OBJECTIVE: To evaluate the performance of a continuous quality improvement collaboration at Ridge Regional Hospital, Accra, Ghana, that aimed to halve maternal and neonatal deaths. METHODS: In a quasi-experimental, pre- and post-intervention analysis, system deficiencies were analyzed and 97 improvement activities were implemented from January 2007 to December 2011. Data were collected on outcomes and implementation rates of improvement activities. Severity-adjustment models were used to calculate counterfactual mortality ratios. Regression analysis was used to determine the association between improvement activities, staffing, and maternal mortality. RESULTS: Maternal mortality decreased by 22.4% between 2007 and 2011, from 496 to 385 per 100000 deliveries, despite a 50% increase in deliveries and five- and three-fold increases in the proportion of pregnancies complicated by obstetric hemorrhage and hypertensive disorders of pregnancy, respectively. Case fatality rates for obstetric hemorrhage and hypertensive disorders of pregnancy decreased from 14.8% to 1.6% and 3.1% to 1.1%, respectively. The mean implementation score was 68% for the 97 improvement processes. Overall, 43 maternal deaths were prevented by the intervention; however, risk severity-adjustment models indicated that an even greater number of deaths was averted. Mortality reduction was correlated with 26 continuous quality improvement activities, and with the number of anesthesia nurses and labor midwives. CONCLUSION: The implementation of quality improvement activities was closely correlated with improved maternal mortality.

Background: Implementing effective antenatal care models is a key global policy goal. However, the mechanisms of action of these multi-faceted models that would allow widespread implementation are seldom examined and poorly understood. In existing care model analyses there is little distinction between what is done, how it is done, and who does it. A new evidence-informed quality maternal and newborn care (QMNC) framework identifies key characteristics of quality care. This offers the opportunity to identify systematically the characteristics of care delivery that may be generalizable across contexts, thereby enhancing implementation. Our objective was to map the characteristics of antenatal care models tested in Randomised Controlled Trials (RCTs) to a new evidence-based framework for quality maternal and newborn care; thus facilitating the identification of characteristics of effective care.

Methods: A systematic review of RCTs of midwifery-led antenatal care models. Mapping and evaluation of these models’ characteristics to the QMNC framework using data extraction and scoring forms derived from the five framework components. Paired team members independently extracted data and conducted quality assessment using the QMNC framework and standard RCT criteria.

Results: From 13,050 citations initially retrieved we identified 17 RCTs of midwifery-led antenatal care models from Australia (7), the UK (4), China (2), and Sweden, Ireland, Mexico and Canada (1 each). QMNC framework scores ranged from 9 to 25 (possible range 0–32), with most models reporting fewer than half the characteristics associated with quality maternity care. Description of care model characteristics was lacking in many studies, but was better reported for the intervention arms. Organisation of care was the best-described component. Underlying values and philosophy of care were poorly reported.

Conclusions: The QMNC framework facilitates assessment of the characteristics of antenatal care models. It is vital to understand all the characteristics of multi-faceted interventions such as care models; not only what is done but why it is done, by whom, and how this differed from the standard care package. By applying the QMNC framework we have established a foundation for future reports of intervention studies so that the characteristics of individual models can be evaluated, and the impact of any differences appraised.

Obestatin is a 23-amino acid C-terminally amidated gastrointestinal peptide derived from preproghrelin and which forms an alpha helix. Although obestatin has a short biological half-life and is rapidly degraded, it is proposed to exert wide-ranging pathophysiological actions. Whilst the precise nature of many of its effects is unclear, accumulating evidence supports positive actions on both metabolism and cardiovascular function. For example, obestatin has been reported to inhibit food and water intake, body weight gain, and gastrointestinal motility, and to also mediate promotion of cell survival and prevention of apoptosis. Obestatin-induced increases in β-cell mass, enhanced adipogenesis and improved lipid metabolism have been noted along with upregulation of genes associated with β-cell regeneration, insulin production and adipogenesis. Furthermore, human circulating obestatin levels generally demonstrate an inverse association with obesity and diabetes, whilst the peptide has been shown to confer protective metabolic effects in experimental diabetes, suggesting that it may hold therapeutic potential in this setting. Obestatin also appears to be involved in blood pressure regulation and to exert beneficial effects on endothelial function, with experimental studies indicating that it may also promote cardioprotective actions against, for example, ischaemia-reperfusion injury. This review will present a critical appraisal of the expanding obestatin research area and discuss the emerging therapeutic potential of this peptide for both metabolic and cardiovascular complications of diabetes.

One of the most important and costly complications of diabetes are foot infections that may be colonized by pathogenic and antimicrobial resistant bacteria, harboring several virulence factors, that could impair its successful treatment. Staphylococcus aureus is one of the most prevalent isolate in diabetic foot infections, together with aerobes and anaerobes.

Purpose To examine patient-reported outcome (PRO) in a selected group of Swedish patients about to receive anti-vascular endothelial growth factor (VEGF) treatment for diabetic macular edema (DME). Material and methods In this cross-sectional study, 59 patients with diabetes mellitus, who regularly visited the outpatient eye-clinics, were included. Sociodemographic and clinical data were collected and the patients completed PRO measures before starting anti-VEGF treatment. PRO measures assessed eye-specific outcomes (NEI-VFQ-25) and generic health-related quality of life (SF-36). Results The participants consisted of 30 men and 29 women (mean age, 68.5 years); 54 (92 %) patients had type 2 diabetes; Five (9%) patients had moderate or severe visual impairment; 28 (47 %) were classified as having mild visual impairment. Some of the patients reported overall problems in their daily lives, such as with social relationships, as well as problems with impaired sight as a result of reduced distance vision. Conclusions Further studies are needed to investigate PRO factors related to low perceived general health in this patient population. It is important to increase our understanding of such underlying mechanisms to promote improvements in the quality of patient care.

Plusieurs études se sont penchées sur les effets de l’activité physique maternelle sur le poids du nouveau-né, un paramètre reflétant l’environnement intra-utérin associé au risque ultérieur d’obésité et de problèmes métaboliques. Devant les taux alarmants d’obésité infantile à travers le monde, l’identification d’interventions préventives efficaces devient un enjeu majeur dans la lutte contre l’obésité et ses complications. L’activité physique maternelle pourrait être une avenue intéressante, étant donné ses effets bénéfiques sur le gain de poids et le profil métabolique maternels et son potentiel de diminution du poids de naissance de l’enfant. Cependant, la dose optimale d’activité physique et ses effets sur la composition corporelle du nouveau-né sont encore méconnus. Par ailleurs, la majorité des femmes enceintes ne rencontrent pas les recommandations en matière d’activité physique durant la grossesse et les femmes obèses, chez qui les bienfaits de l’activité physique pourraient possiblement avoir le plus grand impact, présentent souvent les niveaux les plus bas. Curieusement, peu d’études ont évalué les effets d’une intervention d’activité physique durant la grossesse dans cette population. Ainsi, avant d’envisager l’activité physique comme une intervention thérapeutique non-pharmacologique durant la grossesse, il importe d’en évaluer la faisabilité et la sécurité et d’en connaître extensivement les effets. Notamment, il s’avère primordial de vérifier s’il est possible d’augmenter en toute sécurité les niveaux d’activité physique durant la grossesse, particulièrement chez les femmes obèses, et de distinguer les effets spécifiques de différents stimuli d’activité physique (variant en type, volume, intensité et moment de la grossesse) sur la croissance fœtale. Dans ce contexte, nous avons dans un premier temps entrepris une revue systématique de la littérature des études observationnelles portant sur l’association entre l’activité physique maternelle et les paramètres de croissance fœtale mesurés à la naissance. Dans un deuxième temps, 2 études de cohortes évaluant l’impact du type, du volume, de l’intensité et du trimestre de pratique de l’activité physique ont été menées afin de complémenter et d’approfondir les résultats de la revue systématique. Finalement, une étude d’intervention randomisée a été réalisée afin d’évaluer s’il est possible d’améliorer les niveaux d’activité physique durant la grossesse chez les femmes enceintes obèses. Nos travaux ont permis d’illustrer l’influence variable que différents stimuli d’activité physique maternelle peuvent avoir sur l’anthropométrie néonatale. La revue systématique a montré qu’un volume moyen d’activité physique est associé à une augmentation du poids de naissance comparativement à un volume plus faible, alors qu’un volume élevé est associé à une diminution du poids de naissance, comparativement à un volume plus faible. Nos données suggèrent également que l’association entre l’activité physique maternelle et le poids de naissance varie en présence de certaines caractéristiques maternelles. Notamment, nous avons montré pour la première fois que l’activité physique vigoureuse pratiquée en début de grossesse était associée à une diminution importante du poids de naissance chez les femmes qui reçoivent un diagnostic de pré-éclampsie en fin de grossesse. L’importance de l’intensité de l’activité physique dans la relation entre l’activité physique maternelle et la croissance fœtale a également été soulignée pour la première fois dans notre étude de cohorte avec mesure de la composition corporelle néonatale. Contrairement à l’activité physique d’intensité modérée, l’activité physique vigoureuse en début de grossesse est associée à une diminution du poids de naissance, principalement en raison d’une adiposité néonatale réduite. Finalement, les résultats de l’essai randomisé ont permis d’établir la faisabilité d’une intervention d’activité physique supervisée visant à augmenter la pratique d’activité physique chez des femmes enceintes obèses et le potentiel d’une telle intervention à favoriser le maintien de la condition physique et une meilleure gestion du gain de poids chez ces femmes. L’ensemble de ces résultats permet de mieux cerner l’impact de l’activité physique maternelle sur la croissance fœtale, en fonction des caractéristiques spécifiques du stimulus d’activité physique mais également de la population étudiée. La faisabilité d’une intervention d’activité physique prénatale dans une population de femmes obèses laisse entrevoir de nouvelles possibilités dans la prévention de l’obésité infantile et de ses complications. L’identification d’une dose optimale d’activité physique favorisant la santé de l’enfant à court et à long terme dans diverses populations de femmes enceintes et l’identification des facteurs permettant une meilleure adhérence aux recommandations qui en découleront constituent des pistes de recherche essentielles à la lutte contre l’obésité.

L’obésité est de nos jours un problème croissant à travers le monde. La morbidité qui y est associée est surtout reliée au développement des différentes composantes du syndrome métabolique (SMet), une constellation de facteurs de risque regroupant l’hypertension, la dyslipidémie (concentration faible de cholestérol des lipoprotéines à haute densité (C-HDL) et élevée de triglycérides (TG)), l’hyperglycémie et l’obésité. Cependant, certains sujets obèses demeurent métaboliquement sains. Les facteurs génétiques joueraient donc un rôle important dans le développement de l’obésité et de ses complications. Les facteurs épigénétiques semblent également y avoir des effets. L’analyse de tissu adipeux viscéral (TAV) a donc été réalisée pour mener à la découverte de plusieurs gènes différentiellement exprimés et méthylés entre les obèses atteints et non atteints par le SMet. Les deux gènes candidats NMT1 et DGKZ font partie de ce groupe et leurs associations avec les composantes du SMet ont été testées. Leurs niveaux de méthylation et d’expression génique ont aussi été analysés.

Retinitis pigmentosa 2 (RP2) gene is responsible for up to 20% of X-linked retinitis pigmentosa, a severe heterogeneous genetic disorder resulting in progressive retinal degeneration in humans. In vertebrates, several bodies of evidence have clearly established the role of Rp2 protein in cilia genesis and/or function. Unexpectedly, some observations in zebrafish have suggested the oocyte-predominant expression of the rp2 gene, a typical feature of maternal-effect genes. In the present study, we investigate the maternal inheritance of rp2 gene products in zebrafish eggs in order to address whether rp2 could be a novel maternal-effect gene required for normal development. Although both rp2 mRNA and corresponding protein are expressed during oogenesis, rp2 mRNA is maternally inherited, in contrast to Rp2 protein. A knockdown of the protein transcribed from both rp2 maternal and zygotic mRNA results in delayed epiboly and severe developmental defects, including eye malformations, that were not observed when only the protein from zygotic origin was knocked down. Moreover, the knockdown of maternal and zygotic Rp2 revealed a high incidence of left-right asymmetry establishment defects compared to only zygotic knockdown. Here we show that rp2 is a novel maternal-effect gene exclusively expressed in oocytes within the zebrafish ovary and demonstrate that maternal rp2 mRNA is essential for successful embryonic development and thus contributes to egg developmental competence. Our observations also reveal that Rp2 protein translated from maternal mRNA is important to allow normal heart loop formation, thus providing evidence of a direct maternal contribution to left-right asymmetry establishment.

We compared the effects of a single acute dose, or chronic fetal exposure, to 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) on the male reproductive system of the Wistar(Han) rat. Tissue samples were taken from dams on GD16 and GD21, and from offspring on PND70 and 120. Steady state concentration of TCDD was demonstrated in the chronic study: body burdens were comparable in both studies. Fetal TCDD concentrations were comparable after acute and chronic exposure, and demonstrate more potent toxicity after chronic versus acute dosing. In maternal liver, cytochrome P450 (CYP)1A1 and CYP1A2 RNA were induced. In fetus, there was induction of both CYP1A1 and CYP1A2 RNA at medium and high doses, but inadequate evidence for induction at low dose in either study. The low level induction of CYP1A1 RNA at low dose in fetus argues against AhR activation in fetus as a mechanism of toxicity of TCDD in causing delay in balanopreputial separation, and the greater induction of CYP1A1 RNA in PND70 offspring liver suggests that lactational transfer of TCDD is crucial to this toxicity. These data characterise the maternal and fetal disposition of TCDD, induction of CYP1A1 RNA as a measure of AhR activation, and suggest that lactational transfer of TCDD determines the difference in delay in balanopreputial separation between the two studies.

Background: Aspirin, dipyridamole and clopidogrel are effective in secondary vascular prevention. Combination therapy with three antiplatelet agents might maximise the benefit of antiplatelet treatment in the secondary prevention of ischaemic stroke. Methodology/Principal Findings: A randomised, parallel group, observer-blinded phase II trial compared the combination of aspirin, clopidogrel and dipyridamole with aspirin alone. Adult patients with ischaemic stroke or transient ischaemic attack (TIA) within 5 years were included. The primary outcome was tolerability to treatment assessed as the number of patients completing randomised treatment. Recruitment was halted prematurely after publication of the ESPRIT trial (which confirmed that combined aspirin and dipyridamole is more effective than aspirin alone). 17 patients were enrolled: male 12 (71%), mean age 62 (SD 13) years, lacunar stroke syndrome 12 (71%), median stroke/TIA onset to randomisation 8 months. Treatment was discontinued in 4 of 9 (44%) patients receiving triple therapy vs. none of 8 taking aspirin (p = 0.08). One recurrent stroke occurred in a patient in the triple group who was noncompliant of all antiplatelet medications. The number of patients with adverse events and bleeding complications, and their severity, were significantly greater in the triple therapy group (p,0.01). Conclusions/Significance: Long term triple antiplatelet therapy was asociated with a significant increase in adverse events and bleeding rates, and their severity, and a trend to increased discontinuations. However, the patients had a low risk of recurrence and future trials should focus on short term therapy in high risk patients characterised by a very recent event or failure of dual antiplatelet therapy.

Aim Quality of service delivery for maternal and newborn health in Malawi is influenced by human resource shortages and knowledge and care practices of the existing service providers. We assessed Malawian healthcare providers’ knowledge of management of routine labour, emergency obstetric care and emergency newborn care; correlated knowledge with reported confidence and previous study or training; and measured perception of the care they provided. Methods his study formed part of a large-scale quality of care assessment in three districts (Kasungu, Lilongwe and Salima) of Malawi. Subjects were selected purposively by their role as providers of obstetric and newborn care during routine visits to health facilities by a research assistant. Research assistants introduced and supervised the self-completed questionnaire by the service providers. Respondents included 42 nurse midwives, 1 clinical officer, 4 medical assistants and 5 other staff. Of these, 37 were staff working in facilities providing Basic Emergency Obstetric Care (BEMoC) and 15 were from staff working in facilities providing Comprehensive Emergency Obstetric Care (CEMoC). Results Knowledge regarding management of routine labour was good (80% correct responses), but knowledge of correct monitoring during routine labour (35% correct) was not in keeping with internationally recognized good practice. Questions regarding emergency obstetric care were answered correctly by 70% of respondents with significant variation depending on clinicians’ place of work. Knowledge of emergency newborn care was poor across all groups surveyed with 58% correct responses and high rates of potentially life-threatening responses from BEmOC facilities. Reported confidence and training had little impact on levels of knowledge. Staff in general reported perception of poor quality of care. Conclusion Serious deficiencies in providers’ knowledge regarding monitoring during routine labour and management of emergency newborn care were documented. These may contribute to maternal and neonatal deaths in Malawi. The knowledge gap cannot be overcome by simply providing more training.

A number of infectious diseases amongst travelers and the immigrant populations are a major public health concern. Some have a long incubation period or remain asymptomatic or paucisymptomatic for many years before leading to significant clinical manifestations and/or complications. HIV, hepatitis B and C, tuberculosis or latent syphilis are among the most significant persistent diseases in migrants. Schistosomiasis and strongyloidiasis, for instance, are persistent helminthic infections that may cause significant morbidity, particularly in patients co-infected with HIV, hepatitis B and C. Chagas disease, which was initially confined to Latin America, must also now be considered in immigrants from endemic countries. Visceral leishmaniasis and malaria are other examples of parasitic diseases that must be taken into account by physicians treating incarcerated migrants. The focus of this review article is on the risk of neglected tropical diseases in particularly vulnerable correctional populations and on the risk of infectious diseases that commonly affect migrants but which are often underestimated.
