Introduction Methicillin-resistant Staphylococcus aureus (MRSA) strains have been responsible for many nosocomial outbreaks. Within hospitals, colonized employees often act as reservoirs for the spread of this organism. This study collected clinical samples of 91 patients admitted to the intensive care unit (ICU), hemodialysis/nephrology service and surgical clinic, and biological samples from the nasal cavities of 120 professionals working in those environments, of a University Hospital in Recife, in the State of Pernambuco, Brazil. The main objective of this study was to determine the occurrence and dissemination of methicillin- and vancomycin-resistant Staphylococcus spp. Methods The isolates obtained were tested for susceptibility to oxacillin and vancomycin and detection of the mecA gene. In addition, the isolates were evaluated for the presence of clones by ribotyping-polymerase chain reaction (PCR). Results MRSA occurrence, as detected by the presence of the mecA gene, was more prevalent among nursing technicians; 48.1% (13/27) and 40.7% (11/27) of the isolates were from health professionals of the surgical clinic. In patients, the most frequent occurrence of mecA-positive isolates was among the samples from catheter tips (33.3%; 3/9), obtained mostly from the hemodialysis/nephrology service. Eight vancomycin-resistant strains were found among the MRSA isolates through vancomycin screening. Based on the amplification patterns, 17 ribotypes were identified, with some distributed between patients and professionals. Conclusions Despite the great diversity of clones, which makes it difficult to trace the source of the infection, knowledge of the molecular and phenotypic profiles of Staphylococcus samples can contribute towards guiding therapeutic approaches in the treatment and control of nosocomial infections.

Aspergillosis is an infection caused by saprophytic fungi of the genus Aspergillus, which typically occurs in immunosuppressed individuals, but has also been reported in immunocompetent patients. The main routes of entry are the respiratory tract, skin, cornea, and ear, and the infection may be localized or disseminated by contiguity or vascular invasion. We report a severe case of rhinosinusitis with cutaneous involvement, caused by invasive aspergillosis, in an immunocompetent user of inhaled cocaine. Invasive aspergillosis related to cocaine abuse has not yet been reported in the literature. After itraconazole treatment and surgical debridement, complete clinical remission was achieved. Nasal reconstruction with a skin graft over a silicone prosthesis resulted in a satisfactory esthetic outcome.

Purpose. The purpose of this work was to evaluate the potential of a novel custom-designed rigid gas permeable (RGP) contact lens to modify the relative peripheral refractive error in a sample of myopic patients. Methods. Fifty-two right eyes of 52 myopic patients (mean [TSD] age, 21 [T2] years) with spherical refractive errors ranging from j0.75 to j8.00 diopters (D) and refractive astigmatism of 1.00 D or less were fitted with a novel experimental RGP (ExpRGP) lens designed to create myopic defocus in the peripheral retina. A standard RGP (StdRGP) lens was used as a control in the same eye. The relative peripheral refractive error was measured without the lens and with each of two lenses (StdRGP and ExpRGP) using an open-field autorefractometer from 30 degrees nasal to 30 degrees temporal, in 5-degree steps. The effectiveness of the lens design was evaluated as the amount of relative peripheral refractive error difference induced by the ExpRGP compared with no lens and with StdRGP conditions at 30 degrees in the nasal and temporal (averaged) peripheral visual fields. Results. Experimental RGP lens induced a significant change in relative peripheral refractive error compared with the nolens condition (baseline), beyond the 10 degrees of eccentricity to the nasal and temporal side of the visual field (p G 0.05). The maximum effect was achieved at 30 degrees. Wearing the ExpRGP lens, 60% of the eyes had peripheral myopia exceeding j1.00 D, whereas none of the eyes presented with this feature at baseline. There was no significant correlation (r = 0.04; p = 0.756) between the degree of myopia induced at 30 degrees of eccentricity of the visual field with the ExpRGP lens and the baseline refractive error. Conclusions. Custom-designed RGP contact lenses can generate a significant degree of relative peripheral myopia in myopic patients regardless of their baselin spherical equivalent refractive error.

Purpose: The purpose of this study was to evaluate the effect of orthokeratology for different degrees of myopia correction in the relative location of tangential (FT) and sagittal (FS) power errors across the central 70 of the visual field in the horizontal meridian. Methods: Thirty-four right eyes of 34 patients with a mean age of 25.2 ± 6.4 years were fitted with Paragon CRT (Mesa, AZ) rigid gas permeable contact lenses to treat myopia (2.15 ± 1.26D, range: 0.88 to 5.25D). Axial and peripheral refraction were measured along the central 70 of the horizontal visual field with the Grand Seiko WAM5500 open-field auto-refractor. Spherical equivalent (M), as well as tangential (FT) and sagittal power errors (FS) were obtained. Analysis was stratified in three groups according to baseline spherical equivalent: Group 1 [MBaseline = 0.88 to 1.50D; n = 11], Group 2 [MBaseline = 1.51 to 2.49D; n = 11], and Group 3 [MBaseline = 2.50 to 5.25D; n = 12]. Results: Spherical equivalent was significantly more myopic after treatment beyond the central 40 of the visual field (p50.001). FT became significantly more myopic for all groups in the nasal and temporal retina with 25 (p 0.017), 30 (p 0.007) and 35 (p 0.004) of eye rotation. Myopic change in FS was less consistent, achieving only statistical significance for all groups at 35 in the nasal and temporal retina (p 0.045). Conclusions: Orthokeratology changes significantly FT in the myopic direction beyond the central 40 of the visual field for all degrees of myopia. Changes induced by orthokeratology in relative peripheral M, FT and FS with 35 of eye rotation were significantly correlated with axial myopia at baseline. Keywords: Field

Objectives: This research work intends to clarify the role of artificial saliva, in particularly the role of mucin, a salivary protein, on the surface properties and adhesion ability of Candida spp. oral clinical isolates to abiotic surfaces. Methods: Four oral clinical isolates of Candida spp. were used: two Candida albicans strains (AC; AM) and two Candida parapsilosis strains (AD; AM2). The strains were isolated from patients using oral prosthesis. The microorganisms were cultured in the absence or presence of mucin and artificial saliva, and their adhesion to an abiotic surface (coated with mucin and artificial saliva) was evaluated. Results: The presence of mucin per se onto the abiotic surface decreased the adhesion of all strains, although the combination of mucin with artificial saliva had reduced this effect. No direct correlation between adhesion and the surface free energies of adhesion of the microorganisms was found. Significance: Candida spp. were human commensal microorganisms that became pathogenic when the host immune defenses were compromised. Medical devices were colonized by Candida spp. particularly, oral prostheses, which might lead to the degradation of the prostheses and systemic infections. The salivary secretions that constantly cover the oral cavity influenced Candida spp. adhesion process. Therefore, it was important to understand the interactions between Candida spp., salivary proteins and the characteristic of oral prosthesis when developing materials for oral prostheses.

OBJECTIVE: To evaluate echocardiography accuracy in performing and obtaining images for dynamical three-dimensional (3D) reconstruction. METHODS: Three-dimensional (3D) image reconstruction was obtained in 20 consecutive patients who underwent transesophageal echocardiography. A multiplanar 5 MHz transducer was used for 3D reconstruction. RESULTS: Twenty patients were studied consecutively. The following cardiac diseases were present: valvar prostheses-6 (2 mitral, 2 aortic and 2 mitral and aortic); mitral valve prolapse- 3; mitral and aortic disease - 2; aortic valve disease- 5; congenital heart disease- 3 (2 atrial septal defect- ASD - and 1 transposition of the great arteries -TGA); arteriovenous fistula- 1. In 7 patients, color Doppler was also obtained and used for 3D flow reconstruction. Twenty five cardiac structures were acquired and 60 reconstructions generated (28 of mitral valves, 14 of aortic valves, 4 of mitral prostheses, 7 of aortic prostheses and 7 of the ASD). Fifty five of 60 (91.6%) reconstructions were considered of good quality by 2 independent observers. The 11 reconstructed mitral valves/prostheses and the 2 reconstructed ASDs provided more anatomical information than two dimensional echocardiography (2DE) alone. CONCLUSION: 3D echocardiography using a transesophageal transducer is a feasible technique, which improves detection of anatomical details of cardiac structures, particularly of the mitral valve and atrial septum.

OBJECTIVE: The aim of this work was the follow-up and evaluation of valve replacement in children under 12 years of age. METHODS: Forty-four children less than 12 years old were underwent valve replacement at INCOR-HCFMUSP between January 1986 and December 1992. Forty (91%) were rheumatic, 39 (88.7%) were in functional classes II or IV, 19 (43.2%) were operated upon on an emergency basis, and 6 (13.6%) had atrial fibrillation. Biological prostheses (BP) were employed in 26 patients (59.1%), and mechanical prostheses (MP) in 18 (40.9%). Mitral valves were replaced in 30 (68.7%), aortic valves in 8 (18.2%), a tricuspid valve in 1 (2.3%), and double (aortic and mitral) valves in 5 (11.4) of the patients. RESULTS: Hospital mortality was of 4.5% (2 cases). The mean follow-up period was 5.8 years. Re-operations occurred in 63.3% of the patients with BP and in 12.5% of those with MP (p=0.002). Infectious endocarditis was present in 26.3% of the BP, but in none of the cases of MP (p=0.049). Thrombosis occurred in 2 (12.5%) and hemorrhage in one (6.5%) of the patients with a MP. Delayed mortality occurred in 5 (11.9%) of the patients over a mean period of 2.6 years; four had had BP and one had a MP (NS). Actuarial survival and re-operation-free curves after 10 years were respectively, 82.5±7.7 (SD)% and 20.6±15.9%. CONCLUSION: Patients with MP required fewer re-operation, had less infectious endocarditis and lower late mortality rates compared with patients with bioprostheses. The former, therefore, appear to be the best valve replacement for pediatric patients.

OBJECTIVE: To evaluate the early outcome of mitral valve prostheses implantation and left ventricular remodeling in 23 patients with end-stage cardiomyopathy and secondary mitral regurgitation (NYHA class III and IV). METHODS: Mitral valvular prosthesis implantation with preservation of papillary muscles and chordae tendinae, and plasty of anteriun cuspid for remodeling of the left ventricle. RESULTS: The surgery was performed in 23 patients, preoperative ejection fraction (echocardiography) varied from 13% to 44% (median: 30%). In 13 patients associated procedures were performed: myocardial revascularization (9), left ventricle plicature repair (3) and aortic prosthese implantation (1). Early deaths (2) occurred on the 4th PO day (cardiogenic shock) and on the 20th PO day (upper gastrointestinal bleeding), and a late death in the second month PO (ventricular arrhythmia). Improvement occurred in NYHA class in 82.6% of the patients (P<0.0001), with a survival rate of 86.9% (mean of 8.9 months of follow-up). CONCLUSION: This technique offers a promising therapeutic alternative for the treatment of patients in refractory heart failure with cardiomyopathy and secondary mitral regurgitation.

OBJECTIVE: To assess the short- and long-term results of the use of streptokinase (SK) for the treatment of thromboses in cardiac valvular prostheses. METHODS: Seventeen patients with cardiac prosthetic thrombosis diagnosed by clinical, echocardiographic, and radioscopic findings underwent fibrinolytic treatment with a streptokinase bolus of 250,000 U followed by 100.000 U/hour. Short- and long-term results were assessed by radioscopy and echocardiography. RESULTS: Of the 17 patients, 12 had mechanical double-disk prostheses (4 aortic, 6 mitral, 2 tricuspid), 4 had single-disk prostheses (2 aortic, 1 mitral, and 1 tricuspid), and 1 had a tricuspid bioprosthesis. The success rate was 64.8%, the partial success rate was 17.6%, and the nonsuccess rate was 17.6%. All patients with a double-disk prosthesis responded, completely or partially, to the treatment. None of the patients with a single-disk prosthesis had complete resolution of the thrombosis. The time of streptokinase infusion ranged from 6 to 80 hours (mean of 56 h). The mortality rate due to the use of streptokinase was 5.8% and was secondary to cerebral bleeding. During streptokinase infusion, 3 (17.6%) embolic episodes occurred as follows: 1 cerebral, 1 peripheral, and 1 coronary. The rethrombosis index was 33% in a mean follow-up of 42 months. CONCLUSION: The use of fibrinolytic agents was effective and relatively safe in patients with primary thrombosis of a double-disk prosthesis. A fatal hemorrhagic complication occurred in 1 (5.8%) patient, and embolic complications occurred in 3 (17.6%) patients. In a mean 42-month follow-up, 67% of the patients were free from rethrombosis.

La microdeleción intersticial 22q11.2, es la más frecuentes en humanos; se estima una prevalencia de 1/ 4000 recién nacidos, presentando una marcada variabilidad Clínica que abarca Síndrome de Di George, Velocardiofacial, Cayler, cardiopatías conotruncales aisladas, formas autosómicas dominantes de Opitz BBB y un subtipo de esquizofrenia caracterizada por dismorfias y disfunción cognitiva. Hipótesis: en nuestro medio existe un subdiagnóstico de esta patología especialmente adultos con problemas de comportamiento tardíos, probablemente atribuible al desconocimiento de la gran variabilidad fenotípica. Objetivos: estimar la prevalencia relativa de microdeleción 22q11.2 en nuestro medio. Correlacionar los hallazgos clínicos con la citogenética de alta resolución y molecular. Diagnosticar las formas heredables. Material y Método: se evaluaran prospectivamente los pacientes derivados por los servicios de cardiología, cardiocirugía, inmunología, psiquiatría que reúnan criterios clínicos de sospecha de microdeleción 22q11.2 desde octubre de 2009 hasta septiembre de 2011. Se excluirán pacientes con cardiopatías conotruncales, insuficiencias velopalatinas, inmunodeficiencias y esquizofrenia encuadradas en otros diagnósticos. Se solicitará consentimiento informado. Se realizará evaluación clínica en el consultorio de Genética Médica. Se efectuará citogenética con técnicas de alta resolución e hibridación in situ fluorescente (FISH). A los progenitores de los pacientes positivos se les realizará igual evaluación clínica y de laboratorio. Resultados: se espera realizar diagnóstico de certeza de microdeleción 22q11.2 en el 90% de los Síndromes de Di George/Velocardiofacial, 20% de las anomalías conotruncales aisladas, 7 al 10% de las formas heredables, 2% de pacientes con esquizofrenia; en este último grupo; el porcentaje puede ascender a un 6%, según datos publicados, si se eligen subpoblaciones de pacientes con retraso del desarrollo, dismorfias, antecedentes de trastornos en el aprendizaje y el lenguaje, voz nasal, historia de hipocalcemia y de aplasia o hipoplasia tímica. Importancia del proyecto: realizar diagnóstico precoz de esta entidad en pacientes pediátricos, adultos con esquizofrenia y formas heredables, lo que permitirá realizar un abordaje interdisciplinario integral de estos individuos y su familia, contribuyendo a un uso racional de los recursos disponibles para optimizar la calidad del servicio de salud. Pertinencia: en relación al tema “innovación y desarrollo tecnológico en medicamentos y tecnología médica” se realizarán técnicas de citogenética molecular (hibridación in situ fluorescente-FISH), indicadas para realizar diagnóstico de certeza de la enfermedad, tanto en las formas esporádicas como en las heredables

INTRODUCCIÓN: Durante su evolución, las plantas han desarrollado un sistema químico de defensa con el fin de combatir el estrés del medio ambiente utilizando sus metabolitos secundarios. De todos los productos químicos secundarios sintetizados por las plantas, los terpenos han contribuido significativamente al desarrollo de nuevos compuestos y son producidos por una gran variedad de plantas, algunos animales (insectos y organismos marinos) y microorganismos. Son abundantes en frutas, cereales, verduras y flores, en musgos, algas y líquenes y son un componente importante de las resinas de las plantas, constituyendo uno de los grupos más amplios de fitonutrientes. Los terpenos son los principales componentes de los aceites esenciales de las plantas aromáticas y tienen gran actividad biológica y actúan como antioxidantes protegiendo los lípidos del ataque de radicales libres de especies del oxígeno, como oxígeno singlete, y radicales hidroxilo, peróxido y superóxido. OBJETIVO GENERAL. Determinar la composición química del aceite esencial de S. areira y la actividad anti-oxidante de la fracción rica en terpenos hidrocarburos y sus componentes mayoritarios, en un modelo experimental de pulmón de ratón. OBJETIVOS ESPECÍFICOS: a) Obtener el aceite esencial a partir de hojas de S. areira; b) Identificar y cuantificar los terpenos presentes en el aceite esencial de S. areira; c) Separar la fracción mayoritaria del aceite esencial (AE) (terpenos hidrocarburos); d)Detectar a nivel pulmonar los posibles efectos anti-oxidante de la administración intraperitoneal (i.p.) de la fracción de hidrocarburos obtenidas del aceite esencial de S. areira y de sus componentes mayoritarios, en un modelo inflamatorio. MATERIALES Y METODOS: 1) Obtención de las muestras de S. areira: Serán recolectada en la localidad de Mendiolaza, Córdoba. Un ejemplar de la misma será depositado en el Museo Botánico de la Fac. Cs. Ex. Fís. y Nat., UNC.2) Obtención del AE: El material vegetal será obtenido por destilación por arrastre por vapor de agua en un equipo tipo Clevenger modificado. 3) Fraccionamiento AE: Se separará la fracción mayoritaria del aceite que corresponde a la de los terpenos hidrocarburos con el fin de determinar su actividad biológica. Dicha separación se llevará a cabo por cromatografía en placa delgada (CCD) utilizando n-hexano o cloroformo como sistema de solvente para la fase móvil. También se determinará la actividad de los compuestos mayoritarios, los cuales serán obtenidos de muestras comerciales (ICN Pharmaceuticals) y para el caso de los que no estén disponibles en el comercio, serán aislados por técnicas cromatográficas. 4) Identificación y cuantificación de los terpenos del AE:Para la cuantificación de los terpenos, se realizará un análisis por cromatografía gas-liquido-espectrometría de masas (GC-MS) empleando un equipo Perkin Elmer Q600 equipado con detector de ionización de llama, con una columna capilar Elite-wax (Crossband-PEG) (60m x 0. 25 mm ID x 0. 25 µm df). La interpretación de los espectros de masas se realizará utilizando una biblioteca Adamns, NIST y por comparación con espectros similares tomados de bibliografía. 5) Inducción de inflamación con LPS y tratamiento con una fracción del AE de S. areira: Se procederá a la instilación nasal de LPS (1,67µg/Kg de peso corporal) y a las 2hs, la administración intraperitoneal de la fracción hidrocarbonada de AE (300 mg/Kg) y se determinará a las 3hs: TNF-α; infiltrado celular y dienos conjugados en muestras obtenidas en lavado bronqueo-alveolar en pulmón de ratón. 6) Genotoxidad: Se utilizará Allium cepa L. para evaluar aberraciones cromosómicas. Estadística : Se analizarán los datos con ANAVA: no paramétrico con Kruskal Wallis y Dunn a posterior (InfoStat, 2010). De los resultados se espera obtener un perfil químico de los terpenos hidrocarbonados de S. areira y evaluar su posible acción antioxidante.
