sympathetic cardiac innervation, something that seems to take more and more importance nowadays, namely in order to try to avoid sudden death episodes at intense physical effort. Also the clinical application of Positron Emission Tomography (PET) has becoming more and more widely recognized as promising. Conclusions: It has been concluded that Nuclear Medicine can become an important application in Sports Medicine. Its well established capabilities to early detection of processes involving functional properties allied to its high sensibility and the actual technical possibilities (namely those related with hybrid imaging, that allows to add information provided by high resolution morphological imaging techniques, such as CT and/or MRI) make it a powerful diagnostic tool, claiming to be used on an each day higher range of clinical applications related with all levels of sport activities. Since the improvements at equipment characteristics and detection levels allows the use of smaller and smaller doses, so minimizing radiation exposure it is believed by the authors that the increase of the use of NM tools in the Sports Medicine area should be considered.

The basic function of the human hand is the manipulation and grasping of various objects in all daily activities, including work activities. This is greatly influenced by strength and manual dexterity. However age, gender and other contexts such as work or leisure activities could influence strength. Handgrip strength, a measure of maximum voluntary force of the hand, has proved to be reliable and valid as an objective parameter to evaluate the functional integrity of the hand as part of the musculoskeletal system. It correlates highly with strength in other muscular groups and is therefore considered as a good indicator of overall muscular strength and functional stress and could be used as a predictor of physical disability. Handgrip strength assessment is simple and reliable and used commonly by several investigators and health professionals, in different contexts (medical, nutritional, rehabilitation, professional settings, engineering, etc.) and with different purposes (research, diagnostic, assessment, etc.). In clinical and rehabilitation settings is of vital importance in the determination of effectiveness of several interventions and for monitoring evolution of diseases. Various ways (methods, techniques and equipments) of collecting information on grip strength have been reported. This chapter will review basic concepts on handgrip function, methodologies of assessment, contexts of application and correlates, such as physical activity, health or nutritional status. Several populations and reference values as also the relationships between handgrip and clinical status, aging, risk of disability and diseases, will be discussed.

A investigação em curso visa compreender a influência da implementação de um conjunto de tarefas no desenvolvimento do conceito de ângulo em alunos do 5.º ano de escolaridade, procurando responder às seguintes questões: a) Que conceções revelam alunos do 5.º ano de escolaridade relativamente ao conceito de ângulo?; b) Que estratégias utilizam os alunos do 5.º ano de escolaridade na exploração das tarefas utilizadas?; e c) Que aspetos do conceito de ângulo são desenvolvidos pelos alunos através da realização das tarefas propostas? A presente comunicação incide na primeira questão do estudo. Optou-se por uma abordagem metodológica qualitativa de paradigma interpretativo com a modalidade de experiência de ensino. Selecionou-se quatro alunos para constituir o grupo-alvo. Procedeu-se à avaliação diagnóstica das conceções de ângulo dos quatro alunos através da realização, no 1.º Período do ano letivo de 2011/12, de entrevistas clínicas semiestruturadas individuais, gravadas em vídeo. Além das entrevistas, foram usadas, como técnicas de recolha de dados, a observação participante das aulas, videogravadas, bem como a análise de documentos. Para analisar os dados, foram elaboradas categorias analíticas. Os resultados relativos à avaliação diagnóstica evidenciam conceções erradas de ângulo e respetiva amplitude: (a) os ângulos são os lados de polígonos (1 aluna); (b) o comprimento dos segmentos representativos dos lados está relacionado com o tamanho dos ângulos (2 alunos); (c) o comprimento do arco marcando o ângulo está relacionado com o tamanho dos ângulos (3 alunos); (d) nos polígonos côncavos, sãoângulos apenas os de amplitude inferior a 1800 (3 alunos); (e) em figuras que não são polígonos, os ângulos são os espaços entre os segmentos de reta e as linhas curvas (1 aluno); (f) o ângulo é a área entre dois segmentos representativos dos lados (4 alunos); e (g) os ângulos retos são apenas os posicionados na posição usual horizontal/vertical (1 aluna).

Magnetic resonance (MR) imaging has been used to analyse and evaluate the vocal tract shape through different techniques and with promising results in several fields. Our purpose is to demonstrate the relevance of MR and image processing for the vocal tract study. The extraction of contours of the air cavities allowed the set - up of a number of 3D reconstruction image stacks by means of the combination of orthogonally oriented sets of slices for e ach articulatory gesture, as a new approach to solve the expected spatial under sampling of the imaging process. In result these models give improved information for the visualization of morphologic and anatomical aspects and are useful for partial measure ments of the vocal tract shape in different situations. Potential use can be found in Medical and therapeutic applications as well as in acoustic articulatory speech modelling.
