INTRODUCTION: Brazil has been called a nation in nutrition transitional because of recent increases in the prevalence of obesity and related chronic diseases. With overweight conditions already prevalent among middle-income populations, there exists a need to identify factors that influence nutrition behavior within this group. OBJECTIVE: To address this subject, a research study was implemented among middle-class adolescents attending a large private secondary school in Manaus, Amazonas, Brazil. The study determined the availability and accessibility of snack foods as well as subjects attitudes and preferences towards, and the influence of family and friends on healthy (high-nutrient density) snack choices. METHODS: The 4-stage process included: (a) a nutrition expert focus group discussion that reported local nutrition problems in general and factors related to adolescent nutrition, (b) an adolescent pilot survey (n=63) that solicited information about snacking preferences and habits as well as resources for nutrition information and snack money; (c) a survey of various area food market sources to determine the availability and accessibility of high nutrient density snacks; and (d) a follow-up adolescent survey (n=55) that measured snack food preferences and perceptions about their cost and availability. RESULTS: Results included the finding that, although affordable high nutrient density snacks were available, preferences for low nutrient density snacks prevailed. The adolescents were reportedly more likely to be influenced by and obtain nutrition information from family members than friends. CONCLUSION: From study results it is apparent that a focus on food availability will not automatically result in proper nutritional practices among adolescents. This fact and the parental influence detected are evidence of a need to involve adolescents and their parents in nutrition education campaigns to improve adolescent snack food choices.

INTRODUÇÃO: A utilização pelos Culicidae de recipientes contendo água para a colocação de seus ovos, em área antropogênica, pode indicar plasticidade genética que os direcione evolutivamente no sentido da domiciliação. Nesse sentido, foram coletadas as diferentes espécies de Culicidae que colonizam recipientes alocados em mata ciliar, na área rural. MATERIAL E MÉTODO: Foram instalados recipientes de pneu, plástico, lata e bambu, em mata ciliar, em área rural no Norte do Paraná, Brasil. RESULTADOS: Coletaram-se larvas de Cx. grupo coronator, Cx. declarator, Cx. laticlasper, Cx. (Melanoconion) secção Spissipes, Cx. tatoi, Tr. compressum, Tr. pallidiventer, Ae. terrens, Cx. mollis, Cx. bigoti, Hg. leucocelaenus, Cx. eduardoi, Cx. quinquefasciatus, Li. durhamii e Toxorhynchites sp. As cinco primeiras espécies foram específicas de pneus. As duas espécies de Trichoprosopon ficaram restritas a bambu. Ae. terrens e Cx. mollis foram caletadas em pneu e bambu, Cx. bigoti foi coletada em pneu, lata e bambu, enquanto que Hg. leucocelaenus só não foi encontrada em lata. As quatro últimas espécies foram coletadas em todos os tipos de recipientes. Cx. quinquefasciatus, Cx. eduardoi, Li. durhamii tiveram significante flutuação populacional. CONCLUSÕES: O pneu caracterizou-se como o recipiente mais aceito pelos culicídeos. As áreas onde a mata ciliar esteve mais densa e o locais onde o solo esteve mais úmido foram os pontos com maior número de capturas. A mata ciliar, mesmo muito reduzida e alterada, foi suficiente para abrigar várias espécies de culicídeos. As espécies caputradas podem ser portadoras de plasticidade gênica que as capacitem a colonizar ambientes antropogênicos.

OBJECTIVE: To assess the behavior of induced abortion as a function of certain demographic variables, for the population of fertile women (15 to 49 years old) residing in the Vila Madalena subdistrict S. Paulo (Brazil). MATERIAL AND METHOD: Two population samples were selected. One sample, with 996 women, investigated the incidence of induced abortions during 1987, using the RRT. In the other, involving 1,004 women, the same information was detected through a conventional approach. In both samples, the induced abortion occurring during the reproductive life was recorded in direct fashion. Though this analysis refers only to information about past abortions, that is by 2,000 women -, it should be noted that it is exactly the RRT that lends credibility to the found or results given results. CONCLUSION: The analysis furnishes evidence showing that single women, young women between the ages of 15 and 19, women who have not had live births, women who have a number of children below the expected ideal, women who use contraceptive methods (especially inefficient ones) and women who do not have any restrictions as to abortion constitute the categories most inclined to resort to induced abortion. This grouping suggests the existence of interrelationships between categories, that is, each of these categories is probably composed primarily of the same women, those who are at the beginning of their reproductive lives.

INTRODUCTION: Although there was a considerable reduction in infant mortality in Pelotas, Rio Grande do Sul in the last decade, its perinatal causes were reduced only by 28%. The associated factors of these causes were analised. MATERIAL AND METHOD: All hospital births and perinatal deaths were assessed by daily visits to all the maternity hospitals in the city, throughout 1993 and including the first week of 1994. RESULTS: The perinatal mortality rate was 22.1 per thousand births. The multivariate analysis showed the following risk factors: low socioeconomic level, male sex and maternal age above 35 years . Among multigravidae women, the fetal mortality rate was significantly increased for mothers with a previously low birthweight and a previous stillbirth. For early neonatal mortality the risk was significantly increased by a smaller number of antenatal visits than 5 and low birthweight. CONCLUSIONS: Main risk factors for perinatal mortality: low socioeconomic level, maternal age above 35 years and male sex. For early neonatal mortality the risk was significantly increased by a smaller number of antenatal visits than 5 and low birthweight.

Durante estudos ecológicos sobre mosquitos anofelíneos no município de Bataguassu, Estado de Mato Grosso do Sul, foram encontradas larvas e adultos de Aedes albopictus. Pela primeira vez sua introdução ocorre numa área enzoótica do vírus selvático da febre amarela no Brasil. Isto sugere risco potencial para transferência desse vírus para área urbana infestada com Aedes aegypti.

OBJETIVO: Se realizó una intervención comunitaria bajo la estrategia de promoción de la salud, con el objetivo de desarrollar un programa de educación para la salud con mujeres. MÉTODOS: Se analizó la metodología de educación popular; con la finalidad de generar procesos organizativos y de participación social que mejoren la nutrición y sobrevivencia infantil. RESULTADOS: Los principales resultados, se relacionan con la generación de procesos autogestivos, la conformación de un grupo de promotoras de salud que han impulsado la organización de las mujeres enfocando su trabajo a mejorar la nutrición infantil y la salud familiar. Las promotoras han tomado en sus manos el programa de vigilancia epidemiológica en nutrición infantil y en conjunto con las mujeres han emprendido una serie de acciones para mejorar la nutrición de los niños y los porcentajes de desnutrición en los niños que participan en el programa de nutrición infantil han iniciado un descenso (64% a 62%). CONCLUSIONES: Los programas de nutrición infantil tienen mayores posibilidades de éxito en la medida que logran involucrar a la población en la resolución de esta problemática, eso es posible cuando se utiliza una metodología que propicie la participación de los individuos y se generen espacios que les permitan realizar una práctica transformadora de su realidad. La metodología de la educación popular proporciona las pautas en ese sentido. Es necesario continuar ampliando las experiencias educativas en educación para la salud con este tipo de metodología.

Este estudo, de natureza qualitativa, visa em primeiro lugar a identificação de necessidades de formação contínua de docentes do 1º ciclo do ensino básico, decorrentes da inclusão de alunos com necessidades educativas especiais (N.E.E.) em escolas de meio rural. Pretende ainda criar linhas orientadoras para a construção de um dispositivo de formação contínua que possa contribuir para o desenvolvimento de capacidades, atitudes e conhecimentos relacionados com a inclusão de alunos com N.E.E. Utilizámos a entrevista semi - directiva, de forma a conhecer a percepção que os professores têm sobre as suas necessidades formativas e recorremos a observações naturalistas, de modo a identificar necessidades decorrentes da prática, através da análise de situações reais de aprendizagem em salas de aula inclusivas. Foram entrevistados 8 professores, tendo sido observadas as aulas de dois desses professores. A partir do cruzamento dos dados obtidos por ambas as técnicas, identificámos um conjunto de preocupações, dificuldades, aspirações/desejos relacionados com a inclusão e ainda carências formativas relacionadas com as N.E.E. As entrevistas permitiram-nos explorar, de uma forma geral, a percepção que os professores tinham das suas necessidades, ou seja as necessidades percebidas, enquanto que as observações nos possibilitaram a identificação das necessidades da prática, necessidades inferidas. No presente estudo, foi possível concluir que as observações constituem um meio privilegiado para a identificação de necessidades de formação, ao nível da prática pedagógica de uma sala de aula, na medida em que os professores, nas entrevistas, não expressaram algumas das dificuldades observadas e não identificaram um conjunto de carências formativas que a análise da observação sugere. Partindo dos resultados obtidos, foi possível enumerar as principais necessidades de formação contínua decorrentes da inclusão e propor um dispositivo de formação contínua, apoiado na observação e supervisão, que julgamos poder dar resposta às necessidades formativas dos professores encontradas no presente estudo.

OBJETIVO: Conocer la percepción, recursos y prácticas que sobre el aborto tienen las parteras de un área rural mexicana, el Municipio de Yecapixtla, Morelos, situado en la parte central de México. MÉTODOS: Se utilizó método cualitativo, realizándose entrevistas en profundidad, grupos focales y observación participativa. Los tópicos indagados fueron aspectos socioculturales, sexualidad, reproducción y aspectos de salud en relación al aborto. Se entrevistó en profundidad a 9 parteras y se realizó un grupo focal en el que participaron 16 parteras. RESULTADOS: Éstos dan cuenta de una profunda actitud de rechazo ante el aborto, inducido o espontáneo. El primero se conceptúa como un pecado grave y el segundo como un fracaso en la principal función de la mujer en estas comunidades, la reproducción. Las mujeres que abortan son llamadas "puercas", "cochinas" o "perras" y las parteras se muestran reacias a atenderlas. Sin embargo, es común entre las mujeres de la comunidad la práctica de la "regulación de la regla", es decir, la utilización de sustancias que hacen que aparezca la menstruación si ésta se ha retrasado. Dicha práctica no se considera abortiva. CONCLUSIONES: El conocimiento del mundo de significaciones populares en torno al aborto es imprescindible para construir modelos y estrategias efectivas que, desde los servicios institucionales de salud, refuercen los vínculos entre éstos y las parteras tradicionales, de manera que se mejore el acceso a servicios y la calidad de la atención a las mujeres.

OBJECTIVE: To identify the association between food group consumption frequency and serum lipoprotein levels among adults. METHODS: The observations were made during a cross-sectional survey of a representative sample of men and women over 20 years old living in Cotia county, S. Paulo, Brazil. Data on food frequency consumption, serum lipids, and other covariates were available for 1,045 adults. Multivariate analyses adjusted by age, gender, body mass index, waist-to-hip ratio, educational level, family income, physical activity, smoking, and alcohol consumption were performed. RESULTS: Consumption of processed meat, chicken, red meat, eggs and dairy foods were each positively and significantly correlated with LDL-C, whereas the intake of vegetables and fruits showed an inverse correlation. Daily consumption of processed meat, chicken, red meat, eggs, and dairy foods were associated with 16.6 mg/dl, 14.5 mg/dl, 11.1 mg/dl, 5.8 mg/dl, and 4.6 mg/dl increase in blood LDL-C, respectively. Increases of daily consumption of fruit and vegetables were associated with 5.2 mg/dl and 5.5 mg/dl decreases in LDL-C, respectively. Alcohol beverage consumption showed a significant positive correlation with HDL-C. CONCLUSIONS: Dietary habits in the study population seem to contribute substantially to the variation in blood LDL and HDL concentrations. Substantially CHD risk reduction could be achieved with dietary changes.

Portugal is a culture grounded in strong traditions and family. Yet, social changes like women returning to the workforce and a decreas ed national birth rate are impacting the traditional family structure and care giving environments of children. Female employment has been increasing steadily in P ortugal over the last three decades (Galego & Pereira, 2006) and the total fert ility rate decreasing from 4.1 to 2.8 (INE, 2006). Furthermore, extended family me mbers, like grandparents, no longer reside close by to their children and grandc hildren as in the past, because of a changing labor market. Many of the younger gen eration are leaving their rural communities to flock to urban areas because o f job opportunities, leaving behind older relatives who would have otherwise par ticipated in the daily care of children. Given these social and economic changes, children are spending more time in out-of-home care with non-familial caregive rs. Yet, government regulations and guidelines in early care and educat ion (ECCE) and early intervention (EI) are only just emerging; it contin ues to be a work in progress.

INTRODUCTION: After the era of rubella vaccine, cytomegalovirus (CMV) infection is one of the most frequently causes of mental retardation and congenital deafness. Seroepidemiological studies are necessary to understand the transmission dynamics of the disease. The purpose of the study was to quantify the transmission rate of CMV disease in a community in the state of São Paulo, Brazil. METHODS: Using ELISA test (IgG), a retrospective serological survey looking for CMV antibodies was performed in an non-immunized community. Frozen sera from 443 individuals, randomly selected by cluster sampling technique in the town of Caieiras, São Paulo, were collected from November 1990 to January 1991. Seroprevalence was stratified by age (0-40 years). Mathematical techniques were applied to determine the age-dependent decay function of maternal antibodies during the first year of life, the age-dependent seroprevalence function and the force of infection for CMV in this community. RESULTS: It was observed a descending phase of seropositivity in the first 9 months, but changes in antibody titration were observed between 8 months old and one year of age. The average age of the first infection was 5.02 months of age and 19.84 years, when the age-dependent seroprevalence and the force of infection were analyzed between 10 months of age and 10 years of age and from 10 to 40 years old, respectively. CONCLUSION: CMV infection is highly prevalent among the population studied and infection occurs in the first year of life. This study shows that most women at reproductive age are vulnerable to the first infection, increasing the risk for congenital infection.

The aim of this study was the assessment of exposure to ultrafine in the urban environment of Lisbon, Portugal, due to automobile traffic, and consisted of the determination of deposited alveolar surface area in an avenue leading to the town center during late spring. This study revealed differentiated patterns for weekdays and weekends, which could be related with the fluxes of automobile traffic. During a typical week, ultrafine particles alveolar deposited surface area varied between 35.0 and 89.2 μm2/cm3, which is comparable with levels reported for other towns such in Germany and the United States. These measurements were also complemented by measuring the electrical mobility diameter (varying from 18.3 to 128.3 nm) and number of particles that showed higher values than those previously reported for Madrid and Brisbane. Also, electron microscopy showed that the collected particles were composed of carbonaceous agglomerates, typical of particles emitted by the exhaustion of diesel vehicles. Implications: The approach of this study considers the measurement of surface deposited alveolar area of particles in the outdoor urban environment of Lisbon, Portugal. This type of measurements has not been done so far. Only particulate matter with aerodynamic diameters <2.5 (PM2.5) and >10 (PM10) μm have been measured in outdoor environments and the levels found cannot be found responsible for all the observed health effects. Therefore, the exposure to nano- and ultrafine particles has not been assessed systematically, and several authors consider this as a real knowledge gap and claim for data such as these that will allow for deriving better and more comprehensive epidemiologic studies. Nanoparticle surface area monitor (NSAM) equipments are recent ones and their use has been limited to indoor atmospheres. However, as this study shows, NSAM is a very powerful tool for outdoor environments also. As most lung diseases are, in fact, related to deposition of the alveolar region of the lung, the metric used in this study is the ideal one.

OBJETIVO: Investigar a ocorrência de acidentes do trabalho, na zona rural, e sua associação com alguns fatores de risco. MÉTODOS: O estudo foi realizado na zona rural do município de Pelotas, RS. O delineamento do estudo foi transversal de base populacional. Uma amostra representativa dos trabalhadores rurais foi obtida por meio de amostragem, em estágios múltiplos, utilizando-se os setores censitários da Fundação IBGE. As entrevistas foram realizadas em um período de quatro meses, utilizando-se questionários padronizados e pré-codificados. Os 580 trabalhadores entrevistados pertenciam a 258 famílias da zona rural. RESULTADOS E CONCLUSÕES: A prevalência de acidentes encontrada foi de 11%. Os fatores de risco associados à maior ocorrência de acidentes, na análise multivariada, foram a classe social mais baixa (OR=1,81), a cor não-branca (OR=3,50) e a insatisfação com o trabalho realizado (OR=2,77).

The aim of this study was to contribute to the assessment of exposure levels of ultrafine particles in the urban environment of Lisbon, Portugal, due to automobile traffic, by monitoring lung deposited alveolar surface area (resulting from exposure to ultrafine particles) in a major avenue leading to the town center during late spring, as well as in indoor buildings facing it. Data revealed differentiated patterns for week days and weekends, consistent with PM2.5 and PM10 patterns currently monitored by air quality stations in Lisbon. The observed ultrafine particulate levels may be directly correlated with fluxes in automobile traffic. During a typical week, amounts of ultrafine particles per alveolar deposited surface area varied between 35 and 89.2 μm2/cm3, which are comparable with levels reported for other towns in Germany and the United States. The measured values allowed for determination of the number of ultrafine particles per cubic centimeter, which are comparable to levels reported for Madrid and Brisbane. In what concerns outdoor/indoor levels, we observed higher levels (32 to 63%) outdoors, which is somewhat lower than levels observed in houses in Ontario.

OBJECTIVE: To propose a mathematical method for the estimation of the Basic Reproduction Number, R0, of urban yellow fever in a dengue-infested area. METHODS: The method is based on the assumption that, as the same vector (Aedes aegypti) causes both infections, all the quantities related to the mosquito, estimated from the initial phase of dengue epidemic, could be applied to yellow fever dynamics. It is demonstrated that R0 for yellow fever is, on average, 43% lower than that for dengue. This difference is due to the longer dengue viremia and its shorter extrinsic incubation period. RESULTS: In this study the analysis was expanded to the epidemiological situation of dengue in São Paulo in the year 2001. The total number of dengue cases increased from 3,582 in 2000 to 51,348 in 2001. It was then calculated R0 for yellow fever for every city which have shown R0 of dengue greater than 1. It was also estimated the total number of unprotected people living in highly risky areas for urban yellow fever. CONCLUSIONS: Currently there is a great number of non-vaccinated people living in Aedes aegypti infested area in the state of São Paulo.
