The year 2011 was full of significant advances in all areas of medicine. Whether small or large issues, they all have an impact on daily practice in general internal medicine. For example, intravenous administration of diuretics in heart failure shows no benefit. But double dose may improve symptoms faster. Direct Xa inhibitors are emerging as alternative to anti-vitamin K. beta-blockers reduce overall mortality in COPD and do not worsen lung function significantly. Each year, the chief residents from the Department of internal medicine at the University hospital of Lausanne meet to share their readings. Twelve new therapeutic considerations of 2011 are reviewed here.

Research on achievement goals usually defines mastery goals as the desire to acquire knowledge, and performance goals as the desire to outperform (or not to underperform) others. Educational contexts are most of the time social contexts, involving various persons and groups, of various hierarchical positions, and various cultural and ideological contexts. Surprisingly, most research in the achievement goal field has been conducted at an individual level of analysis. In the present paper, we will review the social consequences and antecedents of goal endorsement. This research indicates that goals strongly affect the way one behaves with co-learners. Moreover, it suggests that more than merely individual dispositions, goals reflect the social relation students have with other persons, institutions, and with the society to which they belong. We conclude this paper by setting an agenda for future achievement goal research.

This study presents new evidence concerning the uneven processes of industrialization innineteenth century Spain and Italy based on a disaggregate analysis of the productivesectors from which the behaviour of the aggregate indices is comprised. The use of multivariate time-series analysis techniques can aid our understanding and characterization of these two processes of industrialization. The identification of those sectors with key rolesin leading industrial growth provides new evidence concerning the factors that governed thebehaviour of the aggregates in the two economies. In addition, the analysis of the existenceof interindustry linkages reveals the scale of the industrialization process, and wheresignificant differences exist, accounts for many of the divergences recorded in the historiography for the period 1850-1913.

BACKGROUND AND AIMS: The structured IBD Ahead 'Optimised Monitoring' programme was designed to obtain the opinion, insight and advice of gastroenterologists on optimising the monitoring of Crohn's disease activity in four settings: (1) assessment at diagnosis, (2) monitoring in symptomatic patients, (3) monitoring in asymptomatic patients, and (4) the postoperative follow-up. For each of these settings, four monitoring methods were discussed: (a) symptom assessment, (b) endoscopy, (c) laboratory markers, and (d) imaging. Based on literature search and expert opinion compiled during an international consensus meeting, recommendations were given to answer the question 'which diagnostic method, when, and how often'. The International IBD Ahead Expert Panel advised to tailor this guidance to the healthcare system and the special prerequisites of each country. The IBD Ahead Swiss National Steering Committee proposes best-practice recommendations adapted for Switzerland. METHODS: The IBD Ahead Steering Committee identified key questions and provided the Swiss Expert Panel with a structured literature research. The expert panel agreed on a set of statements. During an international expert meeting the consolidated outcome of the national meetings was merged into final statements agreed by the participating International and National Steering Committee members - the IBD Ahead 'Optimized Monitoring' Consensus. RESULTS: A systematic assessment of symptoms, endoscopy findings, and laboratory markers with special emphasis on faecal calprotectin is deemed necessary even in symptom-free patients. The choice of recommended imaging methods is adapted to the specific situation in Switzerland and highlights the importance of ultrasonography and magnetic resonance imaging besides endoscopy. CONCLUSION: The recommendations stress the importance of monitoring disease activity on a regular basis and by objective parameters, such as faecal calprotectin and endoscopy with detailed documentation of findings. Physicians should not rely on symptoms only and adapt the monitoring schedule and choice of options to individual situations. © 2014 S. Karger AG, Basel.

Fiscal Year 2010 proved to be a year of many challenges. While the nation and the state dealt with an unprecedented economic downturn, a growing number of Iowa families sought assistance to pay higher education costs. The year saw Iowa’s unemployment rate soar to a 23-year high, contributing to a 22 percent increase in enrollment at Iowa’s colleges and universities. An increasing number of Iowans applied for financial aid to pay for college as evidenced by a 47 percent increase in the number of Free Applications for Federal Student Aid (FAFSA) completed over the past 5 years. The economic downturn also forced the State to make a 10 percent reduction in all general fund appropriations which reduced the total amount of state-funded financial aid available to assist families.

The Iowa community colleges transmit the Fall 2000 enrollments to the Department of Education after the 14th day of the fall term start date. All data in this report, except where noted, are taken from the Management Information System (MIS) electronic data files and are confirmed by the community college transmittal sheet. The Fall 2000 unduplicated credit headcount enrollment of 65,473 demonstrates an increase of 3 percent over Fall 1999 enrollment of 63,809.

Fall enrollment grew 1.2 percent to a record high of 88,104 unduplicated students in fiscal year 2009 (Table 1). College enrollment has grown for 11 consecutive years since enrollment slipped in fiscal year 1997. In the 44‐year history of the modern community college system, enrollment has only fallen four times (Figure 1). Enrollment growth was slightly slower than prior years. In 2006, enrollment grew over three percent, while enrollment grew 2.5 percent last year. Nevertheless, enrollment growth is outpacing the projected nationwide growth in community colleges (Hussar and Bailey, 2008: Table 16). For the second consecutive year, part‐time enrollment exceeded full‐time enrollment. Slightly over half, 50.9 percent, of students are enrolled in less than 12 credit hours. Last year, part‐time enrollment exceeded full‐time enrollment for the first time. The shift represents growing enrollment by working students and joint enrollment— high school students who enroll in community colleges.. Nationally, part‐time enrollment at public two‐year colleges has exceeded full‐time enrollment for more than 15 years. In 2006, part‐time students exceeded full‐time students by 60 percent (Hussar and Bailey, 2008: Table 16). Iowa’s enrollment growth has traditionally been consistent. Enrollment decreased four times—1976, 1983, 1984, and 1997—during the entire 44‐year history. Fulltime enrollment decreased nine times over the same period while part time enrollment only fell twice. The remainder of this report will break down credit enrollment by credit hours, student demographics, the programs in which students are enrolled, how the programs break down demographically, and joint enrollment.
