OBJETIVO: Verificar o padrão de crescimento de crianças nascidas a termo com peso baixo e adequado nos primeiros dois anos de vida e identificar fatores determinantes no momento de desaceleração máxima do crescimento. MÉTODOS: Estudo de coorte prospectiva com 148 lactentes nascidos a termo, em cinco municípios do Estado de Pernambuco. Os recém-nascidos foram recrutados nas maternidades no período de janeiro de 1993 a janeiro de 1994 e tiveram as medidas antropométricas aferidas com um, dois, quatro, seis, 12 e 24 meses. Os fatores de risco foram avaliados por análise de regressão linear multivariada. RESULTADOS: Houve incremento na média dos índices peso/idade e comprimento/idade mais evidente nas crianças com baixo peso do que nas com peso adequado ao nascer, especialmente nos dois primeiros meses de vida. A partir desta idade, observou-se desaceleração progressiva do crescimento até os 12 meses. O padrão de crescimento pôndero-estatural foi semelhante entre todas as crianças. Contudo, as nascidas com peso adequado mantiveram peso e comprimento acima das nascidas com baixo peso. As variáveis socioeconômicas explicaram 23% da variação do índice peso/idade, e o peso ao nascer, 4%. A condição socioeconômica explicou 28% da variação do índice comprimento/idade, seguido do peso ao nascer, altura materna e ocorrência de diarréia. CONCLUSÕES: Intervenções visando ao crescimento adequado devem ser direcionadas à assistência pré-natal e aos fatores socioambientais durante a infância, como forma de garantir a expressão máxima do potencial genético neste grupo populacional.

Besnoitia besnoiti is an apicomplexan parasite responsible for bovine besnoitiosis, a disease with a high prevalence in tropical and subtropical regions and re-emerging in Europe. Despite the great economical losses associated with besnoitiosis, this disease has been underestimated and poorly studied, and neither an effective therapy nor an efficacious vaccine is available. Protein disulfide isomerase (PDI) is an essential enzyme for the acquisition of the correct three-dimensional structure of proteins. Current evidence suggests that in Neosporacaninum and Toxoplasmagondii, which are closely related to B. besnoiti, PDI play an important role in host cell invasion, is a relevant target for the host immune response, and represents a promising drug target and/or vaccine candidate. In this work, we present the nucleotide sequence of the B. besnoiti PDI gene. BbPDI belongs to the thioredoxin-like superfamily (cluster 00388) and is included in the PDI_a family (cluster defined cd02961) and the PDI_a_PDI_a'_c subfamily (cd02995). A 3D theoretical model was built by comparative homology using Swiss-Model server, using as a template the crystallographic deduced model of Tapasin-ERp57 (PDB code 3F8U chain C). Analysis of the phylogenetic tree for PDI within the phylum apicomplexa reinforces the close relationship among B. besnoiti, N. caninum and T. gondii. When subjected to a PDI-assay based on the polymerisation of reduced insulin, recombinant BbPDI expressed in E. coli exhibited enzymatic activity, which was inhibited by bacitracin. Antiserum directed against recombinant BbPDI reacted with PDI in Western blots and by immunofluorescence with B. besnoiti tachyzoites and bradyzoites.

Background: Polymorphisms located in genes involved in the metabolism of folate and some methyl-related nutrients are implicated in colorectal cancer (CRC). Objective: We evaluated the association of 3 genetic polymorphisms [C677T MTHFR (methylene tetrahydrofolate reductase), A2756G MTR (methionine synthase), and C1420T SHMT (serine hydroxymethyltransferase)] with the intake of methyl-donor nutrients in CRC risk. Design: Patients withCRC(n 196) and healthy controls (n 200) matched for age and sex were evaluated for intake of methyl-donor nutrients and the 3 polymorphisms. Results: Except for folate intake, which was significantly lower in patients (P 0.02), no differences were observed in the dietary intake of other methyl-donor nutrients between groups. High intake of folate ( 406.7 g/d) was associated with a significantly lower risk of CRC (odds ratio: 0.67; 95% CI: 0.45, 0.99). The A2756G MTR polymorphism was not associated with the risk of developing CRC. In contrast, homozygosity for the C677TMTHFRvariant (TT) presented a 3.0-fold increased risk of CRC (95% CI: 1.3, 6.7). Similarly, homozygosity for the C1420T SHMT polymorphism also had a 2.6-fold increased risk (95% CI: 1.1, 5.9) of developing CRC. When interactions between variables were studied, low intake of all methyl-donor nutrients was associated with an increased risk ofCRC in homozygous participants for the C677T MTHFR polymorphism, but a statistically significant interaction was only observed for folate (odds ratio: 14.0; 95% CI: 1.8, 108.5). No significant associations were seen for MTR or SHMT polymorphisms. Conclusion: These results show an association between the C677T MTHFR variant and different folate intakes on risk of CRC.

Objectives - Evaluate the nutritional status of patients with inactive or mildly active Crohn's disease (CD), and identify possible causes for potential deficiencies. Methods - A total of 78 CD patients and 80 healthy controls were evaluated in respect of nutritional status, dietary intake, and life styles factors. Results - These 73/78 CD patients were on immunomodulating therapies. Mean body mass index (BMI) was lower in patients as compared to controls (P= 0.006) but 32% of CD patients and 33.8% of controls had a BMI > 25, whereas 8% and 23.8% in each group, respectively, were obese (BMI > 30Kg/m(2)). Fat free mass was significantly decreased in both genders (P < 0.05) whereas fat mass was decreased only in males (P= 0.01). Energy intake was significantly lower in CD patients (P < 0.0001) and we observed significantly lower adjusted mean daily intakes of carbohydrates, monounsaturated fat, fiber, calcium, and vitamins C, D, E, and K (P < 0.05). 29% of patients had excluded grains from their usual diet, 28% milk, 18% vegetables, and 11% fruits. Milk exclusion resulted in a significantly lower consumption of calcium and vitamin K (P < 0.001) and the exclusion of vegetables was associated to a lower consumption of vitamins C and E (P < 0.05). Physical activity was significantly lower in CD patients (P= 0.01) and this lack of physical activity was inversely correlated with increased fat mass percentage (r=-0.315, P= 0.001). Conclusions - Results showed that the most prevalent form of malnutrition in CD patients was an excess of body weight, which was concomitant with an inadequate dietary intake, namely micronutrients, clearly related to dietary exclusion of certain foods.

Thymidylate synthase, as a rate-limiting step in DNA synthesis, catalyses the conversion of dUMP into dTMP using 5,10-methylenotetrahydrofolate as the methyl donor. Two polymorphisms have been described in this gene: a repeat polymorphism in the 5' promoter enhancer region (3R versus 2R) and a 6 bp deletion in the 3' unstranslated region. Both of these may affect protein levels. The present case control study was aimed at investigating the influence of these two polymorphisms on the development of colorectal cancer (CRC), as well as their potential interaction with folate, vitamin B6 and vitamin B12 intake. A total of 196 cases and 200 controls, matched for age and sex distribution, were included in the study. No association was found between CRC and the 28 bp repeat polymorphism, but it was observed that individuals with the 6 bp/del and del/del genotypes had a significantly lower risk of developing the disease (OR=0.47; 95% CI 0.30-0.72). A combined genotype (2R/2R; 6 bp/del+del/del) was also found, which was associated with an even lower risk of developing of the disease (OR=0.42; 95% CI 0.26-0.69). No significant interaction between these polymorphisms and vitamin intake was observed. These results indicate for the first time that the 6 bp/del allele might be a protective factor in the development of CRC, independent of the intake of methyl group donors.

Background - Both genetic and environmental factors affect the risk of colorectal cancer (CRC). Objective - We aimed to examine the interaction between the D1822V polymorphism of the APC gene and dietary intake in persons with CRC. Design - Persons with CRC (n = 196) and 200 healthy volunteers, matched for age and sex in a case-control study, were evaluated with respect to nutritional status and lifestyle factors and for the D1822V polymorphism. Results - No significant differences were observed in energy and macronutrient intakes. Cases had significantly (P < 0.05) lower intakes of carotenes, vitamins C and E, folate, and calcium than did controls. Fiber intake was significantly (P = 0.004) lower in cases than in controls, whereas alcohol consumption was associated with a 2-fold risk of CRC. In addition, cases were significantly (P = 0.001) more likely than were controls to be sedentary. The homozygous variant for the APC gene (VV) was found in 4.6% of cases and in 3.5% of controls. Examination of the potential interactions between diet and genotype found that a high cholesterol intake was associated with a greater risk of colorectal cancer only in noncarriers (DD) of the D1822V APC allele (odds ratio: 1.66; 95% CI: 1.00, 2.76). In contrast, high fiber and calcium intakes were more markedly associated with a lower risk of CRC in patients carrying the polymorphic allele (DV/VV) (odds ratio: 0.50; 95% CI: 0.27, 0.94 for fiber; odds ratio: 0.51; 95% CI: 0.28, 0.93 for calcium) than in those without that allele. Conclusion - These results suggest a significant interaction between the D1822V polymorphism and the dietary intakes of cholesterol, calcium, and fiber for CRC risk.

OBJETIVO: Analisar o impacto da implementação de um programa participativo de promoção da saúde sexual em uma comunidade empobrecida, e descrever como o uso dos espaços públicos e privados para práticas sexuais constitui-se um fator que exacerba a vulnerabilidade ao HIV/Aids. MÉTODOS: Estudo etnográfico conduzido em 2002, em favela localizada no município do Rio de Janeiro. Os 6.000 moradores viviam em condições de vida deficitárias em que se verificou a ausência de políticas públicas, postos de saúde, lazer, oportunidades de emprego e segurança, o que consolida o poder de grupos criminosos. Foram abordadas as condições referentes à saúde sexual e à implantação do programa participativo de promoção da saúde sexual pelo Núcleo Comunitário de Prevenção, criado por uma organização não-governamental. Após dois meses de observação participante, foram realizadas 35 entrevistas semi-estruturadas em profundidade com moradores com idade entre 17 e 65 anos. Foram analisadas 11 histórias de vida de líderes comunitários e agentes comunitários de prevenção e sete grupos focais formados a partir dos grupos pré-existentes na comunidade. O material foi categorizado e analisado qualitativamente. RESULTADOS: A precariedade das moradias favorecia maior exposição às práticas sexuais, acentuando o estigma de ser morador de favela vivenciado pela comunidade. Com a implantação do programa do Núcleo, crianças, jovens e adultos se familiarizaram e passaram a ter conhecimento sobre prevenção do HIV/Aids; e jovens e adultos passaram a ter acesso a preservativos. CONCLUSÕES: Os resultados decorrentes da intervenção mostraram que embora a vulnerabilidade permaneça, a prevenção pode ser inserida na cultura local. A prevenção da Aids pode ser fomentada por meio de uma abordagem territorial com base na participação dos moradores e no fortalecimento da organização coletiva.

OBJETIVO: Analisar os fatores determinantes do acesso de adolescentes gestantes a serviços de atenção primária à saúde, anterior à ocorrência da gestação. MÉTODOS: Estudo transversal baseado em referencial teórico. O acesso a serviços foi analisado em cinco dimensões: geográfico, econômico, administrativo, psicossocial e de informação. Participaram 200 adolescentes primigestas (10 a 19 anos) atendidas em uma unidade básica de saúde do município de Indaiatuba (SP), em 2003. Um questionário com perguntas abertas e fechadas referentes ao acesso ao último serviço de saúde utilizado, anterior à gestação, foi aplicado às participantes no momento de sua primeira consulta de pré-natal. Os dados foram analisados por meio do teste de qui-quadrado de Pearson ou exato de Fisher e por regressão logística múltipla, considerando as cinco dimensões de acesso. RESULTADOS: Mais da metade (63,7%) das adolescentes utilizou algum serviço de saúde para consulta ginecológica. Entre as que nunca consultaram um ginecologista, as justificativas dadas foram falta de informação (43,8%) ou sentimento de medo ou vergonha (37,0%). A principal dificuldade de acesso ao serviço esteve relacionada a barreiras psicossociais, identificadas por 77,0% das adolescentes. CONCLUSÕES: Entre as barreiras de acesso ao serviço de saúde, foram significativas apenas as psicossociais. São necessárias novas estratégias para facilitar o acesso ao serviço de saúde às adolescentes, incluindo ações que diminuam as barreiras de gênero e que se considerem suas características sociodemográficas e o vínculo com seus parceiros.

O objetivo do estudo foi identificar o uso de inibidores da fosfodiesterase-5 por estudantes universitários da cidade de São Paulo (SP), em 2006. Alunos do sexo masculino (n=360) responderam questionário sobre diagnóstico de disfunção erétil, freqüência e motivo do uso, medicamento utilizado, existência de prescrição médica e relato de efeitos adversos. Os resultados mostraram que 53 (14,7%) dos alunos já haviam utilizado esses medicamentos, dos quais 53% relataram uso de sildenafila, 37% tadalafila e 10% vardenafila, adquiridos sem prescrição médica ou diagnóstico de disfunção erétil. Os principais efeitos adversos relatados foram cefaléia (23%) e rubor facial (10%), e as principais motivações para uso foram a curiosidade (70%) e potencialização da ereção (12%).

Solution enthalpies of 1,4-dioxane have been obtained in 15 protic and aprotic solvents at 298.15 K. Breaking the overall process through the use of Solomonov's methodology the cavity term was calculated and interaction enthalpies (Delta H-int) were determined. Main factors involved in the interaction enthalpy have been identified and quantified using a QSPR approach based on the TAKA model equation. The relevant descriptors were found to be pi* and beta, which showed, respectively, exothermic and endothermic contributions. The magnitude of pi* coefficient points toward non-specific solute-solvent interactions playing a major role in the solution process. The positive value of the beta coefficient reflects the endothermic character of the solvents' hydrogen bond acceptor (HBA) basicity contribution, indicating that solvent molecules engaged in hydrogen bonding preferentially interact with each other rather than with 1,4-dioxane.
