Health-related quality of life assessment in people with multiple sclerosis and their family caregivers. A multicenter study in Catalonia (Southern Europe)

Objectives: To measure the health-related quality of life (HRQoL) of multiple sclerosis (MS) patients and their caregivers, and to assess which factors can best describe HRQoL. Methods: A cross-sectional multicenter study of nine hospitals enrolled MS patients and their caregivers who attended outpatient clinics consecutively. The instruments used were the SF-36 for patients and the SF-12 and GHQ-12 for caregivers. Classification and regression tree analysis was used to analyze the explanatory factors of HRQoL. Results: A total of 705 patients (mean age 40.4 years, median Expanded Disability Status Scale 2.5, 77.8% with relapsing-remitting MS) and 551 caregivers (mean age 45.4 years) participated in the study. MS patients had significantly lower HRQoL than in the general population (physical SF-36: 39.9; 95% confidence interval [CI]: 39.1–40.6; mental SF-36: 44.4; 95% CI: 43.5–45.3). Caregivers also presented lower HRQoL than general population, especially in its mental domain (mental SF-12: 46.4; 95% CI: 45.5–47.3). Moreover, according to GHQ-12, 27% of caregivers presented probable psychological distress. Disability and co-morbidity in patients, and co-morbidity and employment status in caregivers, were the most important explanatory factors of their HRQoL. Conclusions: Not only the HRQoL of patients with MS, but also that of their caregivers, is indeed notably affected. Caregivers’ HRQoL is close to population of chronic illness even that the patients sample has a mild clinical severity and that caregiving role is a usual task in the study context

Transforming personal experience and emotions through education to cultural diversity: an interplay between unicity and genericity

When subjects studied at school are close to societal discourses and to the students' social identities, when they have high emotional resonance, is it possible to enable the students to distance themselves from their emotions and personal experience, and to conceptualise them? Examining the relation between emotion and learning through the lens of socio-cultural psychology, the aim of our study was to shed light on "secondarisation" processes, that is, processes that transform personal experience and emotions into conceptualised forms of thinking. We analysed 85 video-recorded lessons in education for cultural diversity involving 12 teachers (of primary and secondary schools). Having identified episodes in which emotions were put into words or personal experience was reported, we analysed the use of pronouns (taken as indicators of secondarisation processes) and found a recurrent pattern: "the unicity-genericity routine". We illustrate the functioning of this routine with various excerpts taken from lessons in education for diversity taught in the classes of two teachers in primary school. The results show that the interplay between unicity and genericity works as a discursive resource for the development of secondarisation processes.

Etnografia: esteatégia metodológica utilizada para contextualizar o cenário cultural do CTI de um hospital universitário

O presente trabalho é parte de um estudo etnográfico realizado em um CTI de um hospital universitário que teve o objetivo de descrever a cultura dos intensivistas. São apresentados domínios e taxonomias culturais elaborados a partir dos relatos dos intensivistas. Os resultados aqui apresentados demostraram que a etnografia é uma estratégia metodológica adequada para contextualização do cenário cultural sob o ponto de vista de quem vivencia o cotidiano do trabalho na terapia intensiva.

O processo de cuidar da pessoa que sofreu queimaduras: significado cultural atribuído por familiares

Os objetivos deste estudo etnográfico foram identificar o significado cultural atribuído por familiares à participação no processo de cuidar da pessoa ultima de queimadura; identificar as estratégias de cuidado que os familiares focalizam frente à participação no cuidado. Os dados foram coletados mediante entrevistas semi-estruturadas com 25 familiares de clientes que sofreram queimaduras, admitidos em uma Unidade de Queimados e observações participantes. Os resultados mostram que o significado cultural atribuído por familiares de pessoas vítimas queimaduras ao processo de cuidar tem duas dimensões: a física e a moral e gera principalmente medo.

A sexualidade como uma construção cultural: reflexões sobre preconceitos e mitos inerentes a um grupo de mulheres rurais

Este artigo apresenta a sexualidade como uma construção cultural. Para fundamentar tal assertiva, é relatada uma pesquisa realizada com um grupo de onze mulheres de uma comunidade rural, no interior do Rio Grande do Sul. Os dados foram coletados por meio de entrevistas semi-estruturadas e as narrativas foram analisadas qualitativamente, buscando, via ótica cultural, o significado dos depoimentos. A sexualidade é, aqui, contemplada a partir das vivências individuais, dos valores, das crenças, dos mitos e dos preconceitos, construídos ao longo da socialização de cada colaboradora. No final do texto, foram acrescentadas algumas reflexões que salientam a importância de tal interpretação cultural sobre os eventos, especialmente sobre a sexualidade para as enfermeiras.

Using a Parameterization of a Radiative Transfer Model to Build High-Resolution Maps of Typical Clear-Sky UV Index in Catalonia, Spain

To perform a climatic analysis of the annual UV index (UVI) variations in Catalonia, Spain (northeast of the Iberian Peninsula), a new simple parameterization scheme is presented based on a multilayer radiative transfer model. The parameterization performs fast UVI calculations for a wide range of cloudless and snow-free situations and can be applied anywhere. The following parameters are considered: solar zenith angle, total ozone column, altitude, aerosol optical depth, and single-scattering albedo. A sensitivity analysis is presented to justify this choice with special attention to aerosol information. Comparisons with the base model show good agreement, most of all for the most common cases, giving an absolute error within 0.2 in the UVI for a wide range of cases considered. Two tests are done to show the performance of the parameterization against UVI measurements. One uses data from a high-quality spectroradiometer from Lauder, New Zealand [45.04°S, 169.684°E, 370 m above mean sea level (MSL)], where there is a low presence of aerosols. The other uses data from a Robertson–Berger-type meter from Girona, Spain (41.97°N, 2.82°E, 100 m MSL), where there is more aerosol load and where it has been possible to study the effect of aerosol information on the model versus measurement comparison. The parameterization is applied to a climatic analysis of the annual UVI variation in Catalonia, showing the contributions of solar zenith angle, ozone, and aerosols. Adaptação cultural e validação do instrumento demandas de atenção dirigida

Este estudo tem como objetivos traduzir e validar o instrumento Requirement for Directed Attention (RDA), desenvolvido por Guirardello, para a cultura brasileira. É um instrumento composto de 39 itens que aborda situações no ambiente de trabalho de unidades de cuidados críticos e médico-cirúrgica que demandam atenção dirigida do enfermeiro. O procedimento metodológico constituiu nas etapas de: a) tradução do instrumento para a língua portuguesa; b) back-translation; c) avaliação por um grupo de juízes e d) pré-teste da versão final do instrumento. O instrumento na versão em Português foi aplicado a um grupo de 44 enfermeiros. Os dados resultaram em alta consistência interna para o total dos itens (a = 0,91) e para os domínios: Ambiente Físico (a = 0,78), Psicológico (a = 0,85) e Comportamental (a = 0,80). Apenas o domínio Informação obteve um a de 0,12. Esses resultados sugerem que a versão em Português do RDA pode ser usada para avaliar as demandas de atenção do enfermeiro.
