Os autores descrevem o tratamento de um paciente portador de fístula arteriovenosa entre vasos poplíteos com mais de 20 anos de evolução, após ferimento por projétil de arma de fogo. O paciente foi submetido a tratamento endovascular pelo implante de stents revestidos Viabahn (Gore®), porém o mesmo não foi bem sucedido devido à diferença de diâmetro entre a artéria poplítea proximal e a distal à fístula. A artéria femoral superficial foi submetida à cerclagem com fios de sutura ao redor do stentgraft previamente implantado. Esse recurso de improvisação permitiu uma abordagem distante do sítio da fístula, diminuindo os riscos do acesso cirúrgico em uma região com distorções anatômicas e vasos sanguíneos ectasiados, o que certamente acarretaria maior risco de lesões iatrogênicas.

The purpose of this study was to compare temperature variation during osteotomies with trephine drills under different irrigation conditions: without irrigation, external irrigation, and double irrigation.Synthetic blocks of bone (type I density) were used for drilling procedures with an experimental computed machine, which measured the maximum temperature in the cortical bone during osteotomy with a bone cut that was 5 mm in both diameter and depth. Three groups were formed depending on the irrigation conditions: no irrigation (G1), external irrigation (G2), and double irrigation (G3). Fifty drillings were performed for each group.The average thermal increase in the groups was 21.7 +/- 1.52 A degrees C in G1, 14.2 A +/- 0.70 A degrees C in G2, and 12.4 A +/- 0.75 A degrees C in G3. The results presented statistically significant differences among all groups (alpha = 0.05).The double irrigation technique resulted in a smaller increase in temperature in the cortical bone model, demonstrating a greater efficiency, which may be beneficial when compared to external irrigation alone.The trephine has been widely used in removing small blocks for bone graft, especially the posterior mandible, where the possibility of heating may be higher due to the density of cortical bone in this area. So it is important that the professionals select instruments that can reduce the risks of complications in the proposed treatment.

The rise of extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli in food-producing animals is a growing concern for public health. We investigated ESBL producers isolated from broiler chickens in Brazil and characterized 19 CTX-M-2-producing E.coli. The ISCR1 was detected upstream of the chromosome-located gene bla(CTX-M-2), associated with sul-1 type integron structure. CTX-M-2-producing E.coli exhibited different PFGE-types and phylogenetic groups, showing a non-clonal dissemination. The sequence types found (ST93, ST155 and ST2309) have been associated with humans and animals worldwide. Herein, we report the chromosomal location of bla(CTX-M-2) on E.coli, highlighting the risks of multidrug-resistant bacteria in food-producing animals.

ISO 26000, published in 2010, focuses on corporate social responsibility. This study presents a systematic review conducted in ISI Web of Knowledge (Web of Science) and Elsevier's Scopus databases to answer the following question: What are the barriers and motivators affecting the adoption of ISO 26000 by organizations? The articles were selected using filters that applied two inclusion criteria. The data were summarized in a table covering the concepts of ISO 26000, the motivators, and the barriers. The motivators were globalization or competition in international markets, congruence with management systems, reputation or image, relationship with employees and improvement of the organizational environment, improvement in the relationship with external stakeholders, competitive advantage and strategy, guide to corporate social responsibility (CSR), and reduction of business risks. The barriers were lack of alignment between CSR and organizational strategy; business (national and international); unfamiliarity with ISO 26000; lack of communication, tools and sensitivity to the subject; short-term focus; knowledge management; fear of not fulfilling the standard; and financial resources. Finally, an agenda for future studies was prepared.

One of the main causes of incapacity in athletes, be they human or equines, is the occurrence of intra-articular lesions. The equines are each time more required in his athletic performance, resulting in intense stress to the structures that composes the locomotor device. The leading cause of human and equine athlete’s functional incapacity is the intra-articular disorders. One of the greatest advances in sports medicine was the development of arthroscopy as a minimal invasive intra-articular surgery. The defining characteristic of diagnostic or surgical arthroscopy is featured by minimal tissue damage and broad inspection of internal structures inside the joint associated with low morbidity and complications. The advantages of surgical arthroscopy over traditional surgery are well known: limited hospitalization, early return to competition, lower risks of post-operative joint rigidity, magnification of inspected structures, joint lavage associated or not with removal of potentially dangerous substances. Arthroscopy cannot replace conventional methods and must not do so; however, the intrinsic limitations of conventional diagnostic techniques, such as radiology and synovial fluid analysis, must be kept in mind, particularly in evaluating damage to cartilage and the synovial membrane. Arthroscopy has now become the accepted method of performing all joint surgery, however it is mainly used for radical surgery, such as osteochondral fragment removal, surgical curettage and arthroplasty
