RESUMO - Enquadramento/objectivos: Apesar do elevado nível de comprometimento em estratégias eficazes para o controlo da tuberculose, em todo o mundo, esta constitui ainda um sério problema de Saúde Pública, com uma estimativa global de 9,4milhões de casos novos em 2008 e 1,8milhões de mortes/ano. O reduzido conhecimento das barreiras e facilitadores para o sucesso terapêutico constitui um importante obstáculo na procura de soluções eficazes de melhoramento da qualidade dos programas de controlo da tuberculose. Este estudo procura contribuir para a identificação atempada de doentes com perfis preditivos de insucesso terapêutico, através da identificação inicial de potenciais determinantes do resultado, com base num modelo epidemiológico e estatístico. Métodos: Foi desenvolvido um estudo de caso-controlo para a população de casos notificados ao Programa Nacional de Controlo da Tuberculose (n=24491), entre 2000-2007. Os factores preditivos de insucesso terapêutico foram identificados na análise bivariada e multivariada, com um nível de significância de 5%; a regressão logística foi utilizada para estimar a odds ratio de insucesso terapêutico, em comparação com o sucesso terapêutico, para diversos factores identificados na literatura, e para os quais os dados se encontravam disponíveis. Resultados: A dependência alcoólica (OR=2,889), o país de origem (OR=3,910), a situação sem-abrigo (OR=3.919), a co-infecção pelo VIH (OR=5,173), a interrupção (OR=60.615) ou falha terapêutica no tratamento anterior (OR=67.345) e a duração do tratamento inferior a 165 dias (OR=1930,133) foram identificados como factores preditivos de insucesso terapêutico. A duração do tratamento inferior a 165 dias provou ser o mais importante determinante do resultado terapêutico. Conclusões: Os resultados sugerem que um doente imigrante, em situação de sem-abrigo, dependente alcoólico, com tratamentos anteriores para a tuberculose e co-infectado pelo VIH apresenta uma elevada probabilidade de insucesso terapêutico. Assim, deverão ser definidas estratégias específicas, centradas no doente por forma a impedir este resultado. A base de dados (SVIG-TB), provou ser uma ferramenta de qualidade para a investigação sobre diversos aspectos do controlo da tuberculose. ------------------------------- ABSTRACT - Background/Objective: Despite the high commitment in good strategies for tuberculosis control worldwide, this is still a serious Public Health problem, with global estimates of 9,4million new cases in 2008 and 1,8million deaths/year. The poor understanding of the barriers and facilitators to treatment success is a major obstacle to find effective solutions to improve the quality of tuberculosis programs. This study tries to contribute to the timely identification of patients with predictive profiles of unsuccessful treatment outcomes, through the initial identification of characteristics probably affecting treatment outcome, found on the basis of an epidemiological and statistical model. Methods: A case-control study was conducted for the population of cases notified to the National Program for Tuberculosis Control (n=24 491), between 2000-2007. Predictive factors for unsuccessful outcome were assessed in a bivariate and multivariate analysis, using a significance level of 5%; a logistic regression was used to estimate the odds-ratio of unsuccessful, as compared to successful outcome, for several factors identified in the literature and to which data was available. Results: Alcohol abuse (OR=2,889), patient´s foreign origin (OR=3,910), homelessness (OR=3,919), HIV co-infection (OR=5,173), interruption (OR=60,615) or unsuccessful outcome in the previous treatment (OR=67,345) and treatment duration below 165 days (OR=1930,133) were identified as predictive of unsuccessful outcomes. A low treatment duration proved to be the most powerful factor affecting treatment outcome. Conclusions: Results suggest that a foreign-born patient, alcohol abuser, who has had a previous treatment for tuberculosis and is co-infected with HIV is very likely to have an unsuccessful outcome. Therefore, specific, patient-centered strategies should be taken to prevent an unsuccessful outcome. The database (SVIG-TB), has proved to be a quality tool on research of various aspects of tuberculosis control.

The concept of demand response has drawing attention to the active participation in the economic operation of power systems, namely in the context of recent electricity markets and smart grid models and implementations. In these competitive contexts, aggregators are necessary in order to make possible the participation of small size consumers and generation units. The methodology proposed in the present paper aims to address the demand shifting between periods, considering multi-period demand response events. The focus is given to the impact in the subsequent periods. A Virtual Power Player operates the network, aggregating the available resources, and minimizing the operation costs. The illustrative case study included is based on a scenario of 218 consumers including generation sources.

Low back problems are associated with decreased quality of life. Specific exercises can improve quality of life, resulting in better professional performance and functionality. The purpose of this study was to evaluate the effect of following a 21-month exercise program on the quality of life of warehouse workers. The population included 557 male warehouse workers from a food distribution company in Oporto, Portugal. Upon application of the selection criteria, 249 workers were deemed eligible, which were randomized into two groups (125 in the intervention group and 124 in the control group). Then, subjects were asked to volunteer for the study, the sample being formed by 229 workers (112 in the intervention group and 117 in the control group). All subjects completed the SF-36 questionnaire prior to beginning the program and on the 11th and 21st months following it. The exercises were executed in the company facilities once a day for 8 min. Data were analyzed using SPSS® 17.0 for Windows®. After 11 months of following the exercise program, there was an increase in all scores for the experimental group, with statistically significant differences in the dimensions physical functioning (0.019), bodily pain (0.010), general health (0.004), and rolephysical (0.037). The results obtained at the end of the study (21 months) showed significant improvements in the dimensions physical functioning (p = 0.002), rolephysical (p = 0.007), bodily pain (p = 0.001), social functioning (p = 0.015), role-emotional (p = 0.011), and mental health (p = 0.001). In the control group all dimensions showed a decrease in mean scores. It can be concluded that the implementation of a low back specific exercise program has changed positively the quality of life of warehouse workers.

We evaluated the components of the Fuenzalida-Palacios antirabies vaccine, which is till used in most developing countries in human immunization for treatment and prophylaxis. This vaccine is prepared from newborn mouse brains at 1% concentration. Even though the vaccine is considered to have a low myelin content, it is not fully free of myelin or of other undesirable components that might trigger adverse effects after vaccination. The most severe effect is a post-vaccination neuroparalytic accident associated with Guillain-Barré syndrome. In the present study we demonstrate how the vaccines produced and distributed by different laboratories show different component patterns with different degrees of impurity and with varying protein concentrations, indicating that production processes can vary from one laboratory to another. These differences, which could be resolved using a better quality control process, may affect and impair immunization, with consequent risks and adverse effects after vaccination. We used crossed immunoelectrophoresis to evaluate and demonstrate the possibility of quality control in vaccine production, reducing the risk factors possibly involved in these immunizing products.

RESUMO: O novo cenário demográfico origina mudanças na estrutura etária, nas relações familiares e de trabalho, nas exigências de cuidados de saúde e de protecção social. Estas constituem fortes desafios a enfrentar pelas políticas públicas, exigindo a preparação dos organismos responsáveis. O “envelhecimento activo” começa a ter lugar como paradigma de intervenção, representando já um marco político mundial, na medida em que pode contrariar ou inverter as tendências nefastas das novas transformações. A investigação incidiu numa reflexão em torno das políticas que procuram responder à intensificação e diversidade dos problemas inerentes ao envelhecimento populacional. Envolvemo-nos numa tarefa de análise detalhada de programas municipais em três concelhos da Região Interior de Portugal, segundo a perspectiva e os determinantes do “envelhecimento activo”. Embora não exista uma solução única e definitiva, se nos detivermos no conceito como estratégia global, conclui-se o dever de considerar diversos determinantes numa perspectiva integrada e individualizada, que percorra todo o curso de vida. Os programas devem procurar enfatizar estilos de vida saudáveis com vista num alcance da maior qualidade de vida possível e ter em conta as situações sócio-económicas, mas sem se afastarem do dever de reforçar a componente reparadora e inclusiva da velhice.-----------ABSTRACT: The new demographic scenario creates changes in the age structure, in the family and work relations, in healthcare and social protection demands. All these changes create a great challenge to public policies, demanding the preparation of the responsible organizations. The “active ageing” starts to be seen as the intervention model, which represents a milestone in global policies as it can counteract or reverse the negative trends of these demographic changes. This research focused on the policies that seek to respond to the intensity and diversity of the population aging problems in Portugal. It was produced a detailed analysis of three local programs following the mainstreaming of “active ageing” in interior country. Although there is no unique and definitive solution, if we take the concept as a global strategy, then we can conclude the obligation to consider several determinants in an integrated and individualized perspective, which covers the entire course of life. These programs should emphasize healthy lifestyles aiming the highest quality of life possible taking into account the socioeconomic situations, without disregarding the duty of reinforce the repairing and inclusive component of ageing.

RESUMO - Contexto: A osteoporose é uma doença sistémica associada à diminuição da resistência óssea que condiciona o aparecimento de fracturas por traumatismos de baixa energia as quais reduzem em muitos casos a esperança e a qualidade de vida, implicando um elevado número de horas de dedicação dos cuidadores e conduzindo, em muitos casos, à necessidade de institucionalização dos doentes. Em Portugal, ocorrem cerca de 40.000 fracturas anualmente, das quais 8.500 do fémur proximal, que se estima consumirem mais de 50 milhões de euros só em cuidados hospitalares1. Objectivo: Avaliar o impacto económico da institucionalização na Rede Nacional de Cuidados Continuados Integrados e Lares da 3ª idade por fractura de etiologia osteoporótica em mulheres no ano 2009, em Portugal. Métodos: O estudo começa por analisar sucintamente os aspectos clínicos, sociais e epidemiológicos da osteoporose, focando a perda de autonomia e qualidade de vida dos doentes vítimas deste episódio e a subsequente necessidade de institucionalização. Descrevem-se os custos associados à institucionalização na RNCCI, em Portugal, ano 2009, utilizando como fonte principal a base de dados da instituição “Gestcare CCI”, complementada com dados da literatura. Apuram-se os custos totais associados ao encaminhamento dos doentes para Lares de 3ª Idade em Portugal no ano 2009, utilizando-se informação proveniente de um painel de Delphi modificado e dados da literatura. A valorização dos recursos tem por base os preços em vigor no território nacional, expressos nos decretos-lei devidamente referenciados. Resultados: No ano 2009 em Portugal foram empregues cerca de 2,5 milhões de euros no internamento das mulheres na RNCCI, na perspectiva da sociedade, sendo cerca de 2,2 milhões atribuíveis à osteoporose (90%). Cerca de 91% dos custos totais são alocados à fractura osteoporótica da anca (2 milhões €). Para a fractura vertebral, úmero e punho os custos foram mais baixos - 7,1%, 1,3% e 1% dos custos totais da osteoporose, respectivamente. Nos Lares de 3ª idade, estimou-se a admissão de aproximadamente 14.372 doentes com fractura osteoporótica em diferentes localizações, em 2009, Portugal, com um custo que oscilou entre os 19 e os 21,6 milhões de euros. A fractura osteoporótica da anca foi a mais incidente e a que representou custos mais elevados para a Segurança Social – entre 17,5 e 19,7 milhões de euros. Considerando como referência os 52 milhões de euros gastos em 2006 no tratamento hospitalar da fractura da anca (DGS, 2006), o encaminhamento das mulheres para a RNCCI e Lares da 3ª Idade corresponde a 42% do bolo total. Assim, os resultados nacionais enquadram-se no que se encontra descrito na literatura internacional - os custos atribuíveis à hospitalização oscilaram entre 17%50 e 63%29 da despesa total da doença e das institucionalizações entre os 16%58 e os 59%51. Conclusões: Em Portugal o impacto económico da institucionalização por fractura osteoporótica, sobretudo por fractura da anca, não é desprezável e mostra que existiriam poupanças significativas se fosse possível reduzir a prevalência da doença em Portugal. Face às alterações demográficas associadas ao envelhecimento da população, é expectável que a incidência e custos com o tratamento das fracturas do colo do fémur, mais associadas à osteoporose, venham a subir nos próximos anos, pelo que o combate à doença deve ser considerada uma prioridade nacional. A decisão pela opção por determinados programas de prevenção da doença ou da comparticipação ou não de determinada terapêutica necessita contudo de ser complementada com a medição da dimensão dos benefícios terapêuticos. --- ABSTRACT - Background: Osteoporosis is a systemic disease associated with the loss off the bone strength and it is one of the major causes of low energy fractures, which in many cases reduce life hope and quality. This happens because it has associated extensive treatments and it usually carries loss of independence, implying many hours of caregivers dedication and leading, in many cases, to the institutionalization of the patients. In Portugal, about 40,000 fractures occur annually, which 8,500 are proximal femur, and that are estimated to consume over 50 million euros only in hospital care. Objective: Evaluate the economic impact of institutionalization on the Integrated Continued Care National Network (RNCCI) and Care Homes associated to osteoporotic fractures in women, in the year 2009 in Portugal. Methods: The study begins by reviewing briefly the clinical, social, and epidemiological studies of osteoporosis and osteoporotic fractures, focusing on the patient autonomy loss and life quality. The total and average costs per episode associated with the institutionalization in RNCCI are described, in Portugal, year 2009, using as main data source the application "Gestcare CCI", complemented with literature data. The total costs associated with the patients referral for the Care Homes in Portugal in 2009 is also calculated, using information from a modified Delphi panel and some literature data. The resources valuation is based on prices prevailing in Portugal. Results: In 2009, women relocation in RNCCI consumed approximately 2.5 million euros, which 2.2 million are attributable to osteoporosis (90%). About 91% of the total costs are allocated to osteoporotic hip fracture (€ 2 million). For vertebral, humerus, and wrist fracture, the associated costs were lower, 7.1%, 1.3%, and 1% of total costs of osteoporosis treatment, respectively. In Care Homes, an intake of approximately 14 372 patients with osteoporotic fracture was estimated, at a cost that is between 19 and 21.6 million euros. The osteoporotic hip fracture was the most frequent and represented higher costs for Social Security - between 17.5 and 19.7 million euros. Taking as reference the 52 million spent in 2006 with hospital treatment of hip fracture (DGS, 2006), referring women to RNCCI and Care Homes represents 42% of the total costs. The results are in accordance with the international literature - costs attributable to hospitalization ranged between 17% and 63% of total expenditure of illness and institutionalization between 16% and 59%. Conclusions: In Portugal, the economic impact of institutionalization for osteoporotic fracture, particularly for hip fracture, is not negligible. It was shown that there would be significant savings if it were possible to reduce the prevalence of the disease in Portugal. Femoral fractures were the second most frequent diagnosis in RNCCI in 2008 and 2009 (16% of all episodes recorded). The execution of RNCCI in 2008 was 75 million euros, and 2.7% consumed by hip fracture and 0.3% by wrist, humerus, and spine fractures. The average cost per episode in 2009, from the perspective of society, for hip fracture, vertebral, humerus, and wrist (or non-osteoporotic) was € 5,195, € 5,160, € 5,030, and € 4,854 respectively. Thus, considering an average cost per episode in RNCCI from January to March of 3230€, the expense related to the treatment of these patients in RNCCI in 2009 was higher. For the Care Homes, an intake of approximately 14 372 patients with osteoporotic fracture in 2009 was estimated, at a cost to Social Security that ranged from 19 to 21.6 million €. The osteoporotic hip fracture was the most frequent and it was shown to absorve higher resources from Social Security - between 17.5 and 19.7 million €. This was followed by the analysis of vertebral and humerus fracture and the results showed that these fractures have a low incidence and low proportion of institutionalization, with a significantly lower cost - only about 4.7% and 3.3% of total expenditure, respectively. With demographic changes associated to ageing, it is expected that the incidence and treatment cost of the femoral neck fractures, more commonly associated with osteoporosis, will climb in coming years, so the fight against the disease should be considered a national priority. The decision to choose a certain disease prevention program or to reimburse a certain drug not should only account about the costs, but also the benefits of it. In fact, the size and impact of this problem, makes it necessary to focus all interventions in the prevention of these episodes either by using an appropriate therapy, either through real programs for disease prevention. Once the problem is installed, we must measure the health gains associated with the patient institutionalization by conducting additional research.

RESUMO - Os programas de acreditação revelam-se essenciais para monitorizar e garantir a qualidade dos serviços de imagiologia. Face à inexistência de um documento nacional de acreditação que responda às necessidades específicas de um serviço de imagiologia e perante a existência de diferentes estratégias de implementação da qualidade a nível internacional, definiu-se como principal objectivo deste trabalho contribuir para a criação de um documento de acreditação específico para serviços de imagiologia adaptado à realidade portuguesa. Nesse sentido, o presente trabalho pretende, através da aplicação de um questionário, desenvolvido para o efeito, a um grupo de peritos (painel Delphi) representante de organizações da área da imagiologia e da qualidade, analisar o consenso entre os elementos relevantes na acreditação de um serviço de imagiologia, nomeadamente ao nível da estrutura, processo e resultado. Como resultados esperados destacam-se a estruturação de um documento de consenso para a acreditação nos serviços de imagiologia em Portugal. Por certo, a aplicação de um quadro normativo emanado deste trabalho, na implementação da qualidade em imagiologia, permitirá que os serviços melhorem a sua prestação, que sejam avaliados e comparados com os congéneres, nacional ou internacionalmente, permitindo aos utentes ter mais e melhor informação que lhes permita optar por serviços com qualidade. -------------------------ABSTRACT - Accreditation programs are essential to monitor and ensure the quality of Imaging Services. Due to the lack of a national accreditation document that meets the specific needs of an imaging service and due to the existence of different international quality implementation strategies, the main purpose of this work is to contribute to the creation of a Portuguese imaging services accreditation document. Therefore, the present study aims to apply a inquiry to a group of experts (Delphi panel), that represents the structures and organizations that should integrate the development of the imaging services accreditation document, to analyze the consensus among experts about characteristics of accreditation, particularly the structure, process and outcome of imaging services. The expected results are the increased interest of decision-makers in promoting structural changes in the area of accreditation in Imaging and the creation of a document that evidences the consensus among experts. Finally, it is hoped that the same group of experts meets for accreditation document. The expected results include designing a consensus document for accreditation in imaging services in Portugal. Certainly, the implementation of a regulatory framework emerging from this work, in the implementation of quality imaging, will allow services to improve their performance, which are evaluated and compared with national or international counterparts. Consequently, patients will have better information that will enable them to opt for quality services.

This article describes a study that investigated the main strategic drivers that influence the implementation of sustainability/social responsibility programs. An online survey was administered to managers of Portuguese organizations with certified management systems. The findings suggest that the implementation of such programs is mainly correlated to: 1.) the approach to understanding and working toward the satisfaction of the community’s needs (in the broad sense of social responsibility); 2.) how systematically sustainability within the organization is identified and managed (e.g., pollution prevention, improved environmental performance, and compliance with the applicable environmental laws); and 3.) the degree to which the organization tries to understand the needs of the employees and works toward satisfying them. In addition to the survey, five interviews with top managers of the surveyed organizations provided some useful insights. There was no consensus on the meaning of sustainability and social responsibility: some described it as an instrumental approach for obtaining better organizational results, while others regarded it as the right thing to do (i.e., it is values driven). In all cases, however, the managers supported a kind of umbrella construct under which different size corporations use different models (for example, the Dow Jones Sustainability Index (DJSI), Global Reporting Initiative (GRI), ISO 14001 environmental management systems), although some managers reported that they simply do not know what to do. All of those surveyed agreed that the lack of a systematic approach could represent a major threat to their organization, making them willing to pay more attention and take more action on the issue of sustainability. An additional suggestion made by managers was to change from a triple bottom line (economic dimension, environmental dimension, social equity dimension) to a quadruple bottom line by adding another dimension: personal and family happiness. This fourth dimension was recognized by the Greek philosopher/thinker Aristotle (384-322 BCE) who thought of happiness as the highest good (virtue) and ultimate goal and purpose of life, achieved through living well, in harmony. Such harmony suggests a balance and a lack of excess—in other words a sustainable existence.

The clothing sector in several countries is still seen, in many aspects as a traditional sector with some average characteristics, nevertheless is a very important sector in terms of labour market. Globalization and de-localization are having a strong impact in the organisation of work and in occupational careers. Very few companies are able to keep a position in the market without changes in organisation of work and workers, founding different ways to face this reality according to size, capital and position. We could find two main paths: one where companies outsource production to another territory, close and/ or dismissal the workers; other path, where companies up skilled their capacities. This paper will present some results from the European project WORKS – Work organisation and restructuring in the knowledge society (6th Framework Programme), focusing the Portuguese case studies in several clothing companies in a comparative analysis with some other European countrie

Radio link quality estimation is essential for protocols and mechanisms such as routing, mobility management and localization, particularly for low-power wireless networks such as wireless sensor networks. Commodity Link Quality Estimators (LQEs), e.g. PRR, RNP, ETX, four-bit and RSSI, can only provide a partial characterization of links as they ignore several link properties such as channel quality and stability. In this paper, we propose F-LQE (Fuzzy Link Quality Estimator, a holistic metric that estimates link quality on the basis of four link quality properties—packet delivery, asymmetry, stability, and channel quality—that are expressed and combined using Fuzzy Logic. We demonstrate through an extensive experimental analysis that F-LQE is more reliable than existing estimators (e.g., PRR, WMEWMA, ETX, RNP, and four-bit) as it provides a finer grain link classification. It is also more stable as it has lower coefficient of variation of link estimates. Importantly, we evaluate the impact of F-LQE on the performance of tree routing, specifically the CTP (Collection Tree Protocol). For this purpose, we adapted F-LQE to build a new routing metric for CTP, which we dubbed as F-LQE/RM. Extensive experimental results obtained with state-of-the-art widely used test-beds show that F-LQE/RM improves significantly CTP routing performance over four-bit (the default LQE of CTP) and ETX (another popular LQE). F-LQE/RM improves the end-to-end packet delivery by up to 16%, reduces the number of packet retransmissions by up to 32%, reduces the Hop count by up to 4%, and improves the topology stability by up to 47%.

A indústria automóvel exige, em geral, elevados índices de produtividade e qualidade. Para corresponder às exigências deste tipo de indústria, são requeridos métodos avançados de produção, tentando eliminar ao máximo operações que não gerem valor acrescentado e que possam introduzir problemas no processo de garantia da qualidade. A maquinagem por arranque de apara é um processo utilizado de forma intensiva na indústria automóvel. No entanto, enquanto em componentes críticos como o motor, os processos estão já altamente otimizados, o mesmo não se verifica na maior parte dos sistemas periféricos, normalmente realizados por empresas mais pequenas que gravitam em torno dos principais fornecedores da cadeia de produção de automóveis. Os sistemas responsáveis pela movimentação dos limpa pára-brisas e elevação dos vidros, entre outros, encontram-se neste grupo. Este trabalho visa essencialmente otimizar o processo de maquinagem de componentes periféricos de automóveis, sujeitos a diferentes operações em vários planos. No entanto, e tal como em muitas outras situações relacionadas com a variedade de versões existentes na indústria automóvel relativamente a cada sistema, pretende-se que o processo seja suficientemente versátil para poder ser aplicado em vários componentes de uma mesma família de produtos, necessitando de um número de ajustes o mais baixo possível. O estudo passou por uma análise profunda das similaridades geométricas dos diferentes componentes, análise dos planos de maquinagem de cada componente, operações envolvidas, elencagem da necessidade específica de ferramentas, elaboração de gabaritos de fabrico e apresentação da solução final, a qual passa pela introdução de um 4º eixo e do seu controlo através do sistema CNC já existente, assim como pela elaboração de novos programas.
